Start with your jurisdiction and role. A sanctions-screening alert is not automatically a blocked transaction, and the correct report depends on whether you are a U.S. person, a UK business, a regulated cryptoasset firm, or another participant. First preserve the transaction record and determine whether the transfer was blocked, rejected, or only flagged. Do not move, return, convert, or release the assets solely because a screening tool produced a match.
For a U.S.-linked asset that you determine must be blocked, report it to the U.S. Department of the Treasury’s Office of Foreign Assets Control (OFAC) through its OFAC Reporting System within 10 business days, then file annual reports while it remains blocked. A rejected transaction is reported separately within 10 business days of rejection. In the UK, a suspected financial-sanctions breach is generally reported to the Office of Financial Sanctions Implementation (OFSI); a regulated cryptoasset firm may also need an NCA suspicious activity report (SAR). These routes are not interchangeable.
1. Establish which rules apply before you report
Sanctions reporting is jurisdiction-specific. Identify where the reporter, service provider, account, wallet custodian, or other relevant person is located and which sanctions regime creates the possible obligation. Then identify your role: individual holder, exchange, custodian, bank, money-service business, cryptoasset firm, or another regulated entity.

The applicable sanctions program and the facts of the transfer determine whether property had to be blocked or a transaction had to be rejected.
- United Kingdom: OFSI guidance concerns UK financial sanctions. Some businesses have a legal duty to report suspected breaches, and failing to report when required can be a criminal offence.
- Other jurisdictions: The procedures below do not establish the law in every country. Obtain the current instructions of the relevant sanctions authority and qualified legal advice where the location, parties, or services create a cross-border issue.
A wallet address appearing in a screening database does not, by itself, prove that a listed person owns the wallet or that a particular transfer was legally blocked. Record the screening basis and obtain a legal determination appropriate to the regime before taking irreversible action.

2. Classify what happened: blocked, rejected, or flagged
The most important operational distinction is the action taken, not merely the presence of an alert.
| Situation | What it means operationally | U.S. reporting trigger | Typical evidence |
|---|---|---|---|
| Blocked property | You determined that you hold property that must be blocked and denied the parties access to it. | File a blocked-property report within 10 business days after blocking; file the annual blocked-property report while the asset remains blocked. | Original transfer instructions, wallet and address details, parties, value, date, sanctions basis, and controls used to prevent release. |
| Rejected transaction | You refused to process a transaction because it was prohibited, but you did not take the property into your possession as blocked property. | File a rejected-transaction report within 10 business days after rejection. | Original transfer instructions, reason for rejection, parties and roles, asset and amount, date, and any screening or communications record. |
| Screening hit only | A tool or analyst identified a possible match, but the legal and factual analysis is not complete. | No automatic OFAC filing follows solely from an alert. Resolve the match and document the decision. | Alert, list version or source, identifying information compared, analyst rationale, and any escalation. |
OFAC’s FAQ 49 explains that blocking and rejection reports are due within 10 business days of the relevant action and must include the original transfer instructions. Its virtual-currency guidance says the obligations for digital currency and traditional fiat currency are the same: “The obligations are the same.” That sentence addresses the type of currency, not the outcome of every screening alert.
3. U.S. procedure: reporting a blocked or rejected crypto transaction to OFAC
Step 1: Confirm the U.S. connection and sanctions basis
Determine why U.S. sanctions rules apply and identify the sanctions program, listed person, blocked property rule, or prohibition involved. Document the facts linking the address or counterparty to the relevant person. If the match is uncertain, preserve the asset and escalate for sanctions analysis rather than treating an unverified match as conclusive.

Step 2: Preserve the original transfer instructions
Save the instructions exactly as received, including the sender’s request, destination address, asset, quantity, network, timestamps, routing information, and any intermediary instructions. Do not rely only on a blockchain explorer link. OFAC specifically requires a copy of the original transfer instructions for blocking and rejection reports.
Step 3: Take only the action required by the determination
If the asset must be blocked, deny access and place controls around it so it cannot be transferred or released without authorization. If the transaction must be rejected, refuse the prohibited processing and retain the relevant records. A screening alert alone is not permission to seize, return, convert, or move cryptocurrency.
Step 4: File through the OFAC Reporting System
Use the OFAC Reporting System (ORS) for mandatory initial blocked-property reports, rejected-transaction reports, and the Annual Report of Blocked Property. Register and follow the current form instructions on that page before filing.

OFAC says a filer may ask to use another method only when it can demonstrate unique and extraordinary circumstances that prevent ORS use. Such requests are presumed denied unless OFAC grants written permission. Do not assume that an email or paper submission is acceptable without that permission.

- Blocked property: Submit the initial report within 10 business days after the blocking action.
- Rejected transaction: Submit the report within 10 business days after the rejection.
- Property still blocked: Submit the annual blocked-property report for assets that remain held. OFAC’s published annual deadline is September 30; verify the current annual instructions in ORS before filing.
The 10-business-day clock starts with the action that creates the reportable event. It does not start merely when an automated screening system generates an alert.
Step 6: Keep the crypto blocked while the case remains unresolved
OFAC’s virtual-currency FAQ and guidance state that a U.S. person who determines it holds virtual currency that must be blocked must deny access, follow the holding and reporting requirements, and use controls that prevent unauthorized release. OFAC does not require converting the cryptocurrency into fiat currency or moving it into an interest-bearing account. Conversion or movement can create additional risks and should not be treated as a reporting step.
4. Does an OFAC report also count as a FinCEN SAR?
Not generally. FinCEN’s unitary-filing interpretation is limited to specified designated-terrorist and narcotics categories. It does not create a universal rule that an OFAC blocking report replaces a suspicious activity report.
An activity may be reportable to FinCEN even when the OFAC match is absent, uncertain, or unrelated to the reason for the suspicious-activity filing. The OFAC report may also omit information that a SAR requires. If your institution has an independent SAR obligation, evaluate and file that SAR under the current rule for your institution type; do not use a single universal deadline without checking that rule.

- Was property blocked or a transaction rejected under OFAC rules?
- Would the conduct be suspicious and reportable to FinCEN independently of the sanctions match?
If both answers are yes, complete both reporting processes and ensure that each filing contains the information required by its recipient.
5. UK procedure: reporting suspected financial-sanctions breaches
Report to OFSI
Use the UK government’s financial-sanctions suspected breach reporting form to report a suspected breach to OFSI. The form is the relevant route for UK financial-sanctions reporting, subject to the facts and the applicable measure.
OFSI asks for an accurate narrative that explains what happened, who was involved, each party’s role, the transaction value and currency, date, remitter, intermediary, ultimate beneficiary, and the steps taken after discovery. For complicated cross-border activity, a diagram can clarify the flow of funds, wallets, intermediaries, and control relationships. If you are unsure whether a legal duty applies, the guidance advises obtaining independent legal advice.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errorsRegulated cryptoasset firms: consider an NCA SAR as well
The UK’s 2025 Cryptoassets Threat Assessment says that UK cryptoasset firms holding virtual assets tied to suspected sanctions evasion or circumvention should report the suspected activity to OFSI. Where the firm operates in the regulated sector, it should also file an NCA SAR through its usual channel when applicable.
That guidance is directed at the UK firm context. It should not be read as imposing the same reporting workflow on every individual who holds cryptocurrency. A regulated firm should keep its OFSI and NCA assessments distinct: OFSI addresses the suspected financial-sanctions breach, while the NCA SAR addresses suspected criminal or money-laundering activity under the firm’s applicable obligations.
Handling assets connected with a suspected breach
Where no exception or licence applies, the UK guidance discusses freezing and reporting assets as part of frozen-asset reporting. Do not transfer, liquidate, return, or otherwise deal with the asset merely to tidy an account. Confirm the applicable licence, exception, ownership facts, and instructions from OFSI before changing the asset’s status.

6. UK trade sanctions use a different reporting route
Not every sanctions concern is a financial-sanctions matter. If the issue involves trade sanctions, the UK government’s trade-sanctions reporting guidance says the destination depends on the measure:
- Specified measures are reported to the Office of Trade Sanctions Implementation (OTSI).
- Many other trade-sanctions breaches are reported to HM Revenue & Customs (HMRC).
- The guidance identifies exceptions for oil-price-cap measures and internet-service measures.
- Distinct rules may apply to legal or financial service providers and money-service businesses.
Do not send a trade-sanctions matter to OFSI simply because it involves a cryptocurrency. Identify whether the suspected conduct concerns funds or economic resources, goods, technology, services, or another controlled activity, then follow the route for that measure.
7. Evidence checklist for any report
Prepare a contemporaneous file containing the information that lets an authority reconstruct the transaction and your response. Retain only what you are lawfully permitted or required to retain, and protect personal data and private keys.
- Blockchain record: transaction hash, chain or network, block or confirmation information, date and time with time zone, asset, quantity, and any linked transactions.
- Wallet identifiers: sending, receiving, intermediary, and custodial addresses; the basis for associating an address with a person or entity; and relevant ownership or control information.
- Valuation: the fiat valuation, currency, pricing source, and valuation time used in the report. Explain whether the value is an estimate and how volatility was handled.
- Participants: sender, recipient, remitter, intermediary, ultimate beneficiary, customer, beneficial owner, and each party’s role.
- Sanctions analysis: list or program matched, identifiers compared, screening date, match rationale, false-positive analysis, and the person or team that approved the decision.
- Action taken: whether the transfer was blocked, rejected, or only flagged; the precise date and time of that action; account or wallet controls applied; and any licence or exception considered.
- Communications: original instructions, customer messages, counterpart communications, internal escalations, and approvals.
- Submission record: filed form, confirmation number, attachments, follow-up requests, and the person responsible for monitoring the blocked asset or unresolved case.
OFAC specifically calls for original transfer instructions and relevant wallet or address ownership details. OFSI asks for a narrative, parties and roles, value, currency, date, and response steps. Supplying a clear chronology reduces the risk that an authority must ask basic questions after filing.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.8. A practical decision path
- Pause irreversible activity. Prevent unauthorized access while you establish the facts. Do not send the asset back to the source address as a default response.
- Identify the regime. Record the reporter’s location, the service’s location, the parties’ connections, and whether the issue is financial or trade sanctions.
- Preserve evidence. Export original instructions and collect on-chain and off-chain records before systems rotate or delete them.
- Resolve the alert. Compare names, aliases, addresses, ownership, control, geography, and transaction context. Record why the match is confirmed, disproved, or unresolved.
- Classify the action. Decide whether the event is a required block, a rejection, or only an investigation. Record the action date because it controls a U.S. 10-business-day deadline.
- Check parallel duties. For a U.S. institution, separately assess any FinCEN SAR duty. For a UK regulated cryptoasset firm, separately assess an NCA SAR.
- Submit to the correct portal. Use ORS for U.S. OFAC reports, OFSI’s form for UK financial-sanctions suspected breaches, and OTSI or HMRC for the applicable UK trade-sanctions measure.
- Monitor after filing. Keep blocked property protected, respond to authority questions, and calendar the annual OFAC report when an asset remains blocked.
9. Common mistakes and how to avoid them
Treating every alert as a legal block
Screening data can be incomplete or produce false positives. An alert is a reason to investigate, not proof that the asset must be frozen. Document the comparison and legal conclusion.
Using the wrong deadline
For OFAC, the initial deadline is 10 business days after blocking or after rejection, depending on which action occurred. The annual report is a separate obligation for property that remains blocked. Calendar each event separately.

Submitting an OFAC report and skipping the SAR analysis
An OFAC filing does not generally replace an independent FinCEN SAR obligation. Review the facts under the institution’s current SAR rule and file separately when required.
Confusing financial and trade sanctions in the UK
OFSI is not the universal destination for every UK sanctions concern. Trade-sanctions measures may route to OTSI or HMRC, with stated exceptions. Identify the measure before submitting.
Moving or converting the asset to make the problem disappear
Moving funds can create a new transfer, destroy evidence, or violate the applicable restrictions. OFAC does not require converting blocked virtual currency to fiat or placing it in an interest-bearing account. Obtain authority instructions or legal advice before any change.

Escalate promptly when the parties, wallets, exchanges, or infrastructure span countries; ownership or control is uncertain; a licence or exception may apply; the asset is held by a regulated business; the transfer involves a high-risk sector; or the deadline is close and records are incomplete. Ask counsel to address the applicable sanctions program, blocking versus rejection, licensing, data-sharing, SAR confidentiality, and preservation duties.
