Call your local emergency number for breathing problems, unconsciousness, seizures, extreme overheating, severe agitation, delirium, inability to respond, rapidly worsening hallucinations, or an immediate risk of harm. If none of those signs is present but the exposure is worrying, contact a poison center or clinician promptly.
Start with the fastest safety decision
Psychedelic and dissociative drugs can produce very different reactions from one person or occasion to another. The amount and potency matter, but so do a person’s biology and existing health, mood, expectations, surroundings, and use of alcohol or other drugs. A product sold as a hallucinogen may also contain an unexpected or counterfeit substance. Because those variables cannot be judged reliably from appearance alone, triage should focus on the person’s breathing, consciousness, temperature, behavior and speed of change rather than on the claimed name of the drug.
Use the three pathways below. They are action choices, not a formula that makes psychedelic use safe.
| Situation | Action | Why speed matters |
|---|---|---|
| Any emergency warning sign | Call the local emergency number now (911 is the US example; the UK uses 999 or an emergency department). | Breathing failure, seizures, loss of consciousness, dangerous overheating, severe delirium and rapidly escalating behavior can become life-threatening. |
| Concerning exposure without an immediate emergency sign | Contact a poison center or healthcare professional promptly. In the US, Poison Control is 1-800-222-1222 and webPOISONCONTROL; in the UK, NHS poisoning guidance directs uncertain cases to NHS 111. | A specialist can assess the substance, timing, co-use and symptoms and tell you whether emergency evaluation is needed. |
| Symptoms that persist or remain distressing after the acute effects | Arrange clinical follow-up, including mental-health assessment when appropriate. | Persistent perceptual or psychiatric symptoms need evaluation even if the initial intoxication has ended. |
When deciding between these pathways, ask four practical questions: Is breathing normal? Is the person awake and responding? Are symptoms changing rapidly or creating immediate danger? Could the product or combination be unknown? A “yes” to a serious warning sign means emergency services, not a wait-and-see approach. If the person is stable but the substance, amount or symptoms are concerning, poison specialists can provide case-specific advice. If the immediate episode has passed but perception, mood or thinking remains abnormal, arrange follow-up rather than assuming it will resolve without assessment.

When to call emergency services immediately
SAMHSA advises emergency care or an immediate 911 call when a person who used a hallucinogen has any of the following. The NHS gives overlapping emergency criteria for suspected poisoning and for severe hallucination-related changes. Use your jurisdiction’s emergency number and describe the symptoms plainly.
| Warning sign | What it can look like |
|---|---|
| Trouble breathing | Breathing is very slow, irregular, labored, absent or otherwise abnormal. |
| Unconsciousness or inability to wake | The person does not respond to voice or gentle touch, or repeatedly falls back into an unresponsive state. |
| Seizure | Convulsions, prolonged stiffening or jerking, or repeated seizures. |
| Very high body temperature | Marked overheating, especially together with confusion, agitation, collapse or abnormal behavior. |
| Severe agitation, aggression or violent behavior | The person is uncontrollably agitated, aggressive, or creating an immediate danger to themselves or others. |
| Confusion, delirium or inability to respond | They cannot follow simple requests, are disoriented, or seem detached from reality and unable to communicate normally. |
| Rapidly worsening hallucinations or sudden confusion | Hallucinations escalate quickly, behavior changes abruptly, or the person becomes severely agitated or aggressive. |
| Thoughts or actions that could harm self or others | The person expresses suicidal or violent intent, moves toward a dangerous act, or cannot be kept away from immediate hazards. |
| Possible unknown or counterfeit substance | The product’s identity is uncertain, it came from an unregulated source, or symptoms are unexpectedly intense or unusual. |
Do not wait for every sign to appear. A single serious symptom is enough to call. Tell the dispatcher what was taken if known, when it was taken, whether alcohol or another drug may be involved, and which warning sign you are seeing. If you are unsure whether the situation qualifies, call emergency services and follow the dispatcher’s instructions.
Do not let the person’s explanation of the dose override what you observe. People may misidentify a substance, forget co-use, or underestimate strength. A calm appearance also does not rule out dangerous overheating, delirium or a later loss of consciousness. Explain the most concerning observable change and whether it is worsening.
What to do while help is coming
The safest first aid is to reduce immediate hazards, observe basic life signs and follow professional instructions. Do not attempt to manage a severe reaction with another drug, a cold bath, physical restraint or an improvised antidote.
- Make the area safer. Move sharp objects, weapons, traffic, heights, open water, fire and other hazards away when you can do so without putting yourself at risk. Keep bystanders calm and limit unnecessary stimulation.
- Stay with the person if it is safe. Speak in a calm, simple way. Do not argue about what they see or challenge hallucinations; focus on reassurance and immediate safety.
- Watch breathing and responsiveness. Note whether the chest is moving normally and whether the person responds. Report any change to the dispatcher. Continue checking while waiting.
- Use the appropriate position. NHS poisoning guidance says to start CPR if someone is unresponsive and not breathing, and to place an unconscious person who is breathing in the recovery position. Follow the dispatcher’s directions, especially if you have not been trained.
- Do not induce vomiting. Do not give food or drink when poisoning is suspected unless emergency professionals specifically instruct you to do so.
- Gather information for clinicians. Keep packaging, labels, remaining material, prescriptions and a record of the approximate time and amount, if available. Do not delay the emergency call to search for a package.
- Protect yourself. Avoid direct contact with unknown powders, liquids or vomit. Tell responders if you suspect an unknown or counterfeit product or multiple substances.
NHS first-aid advice is general poisoning guidance, not a psychedelic-specific treatment protocol. Emergency-dispatch instructions take priority over any general checklist.
If the person becomes more agitated, keep a safe exit for yourself and others rather than blocking or wrestling with them. Tell the dispatcher about weapons, traffic, heights, water, fire or other hazards so responders can plan their approach. If you cannot safely remain nearby, move to safety and keep the emergency line informed.
If there is no immediate emergency sign
A person can be awake and breathing normally yet still need urgent professional advice. Contact a poison center or healthcare professional when the substance, strength or amount is uncertain; symptoms are unusual, persistent or worsening; alcohol or another drug may be involved; or the person has a relevant medical or mental-health condition. Have the product name or package, estimated timing, route of exposure, co-used substances, age and weight, current symptoms and medical conditions ready.

In the United States, Poison Control can be reached at 1-800-222-1222, and its webPOISONCONTROL service provides online guidance. These services are US-specific. Elsewhere, use the poison-information service or urgent medical line for your country; the NHS directs uncertain UK poisoning cases to NHS 111. If symptoms become severe while you are waiting for advice, switch to the emergency pathway.
Poison services are appropriate for uncertainty, not a substitute for emergency care. A specialist may ask about the exact product, formulation, route, time, amount, other substances and the person’s age or weight. Answer as accurately as possible, but do not postpone a 911, 999 or equivalent call while trying to calculate a dose. Service names and numbers differ by country, so save the local poison-information and emergency contacts before an incident when possible.
Precautions that reduce avoidable hazards
Official guidance supports a limited set of broad precautions. They lower some preventable dangers but cannot guarantee a safe experience, prevent contamination or predict an individual reaction.
Do not rely on unknown or counterfeit products
SAMHSA warns against unknown or counterfeit substances. A product’s label, color, tablet shape or claimed source cannot establish its contents. NIH notes that fentanyl and related contaminants are difficult to detect without chemical testing. Visual inspection, a casual home test or confidence in a seller cannot rule out contamination. If the identity is uncertain, treat unusual symptoms seriously and tell responders that the contents may be unknown.
Do not assume a familiar name means a familiar pharmacology. A counterfeit product can contain a different active drug, a stronger concentration or multiple substances. Keep any container or label for responders, but never taste, smell or handle an unknown material to identify it.
Avoid alcohol and other drug combinations
Combining hallucinogens with alcohol or other drugs adds uncertainty and can intensify impairment or medical complications. SAMHSA specifically advises avoiding these combinations. If co-use has occurred, disclose it to emergency staff or a poison specialist rather than assuming it is irrelevant or embarrassing.
Do not drive or operate machinery
Hallucinogens can impair judgment, attention and perception. SAMHSA advises not driving or operating machinery after use. A person who feels calm may still misjudge distance, speed, traffic or physical danger. Arrange a sober, safe environment and transport plan instead of relying on the user to decide when they are capable.

Reduce environmental dangers
NIH describes injury risks linked to altered thought and perception. Keep the person away from roads, balconies, water, fire, tools and other places where a distorted perception could cause injury. This is a hazard-control measure, not a way to make drug use safe.

NIH identifies several factors that change psychedelic and dissociative effects:
- Amount and potency: A product sold under the same name may vary in strength, and the actual contents may differ from the label.
- Individual biology and health: Age, body chemistry, existing medical conditions and prescribed medicines can affect the response.
- Mood and expectations: Fear, distress, confidence and expectations can shape the experience and behavior.
- Environment: Crowds, noise, heat, unfamiliar places, heights and access to hazards can increase injury risk.
- Alcohol or other drugs: Co-use can change effects and makes the situation harder to assess.
These variables explain why a previous uneventful experience does not predict what will happen next time. They also explain why a bystander should describe observed symptoms rather than trying to infer a dose-based outcome.
They also make online timelines and anecdotes unreliable for triage. Two people who took products sold under the same name may have different contents, health factors, co-use or environments. Use current symptoms and their direction of change as the decision points.
Persistent or distressing symptoms after the acute effects
Flashbacks and persistent perception changes
NIH describes flashbacks and hallucinogen-persisting perception disorder, in which perceptual disturbances recur or continue after the immediate drug effects. A person who experiences recurring visual phenomena, severe anxiety about perception changes, or disruption to work, sleep or daily life should arrange a medical assessment. Do not assume that persistence is simply a normal comedown.

NIH notes that psychedelics may trigger schizophrenia-like illness in people with predisposing factors or worsen symptoms in people who have schizophrenia. NHS guidance calls for urgent assessment when hallucinations are accompanied by sudden confusion, severe agitation or aggression, rapidly worsening symptoms, or thoughts of harming oneself or someone else. Emergency care is appropriate for those severe changes; otherwise, arrange prompt clinical follow-up.
Pregnancy, medicines and underlying conditions
NIH says effects during pregnancy are not well understood. Anyone who is pregnant, planning pregnancy, taking prescription medicines, or managing a significant medical or mental-health condition should discuss exposure and symptoms with a clinician. Do not stop prescribed treatment or self-diagnose an interaction based on online anecdotes.

Follow-up is also appropriate when symptoms interfere with sleep, work, relationships or the ability to care for oneself. Bring a factual account of timing, symptoms, co-use and any package or prescription information. A clinician can decide whether medical, toxicology or mental-health evaluation is needed; do not try to label the condition yourself.
What poison-center data can—and cannot—tell you
A 2024 study of reported psychedelic exposures to United States poison centers from 2012 through 2022 found that over half of the reported exposures had symptoms requiring treatment, severe residual or prolonged symptoms, or death. This is a description of poison-center reports, not the percentage of all people who use psychedelics and not an individual probability. People who contact poison centers may differ from people who do not, and the report does not establish a universal risk for every substance or setting. It does show why a serious or unusual reaction should receive professional assessment rather than being dismissed as an expected trip.

The figure combines several serious outcomes and applies only to reported poison-center exposures in that study period. It cannot predict what will happen to a particular person, compare one psychedelic with another, or establish that every exposure requires hospital treatment. Its practical message is narrower: reported cases can be medically significant, so seek advice when symptoms are concerning.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Common mistakes to avoid
- Waiting for certainty: You do not need to identify the exact drug before calling for help.
- Assuming consciousness means safety: A person may be awake while delirious, dangerously overheated or unable to protect themselves.
- Trying to force a comedown: Do not give another substance, induce vomiting or use an improvised treatment.
- Leaving someone alone: Symptoms can worsen quickly, especially when the product or combination is unknown.
- Using restraints without professional guidance: Physical struggle can cause injury and worsen agitation. Prioritize distance, hazard removal and dispatcher advice.
- Driving to seek help when the person is unstable: Call emergency services if a red flag is present so trained responders can manage transport.
- Relying on appearance or a home test: Neither packaging, color nor a casual test can establish that a product is uncontaminated.
- Hiding co-use: Withholding alcohol, medicines or other drugs can delay the right assessment. Share it confidentially with professionals.
A concise bystander checklist
- Look for abnormal breathing, unresponsiveness, seizure, extreme heat, severe agitation, delirium, rapidly worsening hallucinations or danger to self or others.
- Call the local emergency number immediately if any red flag is present.
- Remove immediate hazards and stay nearby only if doing so is safe.
- Monitor responsiveness and breathing; use CPR or the recovery position only as directed by emergency professionals or established first-aid guidance.
- Do not induce vomiting or give food or drink when poisoning is suspected.
- Provide responders with the substance information, timing, co-use and medical history you can obtain safely.
- If no red flag is present, contact the appropriate poison center or clinician promptly.
- Arrange follow-up for persistent perceptual, emotional or psychiatric symptoms.
There is no universal psychedelic-specific triage protocol, safe-use formula or consumer product that replaces professional assessment. The reliable decision rule is to match the speed and severity of symptoms to the appropriate local service, disclose what may have been taken, and seek help early when the situation is uncertain.
Frequently Asked Questions
What are the signs that I should call emergency services after psychedelic use?
Call immediately for trouble breathing, inability to wake, a seizure, very high temperature, severe agitation or violence, delirium or inability to respond, rapidly worsening hallucinations, sudden severe confusion, or thoughts or actions that could harm the person or someone else. One serious sign is enough; use your local emergency number.
What if the person is awake but having frightening hallucinations?
If hallucinations are rapidly worsening, accompanied by sudden confusion, severe agitation or aggression, or create a risk of harm, call emergency services. Otherwise, reduce hazards, stay with the person if safe, and contact a poison center or clinician promptly.
Do these 3 things before closing this tab:

What information should I give Poison Control or paramedics?
Provide the product or package if available, estimated amount and time, route of exposure, other drugs or alcohol, current symptoms, age and weight, and relevant medical or mental-health conditions. Do not delay an emergency call while searching for details.
Can a label, appearance or casual home test prove a psychedelic is safe?
No. Products can be counterfeit or contain unexpected substances, and visual inspection or a casual consumer test cannot rule out contamination. Tell responders when the contents are uncertain.
When is Poison Control appropriate instead of 911?
Use a poison center or clinician promptly for a concerning exposure when the person is breathing normally, responsive and has no immediate emergency sign. If breathing, consciousness, seizures, temperature, behavior or safety worsens, switch to emergency services.
Quick wins for a faster PC:

Yes. NIH describes flashbacks and hallucinogen-persisting perception disorder. Recurring or distressing perceptual, emotional or psychiatric symptoms, especially those disrupting daily life, warrant clinical assessment.
Does the poison-center statistic mean every psychedelic user faces the same risk?
No. The 2024 figure describes reported US poison-center exposures from 2012–2022, not all users or an individual probability. It shows that reported cases can be medically serious and should not be dismissed when symptoms are unusual.
The Bottom Line
Call emergency services for any serious breathing, consciousness, seizure, overheating, delirium, rapidly worsening hallucination or immediate-danger sign. If the person is stable but the exposure is concerning, contact a poison center or clinician promptly; arrange follow-up for persistent symptoms. Unknown contents, co-use and impaired judgment make early professional advice safer than guessing.
