In the United States, anyone can report a concerning health problem that happened after vaccination to the Vaccine Adverse Event Reporting System (VAERS), even when they are unsure a vaccine caused it. A VAERS report supports safety monitoring; it is not a medical diagnosis, a finding of causation, or an application for compensation.

Start by obtaining the person’s immunization record and medical records that show what happened, when symptoms began, how clinicians evaluated and treated them, and what happened afterward. If compensation may be appropriate, use the separate Vaccine Injury Compensation Program (VICP) or Countermeasures Injury Compensation Program (CICP) procedures; submitting VAERS does not start either claim.

What a VAERS report means

VAERS is a U.S. vaccine-safety surveillance system operated by the Centers for Disease Control and Prevention (CDC) and the Food and Drug Administration. It collects reports of health events after vaccination so public-health authorities can look for signals that may need investigation.

The CDC’s explicit warning is: “A report to VAERS does not mean that a vaccine caused an adverse event.” The fact that an illness followed a vaccination establishes timing, not causation. A report can involve a coincidental illness, an underlying condition, an administration problem, a medication interaction, or a vaccine-related event; VAERS itself does not decide which explanation is correct.

Reports can be incomplete or contain errors. CDC describes VAERS as passive surveillance, and says that after investigation most reported events are not associated with vaccination. Treat the submission as a notification for safety review, not as proof that an injury occurred because of a particular product.

Who may report and when to do it

Any person may submit a report: the patient, a parent or other caregiver, a family member, or a health-care professional. CDC and FDA encourage reporting even when the reporter is uncertain about causation. Health-care providers and vaccine manufacturers also have additional reporting obligations for specified situations.

Report a concerning event as soon as you have enough accurate information to describe it. Do not delay urgent medical care while assembling a form. A clinician’s assessment is useful evidence, but a clinician does not have to submit the report for you.

Information to assemble before filing

Accurate timing and product information make a report more useful and make later record requests easier. Create a short timeline and gather the following items where available.

Vaccination details

  • Date each dose was administered.
  • Vaccine name and, if known, manufacturer.
  • Lot number and the administering facility’s name and address.
  • Which dose or series number it was, if relevant.

Event details

  • The date and approximate time symptoms began.
  • A plain, chronological description of symptoms, changes in severity, and any recurrence.
  • Diagnoses or suspected diagnoses made by clinicians.
  • Emergency visits, hospitalizations, tests, procedures, medications, and other treatment.
  • Current status, including whether symptoms resolved, persist, or resulted in disability.

Health background

  • Current illnesses and medications.
  • Relevant medical history, allergies, and prior reactions after vaccination.
  • Other vaccinations, infections, procedures, or major events near the same time.
  • Demographic information requested by the form.

Use dates and direct observations rather than conclusions. For example, write that chest pain began two days after the dose and that an emergency department performed specified tests; do not state that the vaccine caused the pain unless a qualified clinician has made that determination.

How to submit a VAERS report

CDC identifies online submission as the preferred route. It also provides a writable PDF form that can be completed and uploaded. Use the current form and submission instructions on the CDC’s VAERS website because labels and procedures can change.

  1. Open the current VAERS reporting page. Choose the online reporting option, or download the writable PDF if that format is more practical.
  2. Enter the reporter and patient information. Supply contact details that will remain valid after submission. A caregiver can report for another person.
  3. Describe the vaccination. Enter the administration date, vaccine, manufacturer, lot number, facility, and dose information exactly as shown in the immunization record. If a field is unknown, say so rather than guessing.
  4. Describe the event. Give the onset date, sequence of symptoms, evaluations, diagnoses, treatment, and outcome. Include relevant illnesses, medications, medical history, and previous post-vaccination events.
  5. Review for chronology and accuracy. Check that the onset date is not confused with the date of diagnosis or the date of the report. Attach or reference supporting information when the current form allows it.
  6. Submit and save the confirmation. VAERS assigns a unique identification number and provides confirmation. Save or print both, along with a copy of the submitted report.

For serious reports, VAERS staff may seek records from health-care institutions or public-health authorities. The confirmation may be the only communication you receive, so do not assume that no follow-up message means the report was rejected.

How to find the immunization record and lot number

The immunization record is the best source for the product, date, lot, and administering provider. If it is not in your files, request it from the clinic, pharmacy, health department, or other provider that gave the dose. Ask specifically for a complete immunization record, not only an appointment summary.

For vaccines covered by the federal Vaccine Injury Table, federal law requires the permanent record to include the administration date, manufacturer, lot number, and the administering provider’s name and address and, when appropriate, title. CDC’s VAERS form instructions also direct reporters to the administering facility for vaccine and lot information.

Check the record for transcription errors and distinguish a lot number from a billing code, order number, or appointment identifier. If the provider cannot locate a lot number, record that it was unavailable and identify the facility and date rather than inventing a value.

Why V-safe is not a substitute

V-safe may prompt a user to begin a VAERS report after the user indicates that medical care was sought, but CDC states that V-safe is not an official record of vaccination. Obtain the provider’s immunization record independently and use V-safe information only as an additional recollection or prompt.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Medical records to request

Request records from every source that evaluated or treated the event. Include records that establish health before the vaccination as well as records documenting onset and follow-up.

  • Primary-care visits and specialist consultations.
  • Urgent-care and emergency-department notes.
  • Hospital admission, discharge, and operative records.
  • Laboratory, imaging, pathology, and other test results.
  • Medication lists, prescriptions, administration records, and treatment plans.
  • Referral notes, rehabilitation records, and follow-up assessments.
  • Records documenting work or functional limitations when those limitations are clinically recorded.

Ask for the complete chart or the specific date ranges needed, including attachments and test reports. A visit summary alone may omit the underlying results or clinician’s reasoning. Keep the request, the provider’s response, and the date received with the records.

How to organize the evidence

Use one dated timeline and one storage location. A labeled binder or expandable folder is optional; a clearly named digital folder with secure backups works just as well.

  1. Create a timeline with the vaccination date, symptom onset, each medical encounter, tests, diagnoses, treatment changes, and current status.
  2. Save an unaltered copy of the immunization record and every received medical record.
  3. Use descriptive filenames such as 2026-03-14_emergency-department_discharge.pdf.
  4. Keep the VAERS report, confirmation, identification number, correspondence, and any attachments together.
  5. Store authorizations and proof of submission separately from notes so they can be found quickly.
  6. Back up digital files in a secure location and limit access to people involved in care or a claim.

Do not rewrite clinical records. If you need to clarify an apparent error, keep your explanatory note as a separate dated document.

VAERS, VICP, and CICP are different pathways

A VAERS report is for public-health surveillance. It does not file a compensation claim. A person seeking benefits must apply directly to the program that may cover the facts of the case.

Pathway Primary purpose Where/how to file Records and eligibility
VAERS Collect reports for vaccine-safety monitoring and signal investigation. CDC’s online reporting system is preferred; a writable PDF can also be completed and uploaded. Accurate vaccine, timing, event, medical-history, and outcome information. A report does not establish causation or compensation eligibility.
VICP Compensation for eligible vaccine-injury claims under the program’s rules. File a petition with the U.S. Court of Federal Claims and submit a copy to the Health Resources and Services Administration (HRSA), following current HRSA directions. Coverage, injury definitions, documentation, and deadlines are program- and fact-dependent.
CICP Compensation for eligible injuries associated with covered countermeasures under its rules. Apply through HRSA’s CICP process rather than through VAERS or the VICP court petition. Coverage and documentation rules differ from VICP. HRSA requires complete records from one year before receipt of the covered countermeasure through the present.

What a VICP petition requires

HRSA states that a VICP petitioner must file with the U.S. Court of Federal Claims. The filing includes an original and two copies, a cover sheet, medical records, other supporting documentation, and the appropriate filing fee. A copy must also be submitted to HRSA.

Use the current HRSA instructions for the court address, forms, filing method, and fee because procedural requirements can change. A VAERS confirmation number is useful background but does not replace the petition or its evidence.

Check eligibility and timing before filing

VICP eligibility depends on the vaccine, the alleged injury, and the relevant dates. HRSA’s eligibility guidance describes the program’s rules and notes that limited equitable tolling may be available in limited circumstances. Because no specific vaccine, injury, or event date is provided here, no individual deadline can be calculated. Review current HRSA guidance promptly and consider qualified legal advice when a deadline or eligibility issue is uncertain.

What the CICP records process requires

CICP has its own coverage, application, and records process. Do not substitute the VICP petition procedure for a CICP application.

For a CICP review, HRSA specifies complete medical records covering the period from one year before receipt of the covered countermeasure through the present. In a vaccine-injury case, that collection includes the immunization record. This one-year lookback is a CICP requirement; it is not a universal record-retention rule for every VAERS report or every compensation pathway.

Authorizations and delivery

Complete an authorization for each provider. If a provider requires its own authorization form, HRSA says CICP will accept a copy of that form. CICP will not begin its review until all requested records are received, and it does not accept records by email for security reasons. Follow the current CICP instructions for approved delivery methods and keep proof of what was sent.

Deadlines, coverage, and changing procedures

VAERS reporting, VICP petitions, and CICP applications have different purposes and timing rules. There is no single deadline that can be applied to every suspected injury. Vaccine coverage, legal requirements, and submission procedures can change, and eligibility can depend on the exact product, event, diagnosis, and date.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Before relying on an old form, web page, or deadline calculation, check the current CDC VAERS instructions and HRSA eligibility and filing guidance. If the matter may be time-sensitive, obtain advice from a qualified attorney or an appropriate patient-advocacy resource rather than waiting for a VAERS response.

Common mistakes and how to correct them

Assuming a report proves causation

Problem: The narrative says the vaccine caused the condition as an established fact.

Correction: Separate observed timing from medical conclusions. State when the dose occurred, when symptoms began, what clinicians found, and what remains uncertain.

Submitting without the lot number

Problem: The reporter guesses a lot number or leaves out the administering facility.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Correction: Request the immunization record from the provider. If a detail truly cannot be recovered, mark it unknown and preserve the request and response.

Using V-safe as the official immunization record

Problem: A V-safe entry is treated as proof of the product or lot.

Correction: Obtain the provider’s permanent immunization record and compare dates and product details before filing.

Expecting VAERS to start compensation

Problem: The reporter waits for VAERS to award benefits or forward a claim.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Correction: File directly through VICP or CICP if the facts may qualify. Preserve the VAERS confirmation as a separate safety-report record.

Sending an incomplete CICP record set

Problem: Only the emergency visit is supplied, even though CICP requests the complete one-year pre-countermeasure history through the present.

Correction: Request records from every relevant provider, complete each authorization, and verify that all requested records were delivered through an approved method.

Overlooking follow-up documentation

Problem: The initial visit is documented but later improvement, persistence, disability, or additional treatment is not.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Correction: Add subsequent records and update the timeline. Keep the original submission unchanged and identify later information as an update.

Understanding VAERS public data

Public VAERS data are not a list of proven vaccine injuries. Reports may be incomplete, duplicated, or later found unrelated to vaccination. On May 8, 2025, the public data were changed to add later or secondary reports for the same patient, vaccine, and dose. CDC cautions that these additions relate to events already reported and do not mean that additional adverse events occurred.

Do not convert a VAERS count into an estimate of confirmed injuries or use a count alone to establish risk. Interpretation requires clinical review, background rates, investigation, and other evidence beyond the existence of a report.

A practical completion checklist

  • Urgent symptoms were addressed by a health-care professional.
  • A dated timeline separates vaccination, symptom onset, diagnosis, treatment, and follow-up.
  • The immunization record includes the date, product, manufacturer, lot number when available, and administering facility.
  • The VAERS description uses accurate observations and timing without claiming unproven causation.
  • Relevant medical records and test results have been requested from every provider.
  • The VAERS confirmation and identification number are saved with a copy of the submission.
  • Any possible VICP or CICP claim is being handled through that program’s separate rules.
  • Current HRSA guidance has been checked for coverage, filing steps, and deadlines.
  • CICP applicants have completed provider authorizations and arranged an approved, non-email delivery method.

This process creates two distinct records: a safety report for VAERS and, if appropriate, a separate compensation application supported by program-specific evidence. Keeping those purposes and submissions separate prevents the most consequential filing errors.
