Short answer: Treat every locked door in Clive Barker’s Hellraiser: Revival as the end of a clue chain. Search the rooms around it for a named key, an inset, a code, a photograph, a diagram, or a missing statue piece. Conventional locks take the matching item; environmental gates require you to retrieve the missing piece and reshape the Genesis Configuration to match the relief. For the early barbed-wire skull puzzle, press the pins in this order: right side → top-back → left side → top-center. The skull opens with the Tongue Key, which unlocks the fleshy mouth door.

Spoiler warning: The instructions below reveal an early key solution, the graveyard-house safe code, several campaign sequences, and the solution to a repeating-room puzzle.

How Revival’s door puzzles work

Hellraiser: Revival is a first-person survival-horror game for PS5 released on October 8, 2026. Its locks mix familiar survival-horror conventions with surreal environmental challenges. A door may need an ordinary key, a four-digit combination, a picture arrangement, a shaped inset, or a change to the room itself.

The central tool is the Genesis Configuration. Assistant Game Director Vid Rajin of Mad Head Games described it in a PlayStation Blog interview as follows: “We use the Genesis Configuration – a puzzle box – at the centre of things.” It is not merely a combat object. You rotate and transform it to manipulate the environment, align mechanisms, and reproduce the shape shown in a statue or monument relief.

  • Harness the Power of the Box – Combine the unique, hellish abilities of the Configuration with an arsenal of earthly weapons to inflict bone-breaking, flesh-searing pain - or is it pleasure? - upon masochistic enemies.
  • Fight Against Unholy Enemies – Take on hell's wretches, deviants, cultists, and even its most infernal priests in a story-driven horror survival game.

That design explains why a door can remain unusable even after you have found an apparently relevant object. The object may be only half of the solution: a statue’s hand or tongue must first be installed, then the Genesis Configuration must be reshaped to fit the newly completed relief. Other locks are deliberately traditional, with a note or desk diagram supplying the clue for a safe or sliding-picture mechanism. Some puzzles have one binary answer; the developers have also said that other encounters can permit more than one solution.

The examples below are the documented campaign solutions, not a guarantee that every optional or late-game puzzle is catalogued. Guides can change as players discover alternate solutions, so use the environmental logic first and the exact answers as a shortcut when you are stuck.

A reliable method for any locked door

Use this sequence before trying random items or rotating the Genesis Configuration without a clue.

  1. Inspect the lock itself. Look for a keyhole, a mouth, a chain, a socket, a relief, a picture panel, or a number dial. The shape tells you which family of puzzle you are facing.
  2. Search the immediate area. Check desks, bodies, shelves, side rooms, containers, photographs, handwritten notes, diagrams, and objects that can be picked up and examined. The intended clue is often close to the door rather than in a distant objective room.
  3. Read and inspect items fully. A note may provide only part of a combination, while a photograph supplies the second part. Rotate a carried object when the inspection view permits it; the useful marking can be on the back or underside.
  4. Test the named object on the matching opening. A Bedroom Key belongs to the bedroom door, a Tongue Key to the fleshy mouth, and an inset belongs in the ornate door with the corresponding recess. If the game will not accept the item, do not force it: return to the nearby room and look for the matching slot or relief.
  5. For a statue or monument, complete the physical shape first. Place the missing hand, tongue, or other piece, then examine the relief and rotate the Genesis Configuration until its outline matches. The correct orientation is a visual fit, not a general-purpose “use” action.
  6. Recheck the room after every major interaction. Opening a skull, safe, or gate can reveal another key or card rather than the final exit. Follow the new item to the next lock.

The four puzzle families

Puzzle family Evidence that points to the answer Action that completes it Common mistake
Item locks A named key, card, or shaped inset found in the surrounding rooms Use the item on the door, gate, or matching recess Trying a generic key on a lock whose shape belongs to a statue or monument
Code and picture locks Notes, dates, photographs, desk diagrams, or a visible image sequence Enter the numbers or reproduce the indicated picture arrangement Reading only one clue and missing the second part supplied by a nearby object
Genesis Configuration gates A missing statue part and a relief showing the required outline Install the part, then rotate or reshape the configuration to fit the relief Attempting to shape the box before restoring the statue
Spatial and environmental puzzles A room layout, looping corridor, inaccessible walkway, or environmental change Use the indicated route or reverse an expected movement pattern Assuming progress always means moving forward

Skull pin puzzle and the Tongue Key

This is the early solution most players search for. The encounter occurs in the cult compound shortly after the first save point and the clothing change. A fleshy mouth door blocks the route. On a desk nearby sits a barbed-wire skull containing the key.

Exact pin order

  1. Pick up the skull and enter its inspection view. The order below assumes the skull is facing you in the same orientation it has when first picked up.
  2. Press the pin on the right side.
  3. Press the pin on the skull’s top-back. This is the pin at the top of the back of the skull, not simply any pin visible along the upper edge. Rotate the skull during inspection if you cannot see it.
  4. Press the pin on the left side.
  5. Press the pin at the top-center.
  6. After the fourth press, the jaw opens. Take the Tongue Key.
  7. Return to the fleshy mouth door and interact with it to use the Tongue Key.

The orientation warning matters. “Back/top” refers to a physical position on the skull, so choosing a pin that merely looks high on the screen can produce the wrong sequence. If the jaw does not open, leave the inspection view, pick the skull up again, and repeat the sequence from the first pin while keeping the original facing orientation.

Graveyard-house safe: code 9606

The safe in the graveyard house uses two pieces of information rather than a random combination. Read the note and then examine the photograph. The walkthrough identifies the dates as 96 and 06; placed together, they produce the four-digit code 9606.

  1. Find the note in the graveyard house and read it rather than merely collecting it.
  2. Examine the photograph and identify the second date.
  3. Enter 9606 on the safe.
  4. Collect the pistol upgrade inside.

If the safe does not accept the code, verify that you have used the dates from both objects and entered the digits in the displayed order. This is a code lock, so the Genesis Configuration is not involved.

Courtyard gates: statue pieces plus the Genesis Configuration

The courtyard contains gates built around incomplete statues. The official design description uses a tongue or hand as examples of the missing pieces, and the campaign walkthrough describes matching the completed relief with the Genesis Configuration.

General solution

  1. Search the courtyard and adjacent rooms for the statue piece named or depicted by the gate. A gate can remain visually impressive but inactive until its missing hand, tongue, or equivalent part is in your inventory.
  2. Place the recovered piece into the statue. The completed figure exposes or clarifies a relief.
  3. Take out the Genesis Configuration and rotate or transform it until its outline matches the relief’s contours.
  4. Confirm the fit at the relief. A near match is not enough; adjust the orientation until the game recognizes the shape.
  5. Use the resulting mechanism to open the gate, then inspect the newly accessible area for the next clue or key.

Do not treat every courtyard gate as a conventional key door. If there is no keyhole and the surface contains a sculpted recess, look for the missing statue component first. Conversely, if the relief is complete but the box has no apparent matching outline, search for another nearby piece before experimenting further.

Red chained bedroom door and the front entrance

The red chained bedroom door is part of a short chain of objectives rather than a single lock. The required Bedroom Key becomes available after the poison-gas statue puzzle.

  1. Complete the poison-gas statue puzzle and retrieve the Bedroom Key.
  2. Use the Bedroom Key on the red chained bedroom door.
  3. Enter the room and collect the Security Card.
  4. Take the Security Card to the house’s front door and use it there.

If you reach the chained bedroom before solving the poison-gas statue encounter, the door is expected to remain closed. The chain is a progression signal, not a prompt to search for a second random key in the same hallway.

Heart-shaped door and the Heart-Shaped Inset

The ornate heart-shaped door requires a shaped insert, not a regular key. The insert is made through a Genesis Configuration interaction with a monument.

  1. Locate the monument associated with the heart-shaped relief.
  2. Match the Genesis Configuration to the monument’s relief. Rotate and reshape the box until the outline is accepted.
  3. Take the resulting Heart-Shaped Inset.
  4. Return to the ornate heart-shaped door and place the inset in its matching recess.

If the monument does not react, check that you are aligning the configuration to the relief itself rather than to a decorative edge around it. If the door has a heart-shaped socket but you have no inset, the manufacturing step at the monument is still incomplete.

The repeating-room puzzle: turn around

Not every barrier is an object lock. In the demonstrated looping-room sequence, the labyrinth is intertwined with the real world, and the setting changes when Aidan reverses direction. The solution is to turn back instead of trying to move forward.

When a corridor appears to return you to the same room, stop treating the loop as a normal navigation failure. Turn around and walk back through the space. That reversal changes the setting and breaks the tormenting loop. The intended action is a change in direction, not a hidden key or a new combat encounter.

What to do when a door will not open

The item appears correct but is rejected

Inspect the door for a different interaction point. A key may belong to a side lock, while the prominent central object is a relief that still needs a statue piece. Reopen the item’s inspection view and compare its name and shape with the opening.

The Genesis Configuration will not fit

Make sure the associated statue is complete. The hand or tongue is not optional decoration: it is part of the gate’s puzzle state. Then align the box to the relief’s actual outline, not to the surrounding masonry. Rotate through all visible faces rather than holding one orientation and repeatedly confirming.

You cannot find the code

Search for paired evidence. A note, a photograph, and a desk diagram can each supply different parts of a combination. Read every note and inspect photographs in detail; dates and visual marks can be the intended numbers or sequence.

The skull sequence failed

Reset your mental orientation. Pick up the skull again, face it as it appeared when first collected, and use right side, top-back, left side, top-center. The second position is specifically the pin on the top of the skull’s back. Rotate the model to expose that pin instead of substituting a different upper pin.

A room keeps repeating

Try the opposite of your instinct. The documented loop is solved by turning around. The labyrinth sequence uses movement direction as the puzzle input.

How to read clues without wasting resources

  • Prioritize inspectable objects over scenery. A desk diagram, a note, a photograph, or a statue recess is more likely to carry a solution than an unresponsive wall.
  • Record the relationship, not just the number. For the graveyard safe, the useful fact is that the two dates combine as 96 and 06, yielding 9606. Writing down isolated numbers without their order can create a false code.
  • Use shape language literally. Tongue, hand, and heart are descriptions of both the required object and the matching socket. The Genesis Configuration gates reward outline matching.
  • Follow newly obtained access items immediately. The skull gives a Tongue Key; the bedroom gives a Security Card. Each is meant to move you to a specific next barrier.
  • Do not assume every puzzle has one universal interaction. Some locks are binary, while the developers have said other puzzles may permit multiple solutions. If the environment clearly supports another route, observe it instead of forcing a memorized answer.

What is and is not covered by these solutions

The documented examples establish the main logic used by the game: ordinary keys and cards, clue-based codes, picture locks, statue-piece gates, Genesis Configuration shaping, and spatial changes. They do not constitute a verified list of every door, optional room, or late-game puzzle. Third-party walkthroughs provide the exact campaign answers above, while the Genesis Configuration’s role and the environmental examples come from the developer interview. Treat an unfamiliar lock as a fresh clue chain rather than assuming that one of these codes will apply.

For a player who wants to remain spoiler-light, the practical rule is simple: identify the lock type, search the adjacent rooms, complete the physical object or clue, and only then use the door. For a player already blocked at the early mouth door, the four-pin sequence and Tongue Key route above are the direct solution.
