Reduce false positives by treating an automated flag as a review lead—not a confirmed defect—and then improving the evidence, routing, sampling, and governance around that lead. Keep a representative random post-closing sample separate from risk-targeted discretionary reviews, record how reviewers resolve every alert, investigate noisy rules and source data, and validate any change against representative loans before deployment. Fannie Mae’s requirements support this control loop, but they do not prescribe a particular machine-learning model, threshold, or false-positive target.

What a false positive is—and what it is not

In mortgage quality control, a false positive is an automated alert that does not represent a substantiated eligibility, compliance, documentation, collateral, fraud, or process defect after a qualified reviewer examines the file. The alert may have been triggered by an extraction error, an outdated reference value, conflicting documents, a duplicate rule, or a legitimate exception that the system cannot interpret.

An alert is not the same as a defect. A defect exists only after the lender applies its written QC criteria, reviews the available evidence, and records a disposition. This distinction protects two objectives that can otherwise conflict: reducing unnecessary work and preserving detection of loans that are ineligible as delivered or otherwise materially deficient.

Disposition Meaning Control treatment
Confirmed defect Evidence supports a defect under the lender’s eligibility, compliance, or QC policy. Classify severity, assign ownership, and include it in defect-rate and corrective-action reporting.
Explained discrepancy The apparent mismatch is resolved by an authoritative document, permitted exception, or documented business context. Clear the alert with the evidence and rationale retained.
Low-confidence extraction The system’s value is uncertain because text, handwriting, image quality, or field mapping is unreliable. Send the source document for human verification before downstream rules create additional alerts.
Conflicting or stale source Two sources disagree, or the reference data no longer reflects the applicable policy or transaction date. Identify the controlling source, date the resolution, and correct the data feed or rule if needed.
Non-material issue A real variance exists but does not meet the lender’s defect definition or severity threshold. Record the rationale; do not inflate the defect rate with issues outside the written taxonomy.

The suggested subcategories above are implementation choices. Fannie Mae requires lenders to identify loans with defects and categorize them by severity; its highest severity applies to defects that make a loan ineligible as delivered. Your policy should map local categories to that requirement without allowing an automated score to make the final determination.

Keep the quality measurement representative

The most important protection against “fixing” false positives by hiding real defects is preserving the sample that measures loan quality. Fannie Mae states that post-closing QC must include both random and discretionary selections. Its guide says, “The random sample is a critical measure of loan quality,” and that “Discretionary sample selections supplement, but do not replace a lender’s random sample.”

Random sampling for the lender-wide defect rate

Use a random sample that represents the lender’s book rather than the files an algorithm already considers risky. Fannie Mae describes two options:

Option Requirement Important qualification
Percentage sample Select at least 10% of monthly production at random. If 10% is less than one loan, select at least one loan. The sample should represent loan types, branch offices, manually underwritten loans, and loans processed through automated underwriting systems.
Statistical sample Use a statistical model with at least a 95% confidence level and 2% precision rate. Fannie Mae describes a six-month statistical statement for this option; document the model and its assumptions.

Random-sample post-closing reviews are full-file reviews. Select loans at least monthly and complete selection, review, rebuttal, and reporting within 90 days from the month of disbursement or acquisition. Do not replace this sample with only loans that produce automated alerts, loans from a high-loss channel, or cases chosen by an analyst after seeing performance data. Those approaches can be valuable for investigation, but they cannot provide an unbiased lender-wide defect estimate.

Discretionary sampling for concentrated risk

Use discretionary selections to investigate areas with greater potential for error or misrepresentation. Examples include a newly launched product, a changed production channel, a branch with unusual findings, a third-party originator, a manual-underwriting population, or a segment whose payment performance has deteriorated. Revisit the risk assessment as products, channels, volume, controls, and loan performance change.

Keep the two streams visibly separate in your data model and reports. A risk-enriched sample will usually have a higher finding rate; presenting that rate as the lender’s overall defect rate overstates the prevalence of defects. Use the random sample to set the target defect rate and the discretionary sample to explain, challenge, or remediate specific risks.

A seven-step workflow for reducing unsupported alerts

1. Define the defect taxonomy before tuning a rule

Write a decision standard that states what evidence confirms a defect, what severity levels mean, and which exceptions are allowed. Include the authoritative document or data source for each material field. A rule that checks debt, income, occupancy, appraisal, or closing data should identify the policy version and transaction date it uses.

Give reviewers a disposition vocabulary that distinguishes confirmed defect, explained discrepancy, low-confidence extraction, stale or conflicting source, non-material issue, and duplicate alert. These labels make a high cleared-alert rate actionable. Without them, “cleared” can combine a legitimate exception, a data-quality failure, and a reviewer disagreement that require different fixes.

2. Preserve the evidence behind every flag

For each alert, store the source document or data record, the extracted value, the triggering value or comparison, the rule or model version, the threshold in force, and the timestamp. Capture the reviewer’s disposition, severity, explanation, and links or identifiers for corroborating evidence. Preserve the original input rather than only the normalized value so a later reviewer can determine whether the problem was in the document, extraction, mapping, or rule.

This traceability is an operating recommendation, not a claim that Fannie Mae mandates a specific database schema. Fannie Mae does require accurate QC records and an independent audit process that assesses whether procedures are followed and conclusions are applied consistently. A complete alert record lets an auditor reproduce the decision and lets engineering compare outcomes before and after a change.

3. Route by confidence and potential impact

Do not let an uncertain extraction cascade into a chain of high-confidence-looking alerts. Route low-confidence values and conflicting evidence to human verification first. A reviewer can then correct the source value once, preventing multiple derivative findings from consuming separate queues.

Route eligibility-sensitive, fraud-related, collateral, and compliance alerts for explicit reviewer attention even when the model confidence is high. Conversely, an explained low-severity discrepancy can be cleared with a documented rationale. This routing design must be validated against the lender’s own files, error costs, and policy; it does not replace the required full-file random reviews.

4. Separate measurement from targeted investigation

Tag every file with its selection basis: random, discretionary, or both. Calculate the lender’s target defect rate from the random post-closing sample. Use discretionary findings to prioritize root-cause work, not to silently alter the denominator of the representative measurement.

When a file belongs to both streams, retain both tags and define in advance how it is reported. A loan cannot be allowed to disappear from the random sample merely because it also triggered a targeted review.

5. Track alert quality at the rule and data-source level

At minimum, report the following measures by alert family, product, channel, and relevant time period:

Measure Calculation or definition Use
Confirmed-defect rate Confirmed defects divided by reviewed alerts in the defined population. Shows how often an alert identifies a substantiated issue.
Cleared-alert rate Alerts cleared as explained, non-material, duplicate, or unsupported divided by reviewed alerts. Highlights review burden, but requires the detailed disposition labels to be meaningful.
Severity mix Distribution of confirmed findings by the lender’s severity categories. Separates many minor findings from a smaller number of eligibility-sensitive defects.
Reviewer disagreement Cases requiring a second review or producing different initial and final dispositions. Identifies ambiguous policy language, weak evidence, or inadequate training.
Post-change comparison Outcomes before and after a rule, threshold, extraction, or source-data update. Tests whether fewer alerts came with an increase in missed or late-discovered defects.

These are diagnostic measures, not Fannie Mae performance targets. Set the lender’s target defect rate using the random QC sample, document the rationale, measure against it at least quarterly, and review the target at least annually as required by Fannie Mae guidance. Management reporting on prefunding and post-closing findings is required at least monthly.

6. Investigate the cause before changing a threshold

A high cleared-alert rate is a symptom, not a tuning instruction. Examine whether the alerts arise from:

  • poor image quality, OCR errors, or field-mapping changes;
  • duplicate rules that evaluate the same condition in different forms;
  • stale rates, limits, investor overlays, or policy reference tables;
  • legitimate exceptions that are absent from the rule’s decision logic;
  • documents that are authoritative for one purpose but inappropriate for another; or
  • a business rule that no longer matches the product or channel.

Assign an owner and expected resolution date to each recurring cause. If the issue is a data feed, fix the feed rather than weakening every downstream rule. If the policy is ambiguous, obtain a written interpretation and update reviewer guidance before changing code.

7. Validate the trade-off and close the loop

Before deployment, replay the proposed change on historical files with known reviewer outcomes. Compare confirmed defects, cleared alerts, severity, reviewer disagreement, and processing time. Include files from the representative random sample and from relevant discretionary populations; a change that looks safe only in a risk-enriched set is not sufficiently validated.

After deployment, monitor for both fewer unsupported alerts and missed defects. Establish a rollback trigger, an effective date, and an owner for post-release review. Document corrective actions, expected resolution, and implementation timing when trends appear. Report the result to management and feed material changes into the next independent audit.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Evidence and review design that auditors can follow

A reviewer should be able to open one alert and answer five questions without reconstructing the entire system:

  1. What source document or data value triggered the alert?
  2. Which rule or model version evaluated it, and what threshold or policy date applied?
  3. What did the reviewer see in the file, including corroborating or contradictory evidence?
  4. Was the outcome a confirmed defect, and what severity and defect type apply?
  5. Who approved the disposition, when was it recorded, and what corrective action follows if the finding is part of a trend?

Keep reviewer notes factual and specific. “No issue” is not a sufficient rationale for a material alert; identify the document, field, exception, or calculation that resolves it. When a reviewer cannot establish the answer, leave the case in an evidence-needed state rather than forcing a clean disposition that will distort alert-quality metrics.

Governance, reporting, and retention

Management reporting

Provide management with at least monthly prefunding and post-closing findings, trend explanations, open corrective actions, and changes in sample composition. Show random-sample defect rates separately from discretionary findings and from raw automated-alert volumes. Include denominator, review completion date, severity distribution, and any material backlog so a falling alert count is not mistaken for better quality.

Corrective-action records

For each material trend, record the problem statement, affected products or channels, root-cause hypothesis, containment action, permanent fix, accountable owner, expected resolution date, validation method, and closure evidence. Reopen the action if post-change monitoring shows a rise in missed defects or a new concentration of unsupported alerts.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Vendor and independent-audit oversight

If a vendor performs QC reviews, extraction, scoring, or workflow administration, monitor its work rather than treating the service output as self-validating. Sample vendor dispositions, compare them with internal reviewers, verify that rebuttals and corrective actions are retained, and require access to the evidence and version history needed for an audit.

Fannie Mae’s current guide requires lenders to retain written and electronic QC review records for at least three years. Retain reports, reverification documentation, rebuttals, corrective actions, rule versions, and evidence of management review for the same period or longer where another applicable requirement demands it. An independent audit should test whether the written QC plan was followed, whether conclusions were recorded, and whether reviewers applied the standards consistently.

Do not overextend the automated valuation model rule

The interagency automated valuation model (AVM) quality-control rule is narrower than “automated mortgage QC.” Effective October 1, 2025, it applies to covered mortgage originators and secondary-market issuers using AVMs in specified credit decisions or securitization determinations to estimate collateral value. Its five control objectives address confidence in estimates, protection against data manipulation, avoidance of conflicts of interest, random sample testing and review, and compliance with nondiscrimination laws.

The scope provisions exclude AVM use for monitoring mortgage or mortgage-backed-security quality or performance, reviewing completed collateral value determinations, and developing an appraisal by a certified or licensed appraiser under the Federal Reserve’s specified scope. Therefore, do not cite the AVM rule as a blanket requirement for every underwriting, document, fraud, or post-closing QC model. Apply the rule when the lender’s use case falls within its covered valuation activity; apply the lender’s broader QC plan and applicable investor requirements to other automated controls.

Evaluation axis Questions to answer
Measurement integrity Does the design preserve the representative random sample and keep it distinct from targeted discretionary review?
Review burden Does it resolve low-confidence values early and prevent duplicate or derivative alerts from multiplying work?
Missed-defect risk Are proposed changes checked against confirmed defects and monitored for false negatives as well as cleared alerts?
Explainability and evidence Can a reviewer see source data, triggering values, rule or model version, threshold, and disposition rationale?
Adaptability Can risk targeting and rules be updated as products, channels, policy, volume, and performance change?
Governance and audit Can management measure trends, assign corrective actions, oversee vendors, retain records, and demonstrate consistent application?
Integration and workload Does the workflow fit the loan-origination and QC processes without creating unmanageable reconciliation or duplicate entry?

Ask for a controlled demonstration using de-identified files and require the supplier or internal engineering team to explain how evidence is versioned, how overrides are logged, and how a model change is validated. A capability statement is not independent proof of accuracy, savings, or agency approval.

Common failure modes and recovery actions

Optimizing for alert volume alone

Symptom: alerts fall quickly after thresholds are loosened, but confirmed defects or late discoveries rise. Recovery: restore a representative validation set, compare against known reviewer outcomes, and require a missed-defect review before approving further reductions.

Using only high-risk files to measure quality

Symptom: the reported defect rate changes whenever the risk model changes. Recovery: re-establish the random sample, document the selection basis, and report the targeted population separately.

Clearing alerts without evidence

Symptom: “no issue” dispositions dominate and auditors cannot reproduce decisions. Recovery: make source evidence and a reason code mandatory, then coach reviewers on the written taxonomy.

Changing rules without version control

Symptom: analysts cannot explain why two similar loans received different alerts. Recovery: assign immutable rule or model versions, effective dates, and rollback packages; backfill comparisons only when the historical logic is known.

Ignoring source-data drift

Symptom: a previously stable alert family develops a sudden cleared-alert spike after a document, LOS, or reference-table change. Recovery: compare raw inputs and extraction confidence before modifying the business rule.

Treating an AVM requirement as universal

Symptom: an AVM control is applied to unrelated underwriting or QC automation, or a non-AVM process is assumed compliant because it adopted one AVM control. Recovery: map the actual use case to the interagency scope and document the separate requirements that govern the other model.

A practical 90-day implementation plan

Period Work Deliverable
Days 1–30 Inventory alert families, data sources, rule versions, selection bases, reviewer dispositions, and existing defect categories. Confirm the random-sample method and 90-day completion clock. Approved taxonomy, evidence checklist, sample map, and baseline metrics.
Days 31–60 Separate random and discretionary reporting; add confidence and impact routing; analyze cleared-alert and disagreement patterns; select one or two noisy causes for controlled remediation. Prioritized corrective-action plans and a historical validation set with known outcomes.
Days 61–90 Replay proposed rule or data changes, review false-negative risk, obtain governance approval, deploy with version control, and schedule post-release monitoring. Change record, rollback criteria, management report, and independent-audit evidence package.

At the end of the cycle, the success test is not simply fewer alerts. It is a documented reduction in unsupported review work while the representative defect-rate measurement remains intact, confirmed defects continue to surface at the expected severity, and every material disposition can be explained from retained evidence.
