Set up governance around each agent’s identity, purpose, data, tools, authority and accountable owner. Give every agent a distinct, revocable identity; grant only the minimum task-specific permissions; enforce authorization in downstream services on every call; require independent approval for high-impact actions; and log enough context to reconstruct what happened without recording credentials or unnecessary sensitive content.

What AI-agent governance must control

An AI agent can interpret instructions, retrieve information and invoke tools that change systems. Governance therefore has to cover more than a model prompt or a list of approved use cases. It must control who or what the agent is, which user or session initiated a request, what data and operations are in scope, which tools can be called, who may approve consequential actions, and how activity is reviewed afterward.

Plan for foreseeable failure modes rather than assuming the model will behave. A malicious instruction hidden in a document, email or web page can attempt prompt injection. A poorly scoped tool can enable data exposure, privilege escalation, deletion, purchasing, deployment or permission changes. A confident but mistaken response can still produce an unauthorized outcome if the backend accepts it without an independent policy check.

NIST’s preliminary AI cybersecurity profile (initial preliminary draft, December 2025) recommends unique agent identities and credentials, separate authorization policies for AI systems, least privilege and separation of duties. OWASP guidance similarly treats prompt injection, excessive agency and tool misuse as application-security concerns requiring downstream authorization, approval controls, logging and monitoring.

Practical setup sequence

1. Inventory every agent and assign a human owner

Create a register before granting production access. The accountable owner is a person who can answer why the agent exists, approve its scope, arrange reviews and retire it. Record at least:

  • Business purpose and the tasks the agent is allowed to perform.
  • Deployment environment, model name and model version.
  • Connected tools, retrieval indexes, databases, file stores and downstream services.
  • Whether the agent retrieves information only or can create, send, delete, purchase, deploy or change permissions.
  • Its non-human identity, credential owner, lifecycle dates and revocation procedure.
  • Initiating-user or session requirements, approval requirements and logging destination.

Keep development, test and production agents distinguishable. An agent that shares an identity with a different environment makes attribution and revocation unreliable. Review the register whenever the model, prompt, memory, retrieval source, tool, provider or deployment boundary changes.

2. Classify data and actions before granting access

Map the data the agent may see and the operations it may perform. Use your organization’s sensitivity and regulatory classifications, and document handling, retention and sharing boundaries for each one. The exact legal obligations depend on your jurisdiction and sector; a general technical control does not replace legal or privacy advice.

Separate retrieval from state-changing operations. A read request, sending a message, deleting a record, purchasing, deploying code and changing permissions should not be treated as one generic “tool access” category. For each operation, specify the permitted resources, fields, recipients, environment and time window. Decide whether the agent may access an entire tenant, a project, a record set or only an individual resource.

3. Give the agent a distinct identity and credentials

Use a separate non-human identity rather than a person’s account. The identity should be attributable to one agent, owned by a named human or team, revocable without disabling an employee, and represented in audit records. Credentials should be stored in your approved secret-management system, never in prompts, source code, documents or ordinary logs.

There are two common identity patterns. A dedicated agent identity is useful when the agent has a stable service responsibility and you need clear service-level attribution. Delegated or user-context execution is useful when every action must inherit the initiating user’s resource and tenant scope. Some systems combine both: the request carries the user context while the backend also records the agent identity. Choose deliberately and document how revocation, attribution and inherited permissions work.

Identity model Best fit Controls to verify
Dedicated agent identity Stable automated tasks with a defined service boundary Named owner, separate credentials, narrow scopes, independent revocation and clear service attribution
Delegated user identity Actions that must remain within the initiating user’s permissions Authenticated user and session binding, tenant and resource checks, expiry and prevention of privilege widening
Combined agent plus user context Systems that need both service attribution and user-level accountability Both identities recorded and authorized; neither context may be silently dropped on a tool call

4. Start with deny and grant the smallest useful scope

Write an allow-list for each agent: approved data sources, operations, records, tools, environments and time limits. Begin with no access, then add only what a documented task requires. Separate read from write, and isolate especially consequential operations behind a different role or policy.

Least privilege is not satisfied by saying “do not disclose confidential data” in a system prompt. NIST recommends that AI systems have their own authorization policies, while OWASP recommends giving downstream extensions only the minimum permissions necessary. Enforce those limits in the authorization layer and the service that owns the data or operation.

Review permissions on a schedule and after material changes. A new retrieval connector, memory store, tool parameter, model, prompt or deployment can alter the effective risk even when the agent’s name remains the same. Remove unused scopes rather than allowing them to accumulate.

5. Enforce authorization on every downstream call

Make an authorization decision outside the model for every request. An application gateway, tool server or downstream service may perform the check, but every alternate path must be covered so an agent cannot bypass the policy by calling a different endpoint or using a tool’s broad backend credential.

At minimum, verify:

  • The authenticated agent identity and the initiating user or session, when user context is required.
  • The tenant, project or organizational boundary.
  • The exact resource and data fields being requested.
  • The operation, such as read, create, send, delete, purchase, deploy or permission change.
  • The approved scope, policy version and any time or environment restriction.
  • That the request has not been altered between approval and execution.

Do not treat the model’s proposed tool call, natural-language explanation or self-reported role as proof of authorization. The service that owns the resource must reject a request when identity, scope, policy or session context is missing or inconsistent.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

6. Put independent approval gates around high-impact actions

Separate proposing an action from executing it. The agent can prepare a draft, command or transaction, but a human or an independent policy service must approve defined high-impact or irreversible operations. Typical candidates include sending external communications, deleting records, purchasing, deploying to production and changing access permissions.

Bind an approval to the exact actor, tool, target, parameters, time and expiry. A generic “approved for this agent” flag is too broad: changing the recipient, amount, record, environment or command should require a new decision. Use short-lived authorization artifacts and replay protection so an old approval cannot be reused for a different request. If the policy engine, approval service or audit control is unavailable, fail closed instead of executing.

Define who may approve, whether two people are required for especially sensitive actions, and how an approver sees the proposed parameters. Keep approval separate from the agent’s own output so a prompt injection cannot manufacture consent.

7. Log and monitor without creating a new data leak

Maintain an auditable record that lets an investigator reconstruct the decision and outcome. Capture the agent identity, initiating user or session, effective permission or policy, tool and target, approval state, timestamp, request or transaction identifier, and whether the operation succeeded, failed or was denied. Monitor downstream activity as well as the conversation interface; a harmless-looking chat can still produce a dangerous backend call.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not place credentials, access tokens or unnecessary sensitive content in plain-text logs. Prefer references, hashes or minimized fields when the full payload is not needed for accountability. Protect the logs with their own access controls, retention limits and review process. Logging an action does not make it permissible, and retaining every prompt forever can create privacy and discovery risks.

8. Test, review and retire

Test the complete path from instruction to backend effect. Place malicious instructions in documents, emails and web pages the agent can read, then verify that they cannot expand its permissions, exfiltrate data or trigger an unapproved operation. Test confused-deputy cases in which a user with limited rights tries to make the agent act with a broader service identity.

Repeat tests after changes to prompts, tools, memory, retrieval, models or providers. Review the agent like any other privileged account: confirm its owner, active scopes, approval rules, logs, incidents and last review date. Revoke credentials and remove connectors when the purpose ends. A retired entry should remain in the register long enough to preserve accountability, while its operational access is disabled.

Design choices to make explicitly

Where authorization is enforced

A gateway can provide a consistent front door, a tool server can constrain individual capabilities, and a downstream service can make the final resource-level decision. These are not mutually exclusive. Select enforcement points so every route is checked and the data-owning service never trusts an agent-provided claim without verification.

How fine-grained permissions should be

Broad roles are easier to administer but often expose unrelated records or operations. Task-, resource- and operation-specific scopes reduce blast radius and make reviews meaningful. The right boundary is the smallest one that still completes the documented task; do not grant write access merely because an agent has read access.

How much approval is enough

Set thresholds based on impact, reversibility, affected people and regulatory exposure. Low-risk retrieval may proceed automatically, while an external send, deletion, purchase, production deployment or permission change may require explicit approval. Record the threshold and approver authority in policy rather than leaving it to an individual prompt.

How much to retain for audit

Audit depth should be sufficient to establish who or what acted, under whose authority, on which target, with what permission and approval, and what happened. Minimize message content and secrets. Give investigators access through controlled procedures, and define retention according to operational, legal and privacy requirements.

Standards and project status

NIST’s AI Risk Management Framework (AI RMF) 1.0 and its Generative AI Profile, NIST AI 600-1, published July 26, 2024, provide a risk-management structure rather than a prescriptive agent architecture. NIST’s AI Resource Center states that AI RMF 1.0 is being revised, so verify the current edition when adopting it.

NIST’s initial preliminary draft AI cybersecurity profile from December 2025 contains agent-specific considerations for unique identities and credentials, separate authorization policies, least privilege, separation of duties, and permission review. It is a preliminary draft, not a final mandatory standard.

The NIST National Cybersecurity Center of Excellence Agentic AI Identity and Authorization project is developing an SP 1800-series practice guide with example implementations, architectures, build details and laboratory lessons. Its project page describes a February 2026 concept paper and an iterative process; the practical guide is therefore still being developed.

NIST’s Cybersecurity for Open and Secure Artificial Intelligence Systems (COSAiS) project is developing control overlays based on SP 800-53 for proposed single-agent and multi-agent use cases. The project page describes a concept paper available for comment in August 2025, so those overlays should not be represented as finalized controls.

OWASP’s AI Agent Security Cheat Sheet and LLM06:2025 Excessive Agency guidance are useful implementation references for prompt-injection defenses, minimum downstream permissions, user-context execution, approval binding, short-lived authorization, fail-closed behavior, logging and monitoring. A vendor’s framework mapping can help organize implementation, but it does not replace an authorization decision in your own systems.

Common failure modes and fixes

The agent uses an employee’s account

Why it fails: attribution, revocation and scope become inseparable from the employee, and an automated process may inherit permissions that were never intended for it.

Fix: issue a distinct non-human identity, name its owner and define whether user context must also be carried on each request.

Permissions exist only in the system prompt

Why it fails: a prompt is not an enforcement boundary and can be bypassed by malicious content, model error or a direct tool invocation.

Fix: enforce identity, resource, operation and tenant checks in the gateway, tool server and data-owning service as appropriate.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

One token can read and change everything

Why it fails: a compromised tool call or mistaken action has an unnecessarily large blast radius.

Fix: split read and write scopes, narrow resources and operations, and place sensitive writes behind independent approval.

Approval is detached from the actual transaction

Why it fails: an approval for one recipient, amount or record can be replayed against another.

Fix: bind approval to actor, tool, target, parameters, expiry and a one-time authorization artifact; reject changed or expired requests.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Logs contain secrets or only conversation text

Why it fails: secrets create a second attack path, while chat transcripts may not show the effective permission or backend result.

Fix: minimize sensitive content and record identity, session, policy, tool, target, approval and outcome with protected retention.

A governance review checklist

  • Is there one accountable human owner and a current purpose statement?
  • Does the agent have a distinct, revocable identity and protected credentials?
  • Are data sources, records, fields, operations, tools, environments and time limits explicit?
  • Are read, write and irreversible operations separated where appropriate?
  • Does every downstream path enforce authorization independently of the model?
  • Are user, session and tenant boundaries verified on each call?
  • Are high-impact actions approved independently and bound to exact parameters?
  • Does the system fail closed when policy, approval or audit controls are unavailable?
  • Can logs reconstruct identity, permission, approval, target and outcome without exposing secrets?
  • Have prompt-injection and confused-deputy tests been repeated after material changes?
  • Is there a documented review, revocation and retirement process?
