Track Medicare Advantage quality measures with a year-specific inventory of applicable CMS specifications, contract and population-level baselines, evidence checks, assigned follow-up, and outcome validation. Treat care-gap closure as the operational work that improves a measure—not as a substitute for the measure itself. CMS Star Ratings summarize contract performance, HEDIS supports quality work for Medicare Special Needs Plans (SNPs), and CAHPS reports beneficiary experience. These sources answer different questions and should be managed in one coordinated, but not conflated, process.

What you are tracking—and what each result means

A Medicare Advantage quality program usually combines three evidence streams. They differ in unit of analysis, population, timing, and permitted evidence. A contract can have strong clinical results and weak experience results, or the reverse, so one score cannot stand in for the others.

Source Unit of analysis What it measures How teams use it
CMS Star Ratings Medicare Advantage or Part D contract Applicable clinical, process, drug, and experience measures combined into one- to five-star results using CMS cut points Review contract performance, priorities, current requirements, and year-over-year risk
HEDIS Applicable measure population, including SNP populations Specified clinical and process performance Identify gaps, monitor quality-improvement initiatives, compare results, and set targets for SNP work
MA and PDP CAHPS Surveyed plan enrollees and contract results Access, appointments, communication, coordination, and plan experience Find experience and access problems and select improvement actions; CMS also publishes comparisons

Star Ratings are contract-level results. A care manager works at member, provider, and workflow level to produce the numerator evidence that an applicable specification accepts. Therefore, a care-gap list is an intervention queue, not a Star Rating report.

Start with the correct measurement and rating year

Do not carry last year’s thresholds or specifications forward automatically. CMS states that “The cut points are recalculated each year based on performance during the measurement period.” The performance period, submission rules, measure set, weights, and cut points can all change.

At the beginning of a cycle, record these items in a controlled planning sheet:

  • Measurement year for the clinical or process event.
  • Star Ratings year to which the result contributes.
  • Contract number, contract type, and whether the contract is MA-PD, MA-only, or standalone PDP.
  • Version and publication date of every CMS technical note, data table, and measure file used.
  • Any measure that is new, respecified, removed, restored, or assigned different reporting treatment.

The CMS Part C and D Performance Data index provides the current technical notes, Star Ratings data tables, and measure files, and is updated regularly. Use the file version that applies to your reporting cycle rather than a locally copied checklist.

Know the size of the current Star Ratings set

For the 2027 Star Ratings published October 8, 2026, CMS lists these maximum counts:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Contract type Maximum measures
MA-PD 43
MA-only 32
Standalone PDP 13

These are program counts, not a promise that every measure applies to every contract or that every contract receives a rating on every listed measure. Confirm applicability in the current files for the specific contract.

Build a measure inventory from the authoritative specifications

Create one row for every applicable measure and preserve the source language needed to audit it. A useful inventory contains:

  • Measure identity: official name, abbreviation, domain, and the Star Ratings year.
  • Population: enrollment, age, clinical, pharmacy, or SNP eligibility criteria.
  • Denominator logic: inclusion rules, continuous-enrollment requirements, allowable exclusions, and the performance period.
  • Numerator evidence: the event, result, or documentation that satisfies the current specification.
  • Data source: claims, encounter data, pharmacy data, laboratory results, clinical records, survey results, or another permitted source.
  • Submission and validation rules: file timing, data-quality checks, audit artifacts, and reconciliation requirements stated in the specification.
  • Owner and due dates: the team responsible for data, outreach, provider action, clinical review, and final validation.
  • Status: baseline, in progress, numerator confirmed, excluded, unable to reach, or awaiting evidence.

Specifications are measure-specific. A generic “screening completed” flag is not enough if the current rule requires a particular date range, result, provider type, or record element. Keep the exact inclusion, exclusion, and evidence rules beside the operational workflow so staff do not promise credit for activity that cannot be counted.

Establish a reliable baseline before opening care gaps

Pull the latest available performance result for each applicable measure, then reconcile its population to your enrollment and data sources. The baseline should show more than a percentage. Include the denominator count, numerator count, exclusions, data refresh date, source system, and whether the result is preliminary or validated.

Separate four kinds of “gap”

Every apparent gap should be classified before outreach begins:

Gap type Typical symptom First response
Clinical need The member is eligible and the required service or result is genuinely outstanding Offer clinically appropriate follow-up through the care team and document the qualifying evidence
Access barrier The member wants care but cannot obtain an appointment, transportation, language support, or an affordable service Resolve the barrier, record the intervention, and confirm completion
Documentation gap The service occurred, but the plan lacks an acceptable result, date, code, or record Retrieve and validate the source document; do not assume outreach alone changes the numerator
Data-quality or attribution issue Duplicate enrollment, stale provider assignment, delayed encounter, or conflicting records creates a false gap Correct the source data, rerun eligibility, and retain the reconciliation trail

This classification prevents a call campaign from being used to solve a feed problem and prevents a data correction from being counted as a clinical intervention.

Segment the population for action

Prioritize members using variables that are actually present and permitted for the program: measure eligibility, due date, prior result, recent utilization, provider relationship, language, contact preference, transportation or access information, and whether a previous attempt failed. Keep sensitive information restricted to staff with a legitimate operational need. A segment should lead to a different action—for example, scheduling help, record retrieval, medication review, or clinical escalation—not merely create a more elaborate report.

Turn a gap list into an owned closure workflow

Use the following sequence for each member-measure record. It is an implementation pattern; the current CMS and applicable measure specifications remain the authority for what qualifies.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Confirm eligibility. Recheck contract attribution, enrollment, age or clinical criteria, measurement period, exclusions, and duplicate records against the current specification.
  2. Identify the missing evidence. State exactly what is absent: a service, a result, a date, a medication review, a provider record, or a valid data feed.
  3. Assign one accountable owner. Name the care manager, provider liaison, pharmacy team, data steward, or clinical reviewer who must move the case forward. Add a due date and escalation path.
  4. Choose the least burdensome effective intervention. Examples include scheduling with an existing provider, arranging transportation, requesting a chart document, resolving a language need, coordinating a referral, or reviewing medication risk with the appropriate clinician.
  5. Record outreach and outcome separately. Log each attempt, channel, date, response, barrier, appointment, referral, and document request. An attempted call or scheduled appointment is a workflow event, not automatically a numerator event.
  6. Verify the qualifying evidence. Reconcile the completed service or result to the accepted data source and required fields. Reject incomplete, out-of-period, duplicate, or otherwise nonqualifying evidence.
  7. Close or requeue the case. Mark numerator confirmed, valid exclusion, unable to reach, declined, still due, or data correction needed. Set the next action for every nonclosed status.
  8. Measure the result. Update the validated numerator and denominator view, not just the outreach dashboard. Review whether the intervention changed the rate and whether the same failure is recurring.

Use leading and confirmed indicators side by side

Leading indicators show whether the operating process is moving: eligible members loaded, records reviewed, outreach attempts, appointments secured, referrals accepted, documents requested, and barriers resolved. Confirmed indicators show whether the specification was met: qualifying service or result, accepted date, valid evidence, and updated measure status. Display them in separate columns and dashboards. Combining them inflates apparent performance and obscures where the process is failing.

Design the data and evidence controls

A practical tracker can be a governed database, data warehouse view, or controlled work queue. Whatever the technology, preserve an auditable link from the member-measure record to the evidence used for closure.

  • Identity controls: stable member and contract identifiers, attribution date, and duplicate detection.
  • Time controls: measurement-period dates, service date, receipt date, load date, and next review date.
  • Evidence controls: source system, document type, result value, provider, and reviewer decision.
  • Change controls: specification version, rule-change date, user who changed status, and reason for reopening or exclusion.
  • Privacy controls: role-based access, minimum necessary fields, secure transfer, retention rules, and a documented correction process.

Run reconciliations between claims, encounters, pharmacy feeds, clinical records, enrollment, and the care-management queue. Investigate sudden rate changes before treating them as improvement; a late feed, attribution change, duplicate file, or specification update can create the appearance of progress or decline.

Use HEDIS correctly for Medicare SNP quality improvement

CMS describes HEDIS performance data for SNPs as useful for identifying performance gaps, monitoring quality-improvement initiatives, tracking progress, comparing performance, and establishing realistic targets. That makes HEDIS a strong operating input for SNP teams.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not turn a HEDIS result into a universal rule for every Medicare Advantage contract. Confirm the applicable population, measure year, technical definition, and submission requirements in the current materials. The CMS SNP HEDIS overview was last modified September 10, 2024, so verify current CMS and NCQA specifications before implementation.

A SNP HEDIS review cycle

  1. Load the current HEDIS result and denominator detail for the SNP population.
  2. Compare the result with the prior validated cycle and the target approved by the quality committee.
  3. Break the gap into clinical need, access, documentation, and data-quality causes.
  4. Launch targeted interventions with provider and care-management owners.
  5. Check intermediate activity weekly or monthly, but validate HEDIS evidence on the schedule required by the measure.
  6. Document the intervention, outcome, unresolved barriers, and next target for the quality-improvement record.

Use CAHPS to find experience and access barriers

CAHPS is not a clinical numerator feed. The MA and PDP CAHPS survey captures beneficiary views of obtaining needed care, getting appointments, provider communication, care coordination, and overall plan experience. CMS publicly reports results and says contracts use them to identify improvement areas.

Bring CAHPS findings into the same governance meeting as clinical measures, but keep the action logic distinct. A poor appointment-access result may explain why a screening gap persists, while a communication result may point to a provider or language-support intervention. The survey result itself does not prove that an individual member completed a clinical service.

Convert an experience finding into an operational test

  • Translate the survey domain into a measurable process question, such as time to third-next-available appointment or percentage of referral requests receiving a response.
  • Choose a limited pilot population or provider group and define the baseline before intervention.
  • Make the access or communication change, then monitor both the process indicator and applicable clinical measure.
  • Use the next available CAHPS result and internal access data to decide whether to scale, revise, or stop the intervention.

Refresh for the 2027 Star Ratings changes

CMS’s 2027 materials include several measure-set changes that require an inventory refresh. The ratings added Concurrent Use of Opioids and Benzodiazepines and Polypharmacy: Use of Multiple Anticholinergic Medications in Older Adults. Care for Older Adults—Functional Status Assessment was restored after specification changes. A respecified Colorectal Cancer Screening measure moved into the ratings.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

CMS also states that the new Part C Depression Screening and Follow-Up measure begins with measurement year 2027 and appears in the 2029 Star Ratings. Do not add that measure to an earlier-year numerator or assume that a workflow built for another depression measure satisfies its future specification. Track the effective measurement year separately from the year in which a rating is published. See the Contract Year 2027 Medicare Advantage and Part D Final Rule fact sheet and the current performance-data files for implementation details.

Set a repeatable operating cadence

CMS does not prescribe one universal care-gap calendar. A practical cadence keeps rules, data, action, and validation synchronized.

Dashboard measures that support decisions

A useful dashboard lets leaders distinguish volume from verified improvement. Include:

  • Validated rate, numerator, denominator, and exclusions for every applicable measure.
  • Change from the prior validated period, with the measurement period identified.
  • Members newly eligible, members due, members with a confirmed numerator, and members still unresolved.
  • Closure rate by gap cause, provider, region, language, access barrier, and outreach channel when those fields are reliable.
  • Median days from gap identification to assignment, intervention, evidence receipt, and validation.
  • Percentage of cases reclassified as documentation or data-quality issues after review.
  • CAHPS access and communication findings paired with the internal process indicators chosen to address them.
  • Open exceptions caused by late feeds, attribution conflicts, duplicate records, or rule changes.

Set targets only after confirming the year-specific measure definition and baseline. For SNP HEDIS, CMS specifically describes using the data to establish realistic targets; a target based on the wrong year’s denominator or cut point is not realistic or auditable.

Common failure modes and recovery steps

Using an old cut point or specification

Symptom: the dashboard compares current performance with a prior threshold or uses an outdated denominator rule. Recovery: retrieve the current CMS technical note and measure file, version the inventory, rerun eligibility and numerator logic, and label any restated trend.

Counting outreach as closure

Symptom: calls, reminders, referrals, or booked appointments are reported as completed care. Recovery: separate activity from confirmed evidence and require the specification-accepted result before changing numerator status.

Applying every measure to every contract

Symptom: a generic enterprise checklist assigns measures that do not apply to the contract or population. Recovery: filter the inventory by contract type, eligibility, reporting year, and current CMS applicability rules.

Ignoring documentation and feed defects

Symptom: providers report completion while claims, encounters, or records show no qualifying evidence. Recovery: open a data-quality or documentation case, request the precise missing element, and retain the source and reviewer decision.

Treating CAHPS as a clinical measure

Symptom: a favorable survey response is counted as proof of a service, or a poor survey score is treated as proof that every member has the same barrier. Recovery: use CAHPS to select experience and access interventions, then monitor the relevant clinical and process measures independently.

Failing to document rule changes

Symptom: staff cannot explain why a measure appeared, disappeared, or changed rate. Recovery: maintain a CMS update log showing publication date, effective measurement year, affected workflow, owner, training date, and first cycle using the new rule.

Governance and accountability

Assign decision rights before the first gap file arrives. Quality leadership should approve the applicable measure inventory and targets. Data stewards should certify source completeness and reconciliation. Clinical leaders should approve interventions that require clinical judgment. Care-management and provider teams should own outreach and barrier resolution. Compliance or audit staff should test evidence, access, and change controls.

Use a regular review that asks four separate questions:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Are we calculating the right population and rule for this year?
  2. Are we reaching the members and providers who can change the result?
  3. Are completed actions producing evidence the specification accepts?
  4. Did the validated result improve without creating a new access, equity, or data-integrity problem?

Document unresolved limitations instead of hiding them in a percentage. A stable exception queue with an identified owner is more useful than a high closure rate that includes unverified activity.
