Human approval is effective only when it is a real execution control, not a confirmation screen. In a banking workflow, an AI agent may research records, draft a recommendation, call internal tools and prepare a transaction. The workflow should permit those preparatory steps while technically blocking any gated action until an authorized person reviews the specific request and decides what happens next.
Build the control as a risk-based system: inventory the agent and its tools, classify the consequences of each action, define a narrow permission boundary, route requests to an appropriately authorized reviewer, enforce the decision in the execution service, and continuously log, monitor and test the entire chain.
What “human approval” should mean in a banking workflow
A meaningful checkpoint separates proposing an action from executing it. The agent can gather evidence, calculate an amount, prepare a case file or submit an approval request. It must not be able to complete the protected action through another tool, account, API path or fallback process before the required decision is recorded.
The approval applies to a defined request, not to an agent in the abstract. The request should identify the affected customer, account, payment, record or operational process; the exact action; the amount or content; the evidence used; the applicable policy checks; and material uncertainty. If any material detail changes, the old decision should no longer authorize the new request.

There is no single regulator-prescribed approval pattern for every banking agent. Institution size, jurisdiction, customer impact, activity type, model capability, connected systems and reversibility all matter. The design below adapts general governance, authentication, access-control and monitoring principles to agentic workflows; it is not a legal conclusion for a particular bank.
What current guidance does—and does not—require
OCC Bulletin 2026-13
The Office of the Comptroller of the Currency’s April 17, 2026 Bulletin 2026-13 describes revised interagency model-risk guidance issued with the Federal Reserve Board and FDIC. It addresses model development and use, validation and monitoring, governance and controls, and vendor or third-party products for models within its scope.
The bulletin expressly says that “Generative AI and agentic AI models are novel and rapidly evolving.” It also states that the revised guidance does not cover generative or agentic AI and is not prescriptive or enforceable. Do not cite it as a direct requirement that every bank agent have a human approval step. Its risk-based governance concepts are still useful when designing controls, and the agencies indicated that they plan further information-gathering on generative and agentic AI.

FFIEC guidance treats an institution’s access population broadly. The relevant users can include customers, employees, third parties, service accounts, applications and devices. Its practices include risk assessment, authentication appropriate to the risk, layered controls, and monitoring, logging and reporting. It also says the guidance does not establish a new compliance standard or impose new requirements.
For an agent, that means the identity making a tool call, the identity authorizing an action and the identity executing it should be distinguishable. The same principle applies when a vendor hosts a model or supplies an agent platform.
Federal Reserve Board internal example
The Federal Reserve Board’s compliance plan for OMB Memorandum M-25-21 describes the Board’s own governance process. It includes a use-case inventory, risk-impact assessments, records, technical controls and review of high-impact uses. The plan states: “Any high-impact AI use requires explicit CAIO approval.” That sentence describes a federal agency’s internal policy; it is not automatically a requirement imposed on banks.
EU considerations
A search result for Regulation (EU) 2024/1689 contains human-oversight language for high-risk systems, including assigned natural persons being informed and able to decide whether and how to intervene or stop a system. The operative article, scope, application date and amendments should be checked against the current EUR-Lex text before giving EU compliance advice. Do not assume that a search-result excerpt determines whether a banking workflow is covered.
Design the workflow around risk, not around the model name
Start with the consequence of the proposed action. A low-impact internal classification may need sampling and exception review, while an irreversible payment, account restriction or customer-facing decision may require approval for every request. Classify the action before choosing the interface, reviewer, timeout and evidence standard.
| Risk tier | Typical action characteristics | Possible approval pattern | Additional controls |
|---|---|---|---|
| Low | Internal analysis, search, summarization or draft work with no direct customer or ledger effect | Post-review sampling or approval only for exceptions | Read-only credentials, data minimization, output monitoring and documented sampling rules |
| Moderate | Reversible operational change, internal case routing or a customer communication draft | Named reviewer approval before release or change | Evidence display, policy checks, two-person review for selected cases and an auditable decision record |
| High | Payment, transfer, account status change, credit or fraud decision, disclosure of sensitive data, or action with material customer, financial, compliance or security impact | Per-request approval by a role with matching authority; no execution without a valid decision | Strong identity, transaction limits, separation of duties, expiry, replay protection, continuous monitoring and tested stop controls |
| Critical or irreversible | Large-value movement, irreversible deletion, regulatory filing or an action that cannot be reliably compensated | Independent or dual approval, with escalation and explicit confirmation of the final details | Out-of-band verification where appropriate, conservative timeout behavior, emergency stop, rollback or compensation plan, and heightened incident review |
These tiers are a design framework, not a regulator-defined classification. Record why an action received its tier and reassess it when the workflow, model, data, tools, limits or customer impact changes.
Step 1: Inventory the agent and the workflow
Create a record for each production or pilot use case. The record should be specific enough that another control owner can understand what the agent can do without reading source code.
- Business owner and accountable control owner.
- Purpose, users, customer population and jurisdictions.
- Agent, model and vendor versions, including release dates and configuration changes.
- Data sources, retention rules, sensitive fields and evidence references.
- Every connected tool, API, queue, database, core service and downstream system.
- Actions the agent may propose, actions it may execute, and actions that are prohibited.
- Transaction, volume, rate and geographic limits.
- Human roles involved in review, execution, escalation and incident response.
- Fallback, shutdown and manual-processing procedures.
Give the use case a stable identifier. Link design changes, approvals, incidents, tests and model or policy versions to that identifier. A change-review path should determine whether a modification needs new testing, a new risk assessment or renewed authorization.

Step 2: Classify consequences and reversibility
Assess at least six impact dimensions: customer, financial, operational, privacy, security and compliance. Add reversibility: a mistaken draft can be discarded, while a settled payment or disclosure may be difficult or impossible to undo.
Ask concrete questions:
- Could an error move funds, change account access, alter a customer’s eligibility or create a legal obligation?
- Would a reasonable customer expect a person to decide this action?
- Can the action be reversed automatically, and within what time?
- Does the agent handle confidential, personal or authentication data?
- Could an attacker manipulate the input, tool response or approval request?
- What is the maximum plausible harm if the agent repeats the action?
Use the highest consequential dimension to set the minimum control level. A workflow that is low risk in one institution can be high risk in another because of different products, customers, limits or integration architecture.
Step 3: Define a permission boundary
Write down exactly what the agent can read, what it can call, and what it can change. Avoid a general-purpose credential that allows the agent to reach an unlisted endpoint.
Separate identities
Use a distinct identity for the agent or service account, a distinct identity for the human reviewer and a distinct execution identity where the architecture supports it. Do not let the agent borrow a reviewer’s session or credentials. Bind an approval to the reviewer’s authenticated identity, role, request identifier and decision time.

- Grant read-only access for research tasks unless a write permission is necessary.
- Allow only named APIs and operations, rather than unrestricted network access.
- Restrict records, fields, accounts, regions and transaction types to the use case.
- Apply amount, frequency, velocity and cumulative exposure limits.
- Block prohibited actions at the tool or service layer, not just in the agent prompt.
- Log denied calls as well as successful calls.
Layer authentication and authorization according to risk. A reviewer’s successful login should not by itself authorize every action; the execution service should still verify role, scope, request state and policy conditions.
Step 4: Route each request to the right authority
Name the accountable reviewer role and a backup or escalation owner. Match authority to the action’s risk, value, customer impact and regulatory sensitivity. A person who can review a low-value operational correction may not have authority to approve a large payment or a customer eligibility decision.
Consider independence. For high-impact actions, the reviewer should not be the same identity that created or materially altered the request. A dual-control arrangement can require two independent approvals when one person’s error or compromise would be unacceptable.
Define what happens when the assigned reviewer is unavailable, rejects the request, requests changes or cannot resolve uncertainty. Escalation should preserve the original request and decision history rather than creating an untraceable side conversation.
Recommended Free Tools
Step 5: Build an actionable approval request
The reviewer should be able to make an informed decision without reconstructing the agent’s entire chain of thought. Present verifiable evidence and the result of policy checks, not unsupported confidence scores.
Minimum request contents
- Stable workflow and request IDs.
- Proposed action, exact parameters and intended execution time.
- Affected customer, account, case or process, with only the data needed for the decision.
- Source records, calculations and links or references to authoritative evidence inside the institution.
- Material uncertainty, missing information, conflicting records and detected anomalies.
- Applicable policy results, limits and exceptions.
- Expected consequences, reversibility and any downstream notifications.
- Agent, model, policy and tool versions used to produce the request.
Provide explicit choices such as approve, reject, request changes and escalate. Require a reason for rejection, material edits or escalation when that information is needed for control testing and later review.
Do not use a vague “AI checked” label as a substitute for evidence. The reviewer must be able to identify what the agent did, what it did not verify and which facts are uncertain.

Step 6: Enforce the approval at the execution point
The strongest pattern is a server-side gate in the service that performs the consequential action. The agent can create a pending request, but the execution service accepts it only when a valid approval record satisfies the request’s conditions.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
- Create a canonical representation of the proposed action, including amount, recipient, account, content, scope and relevant context.
- Store a request ID and an integrity-protected version or hash of those material fields.
- Route the request to the authorized reviewer and authenticate the decision.
- Record approve, reject, request-changes or escalate with reviewer identity, role and timestamp.
- Before execution, recheck that the approval is unexpired, unused or otherwise valid under the workflow, and matches the current request.
- Reject execution if the amount, recipient, content, account, policy result or other material context changed.
- Mark the approval as consumed when one-time use is required, and prevent replay.
- Return a clear status to the agent and create an audit event for the execution result.
Prompt instructions, a visual badge or a client-side button are not sufficient if another path can call the protected tool. Test direct API calls, retries, alternate tools, compromised sessions and queue replays.
Step 7: Set expiry, timeout and exception behavior
Approvals should not remain valid indefinitely. Set an expiry appropriate to the action’s volatility. A payment instruction or customer communication may become stale quickly; a lower-risk internal change may have a longer review window.
When no reviewer responds, default to no execution for consequential actions. Escalate according to a documented schedule, preserve the pending request and notify the responsible operations team. Do not silently convert an unanswered request into approval.
For exceptions, define:
- An emergency-stop mechanism that can disable the agent, tool or workflow.
- Who can invoke the stop and how that action is authenticated.
- Rollback, cancellation or compensation procedures where technically possible.
- Manual fallback processing and customer-communication responsibilities.
- Conditions for resuming service after containment.
Emergency procedures should be tested, not merely documented. A stop that works only through the same failing dependency as the agent is not a reliable control.
Step 8: Log and monitor the complete decision chain
Capture enough information to reconstruct what the agent proposed, what the reviewer saw, what changed and what executed. Protect logs from unauthorized alteration and apply retention rules appropriate to the records involved.
Record at least
- Workflow, use-case and request identifiers.
- Agent, model, prompt or policy configuration and tool versions.
- Input and evidence references, including retrieval timestamps.
- Requested action and every material parameter.
- Reviewer identity, role, authentication event and decision timestamp.
- Reviewer edits, comments, escalations and overrides.
- Execution identity, system response, result and downstream identifiers.
- Timeouts, denials, retries, duplicate requests, exceptions and stop events.
Monitor for control failure
Alert on denied or repeated attempts, requests that change after approval, unusual reviewer patterns, out-of-hours activity, abnormal volumes, policy bypass attempts, tool failures and execution without a matching decision. Trend approval latency, rejection reasons, escalation rates and incidents by workflow version. Monitoring should cover both the agent and the people or services around it.
FFIEC guidance identifies monitoring, logging and reporting as relevant access-control practices. The Federal Reserve Board’s internal plan also describes records and monitoring for anomalous activity. Neither source supplies a universal retention period, alert threshold or approval-time target, so set those values through the institution’s risk and records programs.
Step 9: Test before production and after changes
Approval logic is a security boundary and needs adversarial testing as well as ordinary functional testing.

Test cases
- Agent attempts to call the protected operation without approval.
- Approval is expired, rejected, duplicated or associated with another request.
- A non-authorized employee attempts to approve.
- Amount, recipient, account, message content or policy context changes after approval.
- Two workers or retries submit the same approved request.
- Identity, policy, queue, model or core-system dependencies are unavailable.
- Malformed evidence or an incomplete request reaches the reviewer.
- The emergency stop is activated during preparation and during execution.
- Logs are delayed, unavailable or tampered with.
- A vendor changes an API, model version or permission behavior.
Test expected denial as carefully as expected success. Record test evidence, defects, compensating controls and the conditions for release. Re-test after material changes to the model, agent instructions, tools, data sources, limits, reviewer roles or execution services.
Step 10: Govern vendors and dependencies
Include third-party models, agent platforms, APIs, identity providers, monitoring services and core-system integrations in the control inventory. Review who can change them, how changes are announced, how versions are pinned, what logs are available, and how service outages affect approval and execution.

Contractual and operational reviews should address access, confidentiality, incident notification, continuity, subcontractors, data location, deletion, testing support and exit or replacement. The OCC’s revised model-risk guidance discusses vendor and third-party product considerations for models within its scope; FFIEC access guidance is relevant whether systems and controls are supplied by the institution or a third party.
Do not assume that a vendor’s “human-in-the-loop” feature creates a bank-controlled approval. Verify where the gate is enforced, which identity makes the decision, whether the vendor can bypass it, and whether the institution receives complete records.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsChoosing an implementation pattern
There is no universally best product or architecture. Compare patterns against the action and the control objective.
| Pattern | How it works | Best fit | Trade-offs |
|---|---|---|---|
| Inline approval queue | The agent submits a request to a queue; a reviewer decides before the workflow continues. | Case work, customer communications and operational changes with moderate latency tolerance | Clear audit trail, but staffing and queue delays can become operational bottlenecks. |
| Dual control | Two independent authorized people approve the same canonical request. | High-value or high-impact actions where one compromised identity is unacceptable | Stronger separation of duties, with higher staffing and latency costs. |
| Policy-plus-exception review | Low-risk requests proceed under deterministic rules; humans review exceptions and threshold breaches. | High-volume, repeatable work with stable limits and measurable exceptions | Efficient at scale, but policy drift or poor exception detection can hide risk. |
| Prepare-and-release | The agent prepares a complete transaction or message; a human releases it through a separate controlled step. | Payments, filings and customer-facing communications | Easy to explain, but the release system must verify that the prepared content has not changed. |
| Manual fallback | When the agent, reviewer service or dependency fails, trained staff complete the task without agent execution. | Critical workflows requiring continuity | Improves resilience, but requires procedures, access, training and reconciliation. |
Score each option on consequence and reversibility, agent autonomy and tool breadth, reviewer expertise and independence, execution-gate strength, identity controls, auditability, monitoring, latency, staffing and third-party dependence. Choose the simplest pattern that reliably prevents unauthorized execution at the selected risk level.
Operating metrics and review cadence
Use measures that reveal whether the control works, not just whether reviewers clicked a button. Useful measures include:
- Percentage of gated executions with a matching valid approval.
- Denied, expired, escalated and changed-after-approval requests.
- Approval latency by risk tier and business period.
- Duplicate, replayed or out-of-policy execution attempts.
- Incidents, near misses and emergency-stop activations.
- Reviewer disagreement, override and correction patterns.
- Model, tool or policy changes that triggered reauthorization.
- Coverage and results of bypass and outage tests.
Review high-impact workflows more frequently and after incidents or material changes. Feed trends into risk assessments, training, access reviews, model or agent validation and vendor oversight. A low rejection rate is not proof that the control is effective; it may indicate that reviewers are approving without sufficient scrutiny or that risky requests are not reaching them.

A practical launch checklist
- Use-case owner, risk owner and escalation owner are named.
- Agent, model, policy, data and tool versions are recorded.
- Allowed and prohibited actions are explicit.
- Agent, reviewer and execution identities are separated.
- Risk tier reflects customer, financial, operational, privacy, security, compliance and reversibility impacts.
- Reviewer authority matches the action and value.
- Approval request includes exact parameters, evidence, uncertainty and policy results.
- Execution service rejects missing, expired, replayed or mismatched approvals.
- Timeout, emergency stop, rollback or compensation and manual fallback are tested.
- Decision, execution, denial, retry and incident records are retained and protected.
- Monitoring covers anomalous behavior, bypass attempts and post-approval changes.
- Vendor and dependency controls are documented.
- Material changes trigger testing, review and reauthorization.
Bottom line
Set up human approval as a technically enforced, risk-matched authorization boundary. Let the agent prepare work, but make the protected service require a current decision from the right authenticated person for the exact request. Pair that gate with least-privilege identities, useful evidence, safe timeout and stop behavior, complete logging, active monitoring, testing and change governance. This approach reflects the general risk, access and oversight principles in current U.S. guidance without pretending that any one document creates a universal banking-agent approval mandate.
