Copilot Notebooks does not automatically read every file, message, or website you can access. It answers from notebook references that your account can open, then selects a relevant subset for each response. A missing file is therefore usually a scope, permission, format, selection, or rollout problem—not proof that the file was deleted. An inaccurate answer can also occur when the right source was available but not selected, was outdated, or did not support the conclusion.
Use the checks below in order: confirm the reference is listed, test access with the same account, verify the source type, account for selection limits, constrain the prompt, and audit every important claim against the cited material.
What “missing” and “inaccurate” mean in Copilot Notebooks
An attached reference is not automatically used
Microsoft describes a notebook as a scoped workspace. Copilot uses references added to that notebook rather than searching all organizational content available to you. A file in a SharePoint site, OneDrive folder, Teams-connected location, or another shared area can be visible in the location and still be absent from the notebook’s usable scope.

Even when an item appears in References, it may not be selected for a particular answer or generated artifact. Shared locations are ranked by relevance, and the service uses only a limited number of accessible files for grounding. The response can therefore omit a relevant document without indicating an indexing failure.

Accuracy is not guaranteed
Microsoft’s Copilot Notebooks FAQ states that “answers may not always be accurate as they are generated based on patterns and probabilities in language data.” Treat a fluent answer as a draft until its important claims can be traced to the source. A citation proves that Copilot found related material; it does not prove that every sentence is supported by that material.
Start with this symptom-to-cause triage

1. Confirm the notebook’s actual scope
- Open the Copilot Notebook in the app or OneNote experience where you created it.
- Open the notebook’s References list. Do not rely on the fact that a file exists somewhere in your organization.
- Search the list for the exact file, page, folder, site, or link you expect Copilot to use.
- If it is absent, add that specific item with the reference method offered in your experience. A direct file reference is preferable when inclusion is important.
Check the exact object, not just its parent location
A folder or site reference does not guarantee that every child item will be used. Copilot can select only some accessible files from a shared location, based on relevance. If a contract, spreadsheet, or policy revision must affect the answer, add that file directly rather than assuming the parent folder is sufficient.
Remember generated artifacts have their own selection
Microsoft notes that a source can be attached yet not selected for a particular generated artifact. If a summary, plan, or other artifact omits a reference, inspect which sources were cited in that artifact instead of assuming that every notebook reference was included.
2. Test permissions as the asking user
- Open the source directly in a separate browser tab or the relevant Microsoft 365 app while signed in with the same account that is asking Copilot.
- Confirm that the complete file opens, not merely its title or preview. Check that you can read the pages, sheets, or sections needed for the answer.
- If the source is in a shared notebook or shared location, test access as each person who needs to use it. Notebook access and file access are separate permissions.
- After access is corrected, return to References and add or re-add the item, then run a narrowly scoped prompt.
Microsoft limits notebook grounding to content the current user is permitted to access. In a shared notebook, a recipient may see the notebook while receiving an access request for one of its linked files. That behavior is expected: sharing the container does not silently grant rights to every underlying document.

3. Verify the source type and how it was added
Documented file types
Microsoft’s reference guidance lists Word documents, PowerPoint presentations, Excel workbooks, PDFs, Loop components or pages, Copilot Pages, and OneNote pages among the supported sources. Availability can differ by account and product surface, so use the reference picker in your tenant as the final check.
For a supported file, add the item through the option shown in your notebook—such as upload, search, OneDrive, or a link—and wait for it to appear in References before asking a question. A local copy that was never uploaded is not a notebook reference.
Web links need special care
Web references must satisfy the access rules for the particular Copilot experience. Public pages or approved organizational sources may work; authenticated, paywalled, or otherwise restricted external content may not. A link to one page also does not guarantee that Copilot can crawl every linked subpage. If a web source is essential, save or upload the authoritative content in a supported file format that your account can open.

Copilot Notebooks can appear in the Copilot app and in OneNote-related experiences. The labels and available reference methods may differ. Record the app surface when troubleshooting: a source option missing in OneNote may be available in the Copilot app, while a feature documented for one experience may not have reached another.

4. Account for reference selection limits
Reference counts are product limits, not measurements of answer quality. They determine how much content can be considered for grounding and may change as Microsoft updates the service. The following figures are from Microsoft Support guidance checked in 2026.

Do not infer that the newest, largest, or most recently added file will win selection. Relevance ranking is answer-specific. When the expected document matters, direct attachment plus an explicit prompt is more reliable than relying on an inherited folder or site reference.
5. Make the prompt identify the evidence
Broad prompts invite broad retrieval. State the source by its visible title, specify the task, and request traceable support. For example:

Use this file only: “Q3 Security Review.pdf”. If the answer is not stated in the file, say that it is not stated instead of inferring it.Ask for the exact section, page, table, or heading when the source contains similar terms. If a document has revisions, name the version or date you intend to use. Microsoft warns that pointing Copilot at an old version can produce an outdated answer.
Use comparison prompts to expose omissions
When an answer seems incomplete, ask Copilot to create a two-column comparison: each claim on one side and the supporting passage on the other. Require it to list claims with no supporting passage separately. This makes unsupported synthesis visible without treating Copilot’s second response as independent verification.
6. Audit every consequential answer
- Open the citations attached to the response and verify that they lead to the expected file and version.
- For each decision-critical statement, locate the supporting sentence, table row, or calculation in the source.
- Check whether Copilot changed scope, dates, units, conditions, or exceptions while paraphrasing.
- Look for blended ideas drawn from different documents, especially when the response uses an unqualified “always,” “never,” “all,” or “guaranteed.”
- Separate what the source says from what Copilot inferred. Ask it to label inferences and unresolved conflicts.
- If a statement cannot be traced to a source, treat it as unconfirmed until a person verifies it. This is the validation rule in Microsoft’s guidance.
Pay particular attention to omitted qualifications. A policy may apply only to a region, plan, date range, environment, or user role. An answer can cite the right document while silently dropping the condition that makes the statement true.

Useful follow-up wording is: “Re-read the cited section. Which sentence supports your claim about retention? If no sentence supports it, withdraw the claim and explain the limitation.” A correction request can improve the draft, but it is still generated text; perform the same source check on the revised answer.

7. Check account, license, administrator settings, and rollout
Notebook features and source options depend on the account, subscription, license, and administrator configuration. Microsoft also says that some capabilities roll out gradually. Two people can therefore see different reference controls while using apparently similar notebooks.
- Confirm the account shown in the Copilot interface is the account that can open the source.
- Record whether the notebook is in the Copilot app, OneNote, or Copilot Chat.
- Check whether your organization has disabled a source connector or restricted external links.
- Compare the behavior with another permitted notebook only if that comparison uses the same account and app surface.
- Ask an administrator to confirm service availability before deleting and rebuilding content.
An upgrade may change availability, but it does not guarantee that a missing reference will be selected or that a generated answer will become accurate. Fix scope and evidence first; treat licensing as a prerequisite check, not an accuracy remedy.
Recovery playbooks for common cases
A file in a shared folder is ignored
- Open the file directly with the asking account.
- Add the file itself to References instead of relying on the folder.
- Prompt with the exact filename and a single question answered by that file.
- Inspect the citation. If another file is cited, ask Copilot to use the directly added file only.
The file appears, but the answer uses an older revision
- Open every cited version and compare its date or revision label.
- Remove ambiguity by naming the required version in the prompt.
- Add the current file directly and ask Copilot to state which version it used.
- Verify dates, numbers, and exceptions against the current document before acting.
A web reference cannot be read
- Open the URL without relying on an existing browser session to confirm whether it is public to the Copilot account.
- Check whether the page is paywalled, requires a separate sign-in, or depends on linked subpages.
- Replace it with an accessible organizational source or upload an authoritative copy in a supported format.
- Ask a test question whose answer is plainly visible on the page, then inspect the citation.
A collaborator sees the notebook but not the source
- Have the collaborator open the linked file directly.
- Use the file’s sharing controls to grant the necessary permission, following organizational policy.
- After permission is granted, have the collaborator refresh the notebook and confirm the file opens under their account.
- Do not assume that your successful answer proves the collaborator has the same grounding scope.
A generated summary leaves out a critical reference
- Review the citations attached to that specific artifact.
- Add the critical source directly if it was inherited through a large location.
- Regenerate with a source-specific instruction and a request to list omitted or conflicting evidence.
- Compare the final summary with the source before distributing it.
Build a repeatable verification record
For work that affects customers, compliance, finance, security, or public statements, keep a small record for each important answer:

- Notebook name and the exact references listed.
- File title, version, and date that should govern the answer.
- Whether the file was directly added or inherited through a shared location.
- The prompt used, including any “use this file only” instruction.
- Citations returned and the passages that support each material claim.
- Unconfirmed statements, conflicts, and the person who resolved them.
This record distinguishes a content problem from a permissions problem and makes a later correction possible when a document is replaced or a product limit changes.
When to escalate
Contact your Microsoft 365 administrator or support channel when a source opens for the same account, is a documented supported type, is directly listed in References, and still cannot be selected after a narrowly scoped test. Include the app surface, account type, source location, permission result, reference count, prompt, citations, and the date and time of the failure. Do not include confidential document contents unless your organization’s support process permits it.
Escalation is also appropriate when a feature documented for your organization is absent, when permissions repeatedly revert, or when different users with the same approved access receive incompatible source availability. Product behavior and rollout status can change, so record the experience rather than assuming that an older help page describes your tenant.
Bottom line
Most missing-source complaints are solved by narrowing the notebook’s scope, testing access as the asking user, adding the exact file, and recognizing which reference limit applies. Inaccurate answers require a second discipline: trace every important statement to the cited source and label anything unsupported as unconfirmed. Copilot can accelerate reading and comparison, but the source—not the confidence of the wording—remains the authority.
