Set an AI security agent’s authority in an enforcement layer outside the model. Give it only the tools, data and permissions required for its assigned task; let low-impact, reversible actions run within explicit limits; and require a human decision before destructive, privileged, financial, administrative or externally visible operations. If an action is unknown, outside scope, cannot be validated, or the approval service is unavailable, the safe result is to stop and escalate—not to let the model guess.

Approval is an authorization control, not a conversational prompt. The execution path must independently verify identity, policy, the exact target and parameters, approval validity and auditability immediately before a side effect occurs.

What an approval limit controls

An approval limit defines the boundary between actions an agent may perform autonomously and actions that require a person or a stronger control. It should be expressed in enforceable policy and execution middleware, not only in system instructions or a model-generated “are you sure?” message.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

OWASP’s guidance for excessive agency makes the distinction explicit: “Implement authorization in downstream systems rather than relying on an LLM to decide if an action is allowed or not.” A model can propose an operation, but a policy service, tool gateway or downstream application must decide whether that operation is authorized.

A useful limit has five parts:

  • Actor: the specific agent identity and, where applicable, the human user whose authority is being delegated.
  • Tool: the exact function or API operation, not merely a broad capability such as “administration.”
  • Target and scope: the systems, records, files, accounts or tenants that may be touched.
  • Parameters and consequence: the normalized values, data volume, recipients, monetary amount or other effect.
  • Time and approval state: when the authorization was issued, when it expires and whether it has already been consumed.

If any of these cannot be established, classify the request as unknown or unvalidated and route it to denial or human review.

Start with least agency, not with an approval button

Human review cannot compensate for an agent that has unnecessary powers. Remove dangerous tools and narrow credentials before deciding which actions a person must approve.

Grant only task-required tools

An agent assigned to inspect documents may need search and read operations. It should not inherit write, delete, permission-management or code-execution functions simply because a connected plug-in exposes them. Separate read and write interfaces, and expose the smallest operation that satisfies the task.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use distinct identities

Give each agent, workflow or deployment an identifiable service identity. Do not run every agent under a shared administrator account. Carry the initiating user’s authorization context to the downstream service when the agent is acting on that user’s behalf, while still enforcing the agent’s own policy constraints.

Use short-lived, task-scoped credentials

Issue credentials for the current task and audience, with the smallest possible permissions and an expiry. Do not place long-lived administrator secrets in prompts, tool configuration or agent memory. OWASP DevSecOps guidance recommends task-scoped short-lived credentials, deny-by-default rules and approval for actions outside explicit allow rules.

Separate proposal from execution

The model should produce a structured action request. A policy component should classify and authorize that request, and an execution component should perform it only after the checks pass. This separation prevents a model from bypassing an approval screen by calling a second tool path.

Use risk tiers as a starting policy

Risk tiers make the boundary understandable to operators and consistent across tools. The table below adapts OWASP’s illustrative action list; it is not a universal standard. Adjust it for your assets, sector, legal duties, recovery capability and tolerance for downtime or exposure.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Action class Default treatment Illustrative examples
Low impact, read-only Permit only when explicitly scoped to the user, task and data set. Log the operation. Search documents; read a file
Limited write Show a preview and obtain approval unless a narrow, reversible workflow has been pre-authorized. Write or update a file
High impact or external side effect Require explicit approval bound to the exact operation before execution. Send email; execute code
Critical or hard to reverse Require approval, independent policy validation and stronger authentication. Add rollback or recovery controls where feasible. Delete database records; transfer funds
Unknown, out of scope or unvalidated Deny and escalate. Never infer permission from model confidence or presumed user intent. Unmapped tool; failed policy, approval or logging check

Do not publish universal dollar amounts, record-count limits or model-confidence cutoffs. The reviewed guidance supports risk-based boundaries but does not establish numeric thresholds that apply to every organization.

Factors that raise an action’s tier

  • It changes access rights, security controls, production configuration or identity data.
  • It can disclose confidential, regulated or personal information to a new recipient.
  • It affects many resources, multiple tenants or a large number of records.
  • It is externally visible, such as sending a message, filing a report or changing a public service.
  • It is difficult to reverse, has no reliable backup or could interrupt operations.
  • It executes code, changes policy or can create additional tools or credentials.
  • The target, parameters or requested authority cannot be independently validated.

Build the approval and escalation path

Implement the following sequence for every operation that can create a side effect. Keep classification and authorization outside the model.

  1. Receive a structured action request. Require the agent to identify the tool, target, intended operation and parameters in a machine-readable form. Reject malformed or ambiguous requests before classification.
  2. Classify the operation independently. Evaluate tool, target, scope, reversibility, data sensitivity and potential impact. Treat an unknown tool or unmapped operation as high risk or deny it according to your policy. An example classification does not itself authorize use of a tool.
  3. Verify identity and delegated authority. Confirm the agent identity, the initiating user or workload, tenant boundaries and current credentials. Check that the user is allowed to request this class of operation and that the agent is allowed to perform it.
  4. Apply deny-by-default policy. Match the normalized request against explicit allow rules. A missing rule is not consent. Check constraints such as permitted resources, recipients, time window, environment and maximum scope.
  5. Create an action-specific preview. Present the actual operation, target, parameters and material consequences to an authorized reviewer. Avoid vague text such as “the agent wants to make a change.”
  6. Bind approval to the exact request. Store the actor, tool, resource, normalized parameters, policy decision, approver, timestamp, expiration and one-time or replay-protected approval identifier. If any material field changes, require a new approval.
  7. Revalidate immediately before execution. The policy or execution service must check current permissions and approval state at the moment of the side effect. Do not let the model mark its own request approved.
  8. Execute with constrained credentials. Invoke only the approved tool operation, with the approved parameters and task-scoped credential. Prevent the agent from substituting a more privileged endpoint.
  9. Record the result and notify the owner. Log the request, decision, execution result, errors and any rollback. Route failures or unusual outcomes to the named operational owner.

Design a review screen people can safely approve

A reviewer should be able to understand the real operation without reconstructing it from a conversation. Display:

  • With which values: normalized parameters, filters, record counts, file paths, code hash, amount, currency and schedule, as applicable.
  • Why and under which rule: task identifier, policy rule, risk tier and stated business purpose.
  • Impact and recovery: expected side effects, data exposure, service interruption and available rollback or restore method.
  • Validity: issue time, expiration, whether approval is one-time, and whether a second approver or step-up authentication is required.

Do not allow an approval to authorize a mutable reference such as “whatever the agent finds next.” Resolve the target and parameters before review, or force a fresh approval after resolution. For critical actions, use short-lived authorization artifacts, replay protection and stronger authentication.

Define when the agent must stop

Escalation is required when the system cannot establish that an operation is safe and authorized. Configure explicit triggers rather than relying on a model’s sense of uncertainty.

Scope and policy triggers

  • The requested tool or operation has no classification or allow rule.
  • The operation exceeds the task’s resources, environment, recipient set or data scope.
  • The request changes permissions, security policy, production configuration or credentials.
  • The action combines multiple individually permitted steps into a high-impact outcome.
  • The model requests a new tool, broader credential or exception to an existing rule.

Validation and integrity triggers

  • The target cannot be resolved uniquely or its ownership is unclear.
  • Parameters changed after approval, or a downstream service returned a different normalized request.
  • Identity, credential, policy lookup, risk classification or approval verification fails.
  • Audit logging is unavailable, incomplete or cannot be written durably.
  • The action is not idempotent and a retry could duplicate an external effect.

Operational and safety triggers

  • The operation is destructive, irreversible or exceeds the available recovery window.
  • Observed behavior conflicts with the task plan, expected data or environmental safeguards.
  • Prompt injection, suspicious instructions in retrieved content or tool-output manipulation is detected.
  • The agent encounters a time-sensitive incident for which no named approver or runbook exists.

Escalate to a named owner for each action class. The exact job titles are an organizational decision: for example, a security operations lead may own containment actions, while a system owner may approve production changes. Define backup approvers and an emergency process in the runbook so “urgent” does not become an excuse to bypass controls.

Fail closed when a control is unavailable

If the policy service, approval store, identity provider, risk classifier or audit path is unavailable, block the side effect. Queue a non-executing request for review if appropriate, return a clear status to the agent and alert the designated owner. Never interpret an outage as approval.

Use separate health states for “policy unavailable,” “approval expired,” “identity invalid,” “logging failed” and “downstream execution failed.” They require different recovery actions and should not be collapsed into a generic success or retry loop.

For operations that may be retried, require an idempotency key and verify whether the first attempt completed before sending another request. For irreversible actions, require an explicit recovery plan or a second confirmation rather than automatic retries.

Make permissions granular enough to enforce

Broad roles such as “security administrator” hide the boundary you are trying to protect. Prefer permissions that identify the operation and resource, such as reading alerts in one tenant, quarantining a named endpoint or updating a specific policy object. Constrain:

  • Resource: tenant, project, repository, host, account or record set.
  • Operation: read, create, modify, send, execute, disable or delete.
  • Environment: test, staging or production.
  • Data: sensitivity class, fields, geography and retention restrictions.
  • Volume and frequency: number of records, recipients or changes in a time window.
  • Delegation: which human or service may request the action and for how long.

Keep read access separate from write and delete access. A document-reading agent should not be able to alter the documents it inspected. A monitoring agent may collect evidence without being able to disable a control. This reduces the damage from prompt injection, a malfunctioning model or a compromised extension.

Test the enforcement path, not just the model

  • Attempt a prohibited operation through every available tool and integration.
  • Change a target or parameter after approval and verify that execution is blocked.
  • Replay an approval token and confirm that one-time controls reject it.
  • Disable policy lookup or audit logging and verify fail-closed behavior.
  • Use expired credentials, cross-tenant targets and production resources from a test workflow.
  • Inject instructions into retrieved documents or tool output and confirm that they cannot grant authority.
  • Simulate partial execution, timeouts and duplicate delivery to test idempotency and recovery.
  • Review alerts and escalation routing with the people who must respond.

Threat-model the agent, its tools, data sources, middleware and downstream services together. CISA’s May 1, 2026 joint guidance on careful adoption of agentic AI services emphasizes limited autonomy, layered defense, identity management, oversight, threat modeling, continuous monitoring and regular security assessment.

Examples of sensible boundaries

Scenario Autonomous allowance Approval or escalation condition
Investigate a suspicious login Read the specified alerts, sign-in records and device inventory; produce a report. Quarantine a device, disable an account or change access policy.
Respond to a malware alert Collect evidence from named hosts and calculate hashes. Delete files, isolate production systems or execute remediation code.
Review a cloud configuration Read configuration and identify deviations against an approved baseline. Change firewall rules, rotate credentials or modify an identity role.
Prepare an incident notification Draft a message for a defined internal audience. Send external email, notify regulators or publish a status update.
Manage a vulnerability queue Read findings and update a task label in a designated project. Close findings, alter severity, grant exceptions or deploy code.

These examples illustrate the boundary between observation or preparation and an operation that changes systems, authority or external state. Your own policy must account for the asset and consequence, not just the tool name.

Use current guidance without treating it as a universal taxonomy

OWASP’s LLM06:2025 guidance identifies excessive functionality, excessive permissions and excessive autonomy as recurring causes of excessive agency, including risks from prompt injection, malfunctioning models and overpowered tool extensions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The OWASP security cheat sheet recommends explicit approval for high-impact or irreversible operations, previews, risk-based autonomy, audit trails and interruption or rollback. For critical actions it adds independent validation, exact-action approval binding, short-lived authorization, replay protection, step-up authentication and fail-closed behavior.

OWASP’s Agent Control Standard page, dated September 1, 2026, describes runtime middleware hooks for declarative policy enforcement and calls for behavior that is inspectable, traceable and instrumentable. It indicates a direction for implementation; it does not establish that any particular product has been independently assessed.

NIST describes AI-agent systems as capable of autonomous decision-making and action with limited human supervision. Its Control Overlays for Securing AI Systems project, updated January 8, 2026, includes single-agent and multi-agent use cases. Use such frameworks to structure controls, then map them to your own systems and obligations.

Deployment checklist

  • Every tool and operation has an owner, risk classification and explicit allow or deny rule.
  • Unneeded tools, write paths, delete functions and administrative permissions are removed.
  • Agent identities are distinct; credentials are short-lived and task-scoped.
  • Human authorization context is preserved where delegation is permitted.
  • High-impact, destructive, privileged and externally visible actions require approval.
  • The preview shows the exact tool, target, parameters, consequences and recovery path.
  • Approval is bound to normalized parameters, actor, timestamp and expiry.
  • Execution rechecks policy and approval immediately before the side effect.
  • Unknown, out-of-scope and changed requests are denied or escalated.
  • Policy, identity, approval and audit failures fail closed.
  • Critical actions use stronger authentication, replay protection and idempotency.
  • Logs support investigation and are protected against tampering.
  • Named owners, backups and emergency procedures exist for each escalation class.
  • Regular tests cover bypasses, prompt injection, outages, retries and partial failures.
