Symptoms can offer clues, but they cannot reliably distinguish flu from COVID-19. Both illnesses can cause fever or chills, cough, sore throat, congestion, fatigue, aches, and headache; testing is needed to confirm either diagnosis. A cold often starts more gradually and is milder, but those patterns are not proof. If you are at higher risk for severe illness, contact a healthcare provider promptly: treatment can be time-sensitive.

This guide reflects U.S. Centers for Disease Control and Prevention (CDC) guidance, including respiratory illness guidance updated September 10, 2026. It can help you decide what to do next, but it cannot diagnose your illness.

What symptoms can—and cannot—tell you

Flu, COVID-19, and the viruses that cause common colds share many symptoms. Fever or chills, cough, sore throat, a stuffy or runny nose, fatigue, aches, and headache can occur with more than one of them. Some people have mild symptoms, and some have none; others become seriously ill.

The CDC puts the central limitation plainly: “You cannot tell the difference between flu and COVID-19 by the symptoms alone because many of the signs and symptoms are the same.” This statement appears on its Similarities and Differences between Flu and COVID-19 page, dated September 17, 2024. A symptom pattern can suggest what is more likely, but it cannot establish which virus is responsible. Cold-like symptoms may also come from respiratory viruses other than the viruses that cause common colds.

Use symptoms to judge how unwell you are and whether you need care—not to rule out flu or COVID-19. In particular, having no fever does not rule out flu. A test can help identify the cause, and the result may matter for treatment decisions and steps to reduce spread.

It is useful to separate three questions that can otherwise get mixed together: What might be causing the symptoms? Is the illness severe or getting worse? Could early treatment be appropriate? Symptom patterns may offer a clue to the first question, but they do not answer it with certainty. Severity and personal risk should guide whether you seek care, while a test and a healthcare provider can help clarify diagnosis and treatment.

Do not wait for a particular symptom—such as fever or a change in taste or smell—before considering testing or seeking advice. COVID-19 and flu can present in different ways, and some people have no symptoms. The absence of one expected feature does not establish that you have a cold or that you cannot spread an infection.

How the usual symptom patterns compare

Pattern Common cold Flu COVID-19
Onset Usually gradual. Often abrupt. Symptoms overlap with flu; timing alone cannot reliably identify it.
Typical severity Usually milder. Often more intense, with aches, chills, and fatigue common. Ranges from no symptoms to severe illness.
Symptoms that may occur Runny or stuffy nose, cough, and sore throat are common. Fever or chills, cough, aches, fatigue, sore throat, congestion, and headache; some people also have gastrointestinal symptoms. Fever or chills, cough, breathing difficulty, fatigue, sore throat, congestion, aches, headache, and gastrointestinal symptoms. Changes in taste or smell are more frequent than with flu.
Can symptoms confirm it? No. Symptoms overlap with other respiratory viruses. No. Testing is needed to confirm. No. Testing is needed to confirm.

These are typical patterns, not a diagnostic checklist. A person can have a cold with substantial symptoms, flu without a fever, or COVID-19 that resembles a cold. A symptom appearing in one column does not exclude the illnesses listed in the others.

Clues that may fit a cold

A gradual start, milder symptoms, sneezing, and nasal congestion are more typical of a cold than flu. That makes a cold a reasonable possibility, not a diagnosis. Other respiratory viruses can cause cold-like symptoms, and a person with flu or COVID-19 may not follow a textbook pattern.

People often ask whether a runny nose or sore throat means they only have a cold. It does not. These symptoms are common in colds, but congestion and sore throat can also occur with flu and COVID-19. If symptoms are mild and there are no concerning warning signs, comfort measures may help while the illness runs its course. If you are at higher risk, however, mild symptoms are still a reason to contact a provider promptly because treatment windows can be short.

Clues that may fit flu

Flu often comes on abruptly and can bring stronger aches, chills, fever, and fatigue. But not everyone with flu has a fever, and flu can include symptoms that also occur with COVID-19. An abrupt onset or intense aches therefore cannot confirm flu.

Some people with flu also experience gastrointestinal symptoms. Their presence does not settle the diagnosis either, since gastrointestinal symptoms can occur with COVID-19. If the distinction could affect treatment, testing is more useful than trying to infer the virus from the mix or intensity of symptoms.

Why COVID-19 is hard to identify by symptoms

COVID-19 can look like flu or a cold, and its severity ranges widely. Taste or smell changes occur more often with COVID-19 than with flu, but that difference does not make them a dependable stand-alone test. Breathing difficulty is a symptom that deserves attention regardless of which infection caused it.

COVID-19 can include fever or chills, cough, fatigue, sore throat, congestion, aches, headache, and gastrointestinal symptoms—many of the same features found with flu. Some people have no symptoms. For that reason, a distinctive symptom can be a clue, but neither the presence nor absence of a single feature can reliably tell you which infection you have.

When testing can help

At-home multiplex tests for COVID-19 and flu are available, according to CDC guidance. A multiplex test checks for more than one virus in a single test. Testing is useful when knowing the cause could affect treatment or precautions, but the result needs to be interpreted in light of the test type and your situation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A test is especially worth discussing when a diagnosis could change what happens next—for example, if you may be eligible for time-sensitive treatment, if symptoms are worsening, or if you need help deciding what precautions to take. Testing does not replace assessment of severe symptoms. Seek care for warning signs even if you have not tested or a result appears reassuring.

Antigen tests and molecular tests

Rapid antigen tests are generally less sensitive than molecular tests, including nucleic acid amplification tests (NAATs). That means an antigen test can return a negative result even when you are infected, particularly when viruses are circulating widely. A negative rapid result is not proof that you do not have COVID-19 or flu.

Follow the test’s instructions for use. If an antigen result is negative but you have symptoms or a reason to suspect infection, ask a healthcare provider whether molecular testing or repeat testing is appropriate. The right next step can depend on your symptoms, risk factors, timing, and access to testing.

A negative result should be read as the result of that test, not as a definitive explanation for your illness. The kind of test matters: molecular tests are generally more sensitive, while an antigen test may miss an infection. A clinician can advise whether another test is appropriate in your circumstances. There is no single next step that is right for every person based only on a negative rapid result.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

At-home combination COVID-19 and flu tests offer a way to check for more than one virus. They are not a substitute for clinical care, and a negative antigen result does not guarantee that neither virus is present. Check the product’s instructions and consult a healthcare provider if you are unsure how to interpret a result or whether additional testing is needed.

What a positive result does and does not mean

A positive result identifies the virus or viruses the test is designed to detect, but a positive result for one virus does not necessarily rule out another infection. Flu and COVID-19 can occur at the same time, although CDC characterizes simultaneous infection as uncommon. If you feel significantly worse or have a symptom that needs urgent attention, do not let a test result delay care.

Testing can help inform treatment and prevention decisions, but it cannot predict exactly how an illness will progress. A positive result is a reason to consider the advice that applies to the infection and your personal circumstances. If you are at higher risk, contact a provider promptly rather than assuming that a mild start means treatment is unnecessary or that a positive result automatically determines your eligibility.

What to do after symptoms begin

Choose your next step based on severity and personal risk, not on a guess from symptoms alone. People at higher risk for severe illness should contact a healthcare provider promptly when respiratory symptoms develop—even if symptoms are mild—because a clinician can assess whether testing and treatment are appropriate.

A practical approach is to notice when symptoms began, consider whether you or the person who is ill may be at higher risk, and seek advice early if treatment might matter. These steps are not a way to diagnose yourself; they help ensure that a provider has useful context when discussing testing and care. Do not wait for a suspected cold to become severe before asking for advice if you are in a higher-risk group.

Contact a provider promptly if you may be at higher risk

CDC identifies older adults, young children, pregnant people, and people with certain chronic conditions or weakened immune systems among groups at increased risk in its influenza and respiratory illness materials. These categories do not mean every person will qualify for treatment. Eligibility is individualized, so seek advice rather than assuming either that treatment is necessary or that it is unavailable to you.

Timing matters. CDC’s common-cold treatment guidance, dated March 11, 2026, says COVID-19 treatment should begin within 5–7 days after symptoms begin and flu treatment within 2 days. These are treatment timing windows—not guarantees of eligibility, benefit, or outcome. Contact a healthcare provider quickly to discuss your circumstances; do not wait for symptoms to become severe if you are at higher risk.

The two windows are different: the guidance gives 5–7 days from symptom onset for starting COVID-19 treatment and 2 days for flu treatment. They are not instructions to start treatment on your own, and they do not mean everyone with symptoms needs prescription treatment. A healthcare provider can determine what applies, including whether testing or treatment is appropriate. Because the relevant window begins when symptoms start, prompt contact is more useful than waiting to see whether symptoms become dramatic.

Older adults, young children, pregnant people, and people with certain chronic conditions or weakened immune systems are examples of groups CDC identifies as having increased risk. The categories are not a complete personal assessment and do not guarantee eligibility. If you are uncertain whether your health history puts you at higher risk, a healthcare provider can help assess that question.

Seek emergency care for warning signs

Get urgent medical attention for difficulty breathing, persistent chest pain or pressure, confusion, inability to wake or stay awake, or seizures. CDC notes that its warning-sign lists are not exhaustive: severe or concerning symptoms merit care even if they are not listed here.

Children may show warning signs differently. Seek prompt medical attention for fast or difficult breathing, bluish lips or face, ribs pulling in with breaths, dehydration, or lack of alertness. A fever in an infant younger than 12 weeks is also a warning sign that needs medical attention.

These warning signs matter regardless of whether you suspect flu, COVID-19, or a cold, and regardless of whether a test is positive or negative. If a person is struggling to breathe, has persistent chest pain or pressure, becomes confused, cannot be awakened, or has a seizure, do not delay care while trying to identify the virus. The warning signs listed here are not exhaustive, so severe or worrying changes deserve attention.

Fast or difficult breathing, ribs pulling in with breaths, bluish lips or face, or unusual lack of alertness are reasons for prompt medical attention. Fever in an infant younger than 12 weeks also requires attention. When a child seems severely ill, do not rely on a symptom comparison chart to decide that the illness is only a cold.

Comfort care when symptoms are mild

Most colds improve on their own with time. Rest, fluids, saline, and a clean humidifier or cool-mist vaporizer are among the comfort measures CDC lists. These may help you feel more comfortable, but they do not identify the virus or cure the infection.

Comfort measures address how you feel, not what caused the illness. They do not replace testing when knowing the virus could change a treatment decision, and they are not a reason to delay care if symptoms become severe. A clean humidifier or cool-mist vaporizer is one option for comfort; cleanliness matters, and it should not be presented as a treatment for the underlying virus.

Antibiotics do not treat viruses, so they are not a treatment for a routine cold. A clinician may recommend them only if they identify a bacterial infection; do not assume that symptoms persisting or feeling unpleasant means antibiotics are needed.

Keep an eye on changes

Notice whether symptoms are improving, worsening, or becoming severe, and seek care if warning signs appear. If you are at higher risk, make the call early rather than waiting to see how a suspected cold develops. If testing is negative but your symptoms or concerns remain, ask a clinician what follow-up is appropriate.

If you are caring for someone else, the same principles apply: do not use a presumed diagnosis to dismiss breathing difficulty, persistent chest pain or pressure, confusion, inability to wake, seizures, or severe dehydration. For a child, watch for the distinct breathing, alertness, and dehydration signs above. A test can help identify a virus, but symptom severity determines when urgent care is needed.

Reduce the chance of spreading respiratory illness

Prevention steps do not reveal which virus caused symptoms, but they can reduce the chance of passing a respiratory illness to others. CDC’s respiratory illness guidance, updated September 10, 2026, includes cleaner indoor air, hygiene, staying up to date with recommended immunizations, and precautions when sick. Masking can add another layer of protection.

Cleaner indoor air can include purifying indoor air. Hygiene and recommended immunizations are also part of CDC’s prevention guidance. Masking is an additional layer of protection, especially as part of a broader set of practical precautions. These actions help address spread; they do not diagnose illness or replace treatment advice.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Choose practical precautions for your circumstances, especially when you are sick or around people who may be more vulnerable to severe illness. Testing, treatment advice, and steps to limit spread address different questions: a test may identify a virus, a clinician can advise about care, and prevention measures can lower transmission risk.

Taking precautions while sick can be sensible even before you know whether the illness is flu, COVID-19, or a cold. The uncertainty about the virus does not make prevention measures useless. Conversely, taking precautions does not tell you which virus caused your symptoms; testing and clinical advice serve different purposes.

Frequently Asked Questions

Can you tell the difference between flu and COVID-19 by symptoms?

No. The CDC says their signs and symptoms overlap, so symptoms alone cannot reliably distinguish them. Testing is needed to confirm either diagnosis.

Do I need a test if I have cold symptoms?

Not every person with cold-like symptoms needs the same testing plan. At-home multiplex tests for COVID-19 and flu are available, and testing can help inform treatment or prevention decisions. If you are at higher risk, contact a healthcare provider promptly; if an antigen result is negative but you still suspect infection, ask whether repeat or molecular testing is appropriate.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Does a negative rapid antigen test rule out flu or COVID-19?

No. Antigen tests are generally less sensitive than molecular tests and can be negative when someone is infected. Follow the test instructions and ask a healthcare provider whether additional testing is appropriate for your situation.

Can I have flu and COVID-19 at the same time?

Yes, simultaneous infection is possible, although the CDC characterizes it as uncommon. A positive result for one virus does not necessarily rule out another infection.

When should I contact a healthcare provider about flu-like symptoms?

Contact a provider promptly if you are at higher risk for severe illness, even if symptoms are mild. Timing matters: CDC guidance says COVID-19 treatment should begin within 5–7 days after symptoms start and flu treatment within 2 days. A provider can assess whether testing or treatment applies to you.

Which symptoms need urgent medical attention?

Difficulty breathing, persistent chest pain or pressure, confusion, inability to wake or stay awake, or seizures warrant urgent attention. For children, warning signs include fast or difficult breathing, bluish lips or face, ribs pulling in with breaths, dehydration, lack of alertness, or fever in an infant younger than 12 weeks. Severe or concerning symptoms merit care even if not on this list.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do antibiotics help a common cold?

No. Antibiotics do not treat viruses and are not treatment for a routine cold. A clinician may recommend them if a bacterial infection is identified.

The Bottom Line

Flu, COVID-19, and colds can look alike, and symptom patterns are clues rather than proof. Testing can help identify the cause, but a negative rapid antigen result may not rule out infection. If you may be at higher risk, contact a healthcare provider promptly because treatment windows are short; seek urgent care for breathing difficulty, persistent chest pain or pressure, confusion, inability to wake, seizures, or severe concerning symptoms.
