Do not sign an AI-generated clinical note until you have reviewed it as a draft. Compare it with the encounter and relevant chart, correct additions and omissions, verify that plans are not written as completed actions, and confirm that the assessment reflects your own clinical judgment. The American Psychological Association’s preliminary guidance says clinicians must review AI-generated documentation for accuracy and edit it as needed before signing; it also places responsibility for documentation accuracy and clinical decisions with the clinician. The guidance is written for psychologists, so apply it alongside your profession’s rules, institutional policy and local law. APA guidance (created December 2025; updated January 2026)

An AI note is a draft, not a clinical record

An ambient scribe or dictation system turns a conversation, dictated summary or other input into proposed documentation. Its fluent prose can save typing, but fluency is not evidence that every statement is true. Until you have checked and edited the note, it is a machine-produced draft. Signing makes the final record your documentation, so the review is a clinical and professional act rather than a proofreading formality.

The APA guide describes itself as preliminary and non-exhaustive. It is a useful framework for review, but it does not replace specialty standards, payer rules, employer policy, record-retention requirements or jurisdiction-specific law. A practice should define those local requirements before deploying a scribe and should document who may edit, sign, amend or correct a note.

Why plausible wording can conceal a serious error

Large language models are optimized to produce coherent language. They can therefore make an unsupported statement sound authoritative, merge separate parts of a conversation, or change a future intention into a completed event. The American Medical Association reported two illustrative failures: one system changed a planned prostate examination into an examination that had occurred; another converted a discussion involving a patient’s hands, feet and mouth into a diagnosis of hand, foot and mouth disease. These are not merely spelling mistakes—the first changes the record of care and the second invents a clinical conclusion. AMA, March 18, 2024

Review for both commission (something added or transformed) and omission (something important left out). A note can be dangerous even when every sentence sounds reasonable if it omits a medication change, a safety discussion, a relevant negative finding or the patient’s refusal. Treat the draft as a set of claims that must be reconciled with what happened, not as a transcript that deserves automatic trust.

Prepare the review before you read the prose

Confirm the patient, encounter and note type

Open the intended patient and the correct date of service before evaluating content. Check that the draft is attached to the right appointment, clinician and location, and that it uses the intended template (for example, an office visit rather than a telephone encounter). A polished note in the wrong chart is a critical safety event; stop, quarantine it according to local procedure and notify the appropriate health-information or safety contact.

Gather the material you are allowed to use

Use the encounter itself, your contemporaneous memory or handwritten prompts, the patient’s statements, examination results, orders and relevant prior-chart context. Do not ask the model to fill gaps from unrelated records merely to make the note complete. Review access permissions before opening recordings, transcripts or vendor dashboards, and follow your organization’s rules for storing or deleting those artifacts.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Know what the system actually heard

Determine whether the draft came from ambient listening, clinician dictation or an uploaded written summary. Audio quality, interruptions, accents, multiple speakers and room noise can affect transcription. If a transcript is available under your policy, use it to resolve an uncertain phrase; do not treat a transcript as proof that the clinical interpretation is correct. The AMA Journal of Ethics discusses transcription errors, accent-related distortions and the additional inferences introduced when a system summarizes a conversation. AMA Journal of Ethics, 2025

A practical, step-by-step review routine

The following routine is a practical synthesis of the APA recommendations and the error patterns reported by the AMA. It is not a validated scoring instrument or a universal checklist. Adapt the order to your specialty and local workflow.

1. Establish the encounter frame

  1. Verify patient identifiers, date, visit type, author and intended recipient.
  2. Check that the chief concern and reason for encounter match the appointment and your recollection.
  3. Confirm that the template contains the sections your organization requires and remove sections that do not apply.

If any identity or encounter detail is wrong, correct or regenerate the draft before spending time on clinical editing. Never sign a note whose header is unresolved.

2. Reconcile the history and symptoms

Compare each clinically meaningful history statement with what the patient reported and with relevant chart context. Check onset, duration, severity, triggers, associated symptoms, pertinent history, allergies, medications and the patient’s goals when they matter to the visit. Watch for a model that turns a possibility (“could be,” “concerned about”) into a fact, assigns a symptom to the wrong person, or carries forward an old condition as current.

Correct quoted language when the exact wording changes meaning. If the patient denied a symptom, ensure the denial is attributed to the patient and is not silently generalized to a broader negative review of systems.

3. Verify examination, measurements and performed procedures

Check every finding against what you actually observed or measured. Confirm laterality, anatomical site, units, ranges, device readings and the distinction between a normal finding and an examination that was not performed. A planned, deferred or refused examination must not appear as completed. The AMA’s prostate-examination example demonstrates why action verbs require special scrutiny. AMA, March 18, 2024

For procedures, verify consent, indication, site, technique, findings, complications and post-procedure instructions to the extent your documentation rules require. Remove templated elements that were not relevant to this encounter.

4. Test the assessment and diagnoses

Ask whether each diagnosis, differential, finding and severity label is supported by the encounter. Separate the patient’s reported concern from your assessment, and separate a differential diagnosis from a confirmed diagnosis. Look for disease names inferred from a cluster of words rather than actually diagnosed; the AMA’s hand, foot and mouth example shows how an apparently sensible association can be false. AMA, March 18, 2024

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Check that the assessment reflects your current judgment, not an automatically copied problem-list item. If uncertainty is clinically appropriate, express it explicitly and at the level you would use without an AI assistant.

5. Audit the plan, timing and responsibility

Read every action verb and time expression. Distinguish:

  • Intended: “will order,” “recommend,” “plan to refer” or “scheduled for.”
  • Completed: “ordered,” “referred,” “administered,” “performed” or “result reviewed.”
  • Conditional: “if symptoms worsen, seek care” or “consider imaging if persistent.”

Verify medication name, dose, route, frequency, start or stop date and prescriber responsibility. Confirm that follow-up interval, return precautions, referrals, tests and patient instructions are accurate. A draft that says a test was ordered when it was merely discussed can create a false expectation and a missing order; the reverse can create an unrecognized clinical obligation.

6. Search deliberately for omissions and invented content

After checking what is present, compare the note with the encounter once more looking only for what is missing. Depending on the visit, that may include a safety assessment, informed-consent discussion, treatment alternatives, patient preference, refusal, critical negative finding, monitoring plan or communication with another professional. Then remove material that was never said, is unrelated to this visit, duplicates another section or implies certainty you do not have.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not “repair” a gap by guessing. If the fact cannot be established from the encounter or permitted chart context, leave it out or document the uncertainty according to local policy.

7. Save in the right record and complete sign-off

Make edits in the authoritative record, not only in a preview window or vendor interface. Confirm that the saved version contains your corrections, that copied-forward text did not reappear, and that the note status is the one your organization uses for a completed encounter. Sign only after the final display matches the record you intend to attest to. If your policy requires an addendum rather than editing a signed note, use that process and preserve the audit trail.

Clinical sections that deserve a focused pass

Section Questions to ask Common AI failure to catch
History and chief concern Did the patient say this? Are onset, duration, context and attribution correct? Old history presented as current; patient concern converted into diagnosis.
Review of systems Were positives and negatives actually discussed, and are they limited to relevant systems? Boilerplate negatives or a negative statement inferred from silence.
Examination and results Was each element performed or measured? Are side, site, units and values correct? Planned examination documented as completed; transposed value or laterality.
Assessment Does each diagnosis and certainty level match your judgment? Association mistaken for diagnosis; differential presented as confirmed.
Plan What was ordered, prescribed, scheduled, completed, deferred or refused? Discussion turned into an order or recommendation turned into completed care.
Instructions and follow-up Can the patient act on the instructions, and are timing and return precautions accurate? Missing safety-net advice or an invented appointment date.

Language traps that warrant line-by-line editing

Time and tense

Words such as “today,” “previously,” “will,” “has been,” “completed” and “pending” carry clinical meaning. Re-read every tense change. A sentence that moves a planned action into the past can falsely document care; one that moves a completed result into the future can hide an available decision point.

Negation and scope

Check “no,” “without,” “denies,” “not,” and “negative for.” Speech recognition can drop a negation or attach it to the wrong phrase. Ensure that a patient’s denial is not rewritten as your independent observation, and that “no evidence of” is not used when the examination was limited.

Certainty and attribution

Replace unsupported certainty with the wording that matches your reasoning. Distinguish what the patient reports, what a caregiver reports, what a test shows and what you infer. A model may blend those sources into a single authoritative voice.

Pronouns, speakers and multiple patients

In family visits, group sessions or consultations, verify who made each statement and to whom each finding belongs. If more than one person is present, check names and roles throughout the note rather than relying on context.

Numbers, units and medication details

Recheck decimal points, units, dose frequency, routes, dates, laterality and ranges against the source. Never accept a plausible number simply because it falls within a familiar range.

When the draft should be rejected or escalated

Pause sign-off and use your local correction or safety pathway when:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • the note is attached to the wrong patient or encounter;
  • an assessment, medication, procedure or result is materially wrong and the source cannot be resolved;
  • the system repeatedly introduces the same specialty-specific error;
  • the transcript or recording includes another patient’s information;
  • the note contains a safety-critical omission, such as missing return precautions or a contraindication;
  • you cannot determine whether an action was completed and the distinction affects care, billing or compliance.

Escalation is not a failure of the reviewer. It prevents an uncertain draft from becoming an authoritative record and gives the practice data for vendor evaluation, training or workflow redesign.

Evaluate the tool as well as each note

Before selecting or expanding an AI scribe, compare the workflow and governance features below. APA guidance recommends assessing privacy, security, data handling, minimization, consent and the product’s fit with practice needs. In its deployment reporting, the AMA says The Permanente Medical Group considered accuracy, ease of use and training, privacy and security, and patient consent; weak template integration and the perceived editing burden were barriers for some clinicians. APA guidance AMA, March 18, 2024 AMA, June 12, 2025

Evaluation area Questions for a practice
Workflow fit Is it built into the EHR, connected through an API or standalone? Does it populate the required note template without risky copy-and-paste?
Input method Does it use ambient listening, clinician dictation or an uploaded summary? How does it handle interruptions, multiple speakers and poor audio?
Accuracy and edit burden What errors appear in your specialty, how often do clinicians need substantial edits, and can reviewers see the source or transcript when policy permits?
Privacy and security What data are collected, where are they stored, who can access them, how long are they retained, and may practice data be used to train a model?
Consent and transparency How are patients informed, how is consent or refusal recorded, and what happens when a patient declines?
Practice fit Can templates, terminology, languages, accessibility needs and specialty workflows be configured? What training and support are available?

Run a controlled pilot with representative clinicians and encounter types. Review samples for omissions, false additions, timing errors, medication accuracy and editing time. Keep a documented issue log and define a stop condition for unacceptable safety or privacy behavior. A high adoption rate is not evidence of accuracy if clinicians are silently correcting the same errors or avoiding difficult encounters.

Consent, privacy and information handling

Ambient documentation can capture voices and sensitive discussion beyond the words ultimately placed in the note. Establish a practice policy covering patient notice, consent or refusal, recording and transcript retention, vendor access, secondary use, breach response and deletion. Minimize the information sent to the service and give staff a clear way to stop capture when a conversation should not be recorded.

Do not assume that one federal rule answers every disclosure question. The AMA Journal of Ethics explains that legal informed-consent duties depend on context and that ethical reasons for disclosure may be broader; it also examines privacy risks and inaccuracies caused by transcription and summarization. Use that analysis as ethics commentary, not individualized legal advice, and obtain advice appropriate to your jurisdiction and institution. AMA Journal of Ethics, 2025

Document the selected product’s evaluation and the controls that protect recordings, transcripts and generated drafts. A patient-facing notice should explain, in understandable language, what the tool does and how a person can ask questions or decline when your policy provides that option.

What the reported benefits do—and do not—show

The Permanente Medical Group evaluated an AI-scribe implementation over 63 weeks from October 2023 through December 2024, covering 7,260 physicians and 2,576,627 encounters. The American Medical Association reported an estimated 15,791 documentation hours saved. In the same deployment, 84% of surveyed physicians reported a positive effect on communication and 82% reported improved overall work satisfaction; 47% of patients said their doctor spent less time looking at the computer, and 39% said the doctor spent more time speaking directly with them. These figures describe one organization’s implementation and survey, not a guaranteed result for every specialty or practice. AMA, June 12, 2025

“Both doctors and patients highly value face-to-face contact during a visit, and the AI scribe supports that.” — Vincent Liu, MD, MSc, quoted by the American Medical Association.

Time saved is valuable only if the review remains thorough. Build review time into scheduling, measure correction effort and monitor safety events rather than treating reduced keyboard time as permission to sign faster.

Build a sustainable review policy

Assign responsibility

Name the clinician who owns the final note, the staff who may prepare or route a draft, and the privacy or health-information contact for incidents. Make clear that delegation of editing does not transfer the signing clinician’s responsibility for accuracy.

Train on failure modes, not just buttons

Use de-identified examples of tense changes, negation loss, wrong-speaker attribution, invented diagnoses, omitted refusals and incorrect medication details. Ask trainees to explain why each error matters and where to escalate it. Training should include the organization’s consent, retention and downtime procedures.

Audit and improve

At regular intervals, sample signed notes for clinically important additions and omissions, stratified by specialty, encounter type and input method. Track edit time, rejected drafts, privacy incidents, patient complaints and repeated error patterns. Feed findings into template changes, clinician coaching and product review; do not use a single “accuracy score” as a substitute for clinical oversight.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Plan downtime and corrections

Maintain a manual documentation path for outages, unavailable transcripts or patient refusals. Define how an erroneous signed note is amended, how downstream orders or communications are checked, and how the event is recorded for quality improvement. The procedure should preserve the original audit trail while making the corrected clinical facts clear.

Final sign-off decision

Sign only when you can answer yes to all of these questions for the specific encounter:

  • Is this the correct patient, encounter and note type?
  • Have the history, examination, results, assessment and plan been checked against what occurred?
  • Are planned, completed, deferred and refused actions clearly distinguished?
  • Have unsupported diagnoses, certainty, negative findings and copied-forward material been removed?
  • Are medications, measurements, timing, follow-up and safety instructions accurate?
  • Is the final version saved in the authoritative record and compliant with local sign-off and amendment policy?

If any answer is no, keep the note unsigned, correct it or escalate it. The value of an AI scribe is that it can reduce documentation effort while leaving the clinician more attention for the patient—not that it eliminates the clinician’s review.
