An AI research review is trustworthy only when you measure two different failures: false positives, where the system flags a correct statement as erroneous, and false negatives, where it fails to flag a real error. A single accuracy, F1, or “confidence” score cannot tell you how often either failure occurs.
Evaluate the review against independently established reference judgments, define exactly what counts as an error, and report claim-level detection separately from report-level completeness. Then compare systems at the threshold and error-cost profile that match your use case. Inspect the evidence behind each decision and keep a qualified human in the loop for consequential findings.
Start with the two mistakes that matter
Set the positive class before calculating any metric. In this guide, the positive class is an actual error in the reviewed material.
False positive: an incorrect error flag
A false positive occurs when a reviewer labels a correct, adequately supported item as wrong. For example, it may reject a date because a source uses a different time zone, call a paraphrase false because the wording is not identical, or flag a legitimate uncertainty as a factual error. False positives consume human review time and can cause editors to “correct” material that was already right.

False negative: a missed real error
A false negative occurs when the source material contains an error and the AI review leaves it unflagged. The error could be a fabricated fact, a citation that does not support the sentence, an outdated figure, a misleading qualification, or an important omission. False negatives are usually more dangerous when the review is intended to prevent publication of harmful or consequential claims.
The other two outcomes
A true positive is a real error correctly flagged. A true negative is a correct item correctly left alone. You need all four counts to understand a system’s behavior.
Why no single accuracy score proves reliability
“Accuracy” changes meaning with the test design. The result depends on the review unit, the mixture of correct and incorrect items, the quality of the reference judgments, the system threshold, and the cost assigned to each mistake.

Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →Outbyte Driver Updater FREEScan for outdated or missing drivers - takes under a minuteDriver Scan →A dataset containing mostly correct claims can produce a high overall accuracy even if the reviewer misses many genuine errors. Conversely, a system tuned to flag almost everything may find more errors while creating an impractical number of false alarms. F1 combines precision and recall, but it still hides which type of failure is driving the result and whether the test reflects your real assignments.
NIST’s AI Risks and Trustworthiness guidance calls for realistic test sets, documented methodology, external-validity checks, and disaggregated results rather than an unsupported headline number. NIST’s 2024 GenAI pilot overview, published June 25, 2025, likewise reports system-dependent detection performance and defines metrics at specified operating conditions.
Define the review unit and error taxonomy
Before testing, write a short specification that says what the AI is judging and what a reviewer must do with borderline cases. A “review” may operate on a sentence, an individual factual claim, a citation-to-claim relationship, or an entire report. These are different tasks and should not share one undifferentiated score.
Choose the unit
- Claim level: Does this proposition accurately follow from reliable evidence?
- Citation level: Does the cited source actually support the proposition, including its date, scope, and qualifications?
- Report level: Does the document contain all required findings and avoid material omissions?
Split compound sentences into atomic claims when possible. A sentence can contain one correct detail and one unsupported number; treating it as a single yes/no item obscures the failure.

Label the kind of problem
Record whether an item is false, unsupported, outdated, misleadingly framed, or materially incomplete. “I did not find evidence in a quick search” is not enough to label a claim false. Require an underlying source or a documented reason that the available evidence cannot support it.
Set materiality rules
Decide in advance which discrepancies count as errors. A rounding difference may be immaterial in a general explainer but material in a financial or safety document. Define acceptable date ranges, geographic scope, edition or version, and what level of uncertainty must be disclosed.
Build independent reference judgments
The reference set is the standard against which the AI is tested. It must be created independently of the AI output being evaluated; otherwise, the system can appear correct simply because its own labels influenced the “answer.”
- Sample realistic work. Draw reviewed outputs from the topics, source types, dates, languages, and difficulty levels you actually expect. Include both clean material and deliberately challenging examples.
- Collect primary evidence. Give qualified reviewers the underlying documents, not just search snippets or the AI’s explanation. Ask them to identify the exact passage, table, or version that determines the judgment.
- Label independently. Have at least two reviewers assess relevant items without seeing the AI label. Capture their decision, evidence, confidence, and reason code.
- Resolve disagreements. Use a written adjudication rule, such as a senior reviewer deciding after reading both rationales. Preserve the original labels so disagreement is measurable rather than silently erased.
- Freeze the test set. Do not change labels to improve a system’s score after seeing its output. Version the examples, evidence, and adjudication decisions.
NIST’s risk-management guidance emphasizes documenting the test method and assessing how well results carry beyond the tested conditions. A small, convenient sample can estimate behavior on that sample; it cannot establish performance for every subject or future source.
Calculate the confusion matrix and report the rates separately
| Reference judgment | AI flags an error | AI leaves item unflagged |
|---|---|---|
| Actual error | True positive (TP) | False negative (FN) |
| No actual error | False positive (FP) | True negative (TN) |
Use these definitions with the positive class explicitly set to “actual error”:
- False-positive rate:
FP / (FP + TN). Of the correct items, how many were wrongly flagged? - False-negative rate:
FN / (FN + TP). Of the real errors, how many slipped through? - Recall or true-positive rate:
TP / (TP + FN). How many real errors were caught? - Precision:
TP / (TP + FP). How many flags were genuine errors? - Specificity:
TN / (TN + FP). How often did the system correctly leave sound items alone?
Always publish the denominators. A false-negative rate based on 12 known errors is not as stable as one based on 1,200, and a rate can be undefined when a test contains no errors of a particular type.
Worked example
Suppose 200 atomic claims are independently judged: 40 contain actual errors and 160 do not. The AI flags 30 of the 40 errors, but also flags 20 correct claims.

- TP = 30, FN = 10, FP = 20, TN = 140.
- False-negative rate = 10 / 40 = 25%.
- Recall = 30 / 40 = 75%.
- False-positive rate = 20 / 160 = 12.5%.
- Precision = 30 / 50 = 60%.
“85% accurate” would be technically true here because 170 of 200 decisions match the reference, but it conceals that one in four real errors was missed and two in five flags were wrong.

Evaluate claim accuracy and report completeness separately
A report can contain no demonstrably false statement and still fail its assignment by omitting a required finding. Conversely, it can mention every required topic while attaching citations that do not support the claims. Treat these as separate dimensions.
Claim-level factuality and citation support
For every material claim, trace the citation to the passage that supports it. Check the source’s wording, date, population, geography, edition, and limitations. A citation that is topically related but does not entail the sentence is not adequate support. Record unsupported citations even when the claim happens to be true for another reason.
Report-level completeness with information nuggets
Convert the assignment into a list of required questions or “information nuggets.” Each nugget should state the answer a complete report must contain, such as a limitation, comparison point, safety condition, or date qualification. Mark each nugget as present and adequately answered, partially answered, or absent. NIST’s On the Evaluation of Machine-Generated Reports describes this question-and-answer approach and pairs it with citation mapping for verifiability.
Do not fold missing nuggets into the claim-level false-negative count unless the test is explicitly designed that way. Report “claim error detection” and “required-content coverage” as different results.

Compare systems at the operating point your use case needs
Most reviewers expose a threshold: raising it may reduce nuisance flags but allow more errors through; lowering it may catch more errors while increasing false alarms. Compare systems at matched thresholds or at matched false-positive rates, not only at their default settings.
Choose the risk priority
- High-consequence publication: prioritize low false-negative rates, then budget human time for additional false positives.
- Large-scale triage: set a false-positive ceiling that reviewers can handle and measure recall at that ceiling.
- Exploratory drafting: a looser threshold may be acceptable if every flagged or unflagged claim receives later human checking.
NIST’s text-to-text evaluation plan lists area under the curve, equal-error rate, true-positive rate at a specified false-positive rate, and Bayes risk for situations with different error costs. Bayes risk makes the trade-off explicit: assign a cost to each false positive and false negative, then select the threshold with the lowest expected cost for the task.

Do not compare unmatched populations
A detector tested on citation mismatches is not directly comparable with one tested on fabricated biographies. Report the task definition, class balance, threshold, and evidence requirements alongside every metric.
Test transfer, uncertainty, and subgroup behavior
Performance on a fixed benchmark is not the same as performance on the broader population of future reviews. NIST’s February 17, 2026 report, Expanding the AI Evaluation Toolbox with Statistical Models, distinguishes fixed-benchmark accuracy from generalized accuracy on similar items drawn from a wider population and emphasizes explicit assumptions and uncertainty.
Stratify the evaluation
Break results out by factors that can change error rates:
- Topic and domain vocabulary.
- Primary, secondary, and user-generated source types.
- Publication date and the presence of changing information.
- Simple versus compound claims.
- Numerical, temporal, causal, and attribution claims.
- Evidence quality, access restrictions, and conflicting sources.
- Language, geography, edition, or product version when relevant.
Use confidence intervals or other uncertainty estimates appropriate to the sampling design. If the sample is small or convenience-based, say so. Repeating a benchmark with a different random seed does not establish external validity if every example comes from the same narrow source pool.
Watch for distribution shift
Monitor production reviews for new topics, source formats, model versions, and prompt changes. A system that performed well on yesterday’s documents may degrade when sources become paywalled, citations move behind dynamic pages, or the task begins requiring current prices or regulations.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Make every automated judgment inspectable
An error flag is useful only when a human can verify it efficiently. Require the system to show the claim under review, its decision, the relevant source passage, the source date and scope, and a concise reason tied to the rule being applied.

Give the reviewer context
NIST’s CAISI guidance reports better reliability when reviewer models receive task context, clear rules, and examples of both wanted and unwanted flags. Include the assignment, definitions, materiality thresholds, and examples of borderline cases in the evaluation prompt or interface.

Use multiple judgments where variability matters
Occasional false positives and false negatives can vary by prompt or reviewer run. NIST CAISI describes aggregating multiple reviewer scores to reduce this variability, followed by human validation. Agreement among automated runs is not proof that a decision is true; it is a signal for prioritizing inspection.
Separate evidence retrieval from verdict generation
Log which sources were retrieved and which passages were actually used. If the system cannot show evidence, label the result “needs verification” rather than treating a fluent explanation as support. Preserve the model version, prompt, retrieval date, and threshold so another reviewer can reproduce the decision.
Guard against persuasive but wrong explanations
Explanations can make fact-checking faster without making it safer. In a 2023 study of 80 crowdworkers, Chenglei Si and colleagues compared LLM explanations with search. Explanations improved efficiency and produced accuracy similar to search, but participants over-relied on explanations when those explanations were wrong. Contrastive explanations reduced over-reliance but did not significantly outperform search alone.
The practical control is evidence inspection: open the cited source, read enough surrounding context to test entailment, and challenge the explanation with an alternative interpretation. A confident rationale is a navigation aid, not a reference judgment.
Interpret benchmark headlines without overgeneralizing
NIST pilot findings
NIST’s 2025 pilot summary notes that three generators produced summaries that fooled every detector in that particular evaluation. This is evidence about those generators, detectors, prompts, and test conditions; it is not a universal failure rate for all detectors or current models. The same pilot reported performance differences among tested systems, which is why the system and condition must accompany the number.
Factcheck-Bench
The 2023 Factcheck-Bench: Fine-Grained Evaluation Benchmark for Automatic Fact-checkers evaluated claim, sentence, and document granularity. Its reported best F1 was 0.63 for the tested automatic fact-checking systems and annotation approach. That figure describes the benchmark setup, not the accuracy of every fact-checker in production, and it does not reveal the false-positive and false-negative rates without the underlying confusion counts.
Large model evaluation studies
NIST’s 2026 statistical-modeling report examined 22 frontier large language models across three benchmarks. Its central lesson is methodological: estimate uncertainty and distinguish performance on the fixed benchmark from expected performance on a wider population. It should not be converted into a universal score for AI research reviews.

- Write the specification. Define the review unit, positive class, error categories, materiality rules, source requirements, and acceptable uncertainty.
- Assemble representative examples. Include clean and erroneous claims across the domains, dates, source types, and complexity levels that matter to your work.
- Create independent reference labels. Have qualified reviewers inspect primary evidence, record rationales, and adjudicate disagreements under a fixed rule.
- Run the AI under version control. Save the model, prompt, retrieval configuration, threshold, and date for each evaluation.
- Compute the four confusion-matrix counts. Publish false-positive rate, false-negative rate, recall, precision, specificity, and denominators rather than one headline score.
- Audit completeness separately. Define required information nuggets and map every material citation to supporting passages.
- Compare at relevant thresholds. Report recall at a chosen false-positive rate or use a cost-sensitive measure when the consequences are unequal.
- Stratify and repeat. Examine topics, source types, dates, claim forms, and other subgroups; quantify uncertainty and test a held-out sample.
- Inspect explanations. Make evidence and reasoning visible, use additional reviewer passes when useful, and require human signoff for important findings.
- Monitor after deployment. Sample production outputs, refresh reference judgments as tasks change, and investigate shifts in either error type.
What to require when choosing a review system
| Evaluation question | What to request | Why it matters |
|---|---|---|
| How often does it raise a wrong alarm? | False-positive rate and precision at the stated threshold, with denominators | Shows the human workload and trust cost of flags |
| How often does it miss a known error? | False-negative rate and recall on independently labeled errors | Measures the protection the review actually provides |
| Does it cover the assignment? | Information-nugget completeness and omission analysis | A report can be factually clean yet materially incomplete |
| Can a human verify it? | Source passages, dates, scopes, and reproducible decision logs | Exposes unsupported explanations and speeds adjudication |
| Will it transfer? | Stratified results, uncertainty, sampling method, and external-validity discussion | Limits claims to populations the evidence supports |
| Does the threshold fit the job? | ROC or precision-recall results, recall at a specified false-positive rate, and cost assumptions | Makes trade-offs visible instead of hiding them in a default setting |
| How much human effort remains? | Agreement procedure, escalation rules, and review time per item | A technically strong system may still be impractical at production volume |
Bottom line for editors and evaluators
Judge an AI research review as a measurement system, not as an oracle. Establish independent reference judgments, define “error” precisely, count false positives and false negatives separately, and test both claim support and report completeness. Match the threshold to the cost of each mistake, quantify uncertainty and transfer limits, and make source evidence inspectable. Keep human verification for consequential findings, especially when an explanation sounds convincing but the underlying passage has not been checked.
