Reduce false positives by finding out exactly why a legitimate message was quarantined or blocked, correcting the underlying authentication, reputation, content, or routing problem, and reporting the confirmed misclassification. In Microsoft 365, use message trace and the available security investigation views before changing policy. Keep any allow entry narrow, temporary, documented, and reversible: Microsoft warns that broad allowlists can deliver malicious mail.
What a false positive is—and why the diagnosis matters
A false positive occurs when an email that should be delivered is classified as spam, phishing, malware, impersonation, or another threat. AI-assisted filters combine many signals, so the same visible symptom—“legitimate email flagged as spam”—can result from very different causes. A failed SPF check calls for DNS or sender configuration work; an impersonation detection calls for identity and policy review; a connector problem calls for mail-flow analysis. Treating every event with an allowlist entry removes the symptom without fixing the signal that caused it.
The procedures below are centered on Microsoft 365 and its Defender investigation and policy controls. Labels, controls, and workflows differ across vendors, so do not assume that a Microsoft setting or threshold has the same meaning elsewhere.
Microsoft’s anti-spam FAQ, updated August 12, 2026, states that failed SPF, DKIM, or DMARC checks and poor sender reputation are common reasons legitimate mail is classified as spam. It also notes that blocking legitimate email in significant volume is unacceptable, while some false positives are inevitable. Your goal is therefore controlled reduction, not a promise of zero false positives.

Use a repeatable investigation sequence
1. Establish exactly what happened
Start with one message and record the sender, recipient, visible From address, sending time, subject, message ID if available, and the final disposition. Distinguish among delivered to Junk Email, quarantined, rejected during SMTP, or delivered after an override. The disposition determines which evidence is available and which control may have acted.
In Microsoft 365, run Message trace from the Exchange admin center. Trace identifies the mail-flow events and can show which transport or filtering policy affected the message. For organizations with the required Defender capability, open Threat Explorer or Real-time detections to inspect the detection technology, delivery action, and any override. A bulk-mail threshold, authentication failure, impersonation rule, and custom transport rule require different remediation.
- Capture the detection name or technology, not just the folder where the message ended up.
- Check whether an administrator or user override changed the final action.
- Compare a blocked message with a similar message that was delivered, if one exists.
- Preserve the original headers when possible; forwarding a message can change the evidence.
2. Classify the likely cause
Use the evidence to place the event in one or more of five categories:
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
- Authentication: SPF, DKIM, or DMARC failed, or the result did not align with the visible From domain.
- Reputation: the sending IP or domain has a poor or uncertain reputation, unusual volume, blocklist exposure, or missing reverse-DNS hygiene.
- Content: links, attachments, wording, formatting, or sending behavior resembles bulk mail, phishing, or malware.
- Mail flow: a gateway, forwarder, connector, or routing change altered how source IP and authentication results were interpreted.
- Policy threshold: a trusted-sender setting, anti-phishing threshold, impersonation policy, or custom rule produced the action.
Do not change a policy until you can explain which category the evidence supports. If the trace shows several detections, address the strongest or most security-relevant signal first.
Correct authentication before weakening filtering
Account for every legitimate sender in SPF
List every service that sends mail using your domain: Microsoft 365, marketing platforms, ticketing systems, payroll services, applications, and third-party relays. An SPF record that omits a legitimate sender can cause SPF failure even when the message is genuine. Coordinate changes with the owner of each service and avoid creating multiple SPF records for one domain.
After updating DNS, allow for normal DNS propagation and verify the result in a trace or header from a newly sent message. An SPF pass alone does not guarantee DMARC alignment or delivery; it is one part of the authentication chain.

Enable DKIM for each sending system
DKIM adds a cryptographic signature that receiving systems can validate. Configure signing for Microsoft 365 and for every external platform that sends with your domain in the visible From address. Check that the signing domain is aligned with that From domain for DMARC purposes. A message can have a valid DKIM signature from a service-owned domain and still fail DMARC alignment if the domains do not match under the policy’s alignment mode.
Use DMARC only after SPF and DKIM are working
Microsoft’s DMARC setup guidance recommends configuring SPF and DKIM before publishing an enforcement policy. DMARC passes when at least one aligned SPF or aligned DKIM check succeeds. Alignment is evaluated against the domain shown to the recipient in the From header, not merely against a hidden envelope sender or an unrelated signing domain.
Begin with a monitoring policy so reports reveal legitimate senders and failures. Review unexpected sources, investigate failures, and correct them before increasing enforcement. Microsoft describes staged changes that increase the percentage of mail affected and move toward quarantine or reject after the results are understood. Those increments are examples from Microsoft’s documentation, not a universal schedule; choose intervals that give your organization enough time to observe recurring senders and business-critical campaigns.
Read DMARC reports as operational evidence
Aggregate reports show which IP addresses send for your domain and how SPF, DKIM, and alignment perform. Use them to find forgotten applications, shadow IT, forwarding paths, and unauthorized senders. Reporting services can make large volumes easier to interpret, but they do not replace correcting DNS, signing, or sender configuration. During a staged policy change, compare report data with help-desk complaints and message-trace results before advancing the policy.
Investigate reputation, content, and sending behavior
Reputation problems often belong to the sender
If authentication passes but the message is still classified as spam, inspect the sending IP and domain reputation. Evidence may include a blocklist listing, missing or inconsistent reverse DNS, a newly established domain, abrupt volume changes, or a low-volume sender with little history. The sender may need to correct its mail-server identity, remove compromised accounts, reduce bursts, or work with its hosting provider. An allow entry in your tenant does not repair reputation for other recipients and can hide a compromise.

Review URLs, attachment types, redirect chains, HTML structure, display names, and calls to urgent action. Legitimate newsletters and automated notifications can resemble bulk or phishing mail when they use shortened links, image-only layouts, or a display name that imitates a trusted person. Ask the sender to use a stable branded domain, consistent From address, valid unsubscribe information for bulk mail, and links that resolve directly to the expected domain. Do not ask a sender to remove security-relevant controls merely to satisfy one recipient’s filter.

Check forwarding and connector topology
Forwarding and third-party gateways can change the apparent source IP and affect how authentication results are evaluated. If mail enters Microsoft 365 through another service, document the order of gateways, connectors, and mailbox delivery. Confirm whether Enhanced Filtering for Connectors applies to your topology. Microsoft documents this feature for supported arrangements to improve interpretation of the original sender and reduce false positives caused by an intermediary. Applicability depends on the actual connector design; verify the current Microsoft guidance before changing it.
Choose remediation by cause, scope, and risk
| Remediation | Use when evidence shows | Security and operational trade-off |
|---|---|---|
| Fix SPF, DKIM, or DMARC | Authentication failed or did not align with the visible From domain | Addresses the root cause and improves delivery for other recipients; requires coordination with every sending system |
| Correct reputation or content | Authentication is sound but IP, domain, volume, links, attachments, or wording trigger filtering | Preserves filtering strength; the sender may need changes outside your tenant |
| Adjust an anti-phishing threshold or trusted sender/domain setting | Trace identifies a specific policy threshold or trusted-entity decision | Can reduce targeted false positives, but broad trust reduces protection and must be monitored |
| Tenant Allow/Block List entry | A legitimate message remains blocked while root-cause work or analysis is underway | Fast and reversible when narrow; an overly broad entry can deliver malicious mail |
| Connector or Enhanced Filtering review | A gateway or forwarding path changes source-IP or authentication interpretation | Can correct systemic routing errors; an incorrect connector change can affect all mail flow |
For every option, document the cause, affected sender or messages, owner, creation date, expiration or review date, expected benefit, and rollback method. A change is successful only if legitimate mail is restored without increasing successful delivery of malicious mail.
Use Microsoft 365 exceptions as temporary risk decisions
Microsoft recommends correcting the root cause and submitting the good message for analysis before relying on an allow entry. If an exception is necessary while investigation continues, use the Tenant Allow/Block List and choose the narrowest practical scope: a specific sender, message indicator, or URL rather than an entire domain or a mail-flow bypass.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitchesDo not assume an allow entry overrides every protection. Microsoft notes that malware and high-confidence phishing protections may still apply. Conversely, an allow entry can let a harmful message through when the message resembles the approved pattern. Review whether the sender’s account, domain, or infrastructure could be compromised before granting trust.
- Prefer a single known sender or narrowly defined indicator over a whole organization domain.
- Set an owner and review date; remove the exception when the sender fixes authentication or content.
- Record the detection that justified the exception and the messages it is intended to affect.
- Test both a known-good message and a controlled malicious simulation after the change, using your organization’s approved testing process.
Tune anti-phishing policies carefully
Review trusted senders and domains
Microsoft’s anti-phishing policy controls include trusted senders and trusted domains. Use these settings only when you understand why the normal detection is firing and why the entity is safe for the defined scope. A domain used by a large provider, a shared cloud service, or a partner with delegated accounts may be too broad to trust as a whole.
Change thresholds in small steps
Phishing thresholds can be adjusted in Microsoft’s anti-phishing policies. Tie a threshold change to a demonstrated detection pattern, make one change at a time, and record the previous value. After deployment, inspect subsequent detections for both legitimate and malicious messages. If you cannot tell which change altered the outcome, too many controls were changed at once.

Do not confuse delivery improvement with protection improvement
A policy that delivers every disputed message may appear successful in a short test while silently increasing exposure. Monitor false negatives—malicious messages that were delivered—as well as false positives. Microsoft’s tuning guidance emphasizes reviewing both outcomes. Keep a rollback path and involve the incident-response owner when a change affects impersonation or high-confidence phishing controls.
Submit confirmed false positives and monitor the result
Use Microsoft’s reporting workflow for a confirmed false positive. Include the original message or headers when the workflow requests them, identify the recipient and disposition, and describe why the message is legitimate. Reporting gives the service evidence for analysis and can improve future classification; it is not a substitute for correcting a broken SPF record or unsafe mail-flow design.
After remediation, search for later messages from the same sender and for similar messages from other recipients. Check:
- Whether the corrected message reaches the intended folder.
- Whether the same sender still fails authentication from another system or region.
- Whether a policy override, connector, or transport rule changed the result.
- Whether suspicious messages now pass because the exception is too broad.
- Whether user reports and incident detections changed after the adjustment.
Keep an audit trail of trace queries, headers, policy versions, DNS changes, reports submitted, and exception removals. Retain enough history to distinguish a durable fix from a temporary improvement caused by changing sender volume.

- Collect identifiers. Obtain sender, recipient, timestamp, subject, message ID, and the final disposition from the user or quarantine record.
- Run Message trace. Identify the delivery action, transport events, and policy or rule that acted on the message.
- Open security investigation views. Where licensed and available, use Threat Explorer or Real-time detections to identify detection technology and overrides.
- Inspect headers and authentication. Record SPF, DKIM, DMARC, alignment, sending IP, and any intermediary shown in the authentication results.
- Classify the cause. Decide whether the evidence points to authentication, reputation, content, routing, or a policy threshold.
- Fix the cause. Correct DNS and signing, ask the sender to address reputation or content, or correct connector and forwarding configuration.
- Report the false positive. Submit the message through Microsoft’s analysis workflow with the relevant evidence.
- Use a narrow temporary exception only if needed. Apply the smallest Tenant Allow/Block List scope, assign an owner, and set a review date.
- Re-test. Send or wait for a legitimate message from the affected path and verify the resulting action.
- Monitor both directions. Review later false positives and false negatives, DMARC reports, traces, and user reports before closing the case or advancing enforcement.
Common failure modes and their fixes
“We allowed the whole domain and the problem disappeared.”
The immediate delivery issue may be gone, but every compromised account or look-alike message using that domain now has a wider path. Replace the broad entry with a corrected authentication or content configuration, then narrow or remove the exception.

Authentication may be only one signal. Recheck reputation, content, volume, URL behavior, and any impersonation or bulk-mail detection in the trace. Confirm that all sending systems—not just the primary Microsoft 365 path—use the corrected records.
How to prevent future false positives
- Maintain an inventory of every authorized sending service and its SPF, DKIM, and DMARC configuration.
- Review DMARC reports on a regular schedule and investigate new sources before they become business-critical.
- Require senders and partners to use stable From domains, aligned authentication, valid reverse DNS, and consistent sending behavior.
- Keep connector and forwarding diagrams current, including the order of gateways and the original-source handling.
- Make anti-phishing and trusted-entity changes through documented change control with an owner and rollback plan.
- Set expiration or review dates for every exception and remove entries that no longer solve an active problem.
- Track false negatives alongside false positives so delivery improvements do not conceal increased risk.
- Train help-desk staff to collect headers, trace identifiers, and quarantine details instead of asking for an immediate allowlist.
Operational standard
The safest pattern is consistent: investigate the detection evidence, repair authentication, reputation, content, or routing, report the confirmed false positive, and only then consider a tightly scoped exception. Stage DMARC enforcement while monitoring reports, tune anti-phishing controls in small steps, and verify both legitimate delivery and malicious-message blocking after every change. This approach reduces unnecessary blocks without turning the email system’s safety controls into a permanent bypass.
