MCP failures are not all permission failures. Start by identifying whether the client is launching a local server over stdio or calling a remote HTTP endpoint, then match the exact symptom to the server log. A generic client message can hide an HTTP rejection that occurred before any MCP message was exchanged.
Use this order: verify the transport and endpoint, read client and server logs together, resolve host validation, fix authentication or scopes, check protocol and method compatibility, and reconnect with a fresh transport. The distinction matters: HTTP 421, 401, and 403 point to different layers, while a local spawn ... ENOENT error means the process could not be launched at all.
1. Establish what kind of MCP connection is failing
Before changing credentials or server code, determine how the client reaches the server. MCP deployments generally use one of two arrangements:
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errors- Local stdio: the client starts a process and exchanges MCP messages through its standard input and output. The executable path, command-line arguments, working directory, environment, and process output are the first things to check.
- Remote HTTP: the client connects to a URL. The current TypeScript SDK documents Streamable HTTP as the remote transport and describes SSE as a fallback for older servers that expose only an SSE endpoint.
Write down the configured command, arguments, working directory, environment variables, and endpoint exactly as the client sees them. For HTTP, record the scheme, hostname, port, path, and whether a reverse proxy sits in front of the MCP server. A typo in the path or a client using SSE against a Streamable HTTP endpoint can look like a generic connection failure even when the host is reachable.

Do not treat a successful TCP or TLS connection as proof that MCP is working. HTTP host validation, authorization, and protocol negotiation happen after basic reachability and can reject the request before the server receives an MCP message.
2. Read both sides of the failure
Capture the client error and the server log from the same connection attempt. Clients often collapse a useful HTTP response into text such as “server returned an error response” or “failed to connect.” The server may show the status code, request host, authorization challenge, or protocol method that explains it.
What to record
- The timestamp of the attempt and the endpoint or command used.
- The transport selected: stdio, Streamable HTTP, or legacy SSE.
- The complete client error, including nested causes and response status.
- The server log line at the same time, including HTTP status and request path.
- Whether the server process started, stayed running, and reached its MCP initialization handler.
- For OAuth-protected endpoints, the token resource or audience, expiry, and requested scopes, without publishing the token itself.
If the server log contains no request at all, investigate DNS, routing, TLS, proxy rules, or process startup before changing MCP authorization. If the server logs an HTTP response, use that response as the primary branch in the troubleshooting map below.
3. Use the symptom to choose the right branch
| Symptom | First checks | Likely diagnostic layer |
|---|---|---|
| Generic remote connection error | Endpoint, reachability, selected transport, server log | HTTP setup, proxy behavior, or transport mismatch |
| HTTP 421 or “Invalid Host header” | Host header, allowlist, reverse proxy configuration | Server host validation |
| HTTP 401 | Bearer token presence, syntax, expiry, resource or audience | Authentication |
| HTTP 403 or insufficient scope | Token scopes and server-required scopes | Authorization |
| Method not found or version-dependent behavior | Protocol revision and method support on both peers | Protocol compatibility |
spawn npx ENOENT or a similar launch error |
Executable path, PATH, command, and process output | Local process startup |
Apply only the branch supported by the evidence. Changing an OAuth token cannot repair a wrong Host header, and changing a protocol revision cannot make an executable appear on the system PATH.
4. Fix local stdio servers that never start
With stdio, the client is also the process supervisor. The MCP server must be executable in the client’s environment, start successfully, and keep its protocol stream separate from diagnostic output.

Check the executable and PATH
The TypeScript SDK getting-started documentation identifies spawn npx ENOENT as a sign that the command is not executable on PATH. The same principle applies to any command: an absolute path that works in an interactive shell may fail when the client starts it with a smaller environment.
- Run the configured command manually using the same user account as the client.
- Check that the executable exists and is executable.
- Compare the interactive shell’s PATH with the PATH inherited by the client process.
- If the command is a package runner or script, use a path the client can resolve rather than relying on shell aliases or profile files.
- Verify every argument, including the server’s configuration path and working directory.
Do not “fix” an ENOENT error by changing server permissions unless the executable is present but denied. ENOENT means the process launcher could not find the command or a required path component; it is not evidence that an MCP token lacks permission.

Inspect process output and stream discipline
After the process launches, inspect its standard error and exit status. A server can start and then terminate because of a missing environment variable, invalid configuration, unavailable dependency, or port conflict. If the server writes logs to standard output while stdio is carrying MCP messages, the client may see malformed protocol data. Configure diagnostics on standard error or in a separate log where the server supports that arrangement.
Restart instead of reusing a dead session
When a local process exits, close the failed transport and let the client’s normal connect flow create a new process. A stale session cannot be repaired by sending another initialize message after its underlying process has gone away.
5. Resolve remote transport and endpoint mistakes
For a remote server, confirm that the client and server agree on the HTTP transport. Streamable HTTP is the documented remote transport in the TypeScript SDK. SSE remains relevant when connecting to an older SSE-only server, but selecting the wrong transport can produce an endpoint or method error even though the URL responds.

Verify the endpoint exactly
- Confirm the hostname resolves to the intended deployment.
- Confirm the port and path, including any path prefix added by a reverse proxy.
- Check that the URL scheme matches the deployment’s TLS setup.
- Ensure the proxy forwards the MCP path and the request headers required by the selected transport.
- Check whether the client is configured for an older SSE endpoint while the server expects Streamable HTTP, or the reverse.
A browser visit is not a complete MCP test: browsers may follow redirects, omit authorization headers, or issue a different HTTP method. Use the MCP client’s connection flow and compare its request with the server log.
Separate network failure from application rejection
If DNS, TLS, or routing fails, the server may never log a request. If the server logs HTTP 421, 401, or 403, the network path reached the application and the next steps belong to host validation or authorization. This separation prevents repeated changes to firewall or proxy settings after the request is already arriving at the server.
6. Fix HTTP 421 and “Invalid Host header”
HTTP 421 with an “Invalid Host header” message indicates host validation in the documented Python SDK case. The server received the request but rejected the hostname before normal MCP processing. A freshly deployed server that refuses every connection is, as the MCP Python SDK troubleshooting documentation puts it, “A Host allowlist until proven otherwise.”
Check the host the server actually receives
Compare the client’s URL with the Host header recorded by the server or reverse proxy. A proxy can preserve the public hostname, replace it with an internal service name, or append a port. The allowlist must be designed for the hostname that reaches the validation layer, not merely the address used in a browser or container configuration.
Configure an intentional allowlist
- Identify every legitimate hostname used to reach the deployment, such as the public name and any controlled internal name.
- Configure the MCP server’s host allowlist using the setting and syntax for the SDK version in use.
- Review reverse-proxy behavior so the forwarded Host value is deliberate and consistent.
- Retest through the same proxy and hostname the client uses.
- Keep the allowlist narrow; binding the server to a non-localhost interface does not by itself define a safe host allowlist.
Do not solve 421 by broadly accepting arbitrary hosts. Host validation is a deployment security control, and the correct setting is SDK- and proxy-specific. Follow the deployment guidance for the Python or other SDK version actually running.

7. Diagnose HTTP 401 authentication failures
In the TypeScript SDK authorization flow, HTTP 401 means the request lacks a usable bearer token. The documented causes include a missing, malformed, or expired token. Treat this as an authentication problem, not a protocol-version problem.
Check the token presentation
- Confirm the client sends an
Authorization: Bearer ...header to the MCP endpoint. - Check for accidental whitespace, quoting, truncation, or a token copied into the wrong configuration field.
- Verify the token has not expired and that the client clock is reasonably accurate.
- Make sure a proxy is not stripping the Authorization header.
- Inspect the server’s OAuth challenge and protected-resource metadata rather than guessing the issuer or resource.
Complete the OAuth flow when the client supports it
Some MCP clients can discover protected-resource metadata, direct the user to an OAuth provider, obtain a token, and retry the connection. Confirm that an OAuth provider is configured for the server and complete the authorization flow before reconnecting. If the client is expected to manage OAuth but repeatedly sends no token, inspect its provider configuration and callback or redirect setup.
Do not paste access tokens into logs or support tickets. Record only the status, challenge, resource or audience information, expiry state, and scopes needed to diagnose the request.

HTTP 403 is different from 401. The TypeScript SDK documentation uses 403 for a token that is accepted but does not have the required scopes. Replacing a valid token with another token of the same insufficient scope will not help; the server’s authorization policy must be matched.

- Read the server’s required scopes for the operation or protected MCP resource.
- Inspect the scopes granted in the access token’s claims or authorization response.
- Check that the token was issued for the expected resource or audience, not merely by the correct identity provider.
- Update the client’s requested scopes or the user’s consent and server-side policy as appropriate.
- Obtain a new token after scope changes; an already-issued token normally retains its original claims until it expires.
A token can authenticate the caller and still be forbidden from a particular operation. Keep transport settings unchanged while correcting scopes. If a supposed scope fix changes the response to 401, recheck token formatting, expiry, and resource audience separately.

9. Check protocol revision and method compatibility
Once transport and authorization succeed, an MCP client can still fail because the peers support different protocol revisions or because the requested method is unavailable in the negotiated revision. Typical clues are “method not found,” an initialization that succeeds followed by an operation failure, or behavior that differs between client and server versions.
Verify both peers’ capabilities
- Identify the protocol revision advertised or configured by the client.
- Identify the revision and capabilities implemented by the server and its SDK version.
- Check whether the method exists in that revision and whether it is enabled by the server.
- Compare the initialization and capability negotiation messages when logging is available.
Do not infer a protocol mismatch from an authorization response. A 401 or 403 is evidence about credentials and scopes, not proof that the two peers speak different protocol eras. Resolve authentication first when the server is explicitly returning an HTTP authorization status; investigate revision and method support when the request reaches MCP and fails at the method layer.
Choose a compatible upgrade or fallback
Use a client and server combination whose documented protocol revisions overlap. If an older server offers only SSE, configure the client’s documented SSE fallback rather than forcing Streamable HTTP. If a method was introduced after the server’s revision, use an available method or upgrade the server and client together. Record the versions during troubleshooting because SDK behavior and protocol details can change.

10. Reconnect cleanly after every fix
After correcting the specific cause, use the client SDK’s normal connect flow and close the failed or completed transport cleanly. Do not reuse a session after the server has restarted, the process has exited, or the HTTP transport has closed.
- Stop or disconnect the failed client transport.
- Restart the local server process if it was launched over stdio, or restart the deployment only when its configuration requires it.
- Clear cached credentials only when the token or OAuth state is demonstrably stale; avoid deleting unrelated client configuration.
- Reconnect using the same endpoint and transport that you just corrected.
- Confirm initialization, capability negotiation, and one low-risk operation before restoring normal workloads.
If the same symptom returns, capture a new client/server log pair. A changed status code is useful evidence that you moved to a different diagnostic layer.
11. A practical decision procedure
- Classify the transport. Is the client launching a process over stdio, calling Streamable HTTP, or using SSE for an older server?
- Confirm the target. Check command, arguments, endpoint, path, proxy, and effective environment.
- Check whether the server saw the request. No server log suggests startup or network reachability; an HTTP log means the request arrived.
- Branch on the response. Handle 421 as host validation, 401 as token validity, and 403 as scope or policy.
- Inspect MCP-level errors. After HTTP and authorization succeed, compare protocol revisions and method support.
- Reconnect fresh. Close stale transports and verify the corrected path with a new session.
This sequence avoids the most common diagnostic mistake: treating every failure as a permission issue. A process that cannot be spawned, a request rejected for its Host header, and a token denied for insufficient scope require different owners and different fixes.
12. Prevent repeat failures
- Document the selected transport, endpoint, proxy path, host allowlist, SDK versions, and supported protocol revisions.
- Keep server logs available alongside client logs and preserve HTTP status details instead of logging only a generic connection message.
- Monitor token expiry and scope changes without recording secret token values.
- Test the public hostname through the actual reverse proxy, not only by connecting directly to an internal address.
- Keep local launch commands explicit and verify them under the same account and environment used by the client.
- After SDK upgrades, recheck transport defaults, authorization metadata handling, host-validation settings, and protocol revision support.
Official SDK pages describe implementation details that can evolve. Apply each setting to the SDK and server version in use, and re-read the relevant transport, deployment, authorization, and protocol documentation when upgrading.
