If an AI conversation seems to be making someone intensely believe something untrue, treat the distress and any safety risk as real—even though the belief may not be. A delusion is a firmly held belief that does not match reality, and it can appear with hallucinations, confused thinking, sleep disruption, suspiciousness, withdrawal, reduced self-care, or a sudden drop in day-to-day functioning. These signs are not a diagnosis. Stop using the chatbot as evidence, involve a trusted person, and arrange prompt assessment by a qualified health professional. Use crisis or emergency services immediately if anyone may harm themselves or someone else.

What “AI-related delusion” means—and what it does not

“AI-related delusion” is a practical description, not a recognized medical diagnosis. It usually refers to a distressing, reality-inconsistent belief that appears during or after intensive interaction with a chatbot, or that a chatbot seems to reinforce. The underlying condition could be psychosis, a mood episode, a medical problem, a medication effect, sleep deprivation, or substance-related. Only a clinician who evaluates the person can determine what is happening.

The National Institute of Mental Health (NIMH) defines psychosis as a set of symptoms in which a person loses contact with reality. A delusion is a strongly held belief that does not match reality; hallucinations are seeing, hearing, or otherwise sensing things that other people do not. Psychosis is not synonymous with schizophrenia. It can occur with several mental illnesses and with physical illnesses, prescription medicines, alcohol or other drugs, or severe lack of sleep.

A person may not realize that a belief or perception is not real. Arguing harder rarely resolves that lack of insight and can make the conversation less safe. The useful question is not “Can I prove the chatbot caused this?” but “What symptoms, risks, and changes require human assessment now?”

Warning signs that warrant a professional assessment

One unusual idea, an interest in technology, or a disagreement with an AI answer is not evidence of psychosis. Concern rises when changes are intense, persistent, escalating, or impair everyday life. NIMH and the NHS symptoms guidance describe signs such as:

  • Unshakeable unusual beliefs: certainty that a chatbot, government, employer, stranger, or device is sending personal messages or targeting the person, despite credible contrary evidence.
  • Hallucinations: hearing voices or seeing, feeling, smelling, or tasting things others do not perceive.
  • Suspiciousness or paranoia: fear that ordinary events, people, or online content are part of a threat or plot.
  • Difficulty thinking clearly: thoughts that become hard to follow, rapidly shifting, or unusually intense.
  • Confused speech: words or answers that no longer connect logically, or communication that is markedly different from usual.
  • Sleep disruption: very little sleep, staying awake to chat or investigate, or a dramatic change in the sleep–wake cycle.
  • Withdrawal and self-care changes: stopping contact with others, neglecting food, hygiene, work, school, or medical needs.
  • Difficulty distinguishing reality from fantasy: treating a chatbot’s speculation, role-play, generated image, or confident statement as proof.
  • A sudden functional decline: new problems with work, school, finances, relationships, or basic daily tasks.

These are reasons to seek assessment, not a checklist that establishes a diagnosis. A clinician will consider the person’s baseline, mood, sleep, medications, substance use, medical history, family history, and the exact experiences.

Why a chatbot may appear to be involved

Generative systems produce fluent, confident language without understanding a person’s circumstances. They can be wrong, mirror a user’s framing, or continue a fictional premise when a user needs reality-based guidance. The American Psychological Association’s guide to navigating AI-generated advice says: Do not rely on AI in a crisis. The guide is available at APA’s guide to navigating AI-generated advice thoughtfully and safely.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The APA’s health advisory on generative AI chatbots and wellness applications for mental health warns that current tools may lack validation, oversight, adequate safety protocols, personal context, and professional judgment. A chatbot should therefore never be treated as a diagnostician, a witness that an extraordinary claim is true, or a substitute for crisis care.

APA Monitor reported on September 1, 2026, in “Understanding ‘AI psychosis’”, that clinicians and researchers have described cases of delusional thinking following extended chatbot interactions. The article also stresses that the direction and causes are unsettled. Interaction may reinforce or amplify an existing vulnerability, coincide with another illness, or simply become part of the person’s explanation; available reports do not establish that chatbots independently cause psychosis.

That article reported an OpenAI estimate that about 0.07% of weekly ChatGPT users—approximately 630,000 people—had conversations showing signs of mania or psychosis in 2025. This is a count of conversations flagged as showing signs, not a count of clinically diagnosed people, and it does not demonstrate causation.

Decide how urgent the situation is

Use the highest level of help that fits the actual risk. Do not wait for certainty about a diagnosis before acting on immediate danger.

Situation Action now Examples of routes
Immediate danger Stay with the person if it is safe, remove yourself from danger, and contact emergency services. Tell the dispatcher about suicidal thoughts, threats, weapons, severe confusion, or inability to care for basic needs. In the United States, call 911 for a life-threatening emergency. In the United Kingdom, call 999. Elsewhere, use the local emergency number.
Suicidal thoughts, escalating fear, or a crisis without immediate physical danger Use a live crisis service and involve a trusted person. Do not leave a high-risk person alone simply because a chatbot says the situation is safe. In the United States, call or text 988. In the United Kingdom, contact an existing crisis team or NHS 111 when appropriate. Other countries have different services.
New or worsening symptoms without an immediate threat Arrange prompt assessment with a doctor, psychiatrist, psychologist, or other qualified mental-health professional. Contact the person’s existing care team if they have one. Primary care, an early-psychosis service, community mental-health team, or the locally available equivalent.
Established symptoms that are stable but affecting life Keep scheduled treatment and report changes in sleep, beliefs, perceptions, functioning, medication, or substance use. The responsible clinician or care team; do not change prescribed treatment independently.

U.S. numbers come from NIMH crisis guidance; NHS routes are specific to the U.K. If you are elsewhere, use your country’s emergency number, crisis line, or urgent-care pathway.

If you are experiencing these beliefs or perceptions

Recognizing that something feels unusual is a reason to get support, not a reason to investigate alone. Take these steps in order:

  1. Tell a real person today. Choose someone you trust and say plainly that your thoughts, perceptions, sleep, or sense of safety have changed. Ask them to stay in contact and help you arrange care.
  2. Stop treating the chatbot as a test of reality. Do not ask multiple systems to vote on whether the belief is true, collect screenshots as proof, or continue a conversation that intensifies fear. Save relevant records only to show a clinician what happened.
  3. Arrange prompt professional contact. Call a primary-care practice, mental-health service, psychiatrist, psychologist, or social worker. Say that you are having unusual beliefs or perceptions and need an assessment; you do not need to choose a diagnosis first.
  4. Give the clinician complete information. Describe the belief or perception in your own words, when it began, how convinced you feel, any voices or visions, sleep changes, mood changes, medication or dose changes, alcohol or drug use, medical symptoms, and how chatbot use changed before the symptoms.
  5. Reduce avoidable strain while waiting. Ask a trusted person to help with food, hydration, transport, sleep, and practical decisions. Avoid alcohol and non-prescribed drugs, and do not drive if you are severely distracted, confused, or sleep-deprived.
  6. Escalate immediately if safety changes. Suicidal thoughts, an urge to hurt someone, command hallucinations, inability to obtain food or shelter, or rapidly worsening confusion require crisis or emergency help rather than another online conversation.

You are allowed to seek help even if you are unsure whether the experience is “real,” whether AI played a role, or whether you will be believed. Assessment is designed to sort out those questions safely.

If you are worried about someone else

Your aim is connection and safety, not winning an argument. A person experiencing psychosis may not recognize that their beliefs or perceptions are symptoms and may resist seeing a doctor. The NHS advises contacting an existing crisis or care team; where no team is available in the U.K., NHS 111 can advise on next steps.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Start with calm, concrete concern

Choose a quiet moment, speak slowly, and describe observable changes: “You have slept two hours a night and seem frightened. I want to help you talk with a clinician.” Ask what they are experiencing and what would help them feel safer. Listen without interrogating.

Acknowledge distress without confirming the belief

You can validate emotion—“That sounds terrifying”—without agreeing that the chatbot, neighbor, or government is actually communicating with them. A useful distinction is: “I can see that this feels completely real to you. I do not have the same evidence, and I would like us to get help checking what is happening.”

Offer specific help

  • Offer to call the doctor, crisis team, or emergency service together.
  • Offer transport, a quiet place, food, water, or company while help is arranged.
  • Ask directly whether they are thinking about suicide, harming anyone, or unable to care for themselves. A direct question does not create suicidal thoughts; it clarifies risk.
  • Share the minimum relevant chatbot information with clinicians: the service used, change in frequency, themes that seemed to intensify, and any screenshots the person agrees to share.

Know when you cannot manage it alone

If the person has a weapon, is threatening harm, is attempting suicide, is so confused that basic safety cannot be maintained, or you feel unsafe, move to a safe location and call emergency services. Do not physically restrain someone except where trained responders direct you to do so.

Responses that commonly make things worse

  • Do not debate every detail. Point-by-point challenges can feel like an attack and rarely restore insight during an acute episode.
  • Do not ridicule, threaten, shame, or secretly test the person. These responses damage trust and can increase isolation.
  • Do not affirm an unverified claim as fact. Repeating that an AI is definitely sending secret messages, or that a conspiracy is proven, can reinforce a dangerous belief.
  • Do not diagnose from a checklist or social-media post. Similar symptoms can have very different causes and treatments.
  • Do not tell someone to start, stop, or alter prescription medication. The NHS treatment guidance says medication decisions and stopping treatment should be discussed with the responsible professional; see NHS treatment for psychosis.
  • Do not use an AI system as a crisis counselor. Ask for human emergency or clinical help instead.

What a professional assessment examines

NIMH states, “There is no one cause of psychosis.” A qualified psychologist, psychiatrist, social worker, physician, or other appropriate clinician will distinguish symptoms from causes rather than assuming that an AI conversation is responsible. Assessment may cover:

  • the exact beliefs, perceptions, thoughts, emotions, and level of conviction;
  • onset, duration, sleep, mood, and changes in work, school, relationships, and self-care;
  • prescription and over-the-counter medicines, recent dose changes, alcohol, cannabis, stimulants, psychedelics, and other substances;
  • medical and neurological symptoms, injuries, infections, hormonal or metabolic problems, and other physical causes;
  • family and personal mental-health history, previous episodes, trauma, and current stress;
  • risk of suicide, violence, exploitation, accidental harm, or inability to meet basic needs;
  • the timing and intensity of chatbot use, including whether the system echoed, encouraged, or challenged the belief.

NIMH estimates that roughly 15 to 100 people per 100,000 develop psychosis each year. That broad population estimate is not an individual risk calculation and cannot show whether AI caused a particular person’s symptoms.

How treatment and support may look

Care is individualized. Depending on the assessment, a plan may include antipsychotic medication, talking therapies, family intervention, self-help or peer groups, rehabilitation, housing or employment assistance, and practical support. The NHS treatment page describes these options and emphasizes professional monitoring of medication benefits and side effects.

Medication is not a do-it-yourself intervention. A prescriber chooses and adjusts it with the person, explains expected benefits and adverse effects, and decides how to taper or stop it when appropriate. Family members can help by recording changes, supporting appointments, and reporting urgent side effects, but should not change doses themselves.

Situations that need special attention

Postpartum symptoms

Psychosis after childbirth is a medical emergency. The NHS estimates that around one in every 1,000 women who give birth experiences postpartum psychosis. Sudden confusion, extreme mood changes, hallucinations, or fixed unusual beliefs after birth require urgent medical help; do not leave the parent or baby in an unsafe situation while trying to obtain reassurance from a chatbot.

Older adults and new medical symptoms

New psychosis-like symptoms in an older adult can accompany neurological or other physical illness. Sudden onset, fever, head injury, new weakness, severe headache, medication changes, or fluctuating consciousness call for urgent medical evaluation rather than an assumption of a primary psychiatric disorder.

Sleep loss and substance use

Several nights of little sleep, intoxication, withdrawal, or stimulant use can produce or worsen unusual perceptions and beliefs. Tell clinicians honestly what was taken and when. This information changes the safety assessment and treatment; withholding it can delay the right care.

Set safer boundaries around AI after the crisis

Once a clinician or crisis team is involved, agree on practical boundaries with the person rather than imposing secret surveillance. Options may include pausing open-ended chatbot conversations, disabling notifications, avoiding prompts that intensify paranoia or grandiosity, and using the device only for neutral tasks such as contacting a care team. Keep human contacts, crisis numbers, and the treatment plan easy to reach.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Any AI output that conflicts with a clinician’s advice should be treated as unverified. Do not paste private clinical records, identifying information, or another person’s messages into a chatbot to obtain a diagnosis. If a system repeatedly mirrors an alarming belief, end the session and bring the concern to a human professional.

Technology can still support ordinary logistics—setting an appointment reminder, drafting questions for a doctor, or translating a crisis service’s instructions—when a person is stable and a human remains responsible for care. It must not decide whether someone is psychotic, determine medication, assess imminent risk, or replace emergency responders.

Prepare useful information for the appointment

A short factual record can help a clinician see patterns without turning monitoring into proof-hunting. With the person’s consent, note:

  • the date and time symptoms began or changed;
  • hours slept and major changes in energy or mood;
  • what was experienced (beliefs, voices, visions, confusion) and how it affected behavior;
  • medicines, missed doses, dose changes, supplements, alcohol, and other substances;
  • chatbot name, approximate duration and frequency of use, and conversations that seemed to increase distress;
  • missed work or school, isolation, neglected self-care, risky acts, threats, or suicidal statements;
  • what calmed the person and what escalated the situation.

Keep the record private and share it with the treating team, not publicly or as a way to prove the person wrong. If documenting a conversation increases fear or compulsive checking, stop and prioritize care.

When to seek help even without an AI connection

The same warning signs deserve assessment whether they began after a chatbot, a social-media exchange, a stressful event, or no obvious trigger. There is no requirement to establish an AI link before calling a professional. A person can have a genuine crisis even if the chatbot conversation was incidental, and a chatbot can be involved without being the cause.

Prompt assessment is especially important when symptoms persist, intensify, disrupt sleep or functioning, or lead to withdrawal and neglect of basic needs. Emergency help is the correct route for immediate danger; routine online research is not.

Reliable care routes by location

In the United States, NIMH directs people in crisis to call or text 988 and to call 911 for life-threatening emergencies. In the United Kingdom, NHS guidance points to an existing crisis team, NHS 111 when no team is available, and 999 for immediate danger. Those numbers are not universal: readers elsewhere should use local emergency services, a national crisis line, or a hospital emergency department.

For non-emergency care, start with a primary-care clinician or a qualified psychologist, psychiatrist, or social worker. Ask whether an early-psychosis, community mental-health, substance-use, perinatal, or older-adult service is appropriate. If the person already has a care plan, follow its contact instructions and tell the team about the new symptoms rather than waiting for the next scheduled appointment.
