Ask the organization that holds the information: contact your doctor, hospital, or health plan for its records, and use your secure Medicare account to download Medicare claims data. These are different sources of information. An account download is not necessarily your complete medical chart, and Medicare says that for Medicare Advantage it will include only drug claims.

Start by identifying the record you need and who is likely to have it. If you need a doctor’s notes or test results, the provider is the place to ask. If you want claims data Medicare makes available online, use your Medicare account. If you want Medicare to discuss your claims or records with another person, a separate authorization form may help—but it does not replace a provider’s or plan’s records-request process.

First choose the right place to request information

The right route depends on which organization has the information and what you need it for. Medicare’s guidance describes access to personal health information held by providers who treat you and bill Medicare, and by health plans that pay for your care, including Medicare. Separately, a secure Medicare account provides a way to download claims data held in that account.

Where to ask What you may get How to start
Your doctor, hospital, or other provider Records the provider holds, which may include medical records, billing information, and other information used to make decisions about you. Contact the provider and follow its records-request process.
Your health plan Plan-held information, potentially including claims, enrollment, billing, medical, or case-management records. Contact the plan and ask how to request your records.
Your secure Medicare account Medicare claims data available through the account—not necessarily a complete clinical chart from every provider. Sign in to your account and use its claims download feature.

Medicare says you or your legal representative generally may view or get copies of personal health information from providers who treat you and bill Medicare, and from health plans that pay for your care. The information may include claims and billing records, enrollment information, medical and case-management records, and other records used to make decisions about you. The record holder determines its request procedure; Medicare does not provide one universal form for all providers and plans.

Think of the choice as four questions: Who holds it? A provider, a plan, or Medicare’s online account may be involved. What kind of information do you need? A clinical record and a claims file are not interchangeable. What format do you want? Ask whether the holder can provide a paper or electronic copy. Will another person act for you? The authorization that lets Medicare talk with a helper is distinct from the process a doctor or plan uses for its records.

Request records from a doctor, hospital, or plan

  1. Identify the record holder. For a clinical chart, start with the doctor, clinic, hospital, or other provider that created or maintains it. For plan-related records, contact the health plan that holds them. If more than one organization has information you need, you may need to contact each one; a request to a doctor does not automatically retrieve the plan’s records, or vice versa.
  2. Describe what you need. Specify the records and date range as clearly as you can. For example, you might request a visit record, test results, billing records, or a broader set of records. This helps the organization understand the scope of your request. If you are unsure whether a particular item is held by the provider or plan, ask which organization is the right one to contact rather than assuming one request will cover both.
  3. Ask for the organization’s current request process. Medicare says a form may be required, but there is no single Medicare form that serves as the request form for every provider or plan. Ask where and how to submit the request and whether the organization offers an electronic copy. The process, submission channel, and any verification steps depend on the record holder.
  4. Submit the request and keep a copy. Follow the holder’s instructions, including any identity or representative-verification steps it requires. Keep a copy of what you submitted and note when it was sent so you can follow up if needed. If you are requesting information for another person, ask the organization what it requires to confirm that person may act for the patient.
  5. Ask about timing and any copy fee. Medicare says a response may take up to 30 days. That is the stated possible response time, not a guarantee that every request will take that long. A copy fee may apply, subject to the limits described below. If you need a particular set of records or format, ask about any applicable fee before proceeding.

Procedures vary by record holder, and Medicare’s general guidance does not give a universal submission address or step-by-step form for each organization. If the first person you contact cannot handle the request, ask which department or office does. Do not treat a Medicare account download or a form authorizing Medicare to speak with someone as a substitute for the provider’s or plan’s own records process.

A useful way to make a request clear is to name the holder, say what categories of information you want, and give the time period if you know it. For instance, you could ask a hospital how to request a copy of records from a particular period, or ask a plan what process it uses for enrollment or case-management records. Those are examples of how to describe the request, not a standard form or a guarantee that a particular organization has every listed record.

Download claims from a Medicare account

A Medicare account download is useful when you want claims information that Medicare makes available online. It is not a substitute for requesting a provider’s clinical chart: claims data and a doctor’s full medical record serve different purposes and may contain different information. A claim-related file should not be assumed to include every note, result, or other item maintained in a doctor’s or hospital’s chart.

Medicare also states that if you have a Medicare Advantage plan, the account data will include only drug claims. Keep that limitation in mind when choosing a route: if you need broader plan-held information or clinical records, contact the organization that holds those records rather than relying on the account download alone.

  1. Create or sign in to a secure Medicare account. Medicare currently lists ID.me, CLEAR, and Login.gov as identity-verification options. Use an email address that belongs to you rather than one shared with another person. An account is personal: protect the credentials that provide access to claims information.
  2. Open the claims download feature. Use the account’s available feature to download your claims data. The precise records available depend on the information held in your account. If the file does not contain the clinical or plan records you were looking for, that is a sign to contact the provider or plan that holds them.
  3. Save or print the downloaded information as needed. Medicare says you can print or share the downloaded data. Treat the file as private information, whether it is stored electronically or printed. Choose a storage location you can protect, and take care when sending or showing the information to someone else.
  4. Decide carefully before connecting an app. Medicare describes Blue Button as an optional way to connect apps. Review an app’s privacy practices and what information and features apply before importing your data. You can stop sharing with an app through your Medicare account. Connecting an app is optional; it is not a requirement for getting claims data.

If you cannot access a smartphone or computer, Medicare’s login help page directs you to call 1-800-MEDICARE (1-800-633-4227) for help with account access. This is the contact route identified for login help; for a provider’s chart or a plan’s own records, contact the organization holding those records.

Before choosing any app, check its privacy practices and confirm which records and features apply to your needs. The existence of an app listing does not mean that it can retrieve every provider’s chart or every type of plan information.

Let someone help you without confusing the forms

If you want Medicare to discuss your claims or records with a person you choose, CMS-10106 is the “Authorization to Disclose Personal Health Information” form. Its purpose is to authorize Medicare to talk with that person. It is not a universal request form for medical records held by your doctor, hospital, or health plan.

That distinction matters when someone is helping with more than one task. The CMS-10106 authorization addresses communication with Medicare. A provider or plan has its own records and its own process for requests involving a legal representative or another person acting on your behalf. Ask each organization what authorization or verification it requires; permission for Medicare to speak with a helper does not, by itself, ask a provider to send that helper a chart.

If you are the person assisting a beneficiary, first be clear about where the requested information is held and what you are authorized to do. For Medicare claims or records, ask about the CMS-10106 authorization. For provider- or plan-held information, follow that organization’s instructions. These are separate requests when the information is held by different organizations.

Timing, fees, and electronic access

Medicare says a records response may take up to 30 days. Copying charges may be allowed, but a fee for a copy cannot exceed the total cost of copying labor and supplies, plus postage if the copy is mailed. Medicare also says viewing, searching, downloading, or sending information through an electronic portal is free in most cases. Ask the record holder whether a fee applies to your particular request and what it covers.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The 30-day figure is a possible response window in Medicare’s general rights guidance, not a promise that every request takes exactly that long or a guarantee that each organization will finish sooner. If you need the information by a particular date, make that timing clear when you contact the holder rather than assuming every request will be completed within a specific number of days. The organization can explain its current process for the records you requested.

It can also help to separate a request for access from the format and delivery questions. Ask whether you can view the information electronically, download it, or receive a copy, and whether a fee applies to the option you choose. Medicare’s guidance says electronic portal viewing, searching, downloading, or sending is free in most cases; it does not say that every record holder must use the same portal or offer the same formats. If a paper copy is mailed, the stated fee ceiling includes postage along with copying labor and supplies.

Protect your information after you receive it

  • Store downloaded files securely and protect your account username and password.
  • Keep printed copies somewhere private and secure.
  • Before connecting an app, review its privacy practices and decide whether you want it to receive your claims information.
  • If you shared claims data with an app and no longer want it connected, use your Medicare account to stop sharing.

Medicare advises users to protect downloaded and shared claims information as they would banking or other private information. That advice applies to electronic files and paper copies: both can reveal personal details. Be deliberate about where a download is saved, who can access it, and whether you want an app to receive the data. Keep account credentials private, and avoid using an email address shared with someone else when setting up the account.

Medicare’s privacy notice says beneficiaries can see and get a copy of information Medicare holds about them. Medicare’s “Your Rights” guidance states: “You have the right to have your personal and health information kept private.” Access and privacy go together: request the information through the correct holder, then protect it once you have it.

Choose your route by the record you need

  • Need a doctor’s notes, test results, or a broader clinical chart? Contact the provider that holds those records.
  • Need plan-held claims, enrollment, or case-management information? Contact the health plan and request its records.
  • Need claims data available through Medicare? Sign in to your secure Medicare account and use the claims download feature, keeping in mind the Medicare Advantage limitation.
  • Need Medicare to speak with a helper? Use CMS-10106 for that authorization, and separately follow the provider’s or plan’s process for any records it holds.

In practice, the routes can complement one another. You might download claims information from your Medicare account and separately contact a provider for the clinical chart behind a particular visit. Or you might ask a plan for information it maintains and use CMS-10106 if you want Medicare to discuss its records with a chosen person. Decide which organization holds each item, then make the corresponding request rather than expecting one channel to provide everything.

Frequently asked questions

Can I get my complete medical chart from my Medicare account?

Not necessarily. The Medicare account download is claims data available through the account; it is not necessarily a complete clinical chart from every doctor or hospital. Contact the provider that holds the chart for its records-request process.

What does Medicare Advantage change?

Medicare says account data for a person with a Medicare Advantage plan will include only drug claims. If you need other plan-held information or a provider’s clinical records, contact the plan or provider that holds it.
