You can self-host OpenEMR on an Ubuntu VPS in 2026 by running the official OpenEMR Docker image with a private MySQL or MariaDB service, or by installing Apache, PHP and MySQL/MariaDB directly on Ubuntu. Docker is the better default when you want repeatable deployments and simpler upgrades; a native installation is appropriate when you need direct control over the web server, PHP configuration and filesystem.
Whichever route you choose, treat the VPS as a healthcare application server: keep the database off the public internet, enforce HTTPS, use separate administrative credentials, restrict file permissions, monitor logs and test encrypted backups before changing versions.
What an OpenEMR VPS actually contains
OpenEMR is not a single binary. A working installation has an application layer, a PHP-capable web server, a relational database and persistent storage for uploaded documents and configuration. On Ubuntu, the native arrangement is usually Apache, PHP and MySQL or MariaDB. The official Docker arrangement packages the application and runs a companion MySQL or MariaDB service.
The Docker image listens on HTTP port 80 and HTTPS port 443. The database should communicate with OpenEMR over a private Docker network or private VPS network; there is no reason to publish the database port to the internet. DNS points a hostname such as emr.example.org to the VPS, while the firewall exposes only the web ports and the SSH access you actually need.

The exact PHP extensions, database requirements and supported operating-system packages depend on the OpenEMR release. The OpenEMR 7.0.4 Linux guide specifies PHP 8.2 or newer, particular PHP extensions and an Apache LimitRequestFieldSize value of 32768. Recheck the requirements for the release you select rather than assuming that a later version has identical requirements.
Choose Docker or a native Ubuntu installation

For a new VPS, start with Docker unless a specific integration requires native installation. Do not mix the two approaches on the same hostname: choose one owner for ports 80 and 443 and one location for the persistent database and document files.

Prerequisites before you install
VPS and operating system
- Provision a currently supported Ubuntu LTS release and apply all available security updates before installing OpenEMR.
- Create a non-root administrative account with
sudoaccess. Use root only for the initial account and system provisioning tasks. - Allocate storage for the operating system, database, OpenEMR files, uploaded documents, logs and backup staging. No universal CPU, RAM or storage sizing figure applies, so size from your patient, document and retention workload instead of relying on a generic minimum.
- Enable automatic security updates where your change-control process permits them, and monitor disk usage so a full volume cannot stop the database.
Network and DNS
- Reserve a stable IPv4 address and, if you will publish IPv6, a stable IPv6 address.
- Create an A record for the OpenEMR hostname. Create an AAAA record only when IPv6 is correctly routed and filtered on the VPS.
- Permit SSH only from trusted administrator addresses when possible. Permit TCP 80 and 443 for the web service; do not publish MySQL or MariaDB’s database port.
Secrets and recovery
- Prepare unique, long values for the database root password, application database password and OpenEMR administrator password.
- Store secrets in a file readable only by the account that runs the deployment, or use your secret-management system. Do not put passwords in shell history, screenshots, tickets or public Compose files.
- Decide where encrypted off-site backups will live and how you will test a restore before the first upgrade.
Provision and harden the Ubuntu VPS
- Log in with the provider’s initial account. Change the initial credential if required, create your administrative user and grant it
sudoaccess. Confirm that a second SSH session can log in with the new account before disabling the initial account. - Apply updates and reboot if required. Run the normal Ubuntu update and upgrade process, review any packages that require a restart, and reboot during a maintenance window if the kernel or core services changed.
- Configure the firewall. Allow SSH, HTTP and HTTPS, then enable the firewall. If SSH is restricted to a management subnet, add that narrower rule instead of opening it globally. Verify the active rules from a second session so you do not lock yourself out.
- Set time and logging. Keep Ubuntu’s time synchronization enabled. Accurate timestamps make audit review, certificate renewal and incident investigation possible.
- Reduce exposed services. List listening sockets and remove or bind services that are not required. The database should listen only on the private interface or Docker network used by OpenEMR.
Healthcare data deserves controls beyond this application tutorial. Determine the privacy, retention, access-audit and breach-response obligations that apply in your jurisdiction and contract structure before putting real patient data on the VPS.
Deploy OpenEMR with the official Docker image
Install Docker and prepare a deployment directory
Install a supported Docker Engine and the Compose plugin using Ubuntu’s current package guidance or the Docker project’s repository instructions. Verify both commands before proceeding:

docker compose versionCreate a directory owned by the deployment administrator for the Compose file, environment file and operational notes. Keep persistent database and OpenEMR document volumes on storage covered by your backup plan.
Use the production Compose pattern
Obtain the official OpenEMR production Compose example for the release you intend to run. Do not copy an old community file without checking its image names, database service, health checks, volumes and environment variables. The documented automatic installation requires MYSQL_HOST and MYSQL_ROOT_PASS. Optional variables define the database name, database user and database password, along with OpenEMR credentials.

Put those values in a protected environment file or secret mechanism. The Compose file should place the application and database on a private network and should persist both the database data and OpenEMR’s uploaded files. Never publish the database service’s port on the VPS host.
Pin the images before the first start
Use a dated OpenEMR image tag or an immutable digest, not an unqualified floating tag. The Docker Hub listing showed 8.4.1-2026-10-14 at the time of the source review, but tags and dependencies are volatile; verify the currently available tag immediately before deployment. The official production Compose example also pins OpenEMR and MariaDB images by digest. Record the exact tag or digest in your change log so a later administrator knows what is running.

Start and verify the stack
- Review the environment file for spelling, permissions and accidental whitespace. Confirm that
MYSQL_HOSTnames the Compose database service, not a public IP address. - Start the services in the deployment directory with
docker compose up -d. - Follow the application and database logs with
docker compose logs -f. Look for a healthy database connection, completed initialization and a web listener without repeated restart loops. - Open the hostname in a browser. If the selected image presents a web installer, complete it with the prepared credentials and record any generated configuration securely.
- Sign in with the OpenEMR administrator account, create a separate day-to-day account, and avoid using the highest-privilege account for routine work.
- Test a representative document upload, patient-record operation, scheduled job and outbound mail or integration setting in a non-production record. Confirm that uploaded files survive a container restart.
Enable HTTPS with the image’s certificate automation
The Docker image documents automatic Let’s Encrypt setup when DOMAIN and, optionally, EMAIL are supplied. Point the DNS A or AAAA record to the VPS first, allow inbound TCP 80 and 443, then start or recreate the stack with those variables present. Certificate issuance will fail when DNS points elsewhere, port 80 is blocked, a reverse proxy already owns the ports, or the hostname is not publicly reachable.
After issuance, browse to the HTTPS hostname, inspect the certificate name and expiry, and confirm that HTTP requests are redirected or handled as intended by the image. Test renewal in a maintenance window and keep an alternate administrative path available if certificate automation stops.
Install OpenEMR natively on Ubuntu
Choose this route when you need direct Apache and PHP control or must integrate OpenEMR with existing host services. Follow the Linux installation guide for the exact OpenEMR release; the 7.0.4 guide is not a guarantee for later versions.
Install and configure the software layers
- Install MySQL or MariaDB. Apply the database project’s security recommendations, create the OpenEMR database and a dedicated application user, and keep the root account for administration only.
- Install Apache or another PHP-capable web server. Reserve the OpenEMR hostname and ensure only the intended virtual host serves it.
- Install PHP 8.2 or newer when using the 7.0.4 guide. Add every PHP extension and setting listed by that release’s guide. Do not infer the list from another OpenEMR version.
- Set Apache’s request-field limit. The 7.0.4 Linux guide specifies
LimitRequestFieldSize 32768. Place it in the appropriate Apache configuration context, validate the configuration, and reload Apache. - Place the release under the web root. The Ubuntu example in the guide uses
/var/www/html. Keep the exact release directory and ownership documented so upgrades do not overwrite unrelated virtual hosts. - Run the OpenEMR installer. Supply the database host, database name, application user and password. Complete the administrator setup only over a protected connection.
Handle permissions safely
The native installer may need specific OpenEMR directories to be writable during setup. Grant only the documented temporary write access, complete installation, then return those paths to restrictive ownership and permissions. The web-server account should not have write access to the entire operating system or to backup destinations.

Configure native HTTPS
Install a certificate for the OpenEMR hostname in Apache or your chosen reverse proxy, enable renewal, and test both renewal and service reload. Redirect or reject plain HTTP after certificate issuance unless the installation guide or a required ACME challenge needs a controlled port-80 response. Verify that cookies, document uploads and any integration callbacks use the HTTPS hostname.
Post-installation security checklist
- Confirm that only SSH, HTTP and HTTPS are reachable from the internet.
- Confirm that MySQL or MariaDB is reachable from OpenEMR but not from arbitrary public addresses.
- Use separate named user accounts and the minimum practical privileges for routine administration.
- Protect the environment file, Compose file containing secrets, database credentials and OpenEMR configuration backups.
- Review container, Apache, PHP, database and Ubuntu logs for authentication failures, repeated crashes and storage errors.
- Test session timeout, password-reset mail and administrator recovery before going live.
- Document who can access the VPS, who can access OpenEMR and how access is revoked.
Back up OpenEMR before every upgrade
Back up both the OpenEMR installation and its database before upgrading. The OpenEMR installation documentation explicitly warns: “Be sure to back up your OpenEMR installation and database before upgrading.” A database-only dump does not preserve uploaded documents, configuration files or deployment metadata; a file-only copy does not preserve transactional database state.
Docker backup scope
- Export a consistent MySQL or MariaDB dump while coordinating with the running application.
- Copy the persistent OpenEMR document and configuration volumes.
- Save the Compose file, exact image tags or digests and the protected environment or secret references needed to recreate the stack.
- Encrypt the backup before it leaves the VPS, apply a retention policy and store at least one copy off-site.
Native backup scope
- Take a consistent MySQL or MariaDB dump.
- Copy the OpenEMR web directory and any separately configured document or attachment paths.
- Save Apache virtual-host configuration, PHP settings, certificate-renewal configuration and a record of installed package versions.
Prove that recovery works
Periodically restore the database and files to a staging VPS or isolated environment. Verify that you can log in, open an existing record, retrieve an attachment and complete a representative workflow. A backup that has never been restored is an assumption, not a recovery plan.
Upgrade without turning the VPS into a live experiment
- Read the release notes and the installation requirements for the target OpenEMR version.
- Take the complete database and file backup described above, then create a VPS snapshot when your provider supports it.
- Rehearse the upgrade on staging with a copy of production data protected according to your privacy obligations.
- Schedule a maintenance window and notify users. Stop writes or place the application in the documented maintenance state.
- For Docker, change the pinned OpenEMR and database image tag or digest in the Compose configuration; for native installation, update the release and required packages according to the official guide.
- Start the updated stack or complete the native migration, watch logs and run login, document-upload, scheduled-job and mail/integration checks.
- Keep the previous image reference or VPS snapshot until the new release has passed your acceptance checks.
Troubleshooting common failures
The browser cannot connect
Check DNS A and AAAA records, VPS firewall rules, the provider’s network firewall and the service listening on ports 80 and 443. In Docker, inspect docker compose ps and logs. In a native installation, validate Apache’s configuration and confirm that another service has not claimed the ports.

The application cannot reach the database
Verify that MYSQL_HOST exactly matches the Compose database service name, that the database container is healthy and that the application and database share a private network. For native installs, check the database host, credentials, user privileges and local socket or TCP settings. Do not solve the problem by exposing the database publicly.

Confirm that the hostname resolves to this VPS, port 80 is reachable for the ACME challenge, the certificate email is valid when supplied, and no second reverse proxy is intercepting the request. Check the container or Apache renewal logs and retry only after correcting the underlying DNS or firewall condition.
Uploads fail or disappear after a restart
Check available disk space, PHP upload limits, Apache request limits and filesystem ownership. In Docker, confirm that the document path is backed by a persistent volume rather than the container’s writable layer. In a native install, verify that the web-server account can write only to the directories OpenEMR documents as writable.
An upgrade loops or shows database errors
Stop repeated restarts, preserve the logs, confirm that the target release supports the installed PHP and database versions, and restore the pre-upgrade backup in staging. Do not delete the old database or volume while investigating.
Docker versus native: operational decision
Docker minimizes configuration drift and makes a dated application image reproducible, but it adds responsibilities for Compose files, image provenance, secrets and persistent volumes. Native installation gives precise control over Apache, PHP and the host filesystem, but every package and permission change becomes part of your maintenance burden. For most first-time Ubuntu VPS deployments, the official Docker production pattern is the shorter path to a supportable system. Select native installation when your team already operates Apache/PHP stacks or needs host-level customization that the image does not expose.
