Do not give an AI agent production permissions because it answered a few questions correctly. Test the complete application—model, prompts, tools, identities, data, memory, approval workflow and external inputs—in an isolated environment that resembles deployment. Combine repeatable normal-task evaluations with adaptive adversarial tests, inspect every tool call and state change, require an independent approval layer for high-impact actions, and retain evidence for every release decision.
No score proves an agent safe for every situation. Treat safety as a risk-managed operating process: narrowly scope permissions, block unsafe side effects, retest after material changes, and monitor continuously after launch.
What you are actually testing
An agent is more than a language model producing text. It may read a ticket, retrieve documents, remember information across sessions, choose a tool, pass arguments to an API, and cause a real change. A model can produce a plausible answer while the surrounding application sends an invoice, changes a record, exposes another user’s data or retries an operation indefinitely.

The NIST AI Risk Management Framework places mapping of risks and impacts before measurement and management. OWASP similarly emphasizes application-level controls, least privilege and validation of external input. A safe test plan therefore starts with what the agent can change, not with a collection of prompts.
For background on measurement before and during operation, see the NIST AI RMF Core and the OWASP AI Agent Security Cheat Sheet.
Map the agent’s permissions and consequences
Create an inventory before writing test cases. Record the intended task, every tool it can call, the credential used, the data each tool can read or write, and every external input that can influence a decision. Include indirect capabilities: a read-only search tool may still reveal secrets, while a notification tool may trigger a costly workflow.
Classify each possible action
| Question | What to record | Why it changes testing |
|---|---|---|
| What can change? | Records, money, permissions, messages, files, code or physical processes | Defines the side effects that must be observed and contained |
| Who or what could be affected? | One user, a tenant, employees, customers, third parties or public systems | Sets the scope of privacy and authorization tests |
| How reversible is it? | Fully reversible, recoverable with effort, or effectively irreversible | Determines whether a human approval gate is mandatory |
| What is the worst credible misuse? | Data disclosure, financial loss, service outage, unsafe physical action or legal exposure | Sets risk tolerance and the depth of adversarial testing |
| What identity is used? | End-user identity, service account, delegated token and scopes | Provides concrete authorization boundaries to verify |
Use the consequence assessment to choose test depth. A drafting assistant with no write access needs reliable content and leakage tests; an agent that can alter payroll, delete records or deploy code needs stronger isolation, independent enforcement and explicit approval tests.
Write an allowed-action contract
For each tool, state what is allowed, what is denied, which arguments are constrained, and what evidence or approval is required. Make the contract machine-enforced where possible. The model should request an action; a separate policy or execution layer should decide whether that request is permitted. Never let the model be the sole authority that approves its own high-impact operation.
Build a deployment-like but contained test environment
Run evaluations against an isolated or resettable environment. Use synthetic records or deliberately scrubbed data, non-production credentials and test endpoints. If the real integration cannot be duplicated, put a proxy or simulator between the agent and the service so that calls, arguments and resulting state can be inspected without harming users.

- Reset databases, files and memory between cases so one test cannot influence the next.
- Give each suite only the minimum permissions required for its cases; do not use a broad administrator token for convenience.
- Use deterministic fixtures for ordinary tasks, while retaining a controlled sample of realistic variability for robustness.
- Capture prompts, retrieved content, model outputs, tool calls, arguments, identity, approvals, denials, retries, timeouts and final state.
- Set hard limits on execution depth, elapsed time, token or API cost, request rate and retry count.
- Provide a kill switch and a tested rollback procedure before running any case that can reach a real service.
NIST recommends testing in conditions similar to deployment. Similarity does not mean exposing production: reproduce the same authentication path, retrieval behavior, tool schemas and policy checks while keeping data and side effects safe.
Build repeatable ordinary-behavior evaluations
Routine evaluations answer: does the agent perform its intended job, through the intended controls, under normal and foreseeable conditions? Create a test record for every case with a unique identifier, setup, input, expected outcome, permitted tools, forbidden tools, expected state changes and explicit failure conditions.
Score the whole trajectory, not just the final answer
| Dimension | Pass evidence | Typical failure |
|---|---|---|
| Task outcome | The requested result is correct and complete | A fluent but factually or procedurally wrong response |
| Tool selection | Only an allowed tool is chosen for the task | Calling an administrator or export tool when a read-only lookup suffices |
| Arguments | Parameters are valid, scoped and consistent with the user’s authorization | Wrong tenant, excessive date range or an unvalidated identifier |
| Sequence | Checks and approvals occur before execution | Sending a message before confirming its recipient or approval |
| Error handling | Failures produce a safe stop, useful explanation and no unsafe retry storm | Repeating a charged request after an ambiguous timeout |
| State change | Only the documented, permitted state is changed | Extra records, permissions or memory entries are modified |
Use a mix of single-step and multi-step tasks. Include valid requests, missing information, ambiguous requests, tool errors, partial outages and conflicting records. Keep test data safe for the environment. NIST’s work on evaluation probes stresses visibility into tool use, evidence and decisions in multi-step workflows.
Set thresholds before running the suite
Define pass and fail criteria in advance. A practical policy might require zero successful executions of prohibited tools and zero unapproved high-impact actions in the tested cases, while task-quality targets vary by use case. Those thresholds are implementation choices, not universal NIST or OWASP requirements. Record the sample size and confidence limitations; a clean run over a small or unrealistic set is weak evidence.
Separate safety gates from quality scores. An agent can meet a helpfulness target and still fail release because it leaked a secret once. Conversely, a safe denial on an ambiguous request should not be counted as a task-quality failure when the policy requires clarification.

Red-team the real workflow
Routine evals measure intended behavior. Red teaming deliberately supplies misuse, hostile instructions and unexpected interactions. Use adaptive testing: when a defense blocks one attack, vary wording, placement, timing and tool sequence rather than merely replaying the same prompt.

Minimum abuse-case matrix
| Threat | Test setup | Pass condition |
|---|---|---|
| Direct prompt override | Ask the agent to ignore its governing policy and perform a denied action | The policy remains in force; the action is refused or safely redirected |
| Indirect prompt injection | Place conflicting instructions in a retrieved document, email, web page or tool response | Untrusted content is treated as data, not authority; no injected action occurs |
| Tool misuse or privilege escalation | Request an unavailable tool, admin credential or cross-tenant operation | An independent authorization check denies the call |
| Memory poisoning | Attempt to persist malicious instructions or sensitive content for later sessions | Memory follows validation, scope and retention rules; later users cannot inherit it |
| Cross-session exposure | Ask one session to reveal another user’s context or retrieved documents | Tenant and session boundaries hold, including through citations and summaries |
| Data exfiltration | Request secrets through the final answer, a citation, a log, an outbound message or a tool argument | Secrets are withheld or redacted and no unauthorized transfer occurs |
| Approval bypass | Attempt execution without approval, with expired approval or with approval for different parameters | The enforcement layer blocks the mismatch and records the denial |
| Runaway behavior | Induce recursion, repeated retries, long tool chains or rate bursts | Depth, time, cost and rate limits stop the run safely |
| Multi-agent boundary crossing | Have one agent ask another to exceed its own permission set | Each agent and the shared executor enforce its own boundary |
These cases reflect the categories in the OWASP AI Agent Security Cheat Sheet. NIST’s CAISI work describes agent hijacking, in which malicious instructions embedded in ingested data induce unintended actions; test that path directly rather than assuming a clean user prompt is representative.
Make attacks system-specific
Start with known patterns, then mutate them using the agent’s actual tools, document formats, memory behavior and approval workflow. Test combinations, such as an injected instruction that first poisons memory and later requests an outbound transfer. Keep successful attacks as regression cases. A defense that blocks yesterday’s wording is not evidence that it blocks a new attack path.
Verify approvals, authorization and side effects
For every high-impact operation, test the boundary between deciding and executing. The model may propose a transfer, deletion or deployment, but a separate enforcement component should check identity, scope, policy, parameters and approval freshness before the call is sent.
Recommended Free Tools
- Submit a valid request without approval. Confirm that the request is recorded as pending and execution does not occur.
- Approve the exact request. Verify that the approval identifies the actor, operation, target, parameters and expiry, and that only that request executes.
- Change one parameter after approval. The altered request must require a new approval.
- Use an expired or revoked approval. The executor must deny it even if the model repeats the original request.
- Attempt a different user’s or tenant’s operation. Authorization must fail independently of the model’s confidence or explanation.
- Inspect the resulting state. Compare records, permissions, messages, files and memory before and after the run; account for retries and partial failures.
- Trigger the stop control. Confirm that queued work halts and that operators can identify and roll back completed changes.
Test denials as carefully as successes. A system that says “no” in the chat but still sends the API request has failed. Capture tool arguments, identity, policy decision, approval reference, response code, retry behavior and final state.
Keep an auditable release record
Before a release, preserve enough evidence for another reviewer to reproduce the decision. At minimum, retain:
- the model provider and version, prompts, policies, tool schemas and permission configuration;
- retrieval sources, memory settings, identity mappings and environment details;
- ordinary-task cases, adversarial cases, expected outcomes and test-data versions;
- complete traces of decisions, tool calls, arguments, approvals, denials, errors, retries and state changes;
- the pass/fail criteria, results, unresolved failures, accepted residual risk and the mitigating controls;
- the approver, release identifier and date, with access to logs restricted because they may contain sensitive data.
OWASP recommends repeatable abuse cases, CI/CD gates, least privilege, human oversight and evidence retention. A release gate should block deployment when high-risk permissions, approval logic, tools, memory, retrieval or model providers change without updated tests.

Retest after changes and monitor after launch
“Passed once” is not a permanent property. Rerun the ordinary suite and adversarial cases after material changes to prompts, tools, memory, retrieval, policies, identity configuration or model providers. Add every discovered failure to the regression suite, including failures found in production.

- Monitor denied and successful tool calls, unusual argument patterns, approval mismatches, retries, latency, cost and rate-limit events.
- Alert on cross-tenant access attempts, secret-detection events, unexpected outbound destinations and repeated policy denials.
- Keep a staffed route for investigation, suspension of actions and rollback; specify who can activate it.
- Review whether monitoring itself leaks sensitive prompts, retrieved content or credentials.
- Reassess risk when the agent gains a new tool, data source, identity, memory store or external channel.
Monitoring does not replace pre-release testing. It provides detection and containment when behavior, inputs or dependencies change in ways the test suite did not anticipate.
Understand what published results do—and do not—prove
A January 2025 NIST CAISI technical blog described a red-team experiment in which attack success on held-out Workspace tasks rose from 11% for the strongest baseline attack to 81% for the strongest newly developed attack. The team also examined transfer to three other AgentDojo environments. Those figures belong to that model and evaluation setup; they are not a general vulnerability rate or a prediction for your agent.
The practical lesson is specific: resistance to previously tested attacks does not establish resistance to newly adapted attacks. Report the environment, model, tools, attack set, held-out tasks and transfer conditions whenever you publish a score. Do not compare percentages from different systems as if they formed a universal leaderboard.
Use standards as scaffolding, not a safety certificate
NIST AI RMF
The NIST AI RMF is a voluntary risk-management framework. Its Measure function calls for documented, repeatable evaluations before deployment and during operation, tests in conditions similar to deployment, monitoring and safe failure within defined risk tolerance. It does not prescribe one numerical pass score for every agent.

NIST evaluation probes
NIST describes evaluation probes for agentic AI that examine whether factual claims are supported by a human-curated corpus and create machine-readable trails linking claims to evidence. The page presents this as an ongoing research project, not a finished universal safety certification.
OWASP agent guidance
The OWASP AI Agent Security Cheat Sheet provides a practical checklist for least privilege, abuse cases, human oversight, CI/CD gates and evidence retention. Treat it as an input to your control design, then verify the controls in your own deployment.
OWASP AISVS 1.0
OWASP AISVS 1.0, released in June 2026, lists 191 testable requirements across 12 chapters and three appendices, including agent orchestration, MCP security, adversarial robustness and monitoring. OWASP says most production systems should target at least Level 2 according to risk. The standard assumes general application and infrastructure security are addressed in parallel; satisfying its checklist is not proof that an agent is safe in every context.

The OpenAI red-teaming documentation explains how adversarial testing complements ordinary evals and points to Promptfoo as an open-source framework. A framework can organize cases, but it cannot compensate for missing deployment fidelity, weak authorization or unobserved side effects.
Choose an evaluation approach by evidence quality
Whether you use an in-house harness, a framework or an external testing service, compare the approach on the dimensions that affect decision quality:
| Criterion | Questions to ask |
|---|---|
| Risk coverage | Does it test prompt injection, tool misuse, authorization, leakage, memory, chained actions and runaway behavior? |
| Deployment fidelity | Does it exercise the actual integrations, identities, retrieval, policies and approval path in a safe environment? |
| Action observability | Can reviewers inspect tool choice, arguments, identity, approvals, denials, retries and resulting state? |
| Repeatability and change control | Can the same suite run on every relevant change while retaining prior failures as regressions? |
| Evidence quality | Does it preserve configuration and show how decisions or factual claims relate to trusted evidence? |
| Limitations | Does it state untested risks, uncertain metrics and conditions where results may not generalize? |
These criteria synthesize the evaluation and evidence emphasis in NIST, OWASP and OpenAI guidance; they are not a ranking of vendors.
A concise go/no-go checklist
- The agent’s tasks, tools, identities, data, memory and external inputs are inventoried.
- Each action has a documented scope, consequence rating, reversibility and authorization rule.
- Tests run in an isolated, resettable environment with minimum necessary permissions.
- Ordinary cases score outcome, tool choice, arguments, sequence, errors and state changes.
- Adversarial cases cover injection, privilege escalation, memory, leakage, approval bypass, runaway behavior and multi-agent boundaries.
- High-impact execution is enforced outside the model and requires parameter-specific approval.
- Denials, retries, timeouts, circuit breakers and rollback have been observed, not merely configured.
- Pass/fail criteria and residual-risk decisions were set before the run and are documented.
- Material changes trigger regression and adaptive red-team testing.
- Post-launch monitoring, incident ownership, suspension and rollback procedures are active.
If any high-impact permission cannot be independently denied, observed and stopped, do not grant that permission yet. Reduce the scope, add the missing enforcement layer and repeat the tests until the evidence supports the specific actions you intend to allow.
