A conversation with a qualified clinician is generally reasonable if you are an adult with obesity, or have overweight plus a weight-related condition, and nutrition, activity, sleep and behavioral changes have not achieved your health goals. Whether a GLP-1 medicine is appropriate depends on more than BMI: your medical history, thyroid-cancer risk, gastrointestinal, pancreas and gallbladder history, kidney vulnerability, diabetes medicines, pregnancy plans, treatment goals, cost, availability and willingness to attend follow-up all matter. The decision should be made with an authorized prescriber, using an FDA-approved product when one is indicated.

What GLP-1 weight-loss medicines are

GLP-1 medicines are prescription treatments used with a reduced-calorie eating pattern and increased physical activity. Wegovy (semaglutide) is a glucagon-like peptide-1 receptor agonist approved by the U.S. Food and Drug Administration (FDA) for chronic weight management in adults and in adolescents aged 12 years and older with obesity. It is also approved for adults with overweight when at least one weight-related comorbidity is present. The FDA label states that it is indicated “in combination with a reduced calorie diet and increased physical activity.”

Wegovy has an additional FDA indication for reducing major cardiovascular events in adults who have established cardiovascular disease and either obesity or overweight. Tirzepatide, marketed as Zepbound for chronic weight management, acts on both GIP and GLP-1 pathways. Semaglutide and tirzepatide are prescription medicines; approved indications, available doses, labeling, insurance rules and supply differ by country and can change.

These medicines are not a replacement for nutrition, physical activity, sleep or behavioral support. Obesity guidance treats pharmacotherapy as an adjunct to those measures and generally as part of ongoing chronic-care management.

Do you meet the usual eligibility framework?

The Endocrine Society uses these BMI thresholds as a clinical framework for considering anti-obesity medication:

Screening result When medication may be considered What the threshold does not mean
BMI 30 kg/m² or higher Pharmacotherapy may be considered alongside diet, exercise and behavioral modification. It does not guarantee that a particular medicine is safe, covered, available or effective for you.
BMI 27 kg/m² or higher Medication may be considered when a weight-related comorbidity is present, such as hypertension, dyslipidemia, type 2 diabetes or obstructive sleep apnea. BMI is a screening measure, not a complete assessment of health, body composition or treatment need.

FDA labeling is medicine-specific. Wegovy’s chronic weight-management indication covers adults with obesity, adults with overweight plus at least one weight-related comorbidity, and adolescents aged 12 years and older with obesity. Your clinician must still confirm that the indication fits your age, health status and treatment goals. A discussion can be worthwhile even when your BMI is near a threshold, because the overall clinical picture—not a single number—determines the decision.

How much weight loss can you reasonably expect?

Clinical trials report averages for defined groups. They are not a personal forecast, and they do not mean that everyone reaches the same result or tolerates treatment long enough to achieve it. The following figures come from trials summarized by the American College of Cardiology/American Heart Association in 2023.

Study and treatment Follow-up Average weight change reported How to interpret it
STEP 8: semaglutide 2.4 mg 68 weeks −15.8% Mean change in the specified trial population.
STEP 8: liraglutide 3.0 mg 68 weeks −6.4% Comparator result, not a prediction for an individual.
STEP 8: placebo 68 weeks −1.9% Result with the trial’s placebo and lifestyle program.
SURMOUNT-1: tirzepatide 72 weeks Up to −20.9%, depending on dose Mean change in eligible adults without diabetes; results were dose-dependent.

The trials are not head-to-head comparisons of semaglutide and tirzepatide under identical conditions. Dose escalation, eligibility criteria, adherence, side effects, lifestyle support and follow-up all affect the outcome. A clinician should help you define a useful goal—such as improving blood pressure, glucose, mobility or cardiovascular risk—rather than treating a trial average as a promise.

Weight can also change after treatment is reduced or stopped, so ask at the outset how maintenance will be handled. A plan that includes continued lifestyle support, monitoring and a realistic discussion of long-term treatment is more useful than a short-term target alone.

What health information must be reviewed first?

Thyroid-cancer history

Wegovy is contraindicated if you or a family member has had medullary thyroid carcinoma, or if you have multiple endocrine neoplasia syndrome type 2 (MEN2). Tell the prescriber the exact diagnosis and which relative was affected; do not assume that every thyroid condition carries the same restriction. Known hypersensitivity to the medicine or its ingredients is also a contraindication.

Pancreas, gallbladder and gastrointestinal history

The FDA label warns about pancreatitis, acute gallbladder disease and severe gastrointestinal reactions. Report previous pancreatitis, gallstones, gallbladder surgery, persistent abdominal pain, severe reflux, vomiting, delayed stomach emptying or another significant gastrointestinal disorder. Your clinician may need to decide whether the potential benefit outweighs the risk or whether a different approach is safer.

Kidney vulnerability and dehydration

Acute kidney injury can occur in the setting of volume depletion. A history of kidney disease, recurrent vomiting or diarrhea, diuretic use or difficulty maintaining fluids deserves specific discussion. Ask how to respond if gastrointestinal symptoms make it hard to drink or keep medicines down.

Diabetes medicines and low blood sugar

Reduced food intake and weight loss can increase hypoglycemia risk when a GLP-1 medicine is used with insulin or an insulin secretagogue. The AGA guideline notes that doses may need adjustment. Do not change insulin or another diabetes medicine yourself; arrange a monitoring and dose-adjustment plan with the clinician managing your diabetes.

Pregnancy plans

For patients using Wegovy for weight reduction or cardiovascular-risk reduction, the current FDA label instructs discontinuing semaglutide at least two months before a planned pregnancy because of its long half-life. Tell the prescriber about pregnancy, attempts to conceive, breastfeeding or the possibility of pregnancy before starting, and ask what contraception and stopping plan are appropriate for your situation.

Other medicines and GLP-1 products

Give the prescriber a complete list of prescriptions, over-the-counter products and supplements. The Wegovy label says concomitant use with other semaglutide-containing products or another GLP-1 receptor agonist is not recommended unless a prescriber specifically directs it. Do not combine products because they appear to contain the same ingredient or because an online seller recommends doing so.

Approved products versus compounded or online offerings

FDA-approved medication should be prescribed by an authorized clinician and dispensed by an authorized pharmacy. The FDA has reported that some patients and clinicians seek unapproved semaglutide or tirzepatide for weight loss and has received adverse-event reports involving compounded products, including doses beyond approved labeling.

A social-media advertisement, telehealth storefront or vial labeled “research” is not equivalent to an FDA-approved product. Before paying or injecting, verify:

  • the pharmacy’s identity and authorization in your jurisdiction;
  • the written dosing instructions, storage requirements and expiration information; and

A practical decision process

  1. Confirm your starting point. Have your height, weight, BMI and waist or other relevant measurements recorded. List weight-related conditions such as hypertension, dyslipidemia, type 2 diabetes, cardiovascular disease or obstructive sleep apnea. Remember that BMI is a screening tool, not the whole diagnosis.
  2. Describe what you have already tried. Bring a realistic account of nutrition changes, activity, sleep, behavioral programs and any prior weight medicines. Explain what helped, what was unsustainable and what health outcome you want to improve.
  3. Complete the safety screen. Review thyroid-cancer and MEN2 history, pancreatitis or gallbladder disease, severe gastrointestinal symptoms, kidney vulnerability, allergies, diabetes medicines, pregnancy plans and every prescription or supplement.
  4. Compare realistic options. Ask about semaglutide and tirzepatide in terms of expected benefit, cardiovascular or glucose priorities, adverse effects, titration, administration, interaction risks, cost, insurance coverage and local availability. The AGA guideline specifically recommends considering comorbidities, patient preferences, costs and access when selecting medication.
  5. Use a verified supply chain. Obtain the prescribed product through an authorized prescriber and pharmacy. Avoid products marketed as unapproved or compounded substitutes for FDA-approved medicines.
  6. Agree on follow-up before the first dose. Establish how dose changes, side effects, glucose checks, nutrition, physical activity, progress and long-term maintenance will be reviewed. Know whom to contact if symptoms become severe or you cannot maintain fluids.

What follow-up should cover

Follow-up is not optional paperwork; it is how the treatment is made safer and more useful. At visits or through an agreed monitoring plan, the care team should review:

  • weight trend and the health outcomes that matter to you;
  • nausea, vomiting, diarrhea, constipation, abdominal pain or other gastrointestinal symptoms;
  • signs that could indicate pancreatitis, gallbladder disease, dehydration or a severe reaction;
  • blood-glucose readings and symptoms of hypoglycemia if you use insulin or an insulin secretagogue;
  • kidney status when dehydration or other risk factors are present;
  • dose escalation, missed doses, storage and administration technique;
  • food intake, protein and overall nutrition, physical activity and sleep; and
  • whether continuing, changing or stopping treatment fits your goals and safety profile.

Seek urgent medical care for severe or persistent abdominal pain, repeated vomiting, inability to keep fluids down, symptoms of a serious allergic reaction or severe low blood sugar. Contact the prescribing team promptly for any concerning symptom rather than improvising a dose change.

When a GLP-1 discussion may need to wait

A clinician may recommend resolving another issue first—for example, clarifying a possible contraindication, stabilizing diabetes treatment, evaluating unexplained abdominal symptoms, addressing dehydration or kidney risk, or making a pregnancy plan. A pause is not a rejection of treatment; it prevents an avoidable complication and gives you a clearer baseline from which to judge benefit.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Likewise, if you cannot obtain a reliable product, afford ongoing treatment or attend follow-up, the practical risks may outweigh the theoretical benefit until those barriers are addressed. Ask about non-drug obesity care and other approved options rather than turning to an unverified source.

Questions to take to your appointment

  • Do my BMI and health conditions fit an approved indication for this medicine?
  • What benefit are we targeting: weight, glucose, blood pressure, mobility or cardiovascular risk?
  • Which personal or family conditions would make this unsafe for me?
  • How will my insulin or other diabetes medicines be monitored and adjusted?
  • What symptoms require a same-day call or emergency care?
  • What is the dose-escalation and missed-dose plan for the exact product prescribed?
  • How will we judge whether it is working and how long should I continue?
  • What is the plan if I become pregnant, plan a pregnancy or need to stop?
  • Which pharmacy will dispense it, and how can I verify that the product is FDA-approved?
  • What ongoing costs, coverage limits and supply issues should I plan for in my area?

Bottom line

GLP-1 weight-loss medication is most worth discussing when obesity—or overweight with a related health condition—continues to threaten health despite appropriate lifestyle efforts, and when you are prepared for medical screening, dose adjustment and ongoing follow-up. BMI opens the conversation; it does not make the decision. A careful review of contraindications, diabetes medicines, pregnancy plans, treatment priorities, access and product authenticity is what determines whether the potential benefit is worth the risks for you.
