To read a UnitedHealth Group earnings report, start with the reporting period and consolidated income statement, then examine the medical care ratio (MCR), operating costs, reported versus adjusted earnings per share, cash flow, balance-sheet leverage, segment results and management’s dated outlook. Revenue growth is only the top line: it does not prove that profit, cash generation or margins improved.

The newest evidenced results covered here are UnitedHealth Group’s second quarter of 2026, released July 16, 2026. The company scheduled its third-quarter 2026 release before market open on October 13, 2026, with an 8 a.m. ET call. Check the investor-relations filing index for that release before relying on any Q3 figure.

1. Identify the report, period and accounting basis

UnitedHealth Group publishes earnings releases alongside quarterly Form 10-Q filings, annual Form 10-K filings, proxy statements and other SEC documents. First determine exactly what you are reading:

Free tools Windows power users keep installed

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Reporting period: for example, the quarter ended June 30, 2026, or the fiscal year ended December 31, 2025.
  • Release date: this tells you when the figures and outlook were made public.
  • Document type: an earnings release is a concise management summary; Form 10-Q and Form 10-K filings provide fuller accounting detail, footnotes and reconciliations.
  • Reported or adjusted: reported figures follow generally accepted accounting principles (GAAP); adjusted figures are company-defined non-GAAP measures that exclude specified items.

Use the same period basis for every comparison. A quarter-over-quarter change compares with the immediately preceding quarter, while a year-over-year change compares with the same quarter a year earlier. Seasonality and changes in membership or government reimbursement can make sequential comparisons especially misleading when viewed alone.

Record the units as well as the dates. UnitedHealth presents some amounts in millions or billions, while per-share figures are in dollars and ratios are percentages. A number without its period, unit and reported-or-adjusted label is easy to misread. Also check whether a table has been reclassified or restated; the footnotes explain changes in presentation that can otherwise look like operating growth.

The investor-relations filing index is the practical starting point when a newer report may have been released. It links the earnings release to the SEC-hosted filing, where you can verify definitions, segment tables, accounting policies and non-GAAP reconciliations.

2. Read the consolidated income statement before the headline

Work down the statement rather than stopping at the release’s first growth percentage. The sequence shows how revenue becomes operating income, net earnings and diluted EPS.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Revenue: the top line

Revenue is the amount recognized from UnitedHealth Group’s businesses, including premium revenue and revenue from Optum services and other activities. For fiscal 2025, total revenue was $447.567 billion, up 12% from 2024. That growth did not translate into higher profit: 2025 operating earnings were $18.964 billion, net earnings attributable to common shareholders were $12.056 billion, and diluted EPS was $13.23. The annual filing describes substantial medical-cost growth and other items affecting results.

For Q2 2026, total revenue was $112.0 billion. Put that figure beside the quarter’s $8.0 billion of operating earnings and the reported net margin of 4.9% rather than treating revenue growth as a conclusion about profitability.

Revenue can rise while costs rise faster. Always place the top line beside medical costs, other operating costs, operating earnings, interest and other non-operating items, taxes, net earnings and diluted EPS. Ask whether growth came from more members, higher premiums, greater utilization, acquisitions, Optum activity or a change in mix; then read management’s explanation instead of assigning a cause from the percentage alone.

Operating earnings and net earnings

Operating earnings show profit from the company’s operating businesses before interest, other non-operating items and taxes. Net earnings attributable to common shareholders reflect those additional items and are the earnings base used for basic and diluted EPS. A change between operating earnings and net earnings can therefore reflect financing costs, investment results, tax effects or other non-operating items rather than a change in the underlying insurance or services operations.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

When operating earnings and net earnings move in different directions, trace the bridge in the statement and footnotes. A tax-rate change or non-operating item can lift net income even if operating performance is flat, while a one-time charge can depress net income without changing the quarter’s operating revenue or medical-cost trend.

Diluted EPS

Diluted EPS expresses earnings available to common shareholders on a per-share basis after the effect of potentially dilutive securities. Compare diluted EPS with the same measure in the year-ago period and check whether share-count changes affected the comparison. EPS is not a cash-flow measure. If net earnings rise but diluted EPS rises faster, a lower diluted share count may be part of the explanation; if EPS lags earnings, dilution or other share-count effects may matter.

3. Understand UnitedHealth’s medical care ratio

UnitedHealth Group defines its medical care ratio (MCR) as medical costs divided by premium revenue. It is an insurance-cost ratio, not the group’s consolidated profit margin.

What the ratio means

A higher MCR means medical costs consumed a larger share of premium revenue, all else equal. A lower MCR generally leaves more premium revenue available for operating costs and profit, but it does not tell you the company’s total margin because consolidated revenue includes non-premium activities and the group incurs many expenses outside medical costs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For 2025, reported MCR was 89.1%, compared with 85.5% in 2024, an increase of 3.6 percentage points. The annual filing discusses Medicare funding reductions, elevated medical-cost trend, member mix, value-based-care contract losses, lower favorable development, Medicare Part D and market morbidity among the drivers, along with offsets. Use those explanations rather than assigning a cause from the percentage alone.

The 2025 results release also identified a 20-basis-point negative impact from loss contracts. On the company’s presentation, reported MCR was 89.1% and adjusted MCR was 88.9% after that item. Keep the reported figure visible; the adjusted presentation is useful only when the reconciliation and the reason for exclusion are clear.

Q2 2026 MCR and prior-period development

In the second quarter of 2026, MCR was 86.7%, versus 89.4% in Q2 2025, a 2.7-percentage-point year-over-year decrease. The release attributed the year-over-year decrease to benefit design and pricing discipline, member mix and medical-cost management initiatives. It also reported $860 million of net favorable prior-period medical development, most related to 2026 dates of service.

Prior-period development means the company’s estimate of earlier medical claims changed as more information became available. Favorable development can reduce the current period’s reported medical costs; unfavorable development can increase them. Compare the amount and the dates of service with management’s explanation before deciding whether a quarter’s MCR is representative of current pricing and utilization.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

That reserve development can materially affect a reported quarter. Do not create an “adjusted MCR” by dividing the $860 million by consolidated revenue: the ratio’s stated denominator is premium revenue, and the company’s release does not establish such a calculation.

Why MCR is not 100% minus profit margin

MCR covers medical costs against premium revenue. It excludes the denominator and cost structure of other UnitedHealth businesses and does not incorporate every consolidated expense. Subtracting MCR from 100% therefore does not produce UnitedHealth Group’s profit margin. The remainder is not a simple pool of profit because administrative costs, technology spending, taxes, interest and other items still have to be paid.

4. Separate operating cost, operating margin and net margin

UnitedHealth reports several group-level ratios with different numerators and denominators. In the 2025 Form 10-K:

Metric 2025 reported value What it measures
Operating cost ratio 13.3% Operating costs relative to the relevant consolidated revenue base
Operating margin 4.2% Operating earnings relative to consolidated revenue
Net earnings margin 2.7% Net earnings relative to consolidated revenue

These ratios answer different questions. Operating cost ratio indicates how much revenue is consumed by operating expenses other than the separately presented medical-cost components. Operating margin captures profit from operations. Net margin also reflects non-operating items and taxes.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For Q2 2026, UnitedHealth reported $8.0 billion of operating earnings and a 4.9% net margin. Its operating cost ratio was 12.7%, compared with 12.3% in Q2 2025. Management said the increase reflected targeted investments in technology, operations and community support. A higher cost ratio can be intentional investment rather than evidence of uncontrolled spending; read the explanation and watch whether later quarters show operating benefits.

Do not mix a segment margin with a group margin. A segment’s operating margin uses that segment’s revenue and earnings, while the consolidated margin includes eliminations and activities outside the segment table. Keep the numerator and denominator together whenever you copy a ratio into your notes.

5. Compare reported EPS with adjusted EPS

Reported EPS is the GAAP result. Adjusted EPS is a non-GAAP, company-defined measure intended to show earnings after removing specified items. Neither should automatically replace the other.

Full-year 2025 example

The 2025 results release reported $13.23 of diluted EPS and $16.35 of adjusted EPS. The reconciliation identified specified portfolio-divestiture, restructuring and other items with a $1.78 diluted-EPS impact.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Read the reconciliation line by line. It should name each exclusion, show its tax effect where applicable and explain how the adjustment reaches adjusted EPS. An excluded item may be unusual, but readers should not assume it can never recur. Repeated restructuring, divestitures or contract-related charges may be economically relevant even when management excludes them from its preferred performance measure.

For comparability, compare reported EPS with the prior period’s reported EPS and adjusted EPS with the prior period’s adjusted EPS. Comparing this year’s adjusted number with last year’s reported number combines two different accounting bases and can exaggerate or hide the change.

Q2 2026 example

For Q2 2026, reported EPS was $6.04 and adjusted EPS was $6.38. Use the detailed release’s reconciliation to identify the exact items behind the difference before comparing the quarter with Q2 2025 or with guidance. If the release changes the list of adjustments, note that change rather than assuming the company’s adjusted series is identical every quarter.

6. Check cash flow and the balance sheet

Operating cash flow

Operating cash flow measures cash generated by operations after working-capital movements. It is related to accounting earnings but is not interchangeable with net income. Claims payments, premium collections, provider settlements and other timing differences can make cash flow move differently from earnings in a particular quarter.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • The 2025 results release reported $19.7 billion of operating cash flow, equal to 1.5 times net income.
  • The Q2 2026 release reported $11.1 billion of operating cash flow, equal to 1.9 times net income.

The Q2 multiple is a quarter-specific relationship, not a forecast of the full-year conversion rate. Compare cash flow over several periods and investigate large working-capital movements before treating a single quarter as a trend. Also distinguish operating cash flow from free cash flow: the figures cited here are operating cash flow and do not, by themselves, subtract capital expenditures.

Debt and capitalization

At June 30, 2026, UnitedHealth reported a 41.2% debt-to-capital ratio. Read this alongside cash, debt maturities, interest expense and cash uses such as acquisitions, dividends and share repurchases. A balance sheet can change materially after quarter-end, so use the date attached to every ratio.

Leverage is a context check, not a stand-alone verdict. Rising debt may support an acquisition or other capital action, while falling debt may coincide with lower cash balances. The notes to the filing provide maturity dates and interest details that a headline debt ratio cannot show.

7. Use UnitedHealthcare and Optum to explain the consolidated result

UnitedHealth Group reports through UnitedHealthcare and Optum, with Optum containing distinct businesses. Segment revenue, operating earnings and operating margin can reveal which operations are driving a group-level change.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

In Q2 2026, UnitedHealthcare revenue was $86.0 billion, operating earnings were $3.9 billion and operating margin was 4.6%. Compare segment changes with membership, benefit design, pricing, utilization and medical-cost commentary. A consolidated improvement can hide weakness in one segment if another segment grows faster; the reverse is also true.

When segment revenue grows but segment operating earnings do not, look for cost pressure, mix changes or investment spending. When earnings grow faster than revenue, check whether the improvement came from sustainable operating performance or a reserve-development item. The release and filing commentary should be read with the segment table, not substituted for it.

8. Treat management guidance as a dated outlook

Guidance is management’s expectation at a specific point in time, not a realized result. As of the July 16, 2026 Q2 release, the full-year 2026 outlook was:

Measure 2026 outlook in the Q2 release
Reported EPS $18.45–$18.95
Adjusted EPS $19.50–$20.00
Operating cash flow Approximately $24 billion

Label these figures as the Q2 release outlook. Before using them, check the newest release for revisions, withdrawals or updated assumptions. Compare actual year-to-date results with the remaining range, and read the assumptions concerning medical trend, membership, reimbursement, taxes and capital deployment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not treat the midpoint as a promise or use the range as a valuation target. Guidance can change when claims experience, reimbursement decisions, enrollment or capital plans change. Keep the release date beside every outlook number so an older range is not mistaken for current management expectations.

9. A repeatable checklist for every UnitedHealth earnings report

  1. Record the release date, reporting period and document type.
  2. Write down reported revenue, medical costs, other operating costs, operating earnings, net earnings and diluted EPS.
  3. Compare each figure with the same period a year earlier; add a sequential comparison only when seasonality and mix make it useful.
  4. Read MCR as medical costs divided by premium revenue, then note the company’s explanation for changes and any prior-period development.
  5. Review operating cost ratio, operating margin and net margin separately.
  6. Place reported EPS before adjusted EPS and inspect every reconciliation exclusion.
  7. Check operating cash flow, its relationship to net income and the dated debt-to-capital ratio.
  8. Review UnitedHealthcare and Optum segment revenue, operating earnings and margins.
  9. Compare actual results with the latest dated guidance, not with an outdated range.
  10. Read the risk factors, footnotes and management commentary for items that may not repeat.

A simple worksheet prevents most comparison errors. Put columns for the current period, year-ago period and, where useful, the immediately preceding quarter. Add a separate column for reported versus adjusted status, then write one sentence explaining each material change. This forces you to preserve the period, unit and accounting basis attached to every number.

10. Common reading errors and how to avoid them

Calling revenue growth profit growth

Revenue can increase while medical costs, operating expenses or taxes increase faster. Pair every revenue statement with operating earnings, net earnings and margins.

Treating MCR as the company’s net margin

MCR applies to medical costs and premium revenue. Use the separately reported operating and net margins for consolidated profitability.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Using adjusted EPS without the reconciliation

Adjusted EPS is useful only when you know which items were removed. Keep the GAAP figure visible and assess whether excluded charges are genuinely unusual.

Annualizing one quarter’s cash flow

Quarterly cash flow reflects timing and working capital. Do not multiply Q2’s $11.1 billion by four and present the result as an annual forecast.

Ignoring segment mix

UnitedHealthcare and Optum have different economics. A group-level percentage cannot explain which business changed without segment data.

Using stale guidance

Guidance changes with new claims experience, reimbursement decisions, membership and capital actions. Replace the Q2 outlook with the newest company outlook when available.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Confusing favorable development with current-period improvement

A reserve release for earlier dates of service can lower the reported MCR in the current quarter. Identify the dates of service and the dollar amount before concluding that current claims trends improved by the same amount.

Mixing dollars, percentages and basis points

A change from 89.4% to 86.7% is a 2.7-percentage-point, or 270-basis-point, move, not a 2.7% decline in medical costs. State the unit of every change.

11. Questions readers commonly ask

How do I read UnitedHealth Group’s earnings report?

Confirm the period and document type, read the consolidated income statement, then review MCR, operating and net margins, reported and adjusted EPS, operating cash flow, leverage, segment results and dated guidance. Compare like with like and read the explanations for material changes.

What does UnitedHealth’s medical care ratio mean?

It is medical costs divided by premium revenue. A higher percentage means more premium revenue was consumed by medical costs, but MCR is not the group’s consolidated profit margin and cannot be subtracted from 100% to obtain net margin.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Is revenue growth the same as profit growth?

No. UnitedHealth’s 2025 revenue rose 12% to $447.567 billion while operating earnings, net earnings and diluted EPS were reported at $18.964 billion, $12.056 billion and $13.23, respectively. Costs and other items determine whether revenue growth reaches the bottom line.

What is the difference between reported and adjusted EPS?

Reported EPS is the GAAP result. Adjusted EPS is a company-defined non-GAAP measure that removes specified items. Use the reconciliation to see each exclusion, its tax effect and whether the item could recur.

Why did Q2 2026 MCR improve?

The company attributed the decrease to benefit design and pricing discipline, member mix and medical-cost management initiatives. Q2 also included $860 million of net favorable prior-period medical development, mostly related to 2026 dates of service, so that item belongs in the interpretation.

What should I look for in UnitedHealth’s cash flow?

Start with operating cash flow and compare it with net income over multiple periods. The 2025 release reported $19.7 billion, or 1.5 times net income; Q2 2026 reported $11.1 billion, or 1.9 times net income. Neither quarterly relationship is a guaranteed full-year conversion rate.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Where can I find the newest UnitedHealth report?

Use the company’s investor-relations filing index and then verify the detailed SEC filing. The Q2 2026 results release is dated July 16, 2026; Q3 2026 was scheduled for October 13, 2026, so confirm whether that release is posted before using Q3 figures.

Can I buy UnitedHealth shares directly from the company?

The issuer says shares are bought through a broker and cannot be purchased directly from UnitedHealth. Reading an earnings report is financial education, not a recommendation to buy or sell the stock.

What the CEO’s language adds

In the January 27, 2026 results release, Chief Executive Officer Stephen Hemsley said: “We confronted challenges directly and finished 2025 as a much stronger company, giving us the momentum to better serve those who count on us and continue to improve our core performance.” Treat that statement as management’s framing, then test it against MCR, operating costs, segment results, cash flow and the balance sheet.

Frequently Asked Questions

How do I read UnitedHealth Group’s earnings report?

Confirm the period and document type, then move from revenue and costs to operating earnings, net income and diluted EPS. Follow with MCR, margins, reported versus adjusted EPS, operating cash flow, leverage, segment results and the latest dated outlook.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Is revenue growth the same as profit growth?

No. Revenue can rise while medical costs, operating expenses, interest or taxes rise faster. Pair revenue with operating earnings, net earnings, margins and cash flow.

What is the difference between reported and adjusted EPS?

Reported EPS follows GAAP. Adjusted EPS is a company-defined non-GAAP measure that excludes specified items, so always read the reconciliation and keep both figures visible.

Why did Q2 2026 MCR change?

UnitedHealth cited benefit design and pricing discipline, member mix and medical-cost management initiatives. The quarter also included $860 million of net favorable prior-period medical development.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Should I annualize one quarter’s operating cash flow?

No. Quarterly cash flow reflects claims, premium collections and working-capital timing. Compare several periods instead of multiplying Q2’s $11.1 billion by four.

Where can I find the newest report?

Check UnitedHealth’s investor-relations filing index and verify the detailed SEC filing. Confirm whether the scheduled October 13, 2026 Q3 release is posted before using Q3 numbers.

Can I buy UnitedHealth shares directly from the company?

No. UnitedHealth says investors purchase its shares through a broker; the company does not sell shares directly.

The Bottom Line

The reliable way to read UnitedHealth Group’s earnings is to connect the numbers: revenue shows scale, MCR shows medical-cost pressure on premium revenue, operating and net margins show profitability, reported and adjusted EPS show two different presentations of earnings, and cash flow and leverage test financial strength. Make every comparison period-specific, segment-aware and tied to the latest release.
