If your hamstring cramps, stop what you’re doing, get into a safe resting position, then gently stretch and massage the muscle until it relaxes. Don’t bounce or force the stretch. Heat may ease lingering tightness; cold may help soreness after the cramp. A sudden cramp can be brief, but a pop, swelling, bruising, weakness, or trouble walking can point to an injury instead.
What to do when your hamstring cramps
- Stop and rest. End the activity that triggered the cramp and sit or lie somewhere safe.
- Gently stretch and massage the muscle. Lengthen the hamstring only as far as feels comfortable, and massage it lightly. The American Academy of Orthopaedic Surgeons (AAOS) advises holding a gentle stretch until the cramp stops. Don’t bounce or push into pain.
- Try the AAOS seated stretch only if it feels comfortable. Sit upright with both legs extended in front and feet neutral. Keeping your back long and chest open, hinge at the hips and slide your hands toward your ankles until you feel a stretch. Keep your knees unlocked. Don’t force yourself into this position during a painful spasm.
- Choose heat or cold based on how the muscle feels. Heat may help tightness; cold may ease soreness after the spasm. If you use a hot or cold pack, protect your skin and follow its directions. Neither is a substitute for medical assessment if injury warning signs appear.
- Drink if you’re dehydrated. Fluids are sensible if you have lost water. After intense exercise or heat exposure, a sports drink or another source of electrolytes may be useful. One cramp alone does not establish dehydration or a mineral deficiency, and salt tablets or supplements are not a universal fix.
How to tell a cramp from a possible hamstring injury
A cramp is a sudden, involuntary tightening that commonly settles within seconds or minutes. A hamstring strain or tear is more concerning when pain starts sharply during activity, especially with a popping sensation, or when tenderness, swelling, bruising, stiffness, or difficulty moving develops. Trouble standing or walking is another warning sign. These clues cannot diagnose an injury remotely.
Stop sport and seek medical assessment if pain is severe or worsening, swelling or bruising is substantial or increasing, or you cannot bear weight comfortably. Don’t treat those symptoms as an ordinary cramp and keep exercising.
When to get medical help
Contact a healthcare professional about cramps that are severe, frequent, prolonged, repeatedly return, fail to improve with gentle stretching and adequate fluids, or occur with swelling, redness, warmth, or muscle weakness. Recurrent cramps may warrant a review of medicines and health conditions; they do not automatically mean you need a supplement.

In England, NHS guidance recommends contacting NHS 111 for a suspected hamstring injury that is very painful or worsening; has substantial or worsening swelling or bruising; makes walking or standing painful; causes marked stiffness or difficulty moving; or is not improving with self-care. This pathway applies to England, not every country.
Ways to reduce the chance of another cramp
- Warm up before stretching or exercising, and build exertion gradually.
- Do regular flexibility work and choose fitness activities suited to your current ability.
- Drink enough during exercise and in hot conditions, while recognizing that hydration cannot prevent every cramp.
Cramps can have several causes, including overuse, heat, medicines, health conditions, and factors that are not known. Stretching, hydration, potassium, magnesium, or any particular product should not be treated as a guaranteed prevention method.
