Start with a near-term action: choose a quit date or arrange a conversation with a quitline coach, clinician, or pharmacist. Then combine a coping plan with an evidence-based treatment you can use safely. For U.S. adults who smoke cigarettes, counseling and medication each help, and using both can more than double the chance of quitting compared with neither. The best choice depends on your health history, previous attempts, preferences, and local access—not on a single universally superior product.

This guide explains how to begin without treating a lapse as a personal failure. It summarizes U.S. guidance for adults who smoke cigarettes and notes where pregnancy, age, health conditions, or other tobacco products require individualized advice.

What to do first: turn an intention into a plan

You do not need to feel perfectly ready before taking the first step. A useful first move is one you can complete within the next few days.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Take stock without judging yourself. For two or three days, note when you smoke, what happens just before each cigarette, how strong the urge feels, and what you were hoping the cigarette would do (for example, relieve stress, provide a break, or fit a social routine). Record earlier quit attempts and what helped or made them difficult. This information gives a counselor or clinician something concrete to work with.
  2. Choose a start approach. You can set a quit date, make a near-term plan to reduce opportunities to smoke while arranging treatment, or schedule a conversation about medicines before deciding on a date. A quit coach, clinician, or pharmacist can help you choose an approach that fits your circumstances.
  3. Arrange support before the first difficult urge. In the United States, call 1-800-QUIT-NOW to reach your state quitline. Coaching can help you identify triggers, practice responses to cravings, and learn what local services or medication offers are available.
  4. Prepare your environment. Remove cigarettes, lighters, and ashtrays from the places where you usually smoke. Tell at least one supportive person what you are trying and the specific help you want, such as a smoke-free walk, a distraction during a break, or encouragement to call your coach.
  5. Put the plan in writing. Include your start date or next appointment, the treatment you are considering, three responses to common triggers, and the name and number of your support contact.

Counseling is not only encouragement. It helps you prepare for stress, urges, routines, and situations in which relapse is more likely. CDC describes counseling and medication as effective interventions, with greater benefit when they are used together (CDC clinical guidance, May 15, 2024).

How to choose between support, medicine, or both

Think of cessation as two coordinated parts: behavioral support helps you change routines and handle urges, while medication can reduce withdrawal and cravings. You can use either, but combining them generally gives you a stronger plan.

Behavioral support

Support may be one-to-one counseling, a group, telephone coaching, text messages, an app, or a web program. A coach can help you map triggers, rehearse what to do when an urge arrives, and recover after a lapse. Digital tools are especially useful when appointments are difficult to schedule. The quality and availability of programs vary, so ask what type of coaching is included and how often you can contact it.

Medication

For U.S. adults who smoke cigarettes, CDC lists seven FDA-approved cessation medicines: five forms of nicotine replacement therapy (NRT)—patch, gum, lozenge, oral inhaler, and nasal spray—and the prescription medicines varenicline and bupropion SR (CDC, last reviewed October 4, 2024). NRT supplies nicotine without cigarette smoke; varenicline and bupropion are pills that do not contain nicotine. Your clinician or pharmacist can account for your medical history, other medicines, preferences, and the current product label.

What “best” means in practice

There is no single best method for everyone. Compare options using these questions:

  • Would steady, background control or a treatment you use for individual urges fit your day better?
  • Do you prefer an over-the-counter option or are you willing to arrange a prescription?
  • What happened in previous attempts—strong morning cravings, problems with routines, side effects, or difficulty obtaining treatment?
  • Do you have health conditions, take other medicines, or have circumstances that require a clinician’s review?
  • What can you reliably obtain where you live, and can you get follow-up support?

CDC notes that a long-acting patch combined with short-acting gum or lozenge can increase quitting chances, and that varenicline is another approach that can help strongly. These are options to discuss, not instructions to combine or start without checking suitability.

U.S. medicine options at a glance

The table describes categories rather than doses. Follow the current package label and professional instructions; products and rules can change.

Option Nicotine? Typical access in the United States How it fits a plan
Nicotine patch Yes; steady delivery Over the counter Provides background nicotine control over the day.
Nicotine gum Yes; short acting Over the counter Used for individual urges; technique matters.
Nicotine lozenge Yes; short acting Over the counter Used for individual urges without chewing.
Nicotine oral inhaler Yes Prescription Short-acting NRT option selected with a prescriber.
Nicotine nasal spray Yes Prescription Short-acting NRT option selected with a prescriber.
Varenicline No Prescription Pill that changes the brain’s response to nicotine; suitability and side effects require review.
Bupropion SR No Prescription Non-nicotine pill; a prescriber must check whether it is appropriate with your health history and medicines.

FDA advises discussing the benefits, risks, and side effects of cessation medicines with a health professional and following product-specific directions (FDA, “Smoking—Medicines To Help You Quit”).

Matching a method to your preferences and access

Access is part of the decision, not an afterthought. Patch, gum, and lozenge are available over the counter in the United States, while the inhaler, nasal spray, varenicline, and bupropion SR require prescriptions. An OTC product may be easier to start quickly, but a pharmacist can still review technique, interactions, and whether your health history calls for a clinician visit. A prescription option may be preferable if you want a non-nicotine medicine or have had difficulty with NRT, yet it requires an appointment and follow-up.

Ask practical questions before committing: Can I obtain the product consistently? Is there a pharmacist or clinic I can contact if symptoms occur? Does my insurance or local program cover visits or medicine? If I cannot pay or travel, does the quitline connect me with a community service? CDC notes that some state quitlines offer free or reduced-cost starter kits, but eligibility and availability change. Do not delay a support call while trying to identify a perfect product.

Previous attempts are useful evidence. If a patch helped but urges still broke through, ask whether a short-acting option or more intensive coaching could address that pattern. If a pill caused troublesome effects, tell the prescriber exactly what happened and ask about alternatives. A method you can use consistently, with support you can reach, is more useful than an option that is theoretically attractive but unavailable.

Understanding patch, gum, and lozenge combinations

A patch delivers nicotine continuously, while gum or a lozenge acts more quickly when an urge breaks through. CDC identifies this long-acting plus short-acting approach as one that can improve chances of quitting (CDC medicine tips).

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Questions to settle before using NRT

  • Which product and strength match your smoking pattern according to the label or a professional’s advice?
  • How will you remember the daily patch or carry the short-acting product?
  • What symptoms should prompt a call to a clinician or pharmacist?
  • Are you using any other nicotine source that changes the plan?

Do not improvise by adding products, changing amounts, or stopping because of a side effect without advice. A pharmacist can demonstrate gum or lozenge technique, check interactions, and help you decide whether a patch-plus-short-acting plan is reasonable.

Short-acting products are not interchangeable in how they are used. Gum generally requires a chew-and-park technique, while a lozenge dissolves slowly; swallowing or chewing either incorrectly can reduce benefit or increase discomfort. Read the specific label and ask for a demonstration rather than copying another person’s schedule. Keep medicines away from children and pets, and bring the package to a pharmacist if you are unsure which directions apply.

Prescription pills: what to discuss with a clinician

Varenicline

Varenicline does not contain nicotine and works through nicotine-related brain pathways. A prescriber can review your medical history, current medicines, possible side effects, and the timing of treatment. Ask what to do if you miss a dose or notice a concerning symptom rather than changing the plan on your own.

Bupropion SR

Bupropion SR is another non-nicotine prescription option. It is not appropriate for everyone, so the prescriber needs to review conditions, medicines, and other risk factors before prescribing. Discuss alcohol use, prior reactions, and what monitoring is needed.

Prepare for the appointment

Bring a complete list of medicines, supplements, and nicotine products; your smoking pattern; prior quit medicines; allergies; and any symptoms or diagnoses you think matter. Say whether you are pregnant, breastfeeding, trying to become pregnant, or under 18. Ask how long support will continue, what follow-up is planned, which effects are expected, and which require urgent attention. If you use more than one tobacco product, describe each one and when you use it. Honest details help the professional avoid applying the wrong instructions.

Situations that need individualized guidance

Pregnancy or breastfeeding

CDC advises people who are pregnant or breastfeeding not to use cessation medicines without discussing them with a doctor. During pregnancy, behavioral counseling is encouraged first; medication requires a careful case-by-case assessment because the balance of evidence, benefits, and potential harms is uncertain (CDC clinical guidance). Contact an obstetric or primary-care professional promptly rather than delaying support.

Under age 18

People younger than 18 should not use these medicines without a doctor’s advice. A pediatric or adolescent clinician can tailor behavioral support and determine whether any medicine is appropriate.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Health conditions or other medicines

Tell the clinician or pharmacist about heart or other medical conditions, mental-health history, allergies, and every prescription, over-the-counter medicine, supplement, or nicotine product you use. Ask what side effects are expected, what is urgent, and when follow-up should occur.

Tobacco products other than cigarettes

CDC’s medicine instructions are framed for adults quitting cigarettes. If you use cigars, chewing tobacco, snuff, hookah, heated tobacco, roll-your-own tobacco, or more than one product, seek tailored advice rather than transferring cigarette instructions. WHO’s 2024 adult guideline covers cigarettes, waterpipes, smokeless tobacco, cigars, roll-your-own tobacco, and heated tobacco products, but regulatory approval and availability differ by country (WHO, July 2, 2024).

A trigger-and-craving plan you can use immediately

Cravings are time-limited events, even when they feel intense. Plan a response for each high-risk situation you recorded.

Trigger Preplanned response
Morning coffee Change the location, drink water first, and call or text your support person during the first week.
Work break Take a short walk, stretch, or use a non-smoking break area.
Stress or anger Delay the decision, breathe slowly, and contact a coach before acting on the urge.
Driving Remove cigarettes and lighters, prepare a different audio routine, and keep an approved short-acting treatment available if your clinician recommends it.
Being around smokers Tell friends your plan, avoid smoking areas temporarily, and arrange an exit or check-in call.

Use the same response repeatedly so it becomes a routine. A counselor or quitline coach can help you adapt the list to your strongest triggers and rehearse the wording you will use when someone offers a cigarette.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Make difficult moments easier to predict

Put support contacts in your phone, set a reminder for your chosen start date, and decide where medicines will be stored. Plan for meals, alcohol or social events, commuting, and the first morning after quitting—times when automatic routines can be strong. Ask a household member not to smoke around you and to avoid offering cigarettes. If a particular place repeatedly leads to smoking, change the route or schedule temporarily. These changes are not permanent rules; they give your new routine time to strengthen.

Getting coaching and medication in the United States

Call 1-800-QUIT-NOW to reach a state quitline. Services can include telephone coaching, planning for triggers, and referrals. Some quitlines offer free or reduced-cost medication starter kits, but offers vary by state and change over time (CDC, “Getting Coaching with Medicine”).

If you prefer digital help, CDC lists online, texting, and app-based support. Ask whether a program provides human coaching, how often messages arrive, how personal information is handled, and whether it connects you to local clinical care. Outside the United States, begin with a local health service or national quitline because medicine names, approvals, and payment systems differ.

Coaching can continue after the quit date. Follow-up gives you a place to report cravings, troubleshoot a trigger, and review side effects before they derail the attempt. If you miss a call or message, re-engage rather than assuming you have dropped out. Repeated contacts are consistent with the chronic, relapsing nature of tobacco dependence.

What to do after a lapse or a difficult first week

A cigarette after your quit date is information, not proof that the attempt is over. Note what preceded it, remove remaining cigarettes, and contact your coach, clinician, or pharmacist. Revise one part of the plan—such as avoiding a particular setting, adding a support call, or reviewing treatment—then continue. Do not compensate by taking extra medicine or combining products without professional advice.

If cravings remain unmanageable, the medicine is not working as expected, or side effects appear, contact a healthcare professional instead of silently stopping or adding treatments. Tobacco dependence can relapse, so repeated intervention and longer-term support are normal parts of care (CDC clinical guidance).

A simple checklist for your next 24 hours

  • Write down three common triggers and one alternative response for each.
  • Choose a quit date or book a conversation with a clinician, pharmacist, or quitline coach.
  • Call 1-800-QUIT-NOW if you are in the United States, or identify your local quit service elsewhere.
  • List every medicine, supplement, and tobacco or nicotine product to discuss with a professional.
  • Tell one supportive person exactly how they can help.
  • Remove cigarettes and smoking cues from your main environment.
  • Decide how you will respond to a lapse before it happens.

More than 60% of the world’s 1.25 billion tobacco users—over 750 million people—want to quit, yet 70% lack access to effective cessation services, according to WHO’s July 2, 2024 announcement. As WHO Director of Health Promotion Dr Rüdiger Krech put it, “The immense struggle that people face when trying to quit smoking cannot be overstated.” Reaching for structured support is a practical response to that difficulty, not a sign that you should have managed alone.

Frequently Asked Questions

Can I quit without medicine?

Yes. Counseling, quitlines, text programs, apps, and web support can help you plan and handle triggers. For U.S. adults who smoke cigarettes, medication can add benefit, and counseling plus medication generally works better than either alone. Discuss the options with a clinician or pharmacist.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Is a nicotine patch plus gum or a lozenge safe for everyone?

Not for everyone. CDC identifies the combination as an approach that can improve quitting chances, but product directions, health history, pregnancy, age, other medicines, and other nicotine use matter. Ask a clinician or pharmacist before combining treatments.

What if I smoke cigars, use hookah, or use smokeless tobacco?

Do not assume cigarette-medicine instructions apply. CDC’s instructions are aimed at adults quitting cigarettes. A clinician or quitline can tailor a plan; WHO’s 2024 guideline includes several non-cigarette products, with availability and approvals varying by country.

How soon should I seek help after a lapse?

As soon as practical. Identify the trigger, contact your coach or healthcare professional, and adjust the plan. Do not take extra medicine or add products to compensate without professional advice.

Which is better: an over-the-counter product or a prescription?

Neither is universally better. OTC patch, gum, and lozenge may be easier to obtain, while prescription NRT inhaler or spray and the pills varenicline and bupropion offer other approaches. A clinician or pharmacist can match access, preferences, health history, and previous experience.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How do I prepare for a pharmacist or clinician visit?

Bring your smoking pattern, previous quit attempts, all medicines and supplements, allergies, health conditions, and any other nicotine or tobacco use. Ask about expected effects, warning symptoms, follow-up, and what to do if a dose is missed or a craving remains strong.

Are quitline medicines free?

Some U.S. quitlines offer free or reduced-cost starter kits, but offers and eligibility vary by state and can change. Call 1-800-QUIT-NOW to ask what is available locally.

Can I use a quit app instead of counseling?

Apps, texting, and web programs can support planning and reminders. Ask whether a service includes human coaching and whether it can connect you to clinical care. Combining digital support with counseling or medication may provide more help than relying on reminders alone.

The Bottom Line

Choose one next step today: set a quit date, call a quitline, or arrange a medicine conversation. Pair practical trigger planning with support, use medicines only as directed, and seek individualized advice when pregnancy, age, health conditions, other medicines, or non-cigarette tobacco products are involved.
