When a chatbot gives you a false or misleading answer, save the exact response, verify the disputed claim against an authoritative source, submit feedback through the chatbot’s current reporting control, and correct every copy you shared. A confident tone, fluent prose, or an apparently precise citation does not make an answer true.
This guide covers the documented routes for ChatGPT, Claude, and Gemini Apps. Interface labels can change, so use the current help instructions if your screen differs. Reporting a response and correcting information that has already reached other people are separate tasks; do both when necessary.
What a false chatbot answer can look like
An error is not limited to an obviously absurd sentence. Consumer chatbots can produce a plausible claim, an incorrect number, a quotation that was never said, a citation that does not support the summary, or an answer that omits a qualification that changes the meaning. The wording may sound certain even when the underlying information is incomplete, outdated, or invented.
False, misleading, incomplete, or merely unhelpful
- False: the statement conflicts with a reliable record, primary document, or other authoritative evidence.
- Misleading: the words may be technically true but omit a material condition, date, jurisdiction, or exception.
- Unsupported: the answer supplies a quotation, source, statistic, or link that cannot be located or does not support the claim.
- Unhelpful: the answer does not address the question. This is a quality problem, but it is not automatically misinformation.
Identify which category applies before reporting. A precise description helps a reviewer distinguish a factual error from a disagreement, a policy issue, or a request for a better answer.

Step 1: Preserve the disputed response
Do not rely on memory or a paraphrase. Keep the original wording and enough surrounding conversation to show what the chatbot was answering.
- Copy the exact sentence, figure, date, quotation, or citation you believe is wrong.
- Record the conversation date, the chatbot and app you used, and any model or mode label visible in the interface.
- Capture a screenshot if the response might change or disappear. Keep the text copy as well, because screenshots are harder to search.
- Mark the specific proposition that needs checking. A long answer may contain several independent claims.
- Remove passwords, private messages, identity numbers, health details, or other sensitive information before sending feedback unless it is essential to explain the issue.
Preserving context prevents a reviewer from treating a factual error as a reaction to a sentence that was never actually produced. It also gives you a stable record when you later publish a correction.
Step 2: Verify the claim before calling it false
Check the underlying evidence rather than judging the answer by its confidence or writing quality. The strongest check is normally the original record or an authoritative primary source relevant to the claim.
Start with the source closest to the fact
- For a law, regulation, court decision, or government program, consult the relevant government or court publication.
- For a scientific result, open the original study or the issuing institution’s record and check its methods, date, and stated limits.
- For a company announcement, product specification, or service policy, use the organization’s current documentation rather than an unsourced summary.
- For a historical quotation, locate a scan, transcript, or archive entry that shows the words in context.
If no primary source is available, compare several reputable secondary sources and state the remaining uncertainty. Do not turn a disagreement between sources into a definitive correction.
Inspect every citation the chatbot supplied
Open the cited page yourself. Confirm that it exists, is about the claimed subject, and actually supports the sentence. Chatbots can fabricate citations or attach a real source to a conclusion the source does not make. Check whether the page has been updated, whether the chatbot used an older version, and whether a footnote or exception was left out.

Separate a changed fact from an answer that was always wrong
Dates, prices, eligibility rules, product features, and public guidance can change. Note the effective date and geographic or legal scope of your evidence. A statement can be accurate for one country or time period and false for another. Preserve that qualification in the correction instead of presenting a narrower finding as a universal rule.
Step 3: Submit feedback through the chatbot’s current channel
The documented routes differ by service. This table describes the reviewed guidance; menus may be renamed or rearranged in a later interface.

Reporting a ChatGPT response
Under the answer that contains the error, select the thumbs-down control. Choose the issue category presented by the interface, then select “Safety or Legal concern” when that is the reason for the report and complete the remaining prompts. Use the separate content-reporting route when the problem concerns prohibited or otherwise reportable platform content rather than ordinary answer quality.
Describe the disputed sentence, explain what your source shows, and identify the relevant date or jurisdiction. If the issue is simply an incorrect fact with no safety or legal dimension, choose the quality option that most closely matches the current menu instead of forcing a legal classification. OpenAI’s older “Chat model feedback” web form is marked deprecated; do not use it as the general current route. The current alternatives are in-product feedback, the applicable content-reporting path, or customer support, depending on the issue.
Reporting a Claude response
Use the thumbs-down control on the individual response and follow the feedback prompts. If you prefer email, send a concise report to feedback@anthropic.com. Include the exact wording, the evidence you checked, and the consequence of the error without sending unnecessary personal data.

Anthropic’s help guidance warns that Claude should not be used as a singular source of truth and recommends particular scrutiny for high-stakes advice. The email address is a feedback channel, not a promise of an individual investigation or a specified reply time.
Reporting an inaccurate Gemini Apps answer
Select thumbs-down on the response and use the feedback form shown by Gemini. Identify the statement that is inaccurate and, where possible, provide the authoritative page or document you used to check it. Keep the report focused on one or two verifiable claims so the reviewer can reproduce the problem.
What to include in a useful report
A report is easier to assess when it is reproducible and specific. Use this structure:
- Location: name the conversation, response, or message and include the date.
- Exact error: quote the sentence and identify the incorrect part.
- Expected fact: state the corrected wording, including date, region, unit, or exception.
- Evidence: provide the title and link of the primary or authoritative source you checked, if the interface permits links.
- Impact: explain whether the error could affect safety, legal rights, finances, health, privacy, or another person’s decision.
- Reproduction details: note whether the answer appeared after a particular prompt, file upload, or follow-up question.
Do not include a long argument when a short, testable description will do. If several claims are wrong, list them separately rather than describing the entire answer as “bad.”
Step 4: Correct information you already shared
Platform feedback does not edit a message that has already been copied into a post, email, document, presentation, or conversation. Correct the external record yourself.

- Find every place where the chatbot’s claim was repeated, including quoted replies and downloaded documents.
- Replace or annotate the incorrect statement rather than silently deleting context when readers may already have relied on it.
- State the correction plainly: identify what was wrong, give the supported version, and link to the evidence.
- Preserve qualifications such as “as of [date],” country, eligibility condition, or unresolved disagreement.
- Tell people who acted on the claim directly when the stakes justify personal notice.
- Keep a copy of the source and your correction so you can update it if the underlying fact changes.
A practical correction has three parts: “The earlier statement that [claim] was incorrect. The available [primary source] says [corrected claim] as of [date]. The source is [title or link].” Do not replace one unsupported assertion with another, and do not imply certainty where the evidence remains unsettled.
Correcting a public post
Edit the original post if the service preserves an edit history; otherwise add a clearly labelled correction immediately below it. Use the same wording in a pinned comment or update when the original is likely to continue circulating. If someone quoted the error elsewhere, reply with the corrected claim and source rather than assuming an edit will reach them.
Correcting a workplace or school document
Tell the owner or recipients which page, slide, row, or paragraph is affected. Replace the file when practical, retain the prior version for audit purposes, and mark the new version with its correction date. For a decision already made, explain whether the error changes the recommendation or requires a new review.

Do not make a medical, legal, financial, safety, employment, immigration, or security decision based only on a chatbot response. Verify the information with the relevant official authority or a qualified professional before acting.

- Legal: check the current law and jurisdiction with a qualified lawyer or official court or government source.
- Financial: confirm terms, rates, taxes, and risks with the institution or a regulated professional.
- Safety: follow the manufacturer’s instructions, emergency guidance, or responsible authority rather than an unverified conversational answer.
In a high-stakes report, say what action could follow from the error. That context helps the platform triage the issue, but it does not transfer responsibility for your decision to the chatbot provider.

What a report can—and cannot—do
The official guidance documents mechanisms for sending feedback or reporting content. It does not promise that a particular conversation will be edited, that you will receive a reply, or that the same error will never recur.
OpenAI says reported domains and other content may be reviewed by its Model Quality team, which may apply filters or other mitigations. “May” describes a possible review or system change, not a guaranteed model-wide fix. The Claude and Gemini guidance likewise describes feedback submission without a promised correction or response time.
Use the report to give the provider actionable information, then complete the separate correction process for anyone who received the false claim. A successful submission and a successful public correction are different outcomes.

Troubleshooting when the interface does not match
The thumbs-down control is missing
Check that you are viewing the individual response, not a shared transcript or exported file. Expand the response or open the conversation in the service’s main app. If the control remains unavailable, consult the provider’s current help page and use the documented support or content-reporting route for that product version.
The menu has different labels
Choose the option that describes inaccurate, misleading, safety, or legal content. Do not claim a legal violation merely to reach a form. Explain the factual problem in the free-text field and retain a screenshot of the labels you saw.
The citation is unavailable
Record the citation exactly as displayed, including its title and URL, then state that you could not verify it. Search independently for the underlying primary record. An inaccessible citation is not proof that the chatbot’s claim is false, but it is a reason not to rely on the claim until verified.
You submitted feedback but nothing changed
Assume only that the report was submitted. Re-check the external information, update your own correction, and use the provider’s current support route if the issue involves immediate safety, privacy, or legal harm. Do not repeatedly submit identical reports expecting an individual response.

- Have you checked an original or authoritative source?
- Does the source match the date, location, version, and scope of the claim?
- Did you inspect any chatbot citation rather than trusting its label?
- Did you select the platform’s current feedback or reporting route?
- Did you remove unnecessary personal or confidential information?
- Did you correct every copy you shared?
- Did a qualified person or authority review the claim if the stakes are high?

These pages explain reporting mechanisms and accuracy limits; they do not turn a chatbot into an authoritative source. The safest practice is to verify the claim, report the specific failure, and make the correction where people actually saw the information.
