The most productive semaglutide appointment ends with a specific, shared plan: which health goal is being treated, whether an FDA-approved product fits, what benefits and risks apply to you, how monitoring will work, and when to follow up. Bring your complete medical and medication history, explain pregnancy or breastfeeding plans, and ask the clinician to identify the exact product and instructions rather than treating all semaglutide products as interchangeable.

This guide is for people in the United States considering semaglutide for weight management, type 2 diabetes, or both. U.S. approvals and labeling can change, so confirm the current product information with your prescriber.

How do I talk to my doctor about semaglutide?

Open with your goal and ask for a product-specific discussion. You could say:

“I’d like to talk about whether semaglutide is appropriate for my health goals. Can we review which product is intended for my situation, the potential benefits and risks for me, and what monitoring or follow-up I would need?”

Then describe what you want to improve: blood glucose, weight, or both. Mention what you have already tried, what worked or did not work, and any practical concerns such as injections, tablets, side effects, pregnancy plans, cost or coverage, and follow-up access. Do not assume that a product someone else uses, a dose seen online, or a compounded preparation is suitable for you.

What is semaglutide, and which product is being considered?

“Semaglutide” is the active medicine name, not a single prescription with one set of directions. U.S. Food and Drug Administration information distinguishes products by formulation and approved use. Ozempic and Rybelsus are FDA-approved for type 2 diabetes, while Wegovy is FDA-approved for chronic weight management. Your clinician should confirm the exact product, formulation, indication and instructions for your circumstances.

Product FDA-approved purpose identified in current U.S. information What to clarify at the visit
Ozempic Type 2 diabetes Why this product fits your diabetes plan, how glucose will be monitored, and how it relates to your other medicines
Rybelsus Type 2 diabetes How the oral formulation should be taken, what other medicines or conditions may affect use, and the monitoring plan
Wegovy Weight management Your weight-management goal, eligibility, pregnancy instructions, warnings, follow-up and how progress will be measured

The table summarizes indications, not interchangeable treatment instructions. Do not substitute one product for another or copy a dose without the prescriber who knows your medical history. Ask the clinician to show you the product name and formulation on the prescription or package.

Is semaglutide right for me?

Your clinician needs enough information to balance expected benefit against contraindications, warnings, interactions and practical considerations. Be candid; a complete history is more useful than trying to give the “right” answer.

Medical and family history

  • Tell the clinician about diabetes, obesity or overweight-related conditions, gastrointestinal problems, pancreatic or gallbladder disease, kidney or liver problems, and any prior reactions to medicines.
  • Report allergies and previous experiences with injectable or oral medicines, including why a treatment was stopped.
  • Give a personal and family history of medullary thyroid carcinoma (MTC) and multiple endocrine neoplasia syndrome type 2 (MEN 2). The FDA Wegovy prescribing information lists a personal or family history of MTC or MEN 2 as a contraindication.
  • Describe current symptoms, especially persistent severe abdominal symptoms, vomiting, swallowing or digestion problems, or symptoms that concern you. The clinician will decide whether they change the treatment choice or evaluation.

The FDA label discusses thyroid tumors in rodents and states that the relevance to humans is unknown. That warning should not be restated as proof that semaglutide causes thyroid cancer in people; ask what the warning means for your own history.

Current medicines and supplements

Bring every prescription, over-the-counter medicine, vitamin, herbal product and supplement, including the strength and how often you take it. Semaglutide can delay gastric emptying, which may affect absorption of some oral medicines. Your clinician may need to consider timing, monitoring or an alternative.

If you use insulin or an insulin secretagogue, ask specifically about low-blood-glucose risk. FDA labeling says concomitant use can increase hypoglycemia risk and that a dose reduction may be necessary. That is a prescriber decision; do not change a diabetes medicine yourself.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Pregnancy, breastfeeding and procedures

Tell the clinician if you are pregnant, could become pregnant, are trying to conceive, are breastfeeding or have a planned procedure. For Wegovy used for weight reduction, the cited FDA label says to discontinue when pregnancy is recognized and at least two months before a planned pregnancy. Product-specific advice may differ, so confirm the exact instructions for the prescription under discussion. Ask how breastfeeding and an upcoming procedure affect timing and whether another plan is safer.

What should I do before the appointment?

A short written record helps the clinician spend the visit making decisions rather than reconstructing your history.

  1. Write your main goal. State whether you are seeking better blood-glucose control, weight management, or both. Add a practical outcome you care about, such as making daily activity easier or improving a diabetes measure your clinician already tracks.
  2. List relevant diagnoses and past treatment. Include prior medicines, their results, side effects, and why they were stopped. Note previous attempts at nutrition, activity, behavioral or other medical treatment without judging yourself.
  3. Record allergies and reactions. Distinguish a diagnosed allergy from an expected side effect or an unrelated symptom if you can.
  4. Make a complete medication list. Include prescriptions, nonprescription products, supplements and medicines taken only occasionally. Photographing labels can reduce omissions.
  5. Document pregnancy and reproductive plans. Include pregnancy, breastfeeding, plans to become pregnant, contraception questions or a planned procedure.
  6. Bring any semaglutide product you already have. If you are using an approved or compounded product, bring the package or a clear record of the name, concentration or form, supplier and written instructions. Do not guess from memory.
  7. Prepare diabetes records if requested. Ask in advance whether home glucose readings would help and which dates, times and circumstances to record. A paper blood glucose logbook can organize readings if your clinician recommends monitoring, but it does not measure glucose or replace a meter, app, prescribed equipment or medical advice.
  8. Choose your questions. Put the most important safety and monitoring questions first in case the appointment is short.

What should I ask before starting an approved semaglutide product?

Product and purpose

  • Which goal are we treating: diabetes, weight management, or both?
  • Which semaglutide product and formulation is approved and appropriate for that goal?
  • Why is this option preferable to the alternatives for my medical history and preferences?
  • What are the administration steps, schedule and storage requirements for this specific product?

Expected benefit and evaluation

  • What benefit is realistic for someone with my health conditions, and how will we measure it?
  • Which measures will we follow—blood-glucose readings, laboratory results, weight, symptoms, function or another outcome?
  • When will we review whether the plan is helping, and what would prompt a change in strategy?

Safety and interactions

  • Do my personal or family history, current symptoms or other conditions create a contraindication or special warning?
  • Could any prescription, over-the-counter medicine or supplement interact with this product or be absorbed differently because of delayed gastric emptying?
  • Which effects are important to watch, which symptoms require a prompt call to the clinic, and which require urgent care?
  • How should I report severe or persistent abdominal symptoms, symptoms suggesting gallbladder disease, or possible low blood glucose?

Practical planning

  • How will follow-up work, and whom should I contact between visits?
  • What should I do if I miss an administration or cannot obtain the prescribed product? Ask for product-specific instructions rather than relying on an online schedule.
  • How do pregnancy, breastfeeding or a planned procedure affect the timing?
  • What alternatives should I consider if this product is ineffective, not tolerated, unavailable or not covered?

If I have diabetes, how should the conversation change?

Ask for a written glucose-monitoring plan before treatment begins. FDA labeling says to monitor blood glucose before starting and during Wegovy treatment in people with diabetes. Your clinician should tell you whether to use a meter, continuous monitor or another method, when to check, what readings to record, and what result or symptom should trigger a call.

Review every glucose-lowering medicine, especially insulin and insulin secretagogues. Using one of these with semaglutide may increase hypoglycemia risk, and the clinician may decide that a dose reduction is necessary. Continue your current medicines exactly as prescribed until the prescriber gives different instructions. Ask how meals, exercise, illness and missed monitoring should be handled.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

If weight management and diabetes treatment are both goals, ask which outcome has priority at each stage and how the plan will avoid losing sight of either one. The clinician may use different products, monitoring intervals or supporting treatments depending on the indication.

If weight loss is my goal, what should we define?

Ask the clinician to turn “lose weight” into a measurable, health-focused plan. Discuss the amount or trend that would be meaningful to you, the time point for review, and outcomes beyond the scale, such as mobility, blood pressure, sleep, or diabetes risk factors. The available evidence does not support promising a particular result for an individual, so a responsible plan sets review points rather than guarantees.

Ask what nutrition, physical activity, behavioral support or other medical care should accompany the prescription. If semaglutide is not appropriate, request a comparison of alternatives using the factors that matter to you: approved indication, expected benefit, contraindications and warnings, route and schedule, interactions, monitoring, pregnancy considerations, access and coverage.

What are the important safety issues to review?

Thyroid history

The current FDA Wegovy label lists personal or family history of MTC and MEN 2 as contraindications. Give the clinician the exact diagnosis and affected relative if known. The label also describes rodent thyroid-tumor findings and says human relevance is unknown; ask for an explanation tailored to your history.

Pancreas and gallbladder

The label contains warnings about pancreatitis and acute gallbladder disease. Ask what symptoms should prompt a same-day call or urgent evaluation, particularly if you have had related disease before. Do not attempt to diagnose the cause of severe or persistent abdominal symptoms at home.

Low blood glucose

Risk can increase when semaglutide is used with insulin or an insulin secretagogue. Ask how to recognize and treat low blood glucose, when to recheck, and when to seek help. Your clinician should determine whether another medicine needs adjustment.

Delayed stomach emptying and oral medicines

Semaglutide can delay gastric emptying and may affect absorption of some oral medicines. Provide a complete list and ask whether any medicine requires special timing, monitoring or replacement. Tell the clinician about significant digestive symptoms or a planned procedure.

What if someone offers me compounded semaglutide?

Ask the clinician to identify exactly what is being offered, who supplied it, why it is being considered and what written instructions apply. FDA says compounded semaglutide is not an FDA-approved drug. Compounding may address an individual medical need when that need cannot be met by an FDA-approved drug or the approved drug is not commercially available, but a compounded product is not automatically equivalent to an approved product.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Bring the vial, label or pharmacy instructions to the appointment. Confirm the concentration, formulation, measuring device, administration route, storage, expiration information, prescriber and compounder. Do not infer a dose from a different product or concentration, and do not combine instructions from social media, a telehealth message and a pharmacy label. Ask who to contact if the directions are unclear or a reaction occurs.

What should the shared treatment plan contain?

Before leaving, make sure you and the clinician agree on:

  • The treatment goal and the exact product or alternative being considered.
  • How and when it will be administered, including product-specific instructions.
  • Which current medicines continue, which require closer monitoring, and who makes any adjustment.
  • How glucose, weight, symptoms or laboratory measures will be tracked.
  • Expected follow-up timing and the contact route for questions.
  • Symptoms that require a prompt call, urgent care or emergency evaluation.
  • Pregnancy, breastfeeding and procedure instructions.
  • What happens if the product is unavailable, unaffordable, ineffective or not tolerated.

Write down the plan or ask for it in the after-visit summary. Verify the product name and instructions when the prescription is filled.

Common mistakes to avoid

  • Using the medicine’s name as if it identifies one treatment. Confirm the product and indication.
  • Leaving out nonprescription products. Supplements and occasional medicines can matter to the review.
  • Changing insulin or another diabetes medicine independently. Ask the prescriber for an adjustment and monitoring plan.
  • Ignoring reproductive plans. Pregnancy timing is part of medication safety, not an optional detail.
  • Assuming compounded means equivalent. Ask about approval status, supplier, concentration and instructions.
  • Expecting a guaranteed weight-loss result. Agree on measures and review dates instead.
  • Waiting for a routine visit to report serious symptoms. Ask in advance which symptoms need prompt or urgent attention.

What is a good outcome from the appointment?

A good outcome is not simply receiving or rejecting semaglutide. It is leaving with a product-specific decision, a clear reason for that decision, realistic measures of benefit, a safety and monitoring plan, instructions for pregnancy or procedures, and a follow-up date. If the clinician recommends against semaglutide, ask what alternative addresses your goal and how that option will be evaluated.
