The safest dose decision is one made with the clinician who knows your prescription, health history, and treatment goals. Before your appointment, record the exact medicine and formulation, dose, dose times, symptom timing and severity, effects on eating or drinking, missed or differently taken doses, and all other medicines. Then ask what the symptoms could mean, whether the current dose should be held longer or adjusted, what to do before the next scheduled dose, and which changes require prompt or urgent care.
This U.S.-focused guide explains how to make that conversation specific. FDA labeling differs by medicine, formulation, indication, and patient; examples for Wegovy (semaglutide) and Zepbound (tirzepatide) are not universal instructions for every GLP-1 medicine.
Why a detailed conversation matters
“I feel bad” does not tell a prescriber enough to distinguish a medication effect from an unrelated illness, a dose-related reaction, dehydration, a blood-glucose problem, or another cause. A precise timeline lets the clinician assess tolerability and decide whether the treatment plan needs a change.
GLP-1 is a broad class. Tirzepatide acts at both GIP and GLP-1 receptors, while products with semaglutide have their own formulations and labeling. The relevant questions therefore depend on the exact active medicine, whether it is a pen or vial, the indication, the prescribed schedule, the most recent dose, and other medicines or conditions that may affect risk.

The goal of the visit is not to negotiate a dose by yourself. It is to give the clinician enough information to make an individualized plan and to leave with clear instructions for the next dose and follow-up.
What to write down before the appointment
Use a note on your phone or paper. This is a preparation aid, not a validated symptom scale and not a reason to delay care when symptoms are severe.
Identify the prescription exactly
- Product name and active medicine.
- Formulation and device, such as a prefilled pen, single-dose pen, or vial.
- Prescribed indication, such as diabetes management or weight management.
- Current dose, units or milligrams as written by the prescriber, and dosing schedule.
- Date and time of each recent dose, including any missed dose or dose taken differently from the instructions.
Describe the symptom timeline
- What symptom concerns you most.
- When it began in relation to starting treatment, changing dose, missing a dose, or another change.
- How often it occurs, how long each episode lasts, and whether it is worsening, improving, or fluctuating.
- What it prevents or changes: eating, drinking, sleep, work, exercise, or usual activities.
- What you have already tried and whether it helped, without changing the prescription on your own.
List everything else that could matter
Bring the names and doses of prescription medicines, over-the-counter medicines, vitamins, and supplements. Point out medicines your clinician has said may affect glucose or interact with treatment. Include relevant health conditions, recent illnesses, and changes in food or fluid intake.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteBring the product information
Have the prescription label, injection device, vial or package, and written directions available. A photo of the label can help if the appointment is by telehealth, but keep the original packaging for an in-person visit when possible.
Make a one-page dose and symptom timeline
A short chronological record can be more useful than a long narrative. Start with the date treatment began or the date of the most recent dose change. For each dose, write the date, approximate time, amount as labeled, and whether it was taken with the intended device. Under that entry, note when symptoms began and whether they followed a meal, another medicine, exercise, an illness, or a missed dose.
Use concrete descriptions rather than labels such as “mild” or “bad.” For example, note how many episodes occurred, how long they lasted, whether you could keep fluids down, whether you missed work, or whether sleep was interrupted. If you monitor glucose, record readings with their dates and times and explain how they relate to meals, other glucose-lowering medicines, and the injection. A trend—improving, worsening, or unchanged—helps the clinician decide what needs attention now.
Bring the timeline even if symptoms have improved. The clinician may still need to understand what happened before deciding whether and when to change the plan.

[starting treatment / a dose change / a missed dose / another change].” - “Could this be related to the medicine, or should we look for another cause?”
- “Given my symptoms and treatment goals, should I remain at this dose longer, change the plan, or take another step?”
- “What should I do about my next scheduled dose, and when should I contact you again?”
- “Which symptoms should prompt a same-day call or urgent evaluation?”
These are communication aids. They are not instructions to hold, skip, restart, increase, or otherwise change a dose without the prescriber’s direction.
How to ask about a dose increase or staying at the current dose
Ask the clinician to explain the decision in terms of both tolerability and treatment response. Useful questions include:
- What is the purpose of the next dose step for my indication?
- Does the timing and severity of my symptoms suggest that escalation should be delayed?
- What improvement would you want to see before changing the dose?
- If the plan changes, exactly when should I take the next dose and what should I do if symptoms continue?
- What is the follow-up method and timing, and who covers questions after hours?
“Should I delay escalation?” is a question for the prescriber. “I will delay my dose” is a self-management decision and may be unsafe. Ask for an explicit plan rather than inferring one from a general schedule found online.
Ask how the decision will be monitored
Clarify what the clinician wants you to track after the visit: symptom frequency, ability to eat and drink, glucose readings if applicable, weight-related or other treatment goals, or signs that require a call. Ask when the team expects an update and whether a nurse, pharmacist, or on-call service can review questions between appointments. A monitoring plan prevents a vague “see how it goes” from becoming uncertainty about the next dose.
Why GLP-1 dose schedules cannot be combined into one rule
Each product’s U.S. prescribing information sets its own starting, escalation, maintenance, and missed-dose instructions. The indication and formulation can also matter. The following examples show why the exact label should guide the conversation; they are not a cross-product dosing plan.
| Product example | What the cited FDA label says | How to use that information in a visit |
|---|---|---|
| Wegovy (semaglutide) | The October 2025 label describes once-weekly escalation beginning at 0.25 mg for weeks 1 through 4, followed by staged increases. If a patient does not tolerate a dose during escalation, the clinician may consider delaying escalation for four weeks. | Ask whether your symptoms justify remaining at the current step longer and what to do with the next scheduled dose. Do not convert this example into instructions for another semaglutide formulation or medicine. Read the FDA Wegovy prescribing information. |
| Zepbound (tirzepatide) | The December 2024 label describes increases in 2.5 mg increments after at least four weeks on the current dose and says treatment response and tolerability should guide maintenance-dose selection. | Ask how your response and side effects affect the maintenance-dose choice. This schedule does not apply automatically to a different product or formulation. Check for a newer label before relying on it. Read the Zepbound prescribing information. |
Labeling can change. At the appointment, ask the clinician or pharmacist to use the current prescribing information and Medication Guide for your exact product.
Symptoms that deserve prompt discussion
FDA labeling for semaglutide and tirzepatide includes warnings about severe gastrointestinal reactions, pancreatitis, acute gallbladder disease, kidney injury related to volume depletion, hypoglycemia in some combinations, and serious hypersensitivity reactions. The warnings do not mean that every symptom is caused by the medicine, and they do not provide one universal triage rule for every GLP-1 product.

How to describe gastrointestinal symptoms
Do not report only “stomach problems.” Tell the clinician whether you have nausea, vomiting, diarrhea, constipation, abdominal pain, bloating, or inability to keep fluids down; when each began; how often it occurs; and how it affects drinking, eating, urination, sleep, and daily activities.

Ask about symptoms that may signal a complication
Tell the clinician promptly about severe, persistent, worsening, or worrying symptoms. Ask specifically which changes require a same-day call, an urgent evaluation, or emergency services in your circumstances. Follow the product’s Medication Guide and the clinician’s instructions rather than relying on a generic online checklist.
Discuss glucose-related risk in context
Hypoglycemia risk can be relevant when a GLP-1 medicine is used with insulin or an insulin secretagogue. Give the clinician your glucose readings if you monitor them, the timing of readings relative to doses and meals, and the names and doses of other glucose-lowering medicines. Do not change those medicines or the GLP-1 dose without instructions.
Mention allergic-type reactions immediately
Report symptoms that concern you as a possible serious hypersensitivity reaction. Ask what your clinician wants you to do if they occur and whether you need urgent assessment.

Pick the symptom - the matching free tool is one click away.
Do not minimize dehydration concerns
Repeated vomiting or diarrhea can make it difficult to replace fluids. Tell the clinician how much you are able to drink and whether urination or normal activity has changed. Kidney-injury warnings related to volume depletion are part of the Wegovy and Zepbound labeling, so inability to maintain fluids deserves specific discussion rather than an assumption that it is an expected nuisance.
Compounded semaglutide requires a separate measurement conversation
Compounded GLP-1 versions do not undergo the same FDA premarket review for safety, effectiveness, and quality as approved medicines. FDA has reported dosing errors with compounded injectable semaglutide, including mistakes when converting milligrams to syringe units or milliliters and confusion caused by different concentrations.
Bring the vial or package, syringe or device, pharmacy directions, and the prescription to the appointment. Ask:
- What concentration is in this vial?
- What is my intended dose in milligrams?
- How does that prescribed dose correspond to the markings on this specific device?
- Who should I contact if the label, syringe, or written directions do not match?
Do not independently recalculate the amount or change the markings you use. FDA states: “The agency also encourages patients to talk with their health care provider or compounder about how to measure and administer the intended dose of compounded semaglutide or tirzepatide.” Read FDA’s concerns with unapproved GLP-1 drugs used for weight loss and its alert about dosing errors associated with compounded injectable semaglutide products: FDA dosing-error alert.
Ask the prescriber or compounder to write the intended amount and the measurement method in the same units you will use. If a replacement vial has a different concentration, ask for new instructions rather than assuming the old syringe marking remains correct. Never treat a compounded product as FDA-approved or assume its directions match a branded pen.

Ask what to do after a missed or differently taken dose
Tell the clinician exactly what happened: the scheduled date and time, when you actually took the dose, whether you took more or less than prescribed, and any symptoms afterward. Missed-dose rules are product-specific, so do not borrow directions from another medicine or assume that taking an extra dose is appropriate.
Ask for a written plan covering the next scheduled dose, what to do if you vomit after an injection or are unsure whether the full dose was delivered, and whom to contact outside office hours. If the instructions are unclear, pause the decision and contact the prescribing team or pharmacist rather than guessing.
Explain why the dose differed
A dose may differ because of a device problem, a confusing label, travel, illness, a pharmacy substitution, or an intentional change. Explain the reason without embarrassment. The clinician needs the facts to assess exposure and decide whether any monitoring is needed. Keep the device and packaging if there is a question about a malfunction or concentration.

Some product labels contain counseling specific to pregnancy or oral contraceptives. Tell the clinician if you are pregnant, planning pregnancy, breastfeeding, or relying on oral contraception, and ask how your exact product’s label applies. Do not assume that advice for one medicine applies to another.
Also state the outcome you are trying to achieve—glucose control, weight-related goals, or another approved indication—and any recent change in that outcome. Dose decisions balance response, side effects, safety factors, and the reason the medicine was prescribed.
Common communication pitfalls to avoid
- Using only a severity label. “Severe” means different things to different people. Add frequency, duration, and functional effects.
- Leaving out a missed or extra dose. Even an accidental difference can change how the clinician interprets symptoms.
- Assuming the brand name is enough. State the active medicine, formulation, concentration if compounded, and device.
- Borrowing another patient’s schedule. Escalation and missed-dose instructions are product-specific, even within the same broad class.
- Changing several medicines at once. Tell the clinician about any change and wait for individualized instructions rather than trying to troubleshoot independently.
- Waiting for the routine visit despite worsening symptoms. Use the clinician’s same-day or urgent-care instructions when symptoms become severe, persistent, or rapidly worse.
These steps make the appointment safer and more efficient; they do not replace medical assessment.

Current FDA update on mood symptoms
In 2026, FDA reported that its comprehensive review found no increased risk of suicidal ideation or behavior associated with GLP-1 receptor agonists and requested removal of relevant warnings from labeling for affected medicines. The agency’s request may not yet appear in every product label available to patients, so treat this communication as the newer FDA position.
Regardless of the labeling change, tell a health professional about new or worsening depression, suicidal thoughts, or unusual mood or behavior changes. If you feel in immediate danger, use emergency services in your location.

Before ending the appointment, repeat the plan in your own words: the dose and schedule, what to do before the next dose, which symptoms require a call or urgent assessment, and the date or condition for follow-up. Ask the clinician to correct anything you misunderstood. A clear plan is more useful than a general instruction to “watch symptoms,” especially when a product has concentration-specific or missed-dose directions.
Frequently Asked Questions
What should I tell my doctor about GLP-1 side effects?
Give the exact medicine, formulation, dose, schedule, and last dose time. Describe when the symptom started, how often it occurs, how long it lasts, whether it is worsening, and its effect on eating, drinking, sleep, work, and usual activities. Include missed or differently taken doses and every prescription medicine, over-the-counter medicine, and supplement.
Should I ask to change my dose if I feel sick?
Ask whether the symptoms could be related to the medicine, whether escalation should be delayed, and what to do about the next scheduled dose. Do not hold, skip, restart, increase, or otherwise change the dose without instructions for your exact prescription.
What should I ask before increasing my GLP-1 dose?
Ask what the next dose is intended to accomplish, whether your current response and tolerability support escalation, what symptoms should improve first, and exactly when to take the next dose. Ask when and how the clinician wants follow-up and who to contact after hours.
Are Wegovy and Zepbound dose schedules interchangeable?
No. The October 2025 Wegovy label and December 2024 Zepbound label have different product-specific schedules. Wegovy describes a weekly escalation beginning at 0.25 mg for weeks 1 through 4 and allows consideration of a four-week escalation delay when a dose is not tolerated. Zepbound describes 2.5 mg increases after at least four weeks on the current dose and uses response and tolerability to guide maintenance selection. These examples do not apply automatically to another product or formulation.
How do I check a compounded semaglutide dose?
Bring the vial, device, package, prescription, and written directions to the prescriber, pharmacist, or compounder. Ask for the concentration, intended dose in milligrams, and the exact correspondence to the markings on that device. Do not independently convert milligrams to units or milliliters or change the amount.
Do mood changes still need to be reported?
Yes. FDA reported in 2026 that its review found no increased risk of suicidal ideation or behavior associated with GLP-1 receptor agonists and requested removal of related warnings from affected labels. Still tell a health professional about new or worsening depression, suicidal thoughts, or unusual mood or behavior changes, and use emergency services if you feel in immediate danger.
The Bottom Line
Bring a precise dose-and-symptom timeline, the actual product information, and a complete medication list. Ask your clinician for product-specific instructions on escalation, missed doses, warning symptoms, and follow-up. Do not change a GLP-1 dose or calculate compounded-product measurements on your own.
