Place a mandatory, fail-closed approval gate immediately before every high-impact or irreversible tool call. The agent should present the exact target, serialized arguments, intended effect, evidence, applicable policy rule, and uncertainty. A trained, authorized person must explicitly approve or reject that specific payload; silence, screen viewing, or a generic “continue” action is never approval. If authorization, policy, target, or payload changes, pause and request a new decision.

This design keeps the agent useful for low-risk work while preventing consequential side effects—such as sending an external message, moving money, changing access, deleting data, deploying code, or issuing a safety-related command—from executing autonomously.

The control you need: a blocking gate at the tool boundary

An AI agent can reason, plan, and prepare an action without being allowed to perform it. The safety boundary must sit between the agent’s proposed tool call and the tool’s execution. When a call crosses your defined risk threshold, the orchestrator freezes the run, creates an approval request, and withholds execution credentials until an authorized human decides.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The gate is fail-closed: an error, timeout, lost session, revoked permission, or missing reviewer does not fall through to execution. A rejection ends that action. A timeout leaves it unexecuted. The agent may revise its proposal, but a revised proposal is a new action requiring a new decision.

This is different from post-action review. Reviewing a log after an irreversible transfer or disclosure cannot prevent the harm. Post-action monitoring remains useful for detection and accountability, but it does not replace pre-execution approval.

What the approval must cover

  • The identity of the agent and the user or system that initiated the run.
  • The exact tool being requested and the complete arguments, including hidden defaults and recipient identifiers.
  • The target resource, intended effect, scope, and whether the operation is reversible.
  • The evidence used by the agent, its confidence or uncertainty, and any missing information.
  • The policy rule that triggered approval and safer alternatives, if available.
  • An expiry time and the policy version used to classify the request.

Decide which actions are high risk

Do not rely on a vague label such as “autonomous” or “important.” Build an inventory of every tool and external side effect, then classify each operation using consistent criteria. A high-risk action usually combines significant potential impact with limited reversibility, sensitive rights or data, or a legal or safety obligation.

Inventory tools and side effects

Action family Examples Default control
Financial Initiating a payment, changing a bank beneficiary, approving a refund, placing an order Per-action approval; require amount, currency, destination, and verification evidence
External communication Sending an email, publishing a post, notifying a customer, filing a complaint Approval before send or publish; show complete rendered content and recipients
Identity and access Granting privileges, disabling an account, rotating a credential, changing a security policy Approval by an authorized role separate from the requester where practical
Data destruction or disclosure Deleting records, purging backups, exporting personal data, sharing confidential files Approval with retention, legal basis, scope, and recovery information
Production or infrastructure Deploying code, changing firewall rules, modifying DNS, stopping a service Approval tied to an exact change set, target environment, rollback plan, and maintenance window
Safety-critical control Commands that can affect people, equipment, physical access, or hazardous processes Qualified human authorization plus an independent emergency stop
Regulated recommendations or decisions Outputs that determine eligibility, treatment, employment, credit, or access to essential services Approval by a competent reviewer under the applicable sector process

Low-risk actions can remain automatic when their effects are bounded and reversible—for example, drafting an internal note without sending it or querying a read-only knowledge base. Record the rationale for that exemption so the boundary is deliberate rather than accidental.

Use a written risk policy

For each tool, define thresholds for impact, reversibility, affected people, sensitive information, legal or regulatory context, and required approver role. A policy can require approval for every call in a sensitive tool, or only when a parameter crosses a threshold.

Risk level Typical characteristics Execution rule
Low Read-only, internal, reversible, no sensitive data, no external side effect Automatic execution with ordinary logging
Moderate Limited external effect or sensitive context, but readily reversible and low impact Automatic only within explicitly bounded limits; otherwise route to review
High Material financial, access, privacy, safety, legal, or operational consequence Mandatory per-action human approval before the tool call
Critical Irreversible or potentially severe harm, safety-critical control, or broad blast radius Qualified approver, possible two-person authorization, short expiry, and tested stop path

Document who may approve each class. The person who requested an action may not have enough independence or authority to approve it, especially when the action changes their own access or benefits them financially.

Build the approval workflow step by step

  1. Intercept the call. The agent submits a structured tool request to a policy enforcement service rather than calling the external system directly. The service normalizes the tool name, target, arguments, initiating identity, and context.
  2. Evaluate policy. Classify the normalized request using the current policy version. If it is high risk or the classifier cannot determine its risk safely, hold it for approval. An unavailable policy service must not be treated as permission.
  3. Freeze the run. Persist the proposed action and pause the agent. Do not let the model continue by issuing a second route to the same side effect. Keep execution credentials unavailable while the request is pending.
  4. Create a payload-bound request. Serialize the exact arguments in a canonical form and calculate a cryptographic hash. Bind the request to the target, policy version, initiating user, approver requirements, and expiry time.
  5. Present decision context. Show the reviewer the agent identity, initiating user, requested tool, complete arguments, target, intended effect, evidence or citations, confidence or uncertainty, policy trigger, and safer alternatives. Render messages exactly as recipients will see them, including attachments and links.
  6. Collect an explicit decision. Provide distinct Approve and Reject controls. Record the reviewer’s identity, role, timestamp, and optional reason. Do not infer approval from opening the request, inactivity, a chat acknowledgement, or a generic “continue” button.
  7. Revalidate immediately before execution. Check that the reviewer is still authorized, the policy is still valid, the target still exists and belongs to the intended scope, the payload hash is unchanged, and the approval has not expired. Any mismatch returns the request to pending status.
  8. Execute once. Use a narrowly scoped credential and an idempotency key where the target system supports one. Prevent duplicate execution if the agent retries after a network failure.
  9. Record the outcome. Append the request, context, policy snapshot, reviewer identity and role, decision, timestamps, payload hash, execution result, and downstream impact to an append-only log.
  10. Resume or terminate safely. On approval and successful execution, return the result to the agent. On rejection, timeout, failed revalidation, or execution error, expose the status without silently attempting a different consequential action.

Example gate logic

request = canonicalize(agent_tool_call)
policy = load_policy(version="current")
risk = policy.classify(request)

if risk.requires_approval or risk.is_uncertain:
    approval = create_request(
        payload_hash=sha256(request),
        target=request.target,
        policy_version=policy.version,
        expires_at=now + approval_window,
        required_role=risk.approver_role
    )
    pause_agent_run()
    decision = wait_for_explicit_decision(approval.id)
    if decision != "approve":
        record_outcome(decision)
        stop_without_execution()

if not revalidate(request, approval):
    invalidate(approval)
    request_new_approval()
else:
    execute_with_scoped_credential(request, idempotency_key=approval.id)

The enforcement service, not the language model, must own the final allow or deny decision. A prompt instruction such as “ask before sending” is useful guidance but is not a security control because the model can misinterpret, omit, or route around it.

Make approval specific, informed, and resistant to drift

Bind approval to the exact payload

Approval for “send the report” is too broad. Bind it to the exact recipients, subject, body, attachments, send time, and account. For a transfer, include source account, destination, amount, currency, reference, and any scheduled date. For a deployment, include the artifact digest, environment, configuration changes, and rollback target.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Canonicalization matters: equivalent representations must produce the same serialized form, while any meaningful change must alter the hash. Normalize ordering and encoding before hashing, and include fields that might otherwise be filled by tool defaults. If a downstream service expands a short identifier into a different target, resolve and display the final target before approval.

Handle changing context

Approval is a point-in-time decision. Recheck authorization, policy, target state, and payload immediately before execution. A new recipient, changed amount, policy update, expired maintenance window, revoked role, or changed evidence invalidates the original decision. The safe response is a new approval request, not an automatic amendment.

Give reviewers enough context without overwhelming them

Put the consequential facts first: what will happen, to whom, with what scope, and when. Offer expandable evidence and technical details, but do not hide material information behind a summary that could mislead. Highlight uncertainty, conflicting sources, missing permissions, and assumptions. If the agent proposes a safer read-only or draft-only alternative, show it beside the requested action.

Compare approval designs before choosing one

Design decision Safer default for high-risk actions Trade-off to manage
Per-action vs. batch approval Per-action approval with exact payload binding Batches reduce queue load but can conceal a dangerous item; use them only for homogeneous, bounded actions with individual visibility
Pre-execution vs. post-action review Pre-execution blocking Prechecks add latency; post-action review cannot undo an irreversible effect
Reversibility and rollback Prefer reversible operations and show the tested rollback A rollback may be incomplete, expensive, or unavailable; do not label an action reversible without evidence
Reviewer authority Role-based, trained approver with separation of duties where needed Extra separation can slow urgent work; define an emergency role without removing accountability
Payload drift Canonical payload hash and immediate revalidation Strict binding causes more re-approvals when dynamic values change, which is the intended safety behavior
Audit-log integrity Append-only records with controlled retention and access Detailed records contain sensitive information and require protection and retention rules
Latency and queues Explicit expiry, visible queue age, and no automatic approval on timeout Long waits can interrupt workflows; design escalation and safe cancellation rather than bypassing the gate
Emergency stop Independent stop control plus credential revocation Stopping in-flight work may leave partial state; document and rehearse recovery

Operate the reviewer process

Assign competence, training, and authority

The reviewer must understand the action domain, the evidence shown, the consequences of approval, and the organization’s policy. A person who can click a button but cannot inspect a destination, interpret a change set, or stop the system is not meaningful oversight. Maintain role definitions, training records, escalation contacts, and an on-call path for urgent requests.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use separation of duties where the risk warrants it

Require a second approver for unusually large transfers, broad privilege changes, production changes with a wide blast radius, or safety-critical commands. Enforce the separation technically: the initiator cannot satisfy the second-person requirement, and an agent identity cannot approve its own request.

Prevent approval fatigue

Route only actions that meet the policy threshold, group genuinely identical low-impact items, and show why each request was triggered. Measure rejection, timeout, escalation, and override rates. A near-perfect approval rate may indicate that reviewers are rubber-stamping, while a high rejection rate may reveal poor agent planning or an overly broad policy.

Keep an audit trail that can explain every decision

Store an append-only event sequence rather than overwriting a request record. At minimum, capture:

  • Request identifier, agent identity, initiating user, session, and tool name.
  • Canonical arguments, target identifiers, payload hash, and policy version.
  • Risk classification, trigger rule, required approver role, and expiry.
  • Evidence references, uncertainty indicators, and alternatives presented.
  • Reviewer identity, role, decision, reason, and timestamps for submission and execution.
  • Revalidation results, credential scope, idempotency key, tool response, and downstream impact.
  • Rejection, timeout, cancellation, override, incident, and recovery events.

Protect logs from alteration by the system whose behavior they record. Restrict access, encrypt sensitive fields, synchronize clocks, and define retention according to your legal, contractual, and incident-response needs. Log enough to reconstruct the decision without copying unnecessary personal or secret data.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Provide an independent stop and recovery path

Approval is not the only control. Provide a stop button outside the agent’s conversation and orchestration loop. It should halt queued actions, block new tool calls, and alert the operations owner. During an incident, revoke or narrow the agent’s credentials so a compromised process cannot simply retry through another path.

Test the stop path regularly: stop a queued request, stop during a slow downstream call, revoke credentials while a run is paused, and recover from a partially completed operation. Record what state remains and who is responsible for reconciliation. A stop control that exists only in documentation, depends on the agent’s cooperation, or cannot reach the credential system is not independent.

The reviewer approves a summary, but the payload changes

Cause: the system stores a natural-language description rather than the serialized arguments.

Fix: hash the canonical payload, display the material fields, and reject any hash, target, policy, or authorization mismatch at revalidation.

A timeout becomes an approval

Cause: queue code treats an empty response or expired request as success.

Fix: model approval as an explicit enum with no implicit default: pending, approved, rejected, expired, cancelled, or invalidated. Only approved can proceed.

Apply the approach to common requests

“Ask before sending an email”

  1. Let the agent draft, but expose only a draft-capable tool until approval.
  2. At send time, capture the final From account, recipients, reply-to address, subject, body, attachments, and scheduled time.
  3. Show a rendered preview and the policy reason, such as external recipients or sensitive attachment.
  4. Bind approval to that complete payload and recheck recipients and attachment hashes immediately before sending.

“Ask before moving money”

  1. Require the agent to prepare a payment instruction without transfer authority.
  2. Display source and destination accounts, beneficiary verification, amount, currency, reference, timing, fees, and evidence for the request.
  3. Route the request to an approver whose role permits that amount and who is not the initiator.
  4. Use a short expiry, idempotency key, revalidation, and a reconciliation process for ambiguous responses.

“Ask before deleting data”

  1. Show the exact records, retention status, legal hold checks, estimated size, and recovery options.
  2. Require approval tied to the resolved record identifiers and deletion mode.
  3. Recheck holds, ownership, and identifiers before execution; invalidate approval if the selection changes.
  4. Record the deletion result and recovery evidence in the audit trail.

Align governance with authoritative frameworks

European Union guidance emphasizes that high-risk systems must support meaningful human oversight across their lifecycle. Recital 73 of the EU AI Act states: “High-risk AI systems should be designed and developed in such a way that natural persons can oversee their functioning, ensure that they are used as intended and that their impacts are addressed over the system’s lifecycle.” It also calls for operational constraints that the system cannot override and for assigned people with the competence, training, and authority to intervene or stop the system when it does not perform as intended.

The European Commission describes high-risk use cases as those that can endanger health, safety, or fundamental rights. Obligations and transition dates depend on the use case and jurisdiction; verify the current Commission guidance for the deployment you operate rather than applying a universal deadline.

NIST’s AI Risk Management Framework is voluntary and is intended to improve the incorporation of trustworthiness into AI design, development, use, and evaluation. Its Playbook organizes implementation activities under Govern, Map, Measure, and Manage. The AI RMF Core treats risk management as continuous and timely across AI-system lifecycle dimensions. In practice, that means revisiting your inventory, policy thresholds, reviewer performance, incidents, and stop exercises as the agent, tools, data, and operating environment change.

Deployment checklist

  • Every side-effecting tool is inventoried, including alternate routes and defaults.
  • High-impact and irreversible actions are explicitly defined and routed to mandatory approval.
  • The agent cannot reach consequential credentials without passing the gateway.
  • Approval requests show exact payload, target, effect, evidence, uncertainty, policy trigger, and alternatives.
  • Only an authorized, trained human can approve; viewing and inactivity do not count.
  • Approval is bound to a canonical payload, target, policy version, and expiry.
  • Authorization, policy, target, and payload are revalidated immediately before execution.
  • Timeouts, rejection, drift, and service errors fail closed.
  • Execution is idempotent where possible and uses least-privilege credentials.
  • Append-only logs capture decisions, hashes, results, and downstream impact.
  • An independent stop control can halt work and trigger credential revocation.
  • Teams exercise failure, recovery, and emergency procedures and review the resulting metrics.
