To stop a sensitive MCP action from reaching production before a person reviews it, pause the tool call before execution, show the reviewer the exact tool and arguments, and resume only after an explicit approval for that call. Keep authentication, authorization, and input validation in the MCP server: a confirmation prompt, model instruction, or tool annotation is not a substitute for those checks.

What an approval gate does—and does not do

An approval gate is a human decision point between a proposed tool call and its execution. In the OpenAI Responses API flow, the application receives an mcp_approval_request, reviews the requested tool and arguments, and returns an mcp_approval_response. Each approval applies to one tool call; it is not blanket permission for later calls.

That decision answers whether the person approves the proposed operation. It does not prove that the caller is allowed to perform it. The MCP server must still authenticate the request, authorize the caller for the target and action, validate the arguments, and enforce those checks before changing or exposing production data.

The MCP draft specification recommends human oversight and confirmation for operations, but it does not prescribe a particular user interaction model. Approval behavior therefore depends on the client or runtime you deploy; do not assume every MCP client supplies the same gate.

1. Identify which actions need review

Start with the effects a tool can cause, not its name. Inventory the tools available in the workflow and the credentials they use. Classify actions by their impact, reversibility, and the production data or systems they can reach.

  • Separate reads that cannot change state from writes and other consequential actions.
  • Consider deletes, deployments, external messages, permission changes, and actions that expose sensitive data as candidates for review.
  • Decide which operations need approval, whether any actions should be unavailable to the workflow, and which authorized reads can proceed without a blocking prompt.

OpenAI’s MCP server guidance says the readOnlyHint should be true only when a tool cannot change state, and destructiveHint should be true for actions with irreversible or difficult-to-reverse outcomes. These annotations should describe actual behavior; they are signals to clients, not enforcement controls.

2. Choose the approval layer your runtime supports

Approval configuration is product- and runtime-specific. The two documented OpenAI options below are related, but not interchangeable across all MCP clients.

Implementation Approval configuration Tool filtering Where to verify behavior
Responses API with hosted or remote MCP calls Configure require_approval; handle the approval request and response in the application flow. A decision applies to one tool call. allowed_tools can restrict which imported tools are available. Confirm the deployed API flow pauses for the proposed call and resumes only after the corresponding approval response.
Agents SDK with a local MCP server The documented require_approval configuration supports always, never, and per-tool policy mappings. Use the policy supported by the SDK configuration and expose only the tools needed. Check the current SDK documentation and test the policy in the runtime version you deploy.

A practical policy is to leave appropriately authorized, non-mutating operations available without a blocking prompt and require review for sensitive or consequential actions. That is an implementation choice, not a universal MCP protocol rule. Requiring approval for every call is also possible where supported, but it adds interaction and latency.

3. Make the pending call specific and reviewable

Before asking for a decision, present enough detail for a person to understand what the proposed call will affect. Show the tool name, target, action, and arguments in a readable form. Avoid a vague “Allow?” prompt that hides the operation or its impact.

  1. Receive the approval request from the configured API or runtime.
  2. Display the specific tool call and its arguments, including the target or record where relevant.
  3. Collect an explicit approval or rejection for that call.
  4. On approval, send the matching approval response through the documented flow; on rejection, do not execute the proposed action.

Keep the reviewed operation aligned with the operation sent to the server. If the application changes arguments after approval, the person may have approved a different action from the one that executes. The approval flow does not by itself guarantee argument immutability, so design the handoff to preserve that correspondence.

4. Enforce access and validate input in the MCP server

The server is the authorization boundary. Authenticate the caller and enforce authorization on every request, regardless of whether the client displayed a prompt. Scope actions to validated credentials, check permission for the requested operation and target, validate the input, and only then perform the operation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

OpenAI’s MCP server guidance states: “Enforce authorization in the MCP server for every request; never rely on the model to decide whether a user has access.” A human approval is not proof of access, and a model choosing a tool is not an authorization decision. Tool annotations likewise do not replace server-side authorization, validation, or confirmation.

5. Treat missing approval as no approval

Define what happens if a reviewer rejects the request, does not respond, or the approval handoff fails. The safe implementation is to stop the proposed action unless the required approval is received. Do not interpret a timeout, missing response, or interrupted workflow as permission to proceed. This fail-closed behavior is an implementation recommendation; it is not a behavior guaranteed universally by the MCP protocol.

6. Test the gate and the server boundary

Use MCP Inspector during development to examine the server and exercise both the human gate and the server’s own checks. Test representative valid and invalid inputs, including calls the reviewer rejects and requests from callers who lack permission.

  • Inspect initialization, advertised tools, input schemas, annotations, results, and errors.
  • Check that the approval prompt displays the relevant target and arguments, and that rejection or an absent decision does not execute the action.
  • Verify authorization for private data and write operations independently of the approval prompt.
  • After deployment, use the Inspector against the production endpoint to check the same core behaviors.

7. Prepare the production deployment

Approval is one control in a broader production setup. OpenAI’s deployment guidance recommends using the host’s secrets system for production credentials, setting timeouts and rate limits for expensive or externally visible tools, removing debug responses and unnecessary personal data, and ensuring logs do not contain access tokens or sensitive tool results.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Review what information a tool sends to a third-party server, since approval of an action does not eliminate the risk of sharing sensitive data. Also consider prompt injection in untrusted content and unexpected changes in remote tool behavior. Keep an eye on what is shared and on the behavior of tools after deployment.

When choosing infrastructure, assess runtime and dependency support, streaming behavior, cold-start and request latency, network access, residency and compliance needs, secrets management, logging and alerting, and rollback or versioning support. The cited implementation guidance does not establish one hosting provider as best.

Keep the controls separate

  • Tool filtering limits which actions are exposed to a workflow.
  • Human approval lets a person review a specific proposed call before it proceeds.
  • Server authorization decides whether the authenticated caller may perform that action on that target.
  • Input validation checks that the request is well-formed and acceptable before execution.
  • Logging and monitoring help operators understand system behavior without exposing secrets or sensitive results.

Use the current API or SDK documentation for the runtime you deploy, because configuration and product behavior can change.
