Stop a runaway AI agent with layered controls: give it a unique, short-lived identity; allow only task-specific tools and data; re-check authorization before every tool call; require fresh human approval for irreversible or high-impact actions; enforce hard limits on steps, retries, tokens, time, rate and cost; isolate code and network access; and maintain a tested pause and revocation path.

A session-start confirmation or a model-generated plan is not a safety boundary. The control that matters is the independent decision immediately before an action executes.

What “runaway” means in an AI agent

An AI agent becomes runaway when its loop continues, expands its scope or performs consequential actions without an effective human or policy boundary. The trigger can be a bad plan, an ambiguous instruction, prompt injection in a document or web page, a tool error that causes retries, or credentials that grant more authority than the task requires.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The danger is not limited to a model producing incorrect text. An agent with write access can alter records; one with a mail connector can send messages; one with deployment credentials can change production; and one with broad data access can expose information while trying to complete an otherwise legitimate request. Security guidance often calls this excessive agency: the system can take more actions, with more authority, than the user intended.

Least privilege and approval gates address different failure modes:

  • Least privilege limits the blast radius. A compromised or mistaken agent has fewer tools, resources and operations available.
  • Approval gates stop sensitive actions at the boundary. A person or separately enforced policy must decide before a write, delete, payment, external send, production change or permission change occurs.
  • Execution limits stop loops. Even a permitted task must end when it reaches its step, retry, time, token, rate or cost ceiling.

These controls are complementary. An approval gate should not compensate for an over-privileged identity, and a narrow identity should not be treated as permission to run indefinitely.

The control architecture

Use an execution service or policy engine between the agent and every tool. The model may propose a call, but it should never be the final authority that decides whether the call runs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Layer What it controls Typical failure it prevents
Identity Which user, agent, workload and owner are acting Unattributed actions and confused-deputy behavior
Least privilege Allowed tools, operations, resources, data classes and environments Unnecessary reads, writes or privilege escalation
Per-action authorization Whether this exact actor may perform this exact operation on this target now Stale, self-expanded or injected instructions
Approval gate Human decision for high-impact, irreversible or externally visible actions Accidental sends, deletes, payments and deployments
Run limits Steps, recursion, retries, tokens, time, rate and cost Infinite loops and runaway spending
Isolation Code execution, memory boundaries and outbound network destinations Data exfiltration and cross-tenant access
Audit and detection Complete action history and anomaly signals Undetected bypass attempts or privilege changes
Response Pause, credential revocation, token invalidation and decommissioning Continued activity after an incident is detected

1. Classify actions before granting access

Start with an action-risk matrix rather than a list of model capabilities. Classify every operation by its effect, not by the tool’s marketing name. A “database connector,” for example, may contain harmless read queries and destructive schema changes that require different controls.

Risk dimension Questions to ask Examples that usually need stronger controls
Read or write Does the operation only retrieve information, or does it change state? Updating a customer record; deleting a file
Reversibility Can the exact prior state be restored reliably? Permanent deletion; publishing a public change
Data sensitivity What classification is read, transformed or transmitted? Credentials, health data, personal data or confidential source code
Financial impact Can the call spend money, create a bill or move funds? Purchases, refunds, transfers or paid API usage
External visibility Will a customer, regulator, supplier or public audience see the result? Sending email; posting a status update; filing a ticket
Blast radius How many records, systems, tenants or users can be affected? Bulk updates; organization-wide configuration
Security or privilege effect Does the action alter access, policy or infrastructure? Changing roles, firewall rules, secrets or production settings

Permit low-risk, read-only inspection when the identity and data scope are narrow. Route operations that are irreversible, security-sensitive, financially consequential, externally visible or broad in scope to an explicit approval decision. OWASP guidance specifically calls for approval around security-relevant configuration, permission and infrastructure changes.

2. Give each agent a distinct, expiring identity

Do not run every agent as the user, as an administrator or through one shared service account. Create an identity that makes the actor and its authority visible in logs.

Identity attributes to record

  • A unique agent identifier that is never reused for a different workflow.
  • The initiating user or service and a named owner responsible for the agent.
  • The agent’s purpose, environment and tenant or workspace.
  • The model, tools, connectors and memory stores in use.
  • Creation time, expiration time and the workflow or ticket that authorized it.

Use short-lived or just-in-time credentials where possible. An identity that expires when the task ends is safer than a standing key that remains valid for months. Bind authorization to the initiating user, agent identity, exact tool, operation, target resource and data classification. Microsoft’s identity guidance summarizes the principle as: “Every user, agent, workload, plugin, and tool receives a verified identity, explicit authorization, and the minimum rights required.”

Make permissions task-scoped

Grant only what the current task needs, and deny everything else by default. A support agent investigating one case might receive read access to that case and the ability to draft a response, but not permission to send mail, edit billing data or search every customer. A deployment agent might be allowed to stage an artifact in a test environment while production deployment remains approval-gated.

Scope element Narrow example Dangerous broad alternative
Tool Read-only issue lookup Entire project-management API
Operation Create a draft, not send it Full create/update/delete/send rights
Resource One ticket, repository or tenant All records in the organization
Environment Development or staging All environments, including production
Data Fields required for the task Unfiltered exports and secrets
Duration Minutes or one workflow run Non-expiring credential

3. Re-authorize every tool call

The execution component should make a fresh decision immediately before each tool runs. It should not rely on the model’s earlier plan, a session-start approval or instructions found in retrieved content.

Minimum decision inputs

  1. Identify the initiating user, agent identity, owner and correlation ID.
  2. Normalize the proposed tool, operation, target, parameters and data being sent.
  3. Check that the identity is active, unexpired and assigned to this purpose and environment.
  4. Evaluate the operation against the current policy, resource scope and data classification.
  5. Check whether a valid approval exists for this exact action, target, parameter set and time window.
  6. Apply run limits and anomaly rules, then allow, deny or hold the call.
  7. Record the decision before execution; record the result afterward.

OWASP states the boundary clearly: “The agent can propose an action, but a policy service or execution component should independently validate scope, privilege, and approval state before execution.” Treat retrieved webpages, documents, email, other agents and tool output as untrusted input. None of them can grant new authority or change policy.

Keep the policy path outside the model

The model can explain why it wants an action, but it must not be able to edit the policy, mark its own request approved, change the target after approval or exchange a read token for a write token. Use a separate service, isolated process or enforceable gateway for authorization. If policy evaluation fails, times out or returns an ambiguous result, fail closed for high-impact operations.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

4. Put approval gates at the action boundary

Ask for approval when the proposed operation is concrete and reviewable, not when a vague plan is first generated. A person should approve the action that is about to execute, with enough context to understand its effect.

What an approval request should show

  • The initiating user, agent identity, owner and workflow.
  • The tool and operation, such as “delete,” “send,” “deploy” or “change role.”
  • The exact target, including account, environment, record set or destination.
  • Parameters and the data that will be transmitted or changed.
  • Expected impact, affected quantity and whether the operation is reversible.
  • Why the agent proposed it and which preceding event triggered it.
  • Approval expiration, correlation ID and the available deny or cancel choices.

Require a fresh decision for writes, deletes, payments, external communications, production changes, role or permission changes, security configuration and other irreversible or sensitive actions. Approval should be bound to the displayed parameters. If the agent changes the recipient, amount, resource range or command after approval, create a new request.

Approval patterns that do not work

  • One approval for an entire session: later calls may have different targets and risk.
  • Approval of a natural-language plan only: the executed parameters can drift from the plan.
  • Hidden or automatic approval: a button that approves every future request removes review.
  • Approval by the same mutable agent: the actor being controlled cannot be the independent approver.
  • Unbounded approval lifetime: a decision made for one context should not authorize a later run.

5. Add hard brakes for runaway loops

Every run needs limits that are enforced by the orchestrator, not merely suggested in the prompt. Set a ceiling for each dimension and stop or quarantine the run when it is reached.

Limit What it bounds Useful response when exceeded
Plan steps Total tool actions in one run Pause and require review of the remaining plan
Recursion or chain depth Nested agent-to-agent or tool-to-tool calls Deny the next nested call and preserve the trace
Retries Repeated attempts after errors or denials Quarantine instead of retrying indefinitely
Tokens Model input and output consumption Stop generation and return a bounded error
Time Wall-clock duration of a run or individual call Cancel outstanding work and revoke temporary credentials
Rate Requests per second, minute or destination Throttle, hold or deny bursts
Cost Budget for models, tools or external services Stop spending and notify the owner

Use lower limits for unfamiliar workflows and raise them only with evidence. Stop or quarantine after repeated policy denials, repeated tool errors, unusual destinations, a sudden expansion in parameters or attempts to bypass approval. OWASP identifies chain-depth, retry, token and cost limits as controls against recursive tool abuse and runaway loops.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

6. Isolate code, memory and network egress

Sandbox code execution

Run generated code in an isolated environment with no implicit access to host files, credentials, processes or internal services. Provide only the files and libraries required for the task. Remove the sandbox after the run or reset it between tenants so one workflow cannot inherit another’s state.

Restrict outbound connections

Use an allowlist or policy gateway for destinations, protocols and methods. A code interpreter that can reach arbitrary internet hosts can upload sensitive data or fetch new instructions. Log destination, port, request size and the identity that authorized the connection. Treat a newly observed or unusual endpoint as a reason to hold the run, not as a prompt for the model to justify it.

Isolate memory by user and tenant

Separate conversation history, vector stores, files and long-term memories by user, tenant and purpose. Apply access control and encryption to memory stores, define retention periods and provide deletion procedures. Retrieved memory is input, not policy: an old instruction or poisoned document must not expand current permissions.

7. Log the complete action chain and monitor it

A useful audit record lets an investigator reconstruct what happened without relying on the model’s summary. Record at least:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Agent identity, initiating user, owner, tenant, environment and credential version.
  • Correlation ID linking the request, plan, approvals, tool calls and results.
  • Tool, operation, target, normalized parameters and data classification.
  • Relevant inputs, outputs, model decision and stated rationale.
  • Authorization result, policy version, approval identity, decision and expiration.
  • Start and end times, retries, latency, token use, rate and cost.
  • Errors, denials, cancellations, credential changes and shutdown events.

Protect logs from alteration and restrict who can view sensitive parameters. Monitor for unusual destinations, expanded resource ranges, repeated denials, privilege changes, excessive retries, cross-tenant access patterns and query behavior unlike the agent’s normal workload. Alerts should identify the run and provide a direct path to pause it.

8. Build and test an emergency stop

A shutdown path is only useful if it works while the agent is busy or partially disconnected. Provide a system-level pause that prevents new tool calls, then revoke the authority that could issue more calls.

  1. Pause admission: mark the run and agent identity as blocked so the execution gateway rejects new actions.
  2. Cancel work: terminate queued calls, interrupt cancellable jobs and stop scheduled retries.
  3. Revoke credentials: disable or rotate short-lived tokens, connector grants and signing keys.
  4. Invalidate pending approvals: remove approval state for the affected run, targets and credentials.
  5. Contain side effects: disable network routes, isolate the sandbox and protect affected resources.
  6. Preserve evidence: retain the action log, policy decisions, prompts, outputs and timestamps.
  7. Assess and recover: determine what changed, restore from known-good state where appropriate, then restart only with a reviewed identity and narrowed policy.

Test pause, revocation, token invalidation, decommissioning and restart procedures before production. Include failures such as a stuck worker, an expired approval, a disconnected user interface and a tool that ignores cancellation. Recovery should not depend on the same agent that caused the incident.

A practical implementation sequence

Teams can introduce these controls without waiting for a complete agent platform rewrite. Use the following order so each stage reduces risk immediately.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Inventory the attack surface. List every model, tool, plugin, connector, memory store, data source, environment and external destination.
  2. Assign ownership. Give every agent a unique identity, owner, purpose, environment and expiration.
  3. Default-deny capabilities. Remove unused tools and grant the smallest resource and data scopes for one workflow.
  4. Insert the policy gateway. Route every tool call through independent authorization and log allow, deny and error outcomes.
  5. Gate consequential actions. Add approval for high-impact, irreversible, sensitive, external, financial, production and permission-changing operations.
  6. Set conservative ceilings. Configure step, recursion, retry, token, time, rate and cost limits; quarantine after repeated failures or denials.
  7. Isolate execution. Sandbox code, restrict egress and separate memory by user and tenant.
  8. Connect monitoring to response. Alert on anomalies and make pause, revocation and credential rotation reachable from the alert.
  9. Rehearse scenarios. Simulate prompt injection, an infinite retry loop, a changed target after approval, a leaked token and a compromised connector.
  10. Re-review material changes. Repeat the review whenever prompts, tools, memory, models or workflows change in a way that can alter authority or data flow.

How to evaluate an agent implementation

When comparing platforms or internal designs, ask for evidence rather than a feature label. A “human in the loop” claim is incomplete unless it specifies which actions are gated and where the decision is enforced.

Evaluation axis Questions to verify
Identity and scopes Does every agent have a distinct, expiring identity? Can permissions be limited by tool, operation, resource, data, tenant and environment?
Per-action authorization Is policy checked immediately before every call, independently of the model?
Approval coverage Are deletes, sends, payments, production changes, security changes and permission changes gated? Is approval bound to exact parameters and an expiry?
Runaway controls Can operators set step, chain, retry, token, time, rate and cost ceilings and quarantine on repeated denials?
Isolation Are code, memory and outbound destinations sandboxed and separated by tenant?
Audit quality Do logs include actor, target, parameters, inputs, outputs, policy result, approval and correlation ID?
Response reliability Can operators pause runs, revoke credentials, invalidate tokens and prevent scheduled retries?
Lifecycle ownership Are purpose, owner, expiration and re-review requirements enforced when the workflow changes?

Common mistakes and fixes

“The prompt says not to delete anything.”

Prompt instructions are useful behavior guidance, not authorization. Put deletion behind a policy decision and a fresh approval, then issue a credential that cannot delete unless that decision is valid.

“We approved the plan, so every step is trusted.”

Plans can change after tool output, retrieved text or an error. Approve the concrete action at execution time and request approval again when its target or parameters change.

“The agent only has read access, so limits are unnecessary.”

Read access can still expose sensitive data, incur uncontrolled cost or create a high-volume denial-of-service pattern. Apply data scope, rate, time, token and cost limits to read-only workflows as well.

Free tools Windows power users keep installed

“A shared service account makes operations simpler.”

Shared credentials remove attribution and make least privilege difficult. Use distinct identities with named owners and short validity, then log the initiating user and agent together.

“Stopping the worker is enough.”

A queued job, scheduled retry or still-valid token may restart activity. Pause admission, cancel work, revoke credentials and invalidate approvals as one response sequence.

“Tool output is trusted because it came from our system.”

Tool output, documents, email and web content can carry prompt-injection instructions. Keep them untrusted, sanitize where appropriate and never let them modify policy or grant authority.

What good looks like

A safe agent can still work autonomously on bounded, low-risk tasks. It has a verifiable identity, a narrow and expiring permission set, an independent policy decision before each call, and a human gate where consequences warrant one. Its execution is finite and observable, its code and network access are contained, and operators can stop and revoke it without asking the agent to cooperate.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

These controls follow the principles in Microsoft Learn’s identity and autonomous-agent guidance and the OWASP AI Agent Security Cheat Sheet and LLM06:2025 Excessive Agency guidance. They are qualitative safeguards, not a guarantee of zero incidents; effectiveness depends on complete inventory, correct policy scopes, reliable enforcement and repeated recovery testing.
