AI inference failures rarely have a single cause. A timeout can originate in the client, DNS or network path, gateway, routing control plane, model-server runtime, or a capacity signal that makes every backend ineligible. Troubleshoot in that order: define the exact scope, prove whether the request reached the provider, decompose latency, then follow the routing decision and backend eligibility signals.
Start with a precise symptom record
Do not begin by restarting model servers or increasing retries. First create a record that lets you compare one failing request with a known-good request.
- Endpoint and model: record the hostname, endpoint, model identifier, deployment, and API operation.
- Tenant scope: note the project, account, workspace, or inference pool, plus service tier and region.
- Time window: use timestamps with a timezone and include the first and last occurrence.
- Request identifiers: preserve client, gateway, trace, and provider request IDs. Redact prompts and tokens if necessary, but do not discard correlation IDs.
- Observed symptom: classify it as slow response, connection failure, timeout, HTTP error, wrong-model selection, or uneven replica load.
- Comparison: test a known healthy model, project, region, or endpoint with a controlled request when policy permits.
Filter telemetry to the affected model, service tier, project or tenant, endpoint, and time range before drawing conclusions. Aggregate dashboards can make a localized routing defect look like a provider-wide outage.
Use a triage table
| Symptom | First evidence to collect | Likely investigation path |
|---|---|---|
| Connection refused, DNS error, or client timeout | Client error, DNS result, connect and read timeout values, proxy logs | Client, DNS, firewall, load balancer, proxy, or network path |
| Provider records no request | Client and provider dashboard request counts for the same minute | Upstream timeout, proxy rejection, TLS, DNS, or connectivity |
| HTTP 400 before model execution | Gateway response body, requested routing method, required headers | Routing configuration or request contract |
| HTTP 503 or “no eligible candidates” | Candidate membership, worker readiness, queue and capacity signals | Registration, health, model loading, queue, cache, or saturation |
| Correct model but uneven replica load | Per-replica selection, affinity keys, configuration version, pod creation time | Affinity, stale configuration, seed differences, or candidate-set divergence |
| Slow but eventually successful | TTFT, total duration, output tokens, token rate, retry count | Queueing, prefill/decode saturation, output size, reasoning work, or retries |
Prove whether the request reached the provider
Compare client-side failures with provider request and error telemetry for the same endpoint and time window. The OpenAI Help Center states: “If you see client-side errors but no corresponding data in Service Health: Requests likely did not reach OpenAI.” Treat that as a routing boundary: if the provider has no record, inspect the path before the provider rather than changing model parameters.

When the provider has no record
- Check whether the client’s connect timeout is shorter than the time needed to establish a connection through your proxy or load balancer.
- Check the proxy’s upstream connect, idle, and read timeouts separately. A proxy can terminate a request while the SDK still considers it in progress.
- Validate DNS resolution from the actual workload environment, not only from a laptop.
- Look for recent firewall, NAT, certificate, service-mesh, or load-balancer changes.
- Compare the request ID generated by the client with gateway access logs. A missing gateway entry points to the client or local network; a gateway entry without an upstream attempt points to the gateway.
When the provider has a record
Follow the request into gateway and router telemetry. A provider-side record confirms arrival, not successful model execution. Continue through route selection, proxy attempts, queueing, inference, and response delivery.
Measure latency as components, not one average
Track percentiles (at least p50, p95, and p99) and error rates. Averages hide the tail that users experience during queue bursts. Record these components for the same model, tier, project, and region:
- Time to first token (TTFT): time from accepted request to the first generated token. It exposes connection, queue, scheduling, and prefill delays.
- Total request time: elapsed time until the final token or terminal error.
- Token velocity: generated tokens per second after the first token. Low velocity points toward decode or backend resource pressure.
- Queue time and inference time: separate waiting for a worker from actual model execution when the gateway exposes both.
- Retry count and retry delay: include attempts that never reached a model server.
Correlate latency with output-token volume and reasoning workload. A request producing many tokens can have a normal TTFT but a long total duration. Reasoning-heavy requests can also consume more time than short direct answers on the same model and tier. Compare like with like before labeling a route slow.
Locate the failing layer
Client and network layer
Look for DNS failures, connection resets, TLS errors, exhausted local connection pools, and client-side deadlines. Distinguish a connect timeout from a read timeout: the former means a connection was not established; the latter means an established request did not produce a response within the configured window. Capture the effective timeout after SDK, service-mesh, proxy, and load-balancer defaults have been combined.
Gateway and proxy layer
Inspect access logs for route matches, upstream target, status, bytes sent, bytes received, and termination reason. A gateway can return an error before the router runs, select a default route when a model field is missing, or retry an upstream failure without exposing the attempt count to the client.

Routing control plane
The control plane decides which model or backend is eligible. Check route configuration versions, controller health, generated routes, and rollout status. A stale replica can make different decisions from its peers even when the source configuration appears correct.
Backend and inference runtime
Inspect worker readiness, model-load state, queue depth, prefill and decode saturation, GPU or accelerator memory pressure, and backend error rates. A selected backend can still fail after routing if the model is loading, the process is unhealthy, or the runtime rejects the request shape.
Capacity and eligibility layer
“No eligible backend” is usually a signal problem, not proof that the routing algorithm is defective. A worker may be running but absent from the candidate set because it has not registered, failed a readiness check, or stopped publishing queue and capacity data.
Inspect router startup and configuration errors
Start with router pod startup logs, then compare the running configuration with the configuration you intended to deploy.
- File-read errors: the pod cannot access the mounted configuration or referenced policy file.
- JSON-parse errors: malformed syntax prevents the router from loading any policy.
- Unknown-field errors: a field belongs to a different product version or schema.
- Algorithm-factory errors: the configured routing algorithm is unavailable or misspelled.
HTTP 400 before selection
Compare the routing method requested by the function or API call with the algorithms enabled in the router. Check required gateway headers and the request fields used to select a model. For affinity-aware routing, verify that the affinity key is present, stable for the intended session, and forwarded unchanged through each proxy.

Configuration changes that appear ignored
Record the creation time and configuration version of every router pod. If a new value is absent from startup summaries, the selected deployment or stateful set may not have restarted. Confirm that the traffic is reaching the workload you updated rather than an older deployment, second region, or duplicate service.

Verify candidate registration
List the workers currently registered with the router and compare that list with the replicas you believe are running. Check registration timestamps, heartbeat freshness, readiness state, and model identity. A worker serving a different model revision or tokenizer may be intentionally excluded.
Verify the signals required by the algorithm
Routing policies can require more than a health check. Review whether each candidate publishes:
- current queue depth and estimated wait;
- available capacity or in-flight request counts;
- model-load state;
- prefill and decode utilization;
- KV-cache usage and cache availability;
- optional prefix-cache indexes or affinity metadata.
Google Cloud documentation describes inference-aware routing using signals such as KV-cache utilization, queue length, and prefix-cache indexes. If a required signal is missing, stale, or outside the router’s accepted range, the candidate can be removed even though its process is alive.
Check saturation and cache behavior
A candidate may be excluded while it loads a model, reaches a queue limit, exhausts accelerator memory, or reports a full KV cache. Compare queue depth with TTFT and token velocity. A growing queue with stable decode speed indicates admission pressure; a flat queue with falling token velocity suggests runtime resource contention.

Trace selection, proxy attempts, and retries
Build a per-request trace containing the selected candidate, every proxy attempt, each retry reason, and the final outcome. NVIDIA Cloud Functions documentation gives the exact diagnostic instruction: “Compare routing selection, proxy attempt, retry, and retry-exhaustion metrics.” This sequence shows whether the router selected a bad target, the proxy could not connect, or retries exhausted an otherwise healthy candidate set.
Replica decisions do not match
When two router replicas choose different backends for equivalent requests, compare their configuration digests, pod creation times, random seed values, startup summaries, and registered candidate sets. Differences in any of those can produce legitimate divergence. Affinity decisions also differ when the key is absent, rewritten, or hashed differently by an intermediary.
Wrong-model routing
Inspect the model identifier at four points: client payload, gateway route match, router selection, and backend request. A missing or normalized identifier can trigger a default route. Confirm that the gateway’s body-based rule parses the request content type and field path you actually send; malformed JSON or compressed bodies may prevent body matching.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Control retries, timeouts, and concurrency
Set connection and read timeouts independently and make the read timeout long enough for a valid inference at the requested output size. Then use bounded retries with backoff, a concurrency limit, and an explicit queue. Do not retry every status: retry only failures that are safe and transient for the operation, and preserve an idempotency strategy where duplicate generation would be harmful.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsWhy short timeouts create more failures
If a valid long-running request exceeds the client deadline, the client may retry while the original inference is still consuming capacity. The result is duplicate work, higher queue depth, and even more timeouts. Align deadlines across client, proxy, gateway, and backend so an outer layer does not cancel a request that an inner layer is still processing.

What to do during capacity errors
For persistent 503 or 529 responses, stop increasing traffic. Return to the last stable rate, cap concurrency, and defer or shed lower-priority work. Retries alone do not add capacity and can intensify overload.
Microsoft’s status history describes an Azure OpenAI incident on May 29, 2026, in which overloaded routing and load-balancing infrastructure became unstable; in some cases, one failed request produced up to 48 additional retry attempts. That figure is specific to that incident, not a general retry rate or recommendation.
Inspect managed gateway routing layer by layer
A managed gateway can contain several independent routing components. AWS documentation for a HyperPod gateway describes a Body-Based Router, a gateway or HTTPRoute, and an Endpoint Picker.

- Gateway and HTTPRoute: inspect route conditions, match status, hostnames, filters, and condition messages. A route can exist but remain unresolved or attached to the wrong gateway.
- Controller and router pods: check health, startup configuration, restarts, and the routes generated from the declared policy.
- Inference-pool membership: confirm that intended model servers are members, ready, and publishing usable capacity data.
- Endpoint Picker: inspect selected endpoints, rejected candidates, and picker state. Uneven load can originate here even when the gateway route is correct.
Fix the earliest failing layer first. A backend change cannot correct a gateway route that never reaches the intended inference pool.
Use a repeatable diagnostic workflow
- Freeze a failing example: endpoint, model, region, project, timestamp, request ID, status, and client timeout.
- Filter provider and gateway dashboards to that exact scope; record p50, p95, p99, error rate, TTFT, total time, and output tokens.
- Determine whether the provider recorded the request. If not, investigate client, proxy, DNS, TLS, and network paths.
- If recorded, trace route match, model selection, candidate set, selected backend, proxy attempts, retries, and terminal status.
- For 400 responses, validate routing method, schema, headers, and affinity key.
- For 503 or 529 responses, inspect registration, readiness, queue, capacity, model load, KV-cache, and saturation signals.
- Compare a failing request with a controlled request to a known healthy model, region, or project.
- Apply the smallest safe change, such as correcting a header, restarting stale router pods, restoring a candidate, or lowering concurrency.
- Recheck the same percentiles and error dimensions after the change, then document the configuration version and time of recovery.
Evaluate a routing platform before adopting it
When selecting or reviewing a routing system, judge operational visibility rather than a claimed “fastest” route. No neutral cross-provider benchmark establishes one gateway as universally best.
Quick Recap
| Capability | Questions to ask | Why it matters during an incident |
|---|---|---|
| Model-aware or body-based routing | Can it reliably parse the model field and content type used by your clients? | Prevents silent default routing and wrong-model execution. |
| Eligibility signals | Does it use readiness, queue, capacity, KV-cache, and model-load state? | Reduces selection of workers that are alive but unable to serve. |
| Decision visibility | Can operators see candidate sets, selected endpoints, and rejection reasons? | Separates routing defects from backend saturation. |
| Retry instrumentation | Are proxy attempts, retry reasons, and retry exhaustion measured separately? | Reveals amplification and hidden upstream failures. |
| Rollout behavior | Are configuration versions, pod creation times, and generated routes visible? | Exposes stale replicas and partial rollouts. |
| Regional operation | Can the same evidence and policy be inspected in every required region? | Prevents a regional failure from being mistaken for a global one. |
Prevent recurrence with an evidence checklist
- Emit one correlation ID through client, gateway, router, proxy, and model server.
- Store route-selection and candidate-rejection reasons with configuration version.
- Alert on p95 and p99 TTFT, not only average latency.
- Alert separately on connect timeout, read timeout, upstream 5xx, no-candidate events, and retry exhaustion.
- Track queue depth, KV-cache utilization, model-load state, and prefill/decode saturation per replica.
- Keep retry budgets and concurrency limits explicit, reviewed, and tested against the longest valid inference.
- Record router pod creation times and rollout completion before declaring a configuration change active.
- Maintain a low-volume health request for each critical model and region, while avoiding health checks that consume significant generation capacity.
