An effective AI incident-response tabletop is a decision rehearsal for one real workflow—not a presentation about artificial intelligence. Choose the workflow, define the harm or operational consequence you need to control, assign authority to the people who would respond, and introduce new facts in timed injects. Participants should practice detection, containment, evidence preservation, provider escalation, fallback operations, and communications. Finish with owned corrective actions and repeat the exercise after material changes.
This approach follows the voluntary, use-case-agnostic NIST AI Risk Management Framework (AI RMF 1.0), the NIST Artificial Intelligence Risk Management Framework: Generative Artificial Intelligence Profile (NIST AI 600-1, published July 26, 2024), and the objectives described for CISA’s June 2024 AI incident tabletop scenario. None of these sources imposes one universal agenda, duration, score, or scenario.
What the exercise must prove
Write the intended outcome before writing the scenario. A useful exercise proves whether your organization can make and execute a small number of consequential decisions under pressure:
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minute- Who can declare an AI-related incident and become incident lead?
- Who can pause, constrain, or remove the AI-assisted workflow from service?
- What records, prompts, outputs, configuration changes, and provider information must be preserved?
- When does the organization switch to a manual or alternate process?
- Who contacts the AI provider, and what contractual support path is used?
- When do legal, privacy, security, communications, leadership, customers, regulators, or other affected people need to be involved?
NIST AI 600-1 recommends rehearsing third-party generative-AI incident-response plans at a regular cadence, learning from retrospectives, and performing post-mortems and after-action assessments. The exercise is successful when it exposes unclear authority, missing information, unrealistic fallback assumptions, or communication delays that can be corrected while no live incident is occurring.

Choose one real AI-assisted workflow
Keep the first exercise narrow enough that participants can describe the system and make decisions without guessing. Examples include drafting customer communications, summarizing support cases, processing documents, or assisting an internal decision. These are exercise examples, not claims about incident frequency.
Document the workflow before the session
Create a one-page system description that answers:
- Purpose: What business task does the workflow support?
- AI function: Does the system generate, classify, summarize, extract, recommend, or take an action?
- Inputs: What data enters the model or third-party service? Identify personal, confidential, regulated, proprietary, or customer information.
- Outputs: Who sees the result, and can it trigger a decision, communication, transaction, access change, or other downstream action?
- Dependencies: Which model, API, identity system, data store, orchestration layer, monitoring service, or provider contract is involved?
- Human control: Which person reviews output, and can that review be bypassed by automation or operational pressure?
- Safe state: What does the workflow look like when it is paused, restricted, or operating without AI?
NIST recommends documenting intended use, context, limitations, possible impacts, third-party risks, dependencies, and fallbacks. If the team cannot answer these questions, that documentation gap is itself an exercise finding rather than something to hide.
Describe consequences, not just technical symptoms
“The model produced an incorrect answer” is not a sufficient exercise objective. State what could happen because of the answer: a customer could receive incorrect instructions, a confidential record could be exposed, an employee could make a flawed decision, or a critical process could stop when the provider is unavailable. Note whether harm is reversible, how quickly it can spread, and who might be affected.
Quick wins for a faster PC:
Repair Windows errors before they cause bigger problemsFix Now →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Clear out junk files and repair common Windows errorsFree Scan →Set two or three observable objectives
Objectives should be testable during the session. Suitable objectives include:
- Identify the person authorized to stop or limit the workflow and the information that person needs.
- Decide which evidence must be preserved, where it is stored, and who requests provider records.
- Determine the trigger for switching to a manual process and verify that staffing and quality checks are credible.
- Agree when to involve the AI provider, security, legal or privacy counsel, executives, communications, or affected stakeholders.
Avoid objectives such as “raise awareness” unless you define observable evidence, such as every participant being able to name the incident lead and fallback trigger. Two or three objectives are usually enough for one session; adding more can turn the exercise into a checklist race.
Assign response ownership and invite the right people
Invite people who would actually perform or authorize the response. A large audience is less useful than clear handoffs and decision rights.
| Role | Exercise responsibility |
|---|---|
| Workflow owner and operators | Explain normal processing, identify affected cases, and describe practical operating constraints. |
| Incident response or security lead | Coordinate triage, declare or classify the incident, direct containment, and maintain the incident record. |
| IT, platform, or engineering staff | Restrict access, disable integrations, preserve technical evidence, and restore or reconfigure services. |
| AI, model, or data owner | Explain model versions, prompts, data flows, evaluation limits, and known changes or failure modes. |
| Business continuity or operations | Authorize and staff manual or alternate processing and track service-level consequences. |
| Legal and privacy | Assess notification, confidentiality, privacy, contractual, retention, and jurisdiction-specific obligations. |
| Communications | Prepare internal and external messages, approvals, and updates that separate facts from uncertainty. |
| Leadership | Resolve risk and resource trade-offs and approve business-impact decisions when authority requires it. |
| Provider contact | Use the contractual escalation path, request relevant records, and coordinate service status or remediation. |
Define who can declare an incident, who can pause the workflow, who owns business continuity, who preserves evidence, who contacts the provider, and who approves communications. NIST recommends defining ownership and communicating third-party generative-AI incident-response plans to relevant AI actors. If a role is absent, name an interim owner and record the governance gap for resolution.

- Sensitive data appears in an AI output or is sent to an unintended destination.
- Harmful, unreliable, or materially incorrect generated content is used downstream.
- The provider is compromised, unexpectedly changed, unavailable, or unable to explain an output.
- A model, prompt, retrieval source, policy, or integration changes without the workflow owner’s expectation.
These are plausible prompts for rehearsal, not rankings of incident likelihood. NIST discusses privacy, harmful-content, third-party, dependency, and fallback risks; CISA’s June 2024 scenario document describes an objective of improving AI incident-response plans, information sharing, and organizational resilience during and after a significant AI incident.
Example inject sequence
| Stage | Information revealed | Questions participants must answer |
|---|---|---|
| Opening | A team flags an unexpected AI-generated result in a live workflow. | What is the initial triage path? Who is incident lead? What is the first safe action? |
| Inject 1 | The result has already informed a downstream action. | Who assesses scope and affected people? Who can pause or limit processing? What cases need review? |
| Inject 2 | Logs or provider records may be required, but the provider’s status is unclear. | What evidence is preserved locally? Who requests provider support? Which contractual contact and escalation time apply? |
| Inject 3 | The AI service is unavailable or cannot be trusted. | Who authorizes fallback? Is manual processing staffed? What quality and approval checks prevent a second failure? |
| Inject 4 | Internal users and potentially affected people ask what happened. | Who approves messages? What is known, unknown, and still being investigated? Which audiences need separate updates? |
Do not invent provider behavior, impact numbers, or legal deadlines merely to make the story dramatic. If the exercise needs an unknown fact, label it as unknown and test how participants obtain it.
Prepare the exercise materials and logistics
Prepare a facilitator packet and a participant packet. The participant packet should contain only information available at the start; the facilitator packet can include inject timing and expected observations.
Participant materials
- A workflow diagram showing inputs, AI service, human review, outputs, and downstream systems.
- Current incident-response, business-continuity, privacy, retention, and communications procedures.
- Provider name, service identifier, contract owner, support channel, escalation route, and known service commitments.
- Contact list for incident, operations, legal/privacy, communications, leadership, and technical owners.
- A decision log template with time, decision, owner, evidence, assumptions, and follow-up.
Facilitator controls
- A one-paragraph opening and four or five injects, each with a release time or decision trigger.
- A list of questions that test authority, evidence, fallback, third-party escalation, and communication.
- A visible clock and a method for capturing unresolved disagreements.
- A separate note-taking channel for decisions, assumptions, delays, missing contacts, and conflicting procedures.
A facilitator presents facts and keeps time; a note-taker records what participants actually decide. The facilitator should not solve the scenario or quietly supply missing policy. A no-fault tone encourages people to expose weaknesses. When participants disagree about whether the workflow can be paused, which records may be retained, or who may notify a provider or regulator, record the disagreement as a finding instead of improvising policy.
Run the tabletop for decisions, not presentations
The official sources establish the need to rehearse, communicate, and improve; they do not prescribe a mandatory duration. The following run-of-show is a practical format that can be shortened or expanded.
1. Open with rules and objectives
Explain that the session is a discussion exercise, not a live production test. State the workflow, objectives, assumptions, confidentiality expectations, and decision log. Tell participants to answer in operational terms: what they would do, who would do it, what information they need, and what they would communicate.
2. Present the opening scenario
Give participants the minimum facts needed for initial triage. Ask the incident lead to state the first action, the person responsible, the evidence to preserve, and the condition that would trigger containment. Do not allow a job title to substitute for a documented authority.
3. Release each inject only after a decision
Pause after every inject. Ask who owns the next decision, which systems or records are affected, what assumption could be wrong, and whether the current plan changes. If the group jumps directly to restoration, ask how it will determine that the output, model, provider, or data path is trustworthy enough to resume.
4. Test the fallback explicitly
Make participants name the manual or alternate process, staff it, define intake and quality checks, and decide how long it can operate. A fallback that exists only in a document is not a credible fallback. Identify any queue, service-level, accessibility, safety, or privacy consequence created by switching away from AI.

5. Test information sharing
Ask the group to draft a short internal update and an audience-appropriate external holding message. Each should distinguish confirmed facts, suspected cause, actions taken, affected scope, user instructions, and the next update point. CISA’s stated tabletop objective includes information sharing and organizational resilience, so communications should be exercised alongside technical containment.

6. Stop when objectives are answered
Do not prolong the scenario simply to reach a planned endpoint. End when participants have tested the objectives or when a critical unresolved dependency is clear. Capture the point at which the group could not proceed and why.
Practice third-party escalation and evidence preservation
AI-assisted workflows often depend on a service outside the organization’s direct control. NIST recommends documenting dependencies and fallbacks and checking whether provider contracts identify incident responsibilities, notification, response times, and critical support availability.
Provider escalation questions
- Which contract, service identifier, and support channel apply?
- Who is authorized to contact the provider and share incident details?
- What information does the provider need to investigate?
- What notification, response-time, maintenance, or service-status commitments exist?
- Can the provider preserve relevant logs, prompts, outputs, model versions, access records, or change history?
- What happens if the normal support channel is unavailable?
Do not assume a provider can retrieve every prompt or output, that logs are retained for a particular period, or that a service status page explains a customer-specific failure. Test the actual contact route and record any contractual or technical uncertainty.
Evidence checklist
- Record the original input, generated output, user, timestamp, model or endpoint identifier, and downstream action where permitted.
- Preserve relevant prompts, retrieval material, policy configuration, filters, approvals, and human edits.
- Capture access logs, application logs, error messages, deployment records, model or prompt changes, and provider correspondence.
- Record what was not available and why, including retention limits or privacy restrictions.
- Protect evidence from alteration and restrict access according to security, privacy, and legal requirements.
Legal and privacy teams should determine how evidence preservation interacts with retention, confidentiality, data-subject rights, breach analysis, and jurisdiction-specific reporting duties. NIST identifies legal and privacy alignment as part of incident planning; this article is not jurisdiction-specific legal advice.
Make fallback and recovery operational
A fallback decision should include a trigger, authority, procedure, staffing model, quality control, and return-to-service criteria.
Define the fallback trigger
Examples include confirmed exposure of sensitive data, repeated unreliable output above a defined tolerance, inability to verify provider integrity, loss of required logs, or provider unavailability beyond an operational threshold. The organization must choose thresholds appropriate to its workflow; NIST does not prescribe one universal value.
Define manual operating controls
- Who receives work while the AI path is paused?
- How are urgent cases prioritized?
- Who performs a second review for high-impact decisions?
- How are manual decisions recorded for later reconciliation?
- What capacity or service-level degradation is acceptable?
Define recovery gates
Resumption should require evidence that the triggering condition is understood or controlled, affected data and outputs have been assessed, required fixes or compensating controls are in place, monitoring is active, and an accountable owner approves return. If the cause remains uncertain, use a constrained mode with narrower data, human approval, lower privileges, or limited volume rather than treating “service restored” as “risk resolved.”
Debrief immediately and turn findings into work
Hold a hotwash immediately after the final inject while details are fresh. Ask:

- What worked as intended?
- What was unclear or delayed?
- Which information or contact was missing?
- Which decision had no clearly authorized owner?
- Was the fallback staffed, controlled, and sustainable?
- Could the organization explain the event without overstating certainty?
Sort observations into five categories: plan and process; people and authority; technology and evidence; third party; and communications. Separate an observation from a corrective action. “The provider contact was missing” is an observation; “the platform owner will validate and publish the contractual escalation path by June 30” is an owned action.
| Finding record | Required entry |
|---|---|
| Issue | What failed, was missing, or caused delay? |
| Impact | Which decision, control, service, or stakeholder was affected? |
| Action | What specific change will close the gap? |
| Accountable owner | One person responsible for completion, not merely a department. |
| Due date | A date appropriate to the risk and operational dependency. |
| Verification | How completion will be tested, such as a document review, contact check, technical test, or repeat inject. |
Update the relevant incident plan, provider record, contact list, workflow documentation, fallback procedure, and communication templates. Track near misses and errors as well as realized harm; NIST AI 600-1 calls for learning from retrospectives, post-mortems, and after-action assessments.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Choose the next scenario using explicit criteria
If several scenarios are available, compare them rather than selecting the most sensational one. The following axes synthesize NIST’s focus on impact, reversibility, third-party dependency, fallback, evidence, and ownership; they are a practical comparison method, not a formal NIST scoring model.
| Axis | Questions to ask | Why it matters |
|---|---|---|
| Impact and reversibility | Could the event affect safety, privacy, finances, rights, trust, or critical operations? Can decisions be undone? | High-consequence or irreversible actions deserve earlier rehearsal. |
| Detection speed | Would a person, automated control, customer, or provider notice first? How long could the issue remain hidden? | Tests monitoring and escalation timing. |
| Third-party dependency | How much investigation, containment, or recovery depends on an external provider? | Exposes contract, contact, and information-sharing gaps. |
| Fallback credibility | Can work continue manually or through an alternate service? Is capacity known? | Distinguishes a documented fallback from an executable one. |
| Evidence availability | Which logs, prompts, outputs, configurations, and provider records exist? | Tests investigation and preservation before records disappear. |
| Coordination load | How many teams, executives, customers, regulators, or partners must align? | Exercises authority and communication handoffs. |
Repeat after material change and on a risk-based cadence
NIST recommends regular rehearsal but does not set a universal interval. Choose a cadence based on impact, change, and risk tolerance. Repeat sooner after a material change to the model, provider, data, prompt or retrieval layer, automation, workflow purpose, staffing, contract, or incident-response organization. A low-impact workflow may use discussion-based reviews, while a high-consequence workflow may need more frequent, scenario-specific rehearsals and technical validation.

Vary the scenario so participants do not memorize one answer. One exercise can emphasize privacy and evidence; another can test provider unavailability and manual capacity; another can focus on harmful content, customer communication, or a silent configuration change. Keep the workflow and decision rights current in every version.
Common failure modes and corrections
Making the exercise a model-accuracy debate
Problem: Participants argue about whether an output is technically “hallucinated” while no one decides what to do.
Correction: Ask what consequence occurred, who can contain it, what evidence is needed, and which users or systems are affected. Technical diagnosis can proceed in parallel with operational control.
Inviting only security staff
Problem: Security identifies a risk but cannot explain the workflow, staff fallback operations, or approve customer messaging.

Correction: Include workflow, operations, AI or data, legal/privacy, communications, leadership, and provider roles as appropriate.

Assuming the provider owns the incident
Problem: The organization waits for a vendor response before preserving local evidence or protecting affected people.
Correction: Define local containment and evidence actions first, then test the contractual provider path and escalation alternative.
Calling a document a fallback
Problem: The manual procedure has no staff, queue limits, approval rules, or quality check.
Correction: Make participants allocate people, prioritize work, record decisions, and state how long the process can operate.
Writing communications after the exercise
Problem: Technical response succeeds but internal users or affected people receive contradictory or overconfident explanations.
Correction: Include a communications inject and require messages to distinguish confirmed facts, uncertainty, actions, and next updates.
Ending without owners and dates
Problem: Everyone agrees the plan needs improvement, but no change happens.
Correction: Assign one accountable owner, a due date, a verification method, and the plan or control that must be updated.
What good looks like at the end
By the close of a useful tabletop, participants can point to the workflow being exercised, name the incident lead and decision authorities, describe the first containment action, identify the evidence to preserve, use the provider escalation route, activate a credible fallback, and explain how communications will be approved. The organization also has a dated action register that changes its plans and is tested again.
NIST AI RMF 1.0 and its Playbook are voluntary guidance rather than mandatory compliance checklists. Apply the exercise to your own systems, contracts, locations, and legal obligations. NIST AI 600-1 specifically supports rehearsing third-party generative-AI response plans, conducting post-mortems and after-action assessments, and tracking errors and near misses; the value comes from converting those recommendations into decisions your people can actually make.
The Bottom Line
Run the tabletop around one real AI-assisted workflow, its consequences, and the decisions that control them. Rehearse ownership, containment, evidence, provider escalation, fallback, and communication; then assign and verify corrective actions. Repeat after material changes and on a cadence proportionate to the workflow’s impact and uncertainty.
