RAG does not eliminate prompt injection. It adds another path for hostile instructions: documents, web pages, tickets, or files can enter the model’s context and influence its behavior. Reduce risk with layered controls across ingestion, storage, retrieval, prompt construction, generation, tool execution, monitoring, and incident response. Treat the model as an untrusted component: enforce identity, authorization, secrets protection, and action limits in code and infrastructure outside the model.
The practical goal is not a perfect filter. It is to limit what can enter the system, who can retrieve it, what the model can propose, what software will execute, and how quickly an operator can detect and contain a compromise.
What prompt injection means in a RAG system
OWASP defines prompt injection as a vulnerability in which crafted prompts alter an LLM’s behavior or output in unintended ways. A direct attack is placed in the user’s request. An indirect attack is placed in external content that the system later retrieves, such as an uploaded document, a compromised web page, a support ticket, or hidden text in an extracted file.

Repair Windows errors before they cause bigger problemsFix Now →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Clear out junk files and repair common Windows errorsFree Scan →RAG systems are exposed to both forms because retrieval places external material beside trusted instructions in the model context. A malicious passage can tell the model to ignore policy, disclose information, misrepresent a citation, or call a connected function. The actual impact depends on the system’s permissions and connected actions; an attack cannot automatically perform every listed outcome.

OWASP’s Retrieval-Augmented Generation (RAG) Security Cheat Sheet describes the change precisely: “RAG does not reduce risk — it redistributes it across the data pipeline, creating new attack surfaces at every stage from ingestion to generation to output.” Microsoft Learn likewise warns that “RAG prompts are vulnerable to indirect prompt injection attacks, where adversarial instructions are embedded in the retrieved documents.”
Map controls to the complete RAG pipeline
Use a control at the stage where a failure can be prevented or contained. A model instruction alone is not an enforcement boundary.
| Pipeline stage | Primary control objective | Examples of enforceable safeguards |
|---|---|---|
| Ingestion and extraction | Prevent unapproved or poisoned material from becoming retrievable | Source approval, provenance records, malware and content screening, hidden-text and Unicode inspection, quarantine, integrity manifests |
| Embedding and storage | Preserve isolation and traceability | Tenant and classification boundaries, access-control metadata, protected write paths, digest tracking, separated indexes where required |
| Retrieval | Return only content the requesting identity may use | Permission checks on every request, security trimming, query-abuse detection, rate limits, identity and query logging |
| Prompt assembly | Reduce the model’s opportunity to confuse data with instructions | Clearly separated sections, sanitized or flagged chunks, bounded context, explicit data-only handling instructions |
| Generation and tools | Limit harmful proposals and prevent unauthorized execution | Structured output validation, least-privilege tools, argument validation in code, action-specific approval, deterministic policy checks |
| Operations | Detect, investigate, and recover from attacks | Adversarial tests, telemetry, cache invalidation, document quarantine, fail-closed behavior, incident procedures |
Compare candidate controls by the stage they cover, whether enforcement happens outside the model, identity and tenant coverage, detection of hidden content, false-positive and latency costs, and observability and testability. OWASP distinguishes foundational, audit-oriented, and advanced controls; select the level that matches your data sensitivity, model agency, and regulatory exposure.
1. Control what enters the corpus
Approve sources before ingestion
Maintain an allowlist or approval workflow for repositories, feeds, uploaders, and automated connectors. Record the source, uploader or service identity, timestamp, business owner, classification, and approval decision. A document should not become searchable merely because a connector can reach it.
Inspect extracted content, not only original files
Run screening after text extraction and normalization. Look for instruction-like language aimed at the model, requests to reveal secrets, attempts to override policy, suspicious encoded strings, invisible Unicode characters, and text positioned outside the visible page area. Retain the original file and the extracted representation so an investigator can compare them.

Use an integrity manifest correctly
Keep approved document digests in a manifest protected from ordinary document writes. Verify a new or changed object against the expected digest before publishing it to retrieval. A matching digest proves consistency with that approved baseline; it does not prove that the content is safe or free of injection. Review and approve baseline changes through a separate process, quarantine mismatches, and alert the security team.
Quarantine uncertain material
Do not silently index content that fails provenance, integrity, malware, or extraction checks. Place it in a quarantine area with restricted access and a reason code. If business users need an answer while a review is pending, return a controlled unavailable result rather than silently falling back to an unvetted source.

2. Enforce authorization during retrieval
Attach permissions to every chunk
Store tenant, owner, role, classification, retention, and revocation metadata with each chunk or with an authoritative reference that can be checked at query time. Apply the requesting user’s or agent’s identity to every retrieval operation. Do not assume that permissions inherited when a document was indexed are still valid later.
Security-trim results before prompt assembly
Filter unauthorized results before they reach the model, not after generation. Use permission-aware indexes or an equivalent deterministic policy layer. For sensitive deployments, separate tenants or classifications in storage as well as in application logic. Managed identities can reduce reliance on embedded credentials, but they do not replace per-user authorization.
Handle revocation and probing
When access is revoked, remove or disable affected chunks promptly and define how already-built caches are invalidated. Normalize and inspect queries for enumeration or probing patterns, apply rate limits, and log identity and query identifiers according to your privacy and retention rules. A relevant result must still be authorized; relevance is never authorization.

3. Treat retrieved passages as untrusted data
Separate instructions from evidence
Build the prompt with distinct, labeled sections for trusted application instructions, the user request, and retrieved passages. Instruct the model to use passages as evidence and not to follow instructions contained inside them. Keep the formatting consistent so downstream logging and testing can identify which text came from which source.
Reduce hostile context
Sanitize or flag suspicious chunks before prompt assembly. Set limits on the number of chunks, total characters or tokens, and per-source contribution. Boundaries prevent one noisy or adversarial source from flooding the context, while relevance thresholds reduce irrelevant material that could create additional attack surface.
Understand the limits of delimiters and filters
Delimiters, XML or JSON wrappers, system instructions, pattern filters, and a second “guardrail” model can reduce exposure, but none is a hard security boundary. Novel, disguised, multilingual, encoded, or context-dependent attacks may evade a filter. OWASP notes that fool-proof prevention is not established; use these measures as layers around deterministic authorization and execution controls.
4. Keep secrets and policy enforcement out of prompts
Never place passwords, API keys, connection strings, signing material, or other secrets in a system prompt or retrieved document. Use a secret manager and pass only the minimum non-sensitive context needed for a specific operation.
Do not rely on a prompt to enforce privilege separation, data-access policy, retention rules, or approval requirements. Implement those rules in auditable code and infrastructure that can deny a request even when the model produces a convincing explanation.

Constrain agent tools
- Give each tool the smallest permission set and narrowest network scope it needs.
- Validate every proposed argument in execution code: type, range, target, tenant, ownership, and current authorization.
- Use allowlists for destinations and operations where possible.
- Require explicit, action-specific human approval for destructive, financial, external-communication, or irreversible actions.
- Make execution idempotent or provide a dry-run path so a rejected or repeated request cannot create duplicate side effects.
Guardrail models can help classify or flag requests, but OWASP warns that they are also susceptible to manipulation. They should supplement, not replace, least privilege, deterministic checks, and human approval.
5. Validate generated output before downstream use
Prefer constrained, typed outputs
Require a defined schema for machine-consumed responses. Reject malformed, incomplete, or extra fields before passing data to another service. Validate enumerations, numeric bounds, destination identifiers, and references against current application state.
Check content against policy
Apply deterministic checks for prohibited disclosures, cross-tenant references, unsupported actions, and missing evidence. If the response must cite retrieved material, verify that cited identifiers actually belong to authorized chunks and that attribution was not altered during generation.
Fail closed when checks cannot run
If authorization, schema validation, provenance lookup, or tool-argument validation is unavailable, do not execute the operation or present the result as trusted. Return a controlled error, queue the request for review, or provide a non-actionable response.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problems6. Monitor the system and prepare recovery
Capture useful security telemetry
Record enough information to reconstruct an event: source and document identifiers, versions or digests, tenant and identity, retrieval query identifier, selected chunks, prompt-template version, model and tool decisions, validation outcomes, and final action. Apply data minimization, access controls, retention limits, and redaction so telemetry does not become a second data-leak channel.

Define containment actions
Document who can disable a source, quarantine a document, revoke an identity, disable a tool, invalidate affected caches, and roll back an index. Include a procedure for identifying answers generated from poisoned material and notifying affected owners. Keep the ability to stop downstream actions independently of the model service.
Watch for signals of indirect injection
- Sudden requests for secrets or system prompts.
- Instructions to ignore application policy or change the conversation role.
- Unexpected tool calls, unusual destinations, or arguments outside normal ranges.
- Cross-tenant retrieval attempts or repeated probing queries.
- Large changes in a source’s text, digest, language, encoding, or retrieval frequency.
7. Test attacks before and after every material change
Build an adversarial test harness into development and release work. Run tests against the actual model, prompt template, retriever, permissions, tools, and data transformations used in production; results from one configuration do not establish safety for another.
Minimum test scenarios
- Place an instruction in a document that tells the model to disclose a secret or ignore policy, then verify that it is treated as data.
- Hide the same instruction with invisible characters, unusual Unicode, encoding, OCR artifacts, or HTML positioning and verify screening and model behavior.
- Attempt cross-tenant retrieval with a valid-looking query and confirm that unauthorized chunks never enter the prompt.
- Revoke a document permission and confirm that retrieval and cached results stop honoring the old access.
- Poison a document after approval and verify digest detection, quarantine, alerting, and cache invalidation.
- Ask the model to invoke a tool with an unauthorized target, malformed argument, excessive quantity, or destructive operation and confirm execution-code rejection.
- Tamper with citations or source identifiers and verify attribution checks.
- Simulate unavailable policy or authorization services and confirm fail-closed behavior.
Retain test cases as regression tests. Re-run them after changing the model, embedding model, chunking, retriever, prompt template, tool permissions, parser, or corpus.

What the published attack statistic does—and does not—show
OWASP’s Prompt Injection Prevention Cheat Sheet reports that Hughes et al. (2024) observed 89% attack success on GPT-4o and 78% on Claude 3.5 Sonnet, using up to 10,000 augmented prompts per request. These are results for the tested models and configurations, not a universal probability that any RAG request will be compromised. They should motivate architecture-level testing rather than be used to predict performance in a different corpus, prompt, tool set, or model version.
