A safe Node.js upload pipeline should reject oversized or malformed requests before doing expensive work, verify and normalize image bytes before trusting them, moderate the image and its caption before publication, and keep image-file compression separate from HTTP response compression. Treat every limit and moderation result as a gate with a defined outcome—not as a setting the client can override.
What the upload pipeline should do
Process uploads in a fixed order so untrusted data does not reach image decoders or moderation services before it passes basic resource and format checks:
- Apply ingress and multipart limits. Cap the request body, file size, number of files, text fields, and field nesting before buffering or processing the upload.
- Validate the file. Treat the filename and declared MIME type as hints only. Check the actual bytes and decode the image with a maintained image library; reject unsupported formats and images above your configured dimensions or pixel count.
- Normalize within resource limits. Resize and rewrite accepted images to the formats and dimensions your product supports, with bounded processing and output size.
- Moderate the caption and image. Send the content through the moderation route selected for your policy. Allow, reject, or hold it for review based on category results and your rules.
- Publish only after all gates pass. Keep rejected and review-pending content out of public storage or delivery paths.
These are distinct controls. An upload-size limit protects ingress and processing capacity; a decoded-pixel limit protects image processing; normalization controls the stored derivative; moderation evaluates content; and HTTP compression affects responses sent over the network.

Set upload limits before buffering
Enforce a request-body ceiling at the proxy or server boundary, then configure multipart limits for the application. Multer’s guidance notes that specifying limits can help protect a site against denial-of-service attacks. Do not rely on a browser-side limit: clients can send requests without using your interface.

At minimum, define limits for:
- Maximum request-body and individual file size.
- Maximum number of files per request.
- Maximum number and size of text fields.
- Maximum field nesting, where the multipart parser supports it.
Choose values based on the product’s actual upload requirements and service capacity; there is no universal safe threshold established here. When a request exceeds a limit, return a controlled client error and avoid exposing parser internals, stack traces, or server paths.
Validate image bytes and decoded dimensions
A filename ending in .jpg or a Content-Type: image/jpeg header does not prove that the uploaded bytes are a JPEG—or that they are a safe image to process. Compare the claimed type with the file’s actual content, including its signature where applicable, and decode the image using a maintained library. Allow only the formats the application needs.
Check dimensions or total decoded pixels after decoding and before expensive downstream work. A relatively small compressed file can expand into a very large image in memory. Reject images above the application’s configured dimension or pixel ceiling, and account for memory and processing cost when choosing that ceiling. OWASP ASVS calls for matching content to the expected type, image rewriting or specialized validation, quotas, and rejecting images above the maximum pixel size.

Do not use the original client filename as a storage path or as a trusted identifier. Generate server-controlled names and retain only metadata the application needs.
Normalize accepted images with bounded work
Use an image-processing stage to resize and rewrite accepted inputs into controlled derivatives rather than serving arbitrary originals. Sharp supports resizing and conversion to web-friendly JPEG, PNG, WebP, GIF, and AVIF, with buffer, stream, and filesystem input/output options.
Set application-specific bounds for decoded pixels, output dimensions, and output bytes, and ensure processing cannot run without limits. Pick these values for your product’s image quality needs and available capacity; the cited documentation does not prescribe universal limits. Keep the original only if the product has a defined need for it and the original is stored and served under appropriate controls.

Moderate captions and images before publication
One request for text and image
OpenAI documents omni-moderation-latest for text and image inputs. Its TypeScript reference shows client.moderations.create accepting a text item and an image_url item. The response includes a flagged result, categories, and category scores. The documentation lists a 20 MB image limit for this moderation path; treat that as a provider-side constraint, not as a recommended upload limit for your own API.
Send the caption and the normalized image you intend to publish, where practical, so the moderation decision corresponds to the content users will see. Interpret the returned categories and scores through your own policy. A flag or score is not a universal rule for publication: define which outcomes are allowed, rejected, or routed to human review.
Image-only moderation alternative
AWS Rekognition’s DetectModerationLabels is a documented synchronous route for still images. Its procedure accepts JPEG or PNG bytes or an S3 object and returns hierarchical labels with confidence values. It addresses image moderation, not the caption, so an architecture using it needs a separately selected text-moderation route.

Any confidence threshold is a policy choice. A stricter threshold may reduce false positives while allowing more content through; a looser threshold may catch more concerning content while sending more acceptable content for rejection or review. Calibrate thresholds against your own policy and review capacity rather than assuming a provider score is a final decision.
Keep child-safety handling separate
OpenAI says not to send known or suspected child sexual abuse material (CSAM) to its Moderation API and says that the API is not designed for CSAM detection or handling. A production service needs a dedicated child-safety safeguard and incident-response path; do not treat the general moderation request as that control.

| Gate | Pass outcome | Failure outcome |
|---|---|---|
| Request and multipart limits | Continue to file validation. | Reject with a controlled size, count, or field-limit error. |
| Type, decode, and dimension checks | Continue to normalization. | Reject unsupported, malformed, or over-limit content. |
| Normalization | Continue with the bounded derivative. | Reject or mark processing failed; do not publish the unprocessed upload as a fallback. |
| Caption and image moderation | Publish if the policy allows. | Reject or hold for review according to the policy outcome. |
If moderation or image processing is unavailable, define the behavior deliberately. For a system that promises pre-publication moderation, the safe default is to keep the item unpublished until the required checks succeed. If you support review, store a private pending state and expose it only to authorized reviewers. Record the gate outcome and the policy decision needed for troubleshooting, while avoiding unnecessary retention of sensitive content.
Do not confuse image compression with HTTP compression
Sharp’s image resizing and re-encoding change an image asset: its dimensions, format, or encoded size. Express’s compression middleware instead compresses eligible HTTP response bodies and, by default, selects compressible responses by content type. It supports deflate, gzip, and Brotli.
These mechanisms solve different problems. Re-encode uploaded images when producing controlled image assets. Use HTTP response compression for eligible responses, and exclude already-compressed or binary bodies where appropriate. Measure CPU use, latency, and payload effects with your own traffic before enabling or tuning response compression broadly.

Compare services on the requirements your implementation must meet, rather than selecting by a score field alone:
- Modalities: whether text and image can be evaluated in one request, or require separate routes.
- Input constraints: accepted image formats and maximum image size.
- Decision data: category taxonomy and whether outputs are flags, category results, confidence values, or scores.
- Workflow: synchronous request behavior and how your application handles pending review or service errors.
- Operations: latency, privacy and retention terms, service-region fit, cost, and escalation requirements.
The documented paths described here establish that OpenAI supports text and image inputs in one moderation request, while Rekognition provides synchronous image labels and confidence values for JPEG/PNG or S3 inputs. They do not establish comparative accuracy, current pricing, data-retention terms, or regional suitability. Verify those requirements against current provider documentation and your deployment context before choosing a service.
