Put a blocking approval gate immediately before the action that sends form data or changes external state. The gate should show an authorized reviewer the destination, the exact values to be sent, and the consequence; then allow the application—not the agent—to resume the pending action only after verifying and consuming that reviewer’s decision.
Where to put the checkpoint
Find the precise operation that commits the side effect: a browser’s Submit action, a form-posting tool call, or an API request that transmits data or changes an external record. Pause immediately before that operation. OpenAI’s Agents SDK describes human review as a way to pause execution before sensitive tool calls (OpenAI Agents SDK: Human-in-the-loop).
Do not assume that waiting until the final button click covers every sensitive step. OpenAI’s computer-use guidance treats typing sensitive information into a form as transmission, even before submission (OpenAI computer-use guide). Your policy should therefore identify sensitive fields as well as the final commit action. Depending on the data and destination, require review before entering sensitive values, before submitting, or at both points.

Classify actions by their impact and reversibility. A low-impact, easily reversible submission may justify lighter oversight; sensitive, consequential, or difficult-to-reverse actions call for explicit approval, and some workflows may require a person to initiate the action. OWASP recommends risk-based oversight tiers, but the guidance does not set a universal scoring formula or threshold (OWASP AI Security and Privacy Guide).

Interrupting every low-impact action can create approval fatigue. Use tiers and, where appropriate, batch low-impact decisions while preserving explicit blocking review for actions whose risk warrants it. OWASP discusses both tiering and batching as ways to manage fatigue (OWASP AI Security and Privacy Guide).
Build the approval gate outside the agent
A prompt such as “ask the user before submitting” is an instruction, not an enforcement mechanism. Put the check in the tool executor, policy service, or another infrastructure layer that can prevent the side-effecting operation from running without verified approval. OWASP states: “Implement checkpoints as infrastructure-layer gates, not LLM instructions — the agent must be technically incapable of proceeding without a verified human approval signal.” (OWASP AI Security and Privacy Guide)

When the agent reaches a gated action, the trusted application should stop execution and create a pending request. Keep the run state and pending request in application-controlled server storage, rather than accepting the agent’s or browser client’s account of what is pending or approved. OpenAI’s human-in-the-loop flow provides the pause-and-resolve pattern for sensitive tool calls (OpenAI Agents SDK: Human-in-the-loop).
What the reviewer needs to see
Show the action the system is actually poised to perform, not only the agent’s summary. OWASP recommends exposing exact commands and tool arguments for review (OWASP AI Security and Privacy Guide). For a form submission, make the destination and submitted values clear, and state the material consequence of sending them.

- Destination: the service, site, account, or recipient that will receive the data.
- Submitted values: the actual field values or a clear, reviewable representation of them.
- Action and consequence: what the submit operation will do, such as create or update a record or send a request.
- Decision controls: an unambiguous way to approve or reject the specific pending action.
Treat tool names, arguments, and other agent-generated display content as untrusted. Escape it before rendering, and apply the application’s secret-handling rules so sensitive values are not exposed unnecessarily. The approval interface must balance meaningful inspection with appropriate protection of secrets.
Authenticate the reviewer and bind approval to the pending action
Derive the reviewer’s identity from trusted application authentication middleware, then check that this person is authorized to decide for the particular run and action. Do not accept a reviewer name or an approval flag supplied by the client as proof of identity or permission. OWASP’s guidance emphasizes protecting approval state and validating the decision against the pending request (OWASP AI Security and Privacy Guide).

Give each pending request an opaque decision identifier. When a decision arrives, verify on the server that the identifier still refers to the stored pending action, and that the reviewer may act on it. The decision should apply to that action and its stored arguments—not to a mutable request that the client can replace after review.

Resolve once, then resume safely
- Create pending state: Store the run state, destination, exact pending action and arguments, and an opaque decision identifier in trusted server-side storage.
- Pause execution: Ensure the side-effecting tool call cannot continue while the request is unresolved.
- Verify the decision: Authenticate the reviewer, authorize them for this run and action, and confirm the identifier still matches a pending request.
- Consume atomically: Change the request from pending to approved or rejected in a single atomic operation. Reject stale, duplicate, or already-consumed decisions.
- Resume from trusted state: On approval, continue from the server-owned run state and execute only the action that was reviewed. On rejection, do not perform it.
Atomic consumption prevents two concurrent requests from both treating one approval as valid, while checking the pending state prevents replay. The agent should not be able to turn a rejection, expired request, or altered client payload into permission to proceed.

Record the decision
Keep an audit record of the approval outcome and relevant action context outside the agent’s control. Apply the application’s rules for secret handling and log retention; there is no universal retention period established by the guidance. OWASP recommends logging human-approval activity while protecting sensitive information (OWASP AI Security and Privacy Guide).
