A standing desk should let you change position, not force you to stand still for hours. The most effective approach is to alternate sitting and standing, set the desk and monitor for a neutral posture, keep frequently used items close, and use supportive footwear plus a stable, non-slip mat when the floor is hard. A footrest or sit-stand stool gives your legs another position instead of making continuous standing the default.
Why standing at a desk can hurt
Standing is not automatically healthier than sitting when it is prolonged and static. The Canadian Centre for Occupational Health and Safety (CCOHS) links regular prolonged standing with sore feet, leg swelling, muscular fatigue and low-back pain. The cause is usually a combination of work design, footwear and flooring rather than one defective product.
Static loading reduces recovery
When you stand in one position, the same muscles support your body continuously. Blood flow to loaded muscles is reduced, fatigue accumulates and the feet and calves absorb pressure without a chance to recover. A restricted workstation can make this worse if the keyboard, mouse or documents force you to stay in one stance.
Reach and twisting transfer stress upward
A desk that is too high makes you lift your shoulders or bend your wrists. A desk that is too low encourages trunk flexion. A monitor or frequently used item placed to the side or behind you can cause repeated twisting and overreaching. Those compensations can turn a foot or calf problem into hip or low-back discomfort.

Set the desk for a neutral standing posture
Adjust the work surface
Raise or lower the surface until your hands, wrists and forearms can remain straight and approximately parallel to the floor while your elbows stay near your sides. You should not need to shrug, reach upward or bend your wrists sharply to use the keyboard and mouse. Make the adjustment while wearing the shoes you normally use for work; thick soles change your effective height.
- Stand close enough that your elbows can remain near your torso.
- Set the keyboard and mouse at the height where your forearms are level and your wrists are not cocked upward or downward.
- Keep the mouse beside the keyboard rather than far forward or to the side.
- Place the monitor directly in front of you so your head and torso do not rotate during normal work.
- Recheck the position after adding a mat, footrest or anti-slip flooring, because each changes the distance between your feet and the surface.
Repeat the setup at sitting height
A sit-stand desk needs two usable positions. Lower the surface, or adjust the chair, so your forearms remain in the same neutral relationship when seated. If one position is comfortable and the other requires bent wrists or raised shoulders, you will tend to avoid changing position and lose the benefit of alternating.
Keep the reach zone compact
Put the keyboard, mouse, telephone, notebook and other frequently used items within easy reach. Move your feet and turn your whole body when you need something farther away; do not repeatedly twist behind your shoulder line. Store occasional-use equipment farther away only if retrieving it does not require an awkward lean.

Check clearance before adding equipment
A footrest or stool must fit under or beside the desk without forcing your knees, feet or chair into a confined space. Cable trays, computer towers and drawers can block a stool or prevent you from bringing your feet into a comfortable position. Clear the area before deciding that a product is incompatible with your desk.
Alternate positions instead of standing continuously
CCOHS identifies the key factor as alternating your position as needed. There is no universally correct standing-to-sitting timer. Suggested intervals reported in occupational guidance range from short standing periods to 20, 30, 45 or 60 minutes. Treat those figures as examples for experimenting, not as a prescription that everyone must follow.
| Situation | Practical starting pattern | How to adjust it |
|---|---|---|
| New to standing work | Use short standing periods separated by sitting. | Shorten the standing block if feet or calves become painful; increase only when the change feels easy for several days. |
| Comfortable with alternating | Try a 20-, 30-, 45- or 60-minute block as one possible interval. | Change position before discomfort becomes intense rather than waiting for a timer to force you. |
| Long meetings or focused tasks | Keep the chair or stool available and sit when symptoms begin. | Use a footrest or shift weight while standing, then walk during the next break. |
Use symptoms and task demands to decide when to switch. A timer can remind you to check your position, but it should not keep you standing through pain. Alternate before your posture deteriorates, and vary the pattern across the day instead of repeating one long standing block.

Walk away from the workstation
OSHA advises standing up and walking around for a few minutes periodically. A walk to refill water, collect a document or take a brief break away from the screen changes the loading on your feet and legs more effectively than simply locking your knees in a different place. Schedule these brief walks around natural task transitions.
Use a third posture
CCOHS states: “A workplace that includes an optional seat (chair, sit/stand stool) and some kind of footrest increases the variety of body positions and encourages frequent changes between them.” A stool lets you unload your legs without dropping fully into a low chair, while a footrest or foot rail lets you alternate which leg carries more weight. Neither replaces walking or normal sitting; both reduce the demand for one fixed stance.
Choose footwear that supports standing work
Fit the shoe to the natural foot shape
CCOHS recommends footwear that allows the natural shape of the foot, holds the heel securely, leaves room for the toes, and provides arch support and cushioning. The shoe should not squeeze the forefoot or let your heel slide with every step. Lace-up fastening can help you secure the heel and adjust the fit when appropriate.

- Toe room: Your toes should be able to spread without pressing against the sides or front.
- Heel security: The heel should stay seated rather than lifting repeatedly as you walk.
- Arch support: The shoe should support your arch without creating a hard pressure point.
- Cushioning: Padding should reduce impact but still let you feel stable.
- Breathability: Materials that manage heat and moisture can make a long shift more tolerable.
- Task safety: Follow workplace requirements for slip resistance, protective toes or other hazards; comfort cannot substitute for required safety features.
Consider insoles on hard floors
On concrete, tile or other hard surfaces, shock-absorbing insoles or shoes with thicker insulating soles can reduce impact. An insole should fit the shoe without crowding the toes or lifting the heel out of its counter. Replace it if it becomes compressed or causes new pressure, rather than assuming a thicker insert is always better.
Do not use comfort as proof of a correct fit
A very soft shoe can feel pleasant for a few minutes while allowing the foot to roll or the heel to move excessively. Judge footwear after ordinary walking and standing, including turns and short trips away from the desk. If discomfort remains localized, one shoe may not suit your foot shape even if the other feels fine.

Use an anti-fatigue mat safely
An anti-fatigue mat can cushion hard flooring and reduce foot fatigue, but it does not replace movement, footwear or a usable sitting option. The wrong mat can introduce a trip hazard or make you feel unstable.

| Feature | What to look for | Why it matters |
|---|---|---|
| Cushioning | Enough give to reduce hard-floor impact without feeling spongy. | Excessively soft foam can make balance and weight shifting harder. |
| Stability | A mat that lies flat and stays in place under normal foot movement. | Sliding or curling edges increase trip risk. |
| Edges | Sloped or beveled edges. | Beveled transitions are less likely to catch a toe or chair wheel. |
| Surface grip | Non-slip top and underside suited to your floor. | Grip supports safe stepping and prevents the mat from migrating. |
| Size | Large enough for your normal stance, footrest and small steps. | A mat that is too small encourages stepping off the cushion or standing on an edge. |
| Cleaning | Material and finish appropriate for spills and the surrounding environment. | Regular cleaning preserves traction and prevents buildup at the edges. |
Install and inspect the mat
- Remove dust and debris from the floor before placing the mat.
- Lay it completely flat, with all edges visible and away from door swings or walkways.
- Check that a chair, stool and footrest can move without catching the edge.
- Test stepping on and off it in normal shoes; remove it if it slides, curls or feels unstable.
- Clean it according to its material and inspect the edges regularly.
Do not use a mat to justify longer standing blocks. Its role is to make a sound setup more forgiving, not to eliminate the need to sit, walk and change weight distribution.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Add a footrest, foot rail or sit-stand stool
Weight shifting is a simple way to avoid loading both legs identically for hours. A footrest or rail gives one foot a supported position; switching sides regularly changes the angle at the ankle, knee and hip. Keep the support stable and low enough that you do not have to lift one knee sharply or lean your trunk.
| Option | Useful when | Checks before use |
|---|---|---|
| Footrest | You want to shift weight while remaining upright. | It should resist sliding, have enough surface for the whole foot and fit beneath the desk. |
| Foot rail | You prefer a fixed support attached to the desk or workstation. | It must not obstruct chair clearance, cables or walking paths. |
| Sit-stand stool | You need frequent partial sitting during computer work. | Check height range, base stability and whether your feet can still rest securely. |
Do not perch on a stool so high that your feet dangle, or lean against a footrest while your shoulders reach forward. Adjust the support until your torso remains upright and your work surface still meets the neutral posture requirements.
Troubleshoot discomfort by symptom
Sore soles or aching arches
- Shorten the next standing block and sit or walk briefly.
- Check toe room, heel security and arch support in your shoes.
- Inspect whether the mat is too hard, too soft or shifting.
- Try a shock-absorbing insole on a hard floor only if it does not crowd the shoe.
Calf tightness or leg swelling
- Stop holding one rigid stance; alternate feet on a stable footrest.
- Walk for a few minutes instead of remaining in place.
- Check that the desk is not so high that you stand on your toes or brace through your calves.
- Use sitting as a normal part of the work cycle, not as a failure of the standing setup.
Low-back discomfort
- Move the keyboard and mouse closer so you do not lean forward.
- Center the monitor and reduce repeated rotation.
- Check that the desk height allows relaxed shoulders and level forearms.
- Change position and walk before discomfort escalates.
Hip or knee discomfort
- Vary the stance width and avoid locking the knees.
- Use a stool or chair for part of the task.
- Confirm that a footrest does not force one leg into an extreme angle.
- Recheck footwear and floor hardness, which can alter how you load the joints.
What the evidence says about back relief
A systematic review and meta-analysis of sit-stand workstations, covering literature available through 17 November 2016 and published in Ergonomics in 2018, found a standardized mean difference of -0.230 for low-back discomfort in pain-free populations. On a 10-point pain scale, effect estimates ranged from -0.30 to -0.51. The authors concluded that sit-stand workstations may reduce low-back pain, while noting that more research is needed to establish the right dosage and effects on other outcomes.
These are average group findings, not a guarantee for one person. They also concern sit-stand workstation interventions rather than a promise that standing continuously will relieve pain. If a desk change causes persistent or worsening symptoms, stop treating it as an equipment problem and obtain individual medical or ergonomic advice.
A practical day-to-day routine
- Before work: Put on the shoes you will actually wear, flatten and inspect the mat, and clear the area around the desk.
- Set standing height: Align the keyboard and mouse with straight wrists and roughly level forearms; center the monitor.
- Set sitting height: Lower the desk or adjust the chair so the same neutral arm position is possible while seated.
- Begin with a short standing period: Switch to sitting before your feet, legs or back become painful.
- Change position: Use the chair or stool, place one foot on the footrest, or walk briefly instead of holding the same stance.
- Repeat through the day: Use a timer only as a reminder to assess your posture and symptoms. The interval can be short, 20, 30, 45 or 60 minutes, depending on what you tolerate.
- Review the setup: If discomfort appears in a new area, first check reach distance, desk height, footwear and mat stability before adding another product.
When to get individual help
Equipment adjustments are reasonable for ordinary fatigue that improves when you sit, walk or change position. Seek clinical advice for severe pain, pain after an injury, persistent or worsening symptoms, marked one-sided swelling, numbness, weakness, or symptoms that interfere with normal walking or sleep. A qualified ergonomist or clinician can assess factors that a desk-height change cannot address.
