Read Apache logs in two passes: first decode the active LogFormat, then use status codes in access.log to find suspicious requests and the matching entry in error.log to explain the cause. Apache’s field order is configurable, so never interpret column positions until you have checked the server and virtual-host configuration.
What Apache logs tell you
Apache normally records two complementary streams. The access log answers what the client requested and what Apache returned. The error log explains failures, warnings and operational events while Apache starts and handles requests. A 404 in an access log says that the response was a missing-resource error; the error log may reveal whether the path was absent, a rewrite rule failed, or a permission or backend problem produced the response.
| Log | Primary question | Typical contents |
|---|---|---|
Access log (often access.log) |
What request arrived, and what response did Apache send? | Client address, authenticated identity fields, timestamp, request line, final status, response bytes, and optionally Referer and User-Agent. |
Error log (often error.log) |
Why did Apache, a module, CGI program or virtual host behave unexpectedly? | Timestamp, module and severity, process or thread IDs, client address, and diagnostic text; CGI standard error can also appear here. |
File names and locations are not guaranteed. CustomLog selects an access-log destination and format, while ErrorLog selects the error-log destination. Each virtual host can override those settings.
Find the active logging configuration first
Before decoding a line, identify the configuration Apache is actually running. Distribution defaults, included files and virtual-host blocks can all change the destination, format or severity.

- Locate every relevant directive in the main configuration and included files. On a Unix-like system, search the configuration tree with a command such as
grep -R -n -E '^[[:space:]]*(LogFormat|CustomLog|ErrorLog|ErrorLogFormat|LogLevel)' /etc/apache2 /etc/httpd 2>/dev/null. Adjust the directories for your operating system. - Check the virtual host that serves the affected hostname. A request may be written to a host-specific file rather than the global file.
- Read the complete
LogFormatreferenced byCustomLog. The label after a format definition, such ascommonorcombined, is only a name; the quoted pattern determines the fields. - Record the active
ErrorLogFormatandLogLevel. If no custom error format is set, Apache uses its default structure. - After changing configuration, validate it with the platform’s Apache syntax check (commonly
apachectl configtestorhttpd -t) before reloading. A syntax error can prevent a restart and will be reported in the service manager or error log.
Do not assume that the third space-separated value is always a username or that the final value is always a User-Agent. Quoted request and header fields can contain spaces, and custom formats can add, remove or reorder fields.
Decode the Common Log Format
Apache documents the Common Log Format with this pattern:
LogFormat "%h %l %u %t quot;%rquot; %>s %b" common
A representative line might look like 203.0.113.18 - - [15/Oct/2026:09:14:22 +0000] "GET /images/logo.png HTTP/1.1" 200 1842. Decode it according to the configured pattern, not by blindly splitting on every space.
| Token | Meaning | How to interpret it |
|---|---|---|
%h |
Remote host or client address | The address Apache recorded for the connection. It may be a proxy or load balancer unless your deployment is configured to restore the original client address. |
%l |
Identd identity | An identd-provided identity when available. It is commonly -. |
%u |
Authenticated remote user | The username supplied by HTTP authentication, if authentication identified one; otherwise commonly -. |
%t |
Request time | The time Apache received the request, including the numeric timezone offset. |
%r |
Request line | The client’s method, requested resource and protocol, such as GET /apache_pb.gif HTTP/1.0. |
%>s |
Final response status | The status code ultimately sent to the client. The > means the final status after internal processing. |
%b |
Response body bytes | Bytes sent without response headers. A hyphen means no content was returned. Use %B when a numeric zero is preferable to a hyphen. |
The request line is one quoted field. The method is usually GET, POST, HEAD or another HTTP method; the middle part is the path and query string as logged; and the final part is the protocol version. A malformed or unusually long request line can itself point to a client, proxy or scanning problem.
What Combined Log Format adds
The commonly used Combined format is:
LogFormat "%h %l %u %t quot;%rquot; %>s %b quot;%{Referer}iquot; quot;%{User-agent}iquot;" combined

It appends two request headers:
- Referer: the page the client says led to the request. It can be missing, suppressed by privacy settings or fabricated.
- User-Agent: the client-supplied browser, bot or library identification string. It is useful for grouping traffic but is not proof of a device or person.
Both values are untrusted input. Treat them as text for diagnostics, escape them when displaying them in a browser, and do not use them alone for authentication, authorization or bot blocking.
Use status codes for first-pass triage
Group access entries by the first digit of the final status. Apache’s documentation describes the classes as successful responses (2xx), redirections (3xx), errors caused by the client (4xx), and errors in the server (5xx). This classification narrows the search; it does not identify the root cause by itself.
| Class | What it usually means | What to inspect next |
|---|---|---|
| 2xx | The request succeeded. | Unexpected 2xx responses can indicate that a fallback page, health check or rewrite served content when you expected an error. |
| 3xx | Apache redirected the client. | Check the Location behavior in the application or rewrite rules and look for repeated requests that form a redirect loop. |
| 4xx | The request could not be fulfilled because of the client request or resource state. | For 404, verify the URL, document root, rewrite mapping, aliases and file permissions. For authentication-related codes, inspect authorization configuration. |
| 5xx | Apache or an upstream application failed while serving the request. | Match the timestamp and path in the error log, then inspect the relevant module, CGI or application log. |
Sort or count by timestamp, path and status rather than looking at one line in isolation. A burst of 500 responses immediately after a deployment is a stronger lead than a single old 500. Keep the timezone offset in the log line when comparing records from different hosts.
Understand the error log structure
Apache describes the server error log as its most important diagnostic log and the first place to look when startup or runtime problems occur. A typical entry is:
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minute[Fri Sep 09 10:42:29.902022 2011] [core:error] [pid 35708:tid 4328636416] [client 72.15.99.187] AH00124: Request exceeded the limit of 10 internal redirects due to probable configuration error
| Part | Meaning |
|---|---|
| Date and time | When Apache emitted the message, including microseconds in this example. Confirm the timezone before correlating with another system. |
[core:error] |
The module that reported the event and its severity. Other modules and levels can appear. |
[pid ...:tid ...] |
The operating-system process ID and, where applicable, thread ID handling the event. |
[client ...] |
The client address associated with the request. |
| Message code and text | A diagnostic identifier such as AH00124 followed by the explanation. The example’s limit of 10 internal redirects is an illustrative entry, not a universal site limit. |
ErrorLogFormat can change this layout, so custom fields may appear or the familiar brackets may be absent. CGI programs can write diagnostic text to standard error, which Apache commonly sends to the same error log; an application traceback there can explain a 500 that looks generic in the browser.

A repeatable investigation workflow
- Define the incident window. Write down the affected hostname, URL, method, client-visible status and the exact time including timezone.
- Confirm the virtual host and format. Use the active configuration to select the correct access and error files and learn their field order.
- Read complete access lines. Preserve quoted fields while parsing. Extract the request method, path and protocol, final status, bytes and any configured headers.
- Prioritize 5xx, then 4xx spikes. Investigate server-side failures first. Next examine sudden 404 increases, redirect loops and unexpected successful responses.
- Match the error log. Search a narrow time range and compare hostname, client address, path and process or thread context. One request can produce several error messages, so read the surrounding lines.
- Correlate application and proxy logs. Apache may only be the front end. Compare the same timestamp and path with CGI, PHP, application, load-balancer or container logs.
- Use a request identifier when possible. Add
%Lto the access and error formats so Apache can place a shared identifier on related records. Deploy the format change consistently and verify it on a test request before relying on it during an incident. - Check what is being suppressed. A missing diagnostic line does not prove that no event occurred; the current
LogLevelmay be filtering it.
Why Apache returns 404 and how to diagnose it
A 404 means Apache returned a missing-resource response, but several layers can produce it. Start with the exact path from the quoted request line, including case and URL encoding.
File or directory is absent
Map the URL to the serving virtual host’s DocumentRoot, Alias or rewrite target. Check the filesystem path and capitalization; a file that exists under a different case can work on one filesystem and fail on another. Confirm that the Apache worker can traverse parent directories and read the file.

One free scan finds every outdated or missing driver and matches the right update for your exact hardware.Free scan · exact hardware matchRewrite or front-controller mismatch
Review rewrite rules for this hostname and path. A rule can remove a prefix, send the request to a non-existent target or repeatedly rewrite a URL. Compare a known-good URL with the failing one and inspect nearby error-log messages at a more detailed, temporary severity if needed.
Missing-file diagnostics are filtered
Apache notes that “File does not exist” messages for 404 responses may be logged at info. With LogLevel warn, those messages can be omitted. The setting LogLevel warn core:info restores informational messages from the core module while leaving other modules at the warning threshold. Raise verbosity only for the diagnostic window, because higher levels increase volume and may expose more request data.
Application-generated 404
If Apache successfully hands the request to an application, the application may generate the 404. In that case the access log still shows the final status, while the application log and routing configuration explain why the route was not recognized.
Why Apache returns 500 or fails during startup
A 500 is a server-side failure. Open the error log at the matching timestamp before changing the client request; it is the primary diagnostic source for Apache startup and runtime problems.

Configuration or module error
Run the configuration syntax check before restarting. Read the first error, not only the final service-manager summary. An invalid directive, missing module or incompatible module version can prevent Apache from starting. If the server starts but one URL fails, identify the module and severity in the request’s error entries.
Permission and filesystem failure
Verify that the worker account can read the document, traverse directories, access certificates and write to any directory required by the application. A permission denial may appear as a generic 500 to the client while the error log names the path and operation.
CGI or upstream failure
Look for CGI standard-error output, timeout messages, connection failures or malformed gateway responses. Match the process and timestamp with the application’s own log. A backend crash can produce a 5xx at Apache even though the requested file exists.
Redirect or rewrite loop
Repeated internal rewrites can exhaust Apache’s limit. The illustrative error above reports 10 internal redirects and identifies a probable configuration error. Inspect rules that add and remove trailing slashes, force HTTP-to-HTTPS redirects or map a front controller to itself.

Watch logs safely while testing
For a short, controlled test on Unix, Apache documents tail -f error_log to follow new error entries. Use the actual path from ErrorLog; the file may be named differently or be managed by the system journal. Stop following the file when the test ends and avoid leaving broad, unauthenticated log views exposed.
When filtering from a shell, constrain by time or status and preserve context. Simple commands such as grep ' 500 ' access.log can help with a known format, but they can also match a number inside another field. A parser that understands quoted fields is safer for automation. If you write one, split on whitespace only outside quotation marks, retain escaped quotes, and handle a hyphen as a missing value rather than as zero.

Never paste raw logs into a web page, ticket system or terminal without considering control characters. Client-supplied URLs, Referer values and User-Agent strings can contain misleading text or terminal escape sequences. Escape output for its destination, redact credentials and tokens, and preserve the original file only in a restricted location.
Retention, rotation and access control
Logs can contain IP addresses, authenticated usernames, full paths, query strings and text supplied by clients. Limit read access to operators who need it, protect backups, and define a retention period that meets your operational and legal requirements. Rotation prevents an active log from consuming the filesystem; verify after rotation that Apache continues writing to the intended file and that compressed archives inherit appropriate permissions.

Decide which fields must be redacted, who can query them, how long they are retained and how clocks are synchronized across hosts. Do not treat a centralized system as a substitute for understanding Apache’s native formats.
Choose an analysis approach
Apache supplies logging; statistics, alerting and cross-host analysis are a separate operational layer. Choose the simplest approach that meets your scale and control requirements.
| Approach | Strengths | Limitations and checks |
|---|---|---|
| Plain files and shell tools | Immediate, inexpensive and available during an outage; useful for a single host and short incident window. | Quoted-field parsing is easy to get wrong, searches are host-local, and retention, permissions and rotation are your responsibility. |
| Scripted or command-line parser | Repeatable counts by status, path or time; can enforce the configured format and produce scheduled reports. | Must be updated when LogFormat changes and tested against escaped quotes, missing fields and custom formats. |
| Centralized log service | Cross-host search, retention controls, alerting and correlation of access, error and application streams. | Introduces ingestion cost and another access-control boundary; confirm redaction, transport security, storage region, retention and parser support before sending data. |
For any approach, validate parsing against the live configuration, keep access and error timestamps in a common timezone, and alert on sustained 5xx rates rather than a single noisy request.

Successful static request
For 203.0.113.18 - - [15/Oct/2026:09:14:22 +0000] "GET /images/logo.png HTTP/1.1" 200 1842, the client address is 203.0.113.18; both identity fields are unavailable; the request arrived at 09:14:22 UTC; the method is GET; the resource is /images/logo.png; the protocol is HTTP/1.1; Apache returned 200; and it sent 1,842 body bytes. This line does not prove that a human viewed the image or that the User-Agent was legitimate; those fields are either absent in Common format or client supplied in Combined format.

Missing resource
A line ending in 404 0 or 404 - tells you the final status and how the configured byte token represents an empty response. Use the exact path to check document-root and rewrite mapping, then search the error log. If no “File does not exist” entry appears, check whether LogLevel is filtering the core module’s informational message.
Server failure
A line such as POST /checkout HTTP/1.1 followed by status 500 identifies the failing request but not the defect. Match its timestamp and virtual host in the error log, read all nearby module messages, and then compare the same request with the backend application log. The access line is the symptom; the error and application entries supply the diagnosis.
Quick Recap
Quick reference checklist
- Confirm

- Interpret

- For 404, verify path mapping, case, permissions, rewrites and informational logging.
- For 500 or startup failure, read the error log first, then module, CGI and application diagnostics.
- Use
%Lin both formats when you need a shared request identifier. - Restrict, rotate, redact and safely escape logs because much of their text is client supplied.
