A mattress is probably too firm for side sleeping when your shoulder or hip takes concentrated pressure, your waist leaves a noticeable gap, or you cannot feel reasonably even contact from your ribs to your pelvis. Those signs are clues rather than a medical diagnosis. A bed can also be too soft: if your hips sink substantially lower than the rest of your body, your spine may be pulled out of alignment. Test the way your body meets the mattress, then check your pillow, sleep position and the condition of the bed before deciding whether you need a topper or a replacement.
What “too firm” means for a side sleeper
Side sleeping puts much of the body’s load on the shoulder, ribs and hip nearest the mattress. A surface that does not yield enough can concentrate force at those points instead of spreading it over a broad area. Sleep Foundation identifies shoulder and hip soreness after side sleeping as a possible sign that the surface is not cushioning those areas adequately. It also describes a possible gap between the waist and mattress when a bed is overly firm.
Firmness is not a universal number. The same mattress can feel supportive to one person and uncomfortably hard to another because body weight, shoulder and hip width, body shape, sleep position and personal comfort preferences change how far someone sinks into the materials. A “medium” label is therefore a starting point, not proof that a bed fits you.

Signs that your mattress may be too firm
Shoulder or hip soreness where you make contact
Notice whether discomfort is concentrated on the shoulder or hip pressed into the bed, rather than spread through your whole body. Record when it starts: immediately, after several hours, or only after repeated nights. If changing sides changes which shoulder or hip hurts, the pattern is worth investigating. Sleep Foundation treats this pattern as a clue that the mattress may not be cushioning the pressure points, not as proof that the mattress caused the pain.
A gap under the waist
In your normal side-sleeping posture, ask whether your waist appears to bridge over the mattress while your ribs and pelvis make contact. A visible or easily felt gap can indicate that the surface is not contouring to your body. Do not force your waist down or twist your spine to create contact; observe the position you naturally take when trying to sleep.
Pressure that improves on a softer surface
If a brief trial on a softer surface reduces shoulder or hip pressure without making your hips sag, that comparison supplies useful information. It still does not establish a diagnosis, because pillows, posture, bedding and an underlying health condition can change symptoms.
Numbness, tingling or repeated repositioning
Feeling compelled to move away from one shoulder or hip, waking to change sides, or noticing transient pins-and-needles at a pressure point can indicate that the surface is concentrating load. These sensations are not unique to mattress firmness, so treat them as prompts to investigate rather than as a pass-or-fail test. Note whether they disappear when you use a different sleep surface or posture.
The ribs-to-pelvis contact check
Occupational Health Clinics for Ontario Workers (OHCOW) describes a practical check: lie on your side and consider whether the mattress contacts your body from the ribs through the pelvis with broadly similar pressure. Use it as an observation of contact, not as a precise measurement of spinal alignment.
- Use your usual posture. Lie on the side in which you normally fall asleep. Keep your legs and arms in their ordinary positions rather than exaggerating a straight or curled posture.
- Scan the contact zone. Notice the ribs, waist and pelvis in sequence. You are looking for a reasonably continuous, even-feeling contact rather than one small area carrying most of the load.
- Identify gaps. If a substantial section, especially around the waist, barely touches the surface while the shoulder and hip bear pressure, the mattress may be too firm for your shape.
- Identify deep areas. If one region feels much more compressed or sits clearly lower than the surrounding body, the surface may be too soft instead.
- Repeat on the other side. Differences between sides can reflect your anatomy or an uneven mattress. A pronounced difference is a reason to inspect the bed rather than relying only on a firmness label.
OHCOW’s guidance distinguishes “substantially less pressure” (a possible too-firm pattern) from “substantially greater pressure” or a body region sitting lower (a possible too-soft pattern). It does not provide a numerical threshold, and the check has not been established as a clinical test.
How to make the home check more reliable
Perform the check when you are rested and use the sheets and protector you normally sleep on. Thick quilts or stacked pads can hide the mattress’s actual contour, while testing directly on a bare mattress may feel unlike your usual setup. Lie still for several minutes, then make the small movements you normally make at night. Ask a partner to observe whether the waist is visibly suspended or whether the hips drop lower, but do not let anyone push on your body or reposition you.

Repeat the observation at different times of day and on both sides. A single night can be distorted by exercise, an old injury or an unusual sleeping position. Keep a simple log of the side you used, the location and timing of discomfort, and how you felt on waking. The goal is to find a consistent pattern, not to produce a precise firmness score.
Do not use a ruler, phone app or pressure map as if it supplied a medical threshold. Consumer measurements may describe a mattress, but they cannot account for your anatomy or determine why pain occurs. A visual gap, uneven contact and changes in symptoms after a reversible adjustment are more useful decision clues.
How to tell too firm from too soft
| What you notice | What it may suggest | What it does not prove |
|---|---|---|
| Shoulder or hip receives concentrated pressure; waist appears unsupported or leaves a gap | The surface may be too firm to contour to your side-sleeping shape | That the mattress is the sole cause of pain |
| Hips sink substantially lower; one body region feels deeply compressed; contact pressure is much greater in one area | The surface may be too soft or no longer supportive | That a firmer mattress will automatically solve the problem |
| Broad, reasonably even contact from ribs to pelvis with pressure points cushioned | The firmness may be a workable fit | That the mattress is medically appropriate or comfortable for every night |
Pain by itself cannot tell you which direction to move. Excessive firmness can leave the shoulder and hip sore, while excessive softness can let the hips sink and pull the spine out of alignment. Judge pressure and support together.
Watch for mixed signals. A mattress can feel hard at the shoulder yet allow the pelvis to sink because its comfort layer is worn unevenly. Conversely, a new, resilient mattress may feel firm at first but still provide continuous contact once your body settles. Evaluate each region rather than assigning one label to the whole bed.

A sleeper’s weight determines how much the comfort layers compress. Body shape matters as well: broad shoulders or hips need enough give to avoid concentrated pressure, while a lighter sleeper may not sink far into a mattress that feels soft to someone heavier. Sleep Foundation’s general guidance places many side sleepers between medium-soft and medium-firm, but presents that range as a guideline rather than a prescription.
When comparing beds, ask how your body actually interacts with each one instead of choosing by the printed firmness score. A lighter person may experience a nominally medium mattress as firm; a heavier person may compress the same model enough to feel little surface resistance. Your preferred feel also matters, provided the bed still supports you without painful pressure or excessive sinking.
Couples should assess each side separately. A partner’s weight can make a shared mattress feel different across the surface, and a center support or edge may have a different firmness from the middle. If one sleeper needs more cushioning and the other needs more support, compare each person’s ribs-to-pelvis contact rather than averaging their impressions. Separate toppers or different comfort layers may be practical only if the mattress and base remain stable.

“Back and stomach sleepers may benefit from a firmer mattress, as extra support can feel more comfortable in these positions. Side sleepers may prefer a mattress that has softness. It’s also important for side sleepers to find an option that ensures the head and neck are in alignment. Whatever is the most comfortable for you to sleep with is worth the investment.”

Check the pillow before blaming the mattress
A pillow that is too low can let your head drop toward the mattress; one that is too high can push it upward. Sleep Foundation recommends a supportive pillow that fills the space between your head and the mattress so your head and neck remain aligned with your spine. On your side, the needed height depends on shoulder width, mattress compression and your anatomy.
Test the pillow separately: lie in your ordinary position and ask a partner to look from behind, or use a photograph, to see whether your neck appears level with the rest of your spine. This is an alignment observation, not a diagnostic test. If changing the pillow alters neck or shoulder discomfort while the hip pressure remains unchanged, the pillow is a separate contributor.
A small pillow or rolled towel between the knees is another option described by Sleep Foundation. It can reduce hip pressure and limit the upper leg twisting the spine. Use it as a comfort experiment, not as evidence that the mattress is suitable.

Change only one variable at a time when possible. If you add a topper, replace the pillow and change sleeping sides on the same night, you will not know which adjustment helped. Give each reversible change several nights unless it causes clearly worse pain, numbness or loss of support.
Inspect the mattress itself
Firmness can change as materials age. Look for visible sagging, a body impression that remains when you get up, or a surface that feels different on one side. A worn or misshapen bed may create both pressure points and inadequate support, making a new topper an incomplete solution. Rotate the mattress only if the manufacturer permits it; do not assume rotation will repair permanent sagging.

Also check the foundation and frame. A damaged or flexible base can alter how the mattress supports you, while a protector or thick pad can slightly change surface feel. Remove extra layers temporarily when testing so you know which component is responsible for the pressure.
Check the mattress at the center, sleeping zones and edges, and look underneath for a broken slat or bowed support. Do not cut into the cover or remove upholstery to inspect the inside. If the bed is under warranty, photograph sagging and review the manufacturer’s stated requirements before moving or rotating it; an improper base can affect a claim.
Choose between a topper and replacing the mattress
A topper can add surface cushioning when the underlying mattress is still flat and supportive. Sleep Foundation describes toppers as a temporary way to change feel; their performance depends on construction, thickness and the amount of softness added. No material or thickness is universally correct for every body.
| Situation | More reasonable first step | Why |
|---|---|---|
| The mattress is structurally sound but your shoulder or hip needs more cushioning | Try a topper, if a return or trial option is available | It changes the surface feel without replacing the support core |
| The bed sags, has permanent impressions or no longer supports your hips | Plan for replacement rather than relying on a topper | A cushioning layer cannot restore lost structural support |
| You are unsure whether pressure or alignment is the main problem | Test pillow and position, then evaluate contact again | Changing firmness before isolating other factors can hide the real cause |
| Your weight, shape or sleeping position differs from a partner’s | Use separate comfort adjustments where possible and evaluate each side | One firmness choice may not fit both bodies equally |
Choose any topper by considering your body weight, side-sleeping position, current mattress and the amount of additional cushioning desired. Trial and error may be necessary. Do not treat a topper as a guaranteed pain treatment.
Before buying, confirm that the topper’s dimensions match the mattress, that fitted sheets can accommodate the added height, and that the retailer allows returns. A topper can make a marginally firm bed more comfortable, but it can also trap heat, change edge feel or make an already-soft bed less stable. Remove it if your hips begin to dip or if getting in and out of bed becomes difficult.
A practical decision sequence
- Log the pattern for several nights. Note which shoulder or hip contacts the bed, when discomfort appears, and whether switching sides changes it.
- Perform the contact check. Look for even contact from ribs to pelvis, gaps at the waist, and areas that sink markedly lower.
- Adjust the pillow. Use a height that fills the head-to-mattress gap and reassess neck alignment without changing the mattress.
- Try knee support. Place a small pillow or rolled towel between the knees if the upper leg twists or hip pressure is prominent.
- Inspect the bed and base. Check for sagging, lasting impressions and uneven support before buying an accessory.
- Test a reversible change. If the bed remains supportive but feels hard at the shoulder or hip, a topper can be a temporary experiment. Keep the receipt or trial rights where available.
- Reassess support. Stop the topper experiment if your hips begin to sink substantially lower or your spine feels pulled out of position.
- Replace when the structure is worn. If the mattress cannot support your body evenly, adding cushioning is unlikely to correct the underlying problem.
Common mistakes to avoid
- Choosing by the label alone: “Plush,” “medium” and “firm” are not standardized experiences across brands or sleepers.
- Assuming all pain means softer: A sagging surface can create pain through inadequate support, and a thicker topper may worsen it.
- Testing only for a minute: Brief showroom impressions do not show how your shoulder, hip and waist feel after hours in one position.
- Forcing alignment: Do not press your waist down, arch your back or straighten your legs unnaturally to make contact appear even.
- Changing everything at once: Separate pillow, knee support and mattress changes so you can identify the useful adjustment.
When to seek medical advice
Shoulder and hip discomfort can have causes unrelated to mattress firmness. If pain persists despite changing position or trying a softer surface, if it is severe or worsening, or if you have symptoms that concern you, consult a qualified clinician. Sleep Foundation notes that some people may find another sleeping position more comfortable and recommends discussing uncertainty about a medical issue with a doctor. A mattress test should never delay evaluation of a potentially significant condition.

What a good fit should feel like
For a side sleeper, a workable mattress balances two goals: enough give for the shoulder and hip to settle without concentrated pressure, and enough support to keep the hips from dropping markedly below the rest of the body. The ribs-to-pelvis check, pillow fit and inspection for wear provide a more useful assessment than the word “firm” on a label. Use the observations to guide a reversible adjustment first, and replace a worn support core rather than trying to mask it with additional cushioning.

Frequently Asked Questions
Is shoulder pain proof that my mattress is too firm?
No. Shoulder pain is a clue that the contact area may not be cushioned, but pillow height, sleeping posture, an injury or another health issue can also contribute. Check for concentrated pressure and a waist gap, then consider other causes.
What firmness is best for side sleepers?
Sleep Foundation places many side sleepers between medium-soft and medium-firm as a general starting range. Weight, body shape, shoulder and hip width and personal preference change the result, so even contact and comfortable support matter more than a label.
Can a mattress be too soft for side sleeping?
Yes. If the hips sink substantially lower, one region feels deeply compressed or pressure is much greater in one area, the surface may be too soft or worn. Excessive softness can pull the spine out of alignment.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsWill a topper fix a mattress that is too firm?
A topper can temporarily add cushioning when the mattress is still flat and supportive. It cannot restore a sagging support core, and no material or thickness is universally right. Use a trial or return option when available and stop if your hips begin to sink.
How high should my pillow be when I sleep on my side?
It should fill the space between your head and the mattress so your head and neck stay aligned with your spine. The needed height depends on shoulder width, mattress compression and your anatomy; observe alignment rather than relying on a fixed measurement.
When should I replace the mattress instead of adding cushioning?
Replacement is more appropriate when the bed sags, has permanent impressions, feels uneven or no longer supports your hips. A topper changes surface feel but cannot repair structural loss.
The Bottom Line
Look for a consistent pattern: concentrated shoulder or hip pressure and a waist gap suggest insufficient cushioning, while markedly lower hips suggest inadequate support. Check the pillow, knee position and mattress condition, then try one reversible change at a time. Persistent or concerning pain warrants medical advice rather than a firmness guess.
