You usually cannot detect a made-up chatbot answer from its tone. A response can be fluent, detailed and completely wrong, with invented quotations, studies, dates, facts or citations. Treat every important sentence as a claim to verify rather than treating confidence as evidence.
The reliable test is external: break the answer into claims, open the cited sources, check the exact supporting passage and date, and compare consequential or surprising claims with an independent authoritative source. The higher the cost of an error, the higher your verification standard should be.
What “making things up” means
NIST calls this phenomenon confabulation: a generative AI system confidently presents erroneous or false content. “Hallucination” and “fabrication” are common everyday terms for the same problem. The output may contain a wrong fact, contradict itself, drift away from the prompt, invent a chain of reasoning or cite a source that does not exist.

A chatbot generates likely sequences of words; it does not possess a built-in guarantee that each statement is true. OpenAI’s Help Center guidance warns that ChatGPT can sound confident while being wrong and specifically mentions invented quotes, studies, citations, dates and facts. A polished explanation therefore tells you about the writing, not the reliability of the claims.

No single symptom proves that an answer is fabricated. The question to resolve is not “Does this sound like AI?” but “What evidence supports each claim, and does that evidence actually say what the answer says?”
Signals that should trigger a fact-check
Unusually specific details without a traceable source
Be cautious when an answer supplies an exact quotation, statistic, court citation, article title, study result or person-specific detail but gives no source you can locate. Specificity creates an impression of authority; it is not proof. Copy the distinctive wording into a search or look for the named document in the publisher’s own archive, then verify the original rather than accepting a search snippet.
A citation that does not support the attached sentence
A link can be real and still be wrong for the claim. The page might discuss a related subject without containing the quoted wording, number or conclusion. It might support a narrower statement than the chatbot made, or be too old for a claim about current policy, prices, software versions or events.

Outbyte PC Repair FREEClear out junk files and repair common Windows errorsFree Scan →Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →Confidence in response to an ambiguous question
Overconfident answers to vague or underspecified questions are a known failure mode. If a prompt leaves out a country, date, product edition, programming language, measurement unit or definition, a responsible answer should ask for clarification or state an assumption. A precise answer that silently chooses one interpretation deserves extra checking.
Internal contradictions or prompt drift
Read the whole response for conflicts: one paragraph says a feature is available and another says it was removed; a timeline places an event before its stated cause; a calculation uses a different unit halfway through. NIST includes contradictions and divergence from the prompt in its description of confabulation. Compare the answer with the facts you supplied, not just with itself.
Claims about changing information
Anything involving a current office holder, regulation, product interface, security vulnerability, service outage, price, deadline or release needs a dated authoritative source. A model’s stored knowledge may not include later events unless a search tool is used, and search results still require inspection. An undated answer to a time-sensitive question is a reason to pause.
A suspiciously complete explanation
Long answers can hide one unsupported leap among many accurate-sounding sentences. Watch for a smooth causal story that contains no qualifications, especially when the subject is disputed or technically complex. A chatbot may connect real names, organizations and statistics into a relationship that none of the sources actually establish. Length, structure and confident transitions are not evidence of a verified chain of reasoning.

Requests for personal or secret information
If a chatbot claims to know a private person’s medical history, a colleague’s motive, a confidential company decision or the contents of a document you have not supplied, treat the claim as unsupported. Do not “confirm” it by providing more sensitive data. A plausible-sounding inference about a person is not a source, and repeating it can create privacy or reputational harm.
A practical verification workflow
Use the following process whenever an answer will influence a decision, publication, code deployment or communication. For a low-stakes curiosity, you can use a lighter version; for health, legal, financial or safety matters, follow every step.
- Break the response into individual claims. Mark names, dates, numbers, quotations, causal explanations, instructions and recommendations. A paragraph often contains several claims, and one citation may not support all of them.
- Identify the claims that matter most. Start with statements whose failure would change your decision, cost money, create a safety risk or misrepresent a person or organisation. Also prioritise surprising, obscure or highly specific assertions.
- Identify what would count as proof. Before searching, decide whether you need a statute, a regulator notice, a product manual, a dataset, a court opinion, a published study or a professional assessment. This prevents you from accepting a convenient but irrelevant page.
- Open every important citation. Do not rely on the citation text, a footnote number or a search preview. Confirm the document’s title, author or institution, publication or update date, and the relevant passage.
- Test the exact support. Ask whether the passage says the same thing, with the same scope, conditions and certainty. A source that says “may” does not establish “will”; a study of one population does not automatically describe another; a historical page does not establish today’s status.
- Judge the source’s authority for that particular claim. Prefer the original data, official document, regulator, court, named researcher or primary publication over a summary that repeats someone else’s wording. Authority is claim-specific: a government agency may be authoritative for a regulation, while a manufacturer may be the primary source for a product specification.
- Compare consequential claims independently. Find a second reputable source that did not simply copy the first. Agreement between two summaries that share the same underlying error is not independent confirmation. When possible, compare with the original dataset, filing, standard, statute or research paper.
- Check the date and scope. Record the jurisdiction, edition, version, population, measurement conditions and date that apply. A correct statement can become misleading when its time or geographic qualification is removed.
- Try to falsify the claim. Search for official corrections, later versions, contrary rulings, retractions or documented exceptions. Looking only for confirming pages can reproduce the chatbot’s original error.
- Resolve, label or discard. Keep a claim when the evidence supports it, rewrite it to match the evidence when it was overstated, label it as uncertain when sources conflict, and remove it when you cannot establish it. Do not fill an evidence gap with the chatbot’s confidence.
How to audit an AI citation
OpenAI cautions that search results and citations can be incomplete, outdated or incorrect. A citation list is therefore a starting point, not a guarantee. Use this compact audit for each consequential reference:
| Check | Question to answer | Failure that should change your response |
|---|---|---|
| Existence | Does the page or document actually exist and load from a credible publisher? | The title, author or URL cannot be found, or the page is unrelated. |
| Identity | Do the title, author or institution and publication/update date match the citation? | The chatbot combined details from different documents or supplied a fabricated date. |
| Passage | Can you locate the text, table or data that supports the attached sentence? | The source discusses a neighbouring topic but not the quoted claim. |
| Scope | Do the source’s conditions, population, jurisdiction and level of certainty match the answer? | A limited result was presented as universal, or “possible” became “certain.” |
| Currency | Is the source current enough for the question? | An old policy, version or statistic is being used to answer a current question. |
| Independence | Does another authoritative source agree without merely repeating the same summary? | Important claims remain supported by only one unverified chain of reporting. |
If a chatbot provides a quotation, search the exact wording in the original document. If it provides a number, find the table, methodology or underlying dataset. If it names a law, case or standard, verify the official text and jurisdiction. Formal-looking references are easy to generate and easy to misread.

Do not mistake search results for sources
A search engine may show a matching phrase, but the result can be a snippet, a secondary page or a document with a different context. Google tells Gemini users to double-check answers; a matching search result is not necessarily the page the model used. Open the result, read enough surrounding text to understand the qualification, and follow links back to the primary document. If the original is paywalled or unavailable, say so rather than claiming that a snippet proves the point.
Do not confuse factual accuracy with AI authorship
Two different questions are often mixed together:
| Question | What it tests | What evidence is appropriate |
|---|---|---|
| Is this statement true? | The accuracy, currency and scope of the claims. | Primary documents, authoritative sources, reproduced data and independent corroboration. |
| Was this text generated or assisted by AI? | Whether the wording resembles output associated with an AI system. | An authorship or detection analysis, interpreted within its tested limits. |
A detector score cannot establish that a claim is true or false. In NIST’s first text-summarisation pilot, three generators produced summaries that fooled every detector in that evaluation. That is a reported result for that pilot, not a universal failure rate for every detector or model. It demonstrates why detector output should never replace checking the underlying facts.
Likewise, human-written text can contain errors, and AI-assisted text can contain accurate statements. Fact-check the claims regardless of who or what appears to have written them. A detector can also produce false positives, particularly when prose is short, formulaic, translated or written by a non-native speaker; even a correct authorship classification would say nothing about whether the content is accurate.

| Situation | Minimum sensible check | When to escalate |
|---|---|---|
| Low-stakes explanation or brainstorming | Check an unfamiliar name, number or quotation before repeating it. | The answer will be published, taught or used to make a purchase. |
| Current product, software or policy information | Use the current official documentation or notice and record its date and version. | The answer affects compatibility, security, compliance or a deadline. |
| Technical implementation | Confirm the command or API in the versioned documentation and test it in a safe environment. | The change can delete data, expose credentials or affect production systems. |
| Medical, legal, financial or safety decision | Use an official source and consult a qualified professional. | Always escalate; do not treat the chatbot as the final authority. |
NIST identifies consequential settings, including health-related decisions, as heightened-risk contexts. OpenAI similarly advises checking important information against reliable sources. A chatbot may help you formulate questions or summarise material, but it should not be the final decision-maker where an error can cause serious harm.
Ask for uncertainty without treating the answer as self-verification
A useful prompt can make assumptions and evidence easier to inspect, but it cannot turn the chatbot into an independent fact-checker. Ask it to:
- separate sourced facts from inference and recommendation;
- list the assumptions it made about date, location, version or terminology;
- mark each claim as certain, uncertain or requiring confirmation;
- provide a source for each important factual claim rather than one citation for a whole paragraph;
- quote the relevant passage or explain that it cannot locate one; and
- ask a clarifying question when the prompt has multiple reasonable interpretations.
Then perform the external checks yourself. Asking the same system “Are you sure?” is not independent evidence, even if the second answer is more cautious or includes a longer explanation. A reformatted answer, chain-of-thought-style explanation or confidence label is still generated by the same system and can repeat the original error.
OpenAI’s research essay dated September 5, 2025 argues that accuracy-only evaluations can reward guessing over uncertainty and that uncertainty should be treated distinctly from confident error. In practical terms, “I don’t know,” “the source is out of date” or “this depends on the jurisdiction” can be a responsible result rather than a failure.

Worked examples
Example: a precise quotation
Suppose a chatbot attributes a memorable sentence to a court ruling. Treat the quotation as an unverified claim. Locate the official decision, search the exact wording, check whether the sentence appears in the judgment and confirm that the cited case and jurisdiction match. If the words appear only in a later commentary, report them as a commentator’s wording rather than as the court’s quotation. Check whether the chatbot silently shortened the sentence or removed a limiting phrase; either change can reverse the meaning.
Example: a current rule
If an answer states that a service changed its retention policy this year, check the service’s current policy page or official notice and note the effective date. Look for regional or account-tier exceptions. A page that was accurate last year cannot by itself establish the current rule. Save or link the dated notice you relied on, because an interface or policy may change again after you finish your review.

Example: an underspecified technical question
For “What command fixes this error?”, first identify the operating system, software version, exact error text and desired result. A command that is safe in a disposable test environment might destroy data in production. Ask for the missing context, verify the command in the versioned documentation and test it without using live credentials or irreplaceable files. Read the command character by character; a plausible option, path or wildcard can have destructive effects.
Example: a health recommendation
If a chatbot recommends changing a medicine dose, identify the exact drug, formulation, patient factors and stated source, then stop short of acting on the answer. Check the current information from a regulator, hospital or licensed clinician. Emergency symptoms require appropriate emergency services, not a longer chatbot conversation. The model can help you prepare questions, but it cannot examine you or accept responsibility for the decision.
What to do when sources disagree
Disagreement is not permission to choose the answer that sounds best. First check whether the sources cover different dates, jurisdictions, populations, definitions or versions. A regulator’s current rule may properly differ from a historical study; a preliminary result may conflict with a later systematic review. Record the disagreement and the reason for it in your notes.
Prefer the source with the appropriate authority and the closest scope, and state the remaining uncertainty in your final wording. For a consequential decision, ask a qualified professional to interpret the conflict. If you cannot resolve it, postpone the decision or choose the safer option rather than letting the chatbot arbitrate between sources.
Limits of what can be inferred
There is no general hallucination percentage established here for all chatbots. Error rates vary with the model, version, task, prompt and evaluation method. A result from one benchmark or detector should not be turned into a universal rate.
Nor can fluency, length, a confident tone, a citation count or a detector label prove truth. These are presentation or classification signals. Only evidence that resolves the underlying claim can do that, and sometimes the evidence remains conflicting or unavailable. In those cases, preserve the uncertainty instead of inventing a conclusion.

- Have I separated the response into claims that can be checked?
- Which claim would matter most if it were wrong?
- Does every important citation exist, and does its passage support the exact wording?
- Have I checked the source’s author, institution, date, jurisdiction and version?
- Is the information current enough for the question?
- Does an independent authoritative source agree?
- Did the chatbot make an assumption or answer an ambiguous question without saying so?
- Am I using an AI-authorship detector as if it were a truth test?
- For a medical, legal, financial or safety decision, have I consulted an appropriate professional or official source?
- Have I preserved uncertainty where evidence is incomplete instead of forcing a yes-or-no answer?
The practical rule is simple: believe evidence, not confidence. When a claim cannot be verified, label it as uncertain or leave it out.

Frequently Asked Questions
How can I tell if ChatGPT is hallucinating?
Do not rely on tone or confidence. Break the answer into claims, open its citations, verify the exact supporting passages and dates, and compare important claims with an independent authoritative source. Contradictions, prompt drift, invented quotations and unusually precise details without traceable sources are reasons to check, not proof by themselves.
Can I trust AI citations?
Treat them as leads, not guarantees. Confirm that the cited page exists, that its title, author and date match, and that the passage actually supports the claim with the same scope and certainty. Search results, snippets and citation formatting can all be incomplete, outdated or incorrect.
Why does a chatbot sound sure when it is wrong?
Fluent language and confidence are not reliability signals. Generative systems predict plausible wording and can present erroneous facts, dates, studies, quotes or logic in a polished style. OpenAI specifically warns that ChatGPT may sound confident while being wrong.
Is an AI detector a way to fact-check an answer?
No. A detector estimates whether wording resembles AI-generated text; it does not test whether the claims are true. NIST’s first text-summarisation pilot reported three generators whose summaries fooled every detector in that evaluation, showing why detector results cannot replace source checking.
What should I do when an AI answer has no sources?
Treat every important factual statement as unverified. Ask for sources and assumptions, then locate the original documents yourself. If you cannot establish a claim from an authoritative source, label it uncertain or omit it rather than repeating the chatbot’s assertion.
How do I check a chatbot’s answer about current events or policies?
Use the current official source, notice or documentation and record its publication or effective date. Check the relevant country, region, account tier, product version or deadline. A model’s stored knowledge may not include later changes, and a search result still needs to be opened and read.
Is asking the chatbot “Are you sure?” an independent check?
No. The follow-up is generated by the same system and can repeat or rephrase the original error. It may help expose assumptions or uncertainty, but you still need external evidence and, for high-stakes matters, qualified professional advice.
What if two reliable sources disagree?
Check whether they refer to different dates, jurisdictions, populations, definitions or versions. Prefer the source with the right authority and scope, document the disagreement and state the uncertainty. For medical, legal, financial or safety decisions, ask an appropriate professional to interpret the conflict.
The Bottom Line
Judge a chatbot answer by evidence, not by fluency, confidence, citation count or detector scores. Verify the exact claim against current, authoritative and independent sources, and raise the standard whenever an error could cause harm.
