Reduce false negatives by treating them as a coverage, data, decision, and feedback problem—not as a request for a more aggressive model alone. Define exactly what a missed fraud event is, map every payment flow and fraud scenario you must detect, make transaction and account signals available at decision time, investigate confirmed misses and low-risk approvals, tune approve/challenge/hold/decline actions against both fraud loss and customer impact, and repeat the process whenever data, behavior, or controls change.
No single algorithm or threshold is best for every merchant, payment service provider, or financial institution. The reliable objective is an auditable improvement loop that measures fraud caught and fraud missed together with false declines, approval impact, investigation workload, and recovery.
What a false negative is—and why AI systems produce them
A false negative is a fraudulent payment that the detection system classifies as low risk and allows, or fails to route for a control that would have stopped it. It is different from a false positive: a false positive sends a legitimate payment into a challenge, review, or decline path. Both matter, but they have different costs and require separate measures.

Missed fraud usually comes from one or more of four gaps:

- Economical &Reusable: Built durable materials with a long-lasting battery, our CAGIWIRU air duster it's a sustainable alternative to disposable air cans.
- Coverage: the model or monitoring process does not represent a payment rail, channel, account state, or fraud scenario that is occurring in production.
- Data: a useful signal is absent, stale, delayed, misjoined, inconsistently recorded, or unavailable before the decision.
- Decisioning: a score is translated into an approval threshold that is too permissive for a high-risk segment, or the available action is too weak.
- Feedback and operations: confirmed fraud is not converted into reliable labels, low-risk outcomes are not sampled, or alerts cannot be investigated in time.
Adding more attributes does not automatically close any of these gaps. Data matching depends on quality, and analytical results must be tested for validity. A stricter global threshold can catch more fraud while rejecting genuine customers, increasing manual work, and reducing approval rates.
Define the miss before changing the model
Fraud operations, data science, finance, and customer-support owners should agree on the event being counted. Write the definition into the metric specification and reporting queries.
- Specify the event: count a confirmed fraudulent transaction that the system approved or otherwise failed to challenge, hold, or decline. Keep attempted transactions that were blocked in a separate “caught” measure.
- Set the observation window: document how long after authorization a transaction can be confirmed as fraud. There is no universal label window; the right period depends on dispute timing, investigation practice, reversals, and the payment rail.
- Document label sources: identify which customer reports, disputes, reversals, law-enforcement referrals, investigator decisions, and account-takeover findings create a positive label.
- Preserve decision-time evidence: store the features, rules, model version, action, authentication result, and timestamps that were available when the transaction was scored. Do not reconstruct a miss using information that arrived later.
- Separate outcomes: distinguish unauthorized use, payments authorized under false pretenses, account takeover, synthetic identity, mule-account activity, and merchant abuse when those categories affect controls or reporting.
Delayed or disputed labels are a local design choice. What matters is that the definition is stable enough to compare cohorts and that late confirmations can be linked back to the original decision without silently changing historical results.

Map coverage by payment flow and fraud scenario
Start with a coverage map rather than a model list. For every flow, record where the decision occurs, what data is available, what action can be taken, and which fraud patterns are plausible.
| Coverage dimension | Questions to answer | Why it affects false negatives |
|---|---|---|
| Payment rail and instrument | Card, account-to-account, ACH, bank transfer, wallet, recurring debit, or other instrument? | Timing, reversibility, authentication, and available monitoring differ by rail. |
| Channel | Web, mobile app, call center, point of sale, API, batch file, or partner-initiated payment? | A control built for one channel may not see device, session, or operator signals in another. |
| Account lifecycle | New, established, dormant, recently recovered, recently edited, or closed account? | Takeover and mule activity often appear around account or beneficiary changes. |
| Transaction type | Purchase, payout, refund, transfer, cash-out, recurring payment, or credit? | Amount, velocity, counterparties, and expected behavior vary by type. |
| Fraud scenario | Stolen credentials, social engineering, synthetic identity, mule account, friendly fraud, or coordinated attack? | A model can perform well on known card theft while missing authorized push-payment scams. |
| Geography and legal scope | Which countries, entities, and data-sharing permissions apply? | Signals and controls must respect applicable privacy, banking, and payment regulations. |
Compare this map with the data actually available at authorization and with the evidence investigators obtain later. Saudi Central Bank (SAMA) standards identify customer records, transaction and payment systems, identity and access-management systems, and external databases as possible inputs. The useful question is not whether a field exists somewhere in the enterprise, but whether it is reliable, joined to the right entity, and delivered early enough to influence the decision.
Close data and monitoring blind spots
Audit the input path
For each material feature, measure freshness, missingness, join success, clock skew, and coverage by channel and customer segment. A device identifier that is absent on a mobile-to-web handoff, an account-change event that arrives after authorization, or a counterparty identifier that changes format can create systematic misses while aggregate model quality still looks acceptable.

- Check whether account, beneficiary, password, contact, and recovery changes are captured with actor, timestamp, and previous value.
- Check device, session, IP, location, authentication, and behavioral signals for consistency and lawful use.
- Check velocity and repetition across transactions, accounts, instruments, beneficiaries, and counterparties—not only within one account.
- Check external-data matches for false joins, stale records, duplicate identities, and unexplained coverage gaps.
- Check that features used in training are available in production with the same definition and latency.
Monitor relationships and anomalies
Single-transaction rules miss coordinated activity. Add monitoring for unusual account changes, repeated similar payments in a short period, linked accounts or devices, sudden changes in amount or location, and activity involving new, dormant, or possible mule accounts where lawful and appropriate. The UK Public Sector Fraud Authority practitioner standard describes data matching, exception reporting, device or voice detection, and anomaly monitoring as complementary techniques.
Data matching is only as credible as the data being matched. Validate samples of matches and non-matches, measure error rates, and test whether an apparent anomaly persists after correcting for legitimate seasonal, payroll, travel, or business-cycle behavior.
Investigate confirmed misses and sample low-risk approvals
Every confirmed miss should produce a structured case record. Reconstruct the decision using only information available at the time, then classify the root cause.
- Locate the transaction: link authorization, authentication, account, device, beneficiary, and subsequent dispute or investigation records.
- Rebuild the decision: record the score, rules fired, action taken, model and feature versions, latency, and any downstream control.
- Identify the missing opportunity: classify the cause as absent data, bad data, failed join, model coverage, threshold, decision latency, unavailable action, or workflow failure.
- Look for a cohort: search for similar low-risk approvals by channel, account age, device, counterparty, amount, geography, and time period.
- Assign an owner and due date: a data defect, rule change, model change, and operational fix should each have a responsible team and a validation plan.
Do not examine only known fraud. Periodically sample transactions that scored low risk and review exception reports for emerging or novel patterns. Random sampling can reveal suspicious behavior that has not yet generated a customer complaint or dispute. Record the sampling rate and selection method so that the resulting estimate is interpretable.
Tune thresholds and use layered actions
Instead of a single approve-versus-decline cutoff, define proportionate actions for risk bands and payment contexts. Compare outcomes by segment and flow; a threshold that is safe for a mature subscription customer may be too permissive for a new beneficiary or a dormant account.

|---|---|---|
| Approve | Low observed risk with no triggering exception. | Confirmed fraud that passed, later recovery, and customer impact if the decision is reversed. |
| Step-up authentication | Moderate risk where a customer can prove control of an account or device. | Challenge completion, abandonment, fraud after successful authentication, and accessibility impact. |
| Hold for review | High-value, unusual, or linked activity where a timely human decision is possible. | Review latency, investigator agreement, queue age, released fraud, and legitimate payments delayed. |
| Decline or block | Very high risk or a policy-prohibited pattern. | Confirmed fraud prevented, false declines, repeat attempts, appeals, and lost approval. |
Use cost and capacity analysis rather than intuition. Lowering a threshold may reduce missed fraud but increase false positives, customer friction, manual reviews, and lost approvals. Higher inherent risk can justify stricter criteria or additional controls, while alert capacity and the effect on genuine customers constrain what is operationally sustainable. Visa describes risk-based automated decisioning as a balance intended to increase approvals and reduce false declines; that principle does not establish one threshold for every institution.

Layer controls so that a weak signal in one layer can be compensated for elsewhere: risk scoring, authentication, real-time monitoring, exception reporting, secondary checks, post-transaction review, and escalation. A score that arrives after an irrevocable payment cannot substitute for a pre-authorization control.
Make alerts actionable and investigations fast
A detection improvement is ineffective if the organization cannot act before funds move or cannot investigate the alert with adequate evidence.
- Route by risk and expertise: send account-takeover, mule, merchant, and payment-rail cases to queues with the right playbooks.
- Show evidence: include the triggering features, linked entities, historical baseline, rule or model version, and relevant authentication events.
- Set service levels: define response times for real-time holds, high-value transfers, customer contact, and post-transaction review.
- Provide escalation: investigators need a path to compliance, security, payments operations, and customer support.
- Staff for peaks: campaigns, holidays, incidents, and model changes can multiply alert volume.
- Close the loop: capture investigator outcomes, override reasons, customer confirmation, recovery status, and evidence quality.
SAMA guidance calls for adequate resources, skills, training, and workflow to allocate alerts, with examples including secondary checking and real-time blocking. Human review should be governed: measure agreement between investigators, audit overrides, and distinguish a justified release from an unrecorded dismissal.
Measure the right outcomes
Report false negatives as operational outcomes, not only as a model metric. Use consistent definitions, observation windows, and segment breakdowns.
| Metric | Definition to document | Useful breakdowns |
|---|---|---|
| Confirmed fraud passed | Confirmed fraudulent transactions approved or otherwise not stopped, within the agreed label window. | Rail, channel, scenario, account age, geography where lawful, model version. |
| Fraud caught | Confirmed or subsequently validated fraud stopped by a rule, model, authentication, hold, or investigation. | Control layer, action, time to decision, value band. |
| False-positive or decline impact | Legitimate activity challenged, held, or declined, using a defined confirmation method. | Customer segment, channel, action, abandonment, approval conversion. |
| Loss and recovery | Gross loss, recoveries, reversals, and net loss tied to the event and accounting period. | Rail, fraud type, decision, recovery route and timing. |
| Alert operations | Volume, queue age, review time, escalation rate, investigator outcome, and override rate. | Team, severity, hour, payment flow, peak period. |
| Data health | Feature availability, freshness, join success, drift, and feed latency. | Source system, version, channel, geography, deployment. |
Review these measures periodically and after material changes. Saudi Central Bank standards state: “The effectiveness of fraud detection standards and related controls should be measured and periodically evaluated.” That evaluation should lead to a documented decision—retain, tune, replace, add a layer, or retire a control—not merely a dashboard refresh.
Retest after changes and emerging threats
Re-evaluate when payment behavior, fraud tactics, customer mix, data feeds, model versions, rules, authentication steps, or operating procedures change. Use a pre-change baseline and a post-change observation period, and keep a rollback path for a model or rule that increases customer harm or operational overload.

For ACH participants covered by Nacha’s Phase 2 material, risk-based fraud-monitoring requirements took effect June 19, 2026; because that date was a federal holiday, the practical processing date was June 22, 2026. Nacha describes procedures intended to identify unauthorized entries or entries authorized under false pretenses and requires review at least annually, with updates for evolving risk. It does not require every ACH entry to be screened individually or require all monitoring to occur before processing. Applicability depends on the participant’s role and the applicable phase.

Nacha’s guidance supports a risk-based process rather than a universal pre-processing screen. Examples of suspicious credits include an atypically high-dollar entry, repeated similar credits in a short period, or such activity involving a new, dormant, or possible mule account. Document how your procedures identify both unauthorized entries and entries induced by false pretenses, and review them at least annually when the Phase 2 requirements apply.
European Union payment services
Commission Delegated Regulation (EU) 2018/389 requires in-scope payment service providers to maintain transaction-monitoring mechanisms for unauthorized or fraudulent transactions, based on normal user behavior and specified risk factors. Security measures must be documented, periodically tested, evaluated, and audited. Determine whether your entity, service, and transaction fall within the regulation before treating it as a universal requirement.
Saudi Arabia
SAMA Fraud Detection Standards are framed for SAMA member organizations. They call for risk-aligned controls, updates when the risk landscape changes, effectiveness measurement, and proportionate treatment of higher inherent risk. Use the standards together with the organization’s applicable regulatory obligations and governance requirements.
United Kingdom public-sector practice
The UK Public Sector Fraud Authority practitioner standard is operational guidance, not a universal payments regulation. Its recommendations on exception reports, data matching, sampling, validation, human oversight, and anomaly monitoring are useful design patterns when adapted to the organization’s legal and payment context.

Visa reports a UK account-to-account fraud-detection pilot in which partners saw a 40% average uplift in fraud detection at a 5:1 false-positive rate. The same page says its model identified 54% of fraudulent transactions that had already passed through sophisticated bank and payment-service-provider systems and that Visa Advanced Authorization evaluates more than 500 risk attributes per transaction. These are vendor-reported pilot and product claims; the page does not state a publication year, and they are not independent benchmarks or expected results for a particular institution. Treat them as questions to validate under your own definitions, population, and observation window.
Common approaches that increase risk instead of reducing misses
Raising sensitivity everywhere
A global threshold reduction can inflate false positives and manual queues. Fix: target the segments and scenarios where confirmed misses concentrate, then compare incremental fraud caught with customer and capacity costs.
Collecting data without governance
More personal or external data can create legal, security, and quality problems. Fix: document purpose, lawful basis, access, retention, provenance, and validation before adding a signal.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Training on post-transaction information
Using dispute outcomes, later device changes, or investigation findings as if they were available at authorization creates leakage and overstates performance. Fix: enforce point-in-time feature construction and preserve decision-time snapshots.
Optimizing only for a model score
AUC, precision, or recall does not tell operations whether losses, false declines, or queue time improved. Fix: connect model metrics to confirmed fraud, approval impact, net loss, recovery, and workload.
Ignoring low-risk outcomes
Reviewing only alerts reinforces known patterns and misses novel fraud. Fix: use documented random sampling and exception reports for low-risk approvals.
Assuming monitoring must be universal or pre-processing
Payment-rail obligations differ. Nacha explicitly does not require every ACH entry to be screened individually or all monitoring to occur before processing. Fix: map the organization, role, rail, and jurisdiction before selecting controls.
A practical 90-day improvement cycle
- Days 1–15: establish the baseline. Agree on the miss definition, label window, loss and recovery treatment, false-positive definition, and reporting segments. Inventory model, rule, authentication, monitoring, and investigation versions.
- Days 16–30: map exposure. Document payment flows, fraud scenarios, decision points, available actions, data sources, legal constraints, and known latency or join failures.
- Days 31–45: mine misses and low-risk samples. Reconstruct confirmed misses, run a documented random sample of low-risk approvals, validate matches, and rank root causes by preventable loss and frequency.
- Days 46–60: design targeted changes. Choose data fixes, rule or threshold changes, additional authentication, workflow improvements, or model retraining. Define success, guardrails, rollback criteria, and staffing needs before deployment.
- Days 61–75: test safely. Replay historical decisions with point-in-time data, test segment effects, run controlled exposure where permitted, and verify alert evidence and routing.
- Days 76–90: deploy and review. Roll out with monitoring, compare against the baseline, inspect customer and investigator impact, document the decision, and schedule the next periodic evaluation.
Repeat the cycle when threat patterns, payment behavior, feeds, models, rules, authentication, or regulations change. The goal is not the most restrictive system; it is the highest sustainable reduction in confirmed fraud that passed, with transparent trade-offs for genuine customers and the teams who must operate the controls.
