Use the least health information an AI app needs, remove direct identifiers, deny unnecessary permissions, and verify how the service stores, shares, trains on, reviews, and deletes your data. “HIPAA compliant” is not a universal safety label: many consumer apps are outside HIPAA, yet may still fall under the Federal Trade Commission (FTC) Act and the Health Breach Notification Rule. Treat every prompt, uploaded record, voice clip, image, and connected health feed as sensitive information.
Why AI medical apps require extra caution
An AI medical app can turn a question into a response, summarize a record, interpret an image, or connect information from a wearable or health platform. Each feature can create additional copies of identifiable health information in app databases, logs, backups, analytics systems, support tools, or model-improvement pipelines. The privacy risk is therefore not limited to what appears on your screen.
Health information can reveal diagnoses, medications, reproductive care, mental-health treatment, insurance status, genetic traits, and family details. A prompt that looks anonymous may become identifiable when combined with an email address, account identifier, device data, location, or a connected medical record. The safest default is to share only what is necessary for one task, for one time, with one service.

The FTC says, “Consumers care about the privacy and security of their health-related information.” It also says that a company making privacy promises, expressly or by implication, must live up to them. Read the service’s actual policy and settings rather than relying on a badge, app-store description, or a claim such as “HIPAA secure.”

Is an AI medical app covered by HIPAA?
HIPAA coverage depends on the app’s role and its data flow, not on the fact that it handles health information. HIPAA’s Privacy and Security Rules apply to covered entities, such as certain health plans, health-care providers, and clearinghouses, and to business associates performing defined services for them. A direct-to-consumer app that you download and use independently may not be a covered entity or business associate.
That does not make an uncovered app unregulated. The FTC Act can apply when a company’s privacy or security practices are unfair or deceptive. The FTC’s Health Breach Notification Rule can also reach certain consumer health apps outside HIPAA. In April 2024, the FTC announced final rule changes clarifying that a qualifying breach can involve unauthorized acquisition or unauthorized disclosure of identifiable health information.
Look for a concrete explanation of the company’s legal role, not an unsupported certification slogan. The FTC has warned about marketing terms such as “HIPAA Compliant,” “HIPAA Secure,” and “HIPAA Certified” when they are not backed by specific, verifiable practices. A trustworthy explanation identifies what information is collected, who receives it, how long it is retained, how it is deleted, and how incidents are handled.

The answer can differ by product, account type, health-care partner, and country. If the app is supplied through a clinic or insurer, ask that organization whether it is acting as a covered entity or business associate and which data flows are covered.
Map the app’s data flow before entering anything
Do not start by pasting a medical record. First identify the path from your device to the service and back. Record the answers in a note or screenshot so you can revisit them after an app update.
| Question | What to check | Why it matters |
|---|---|---|
| What is collected? | Prompts, chat history, uploaded documents, photos, voice recordings, device identifiers, diagnostics, cookies, and payment or account details. | Every additional data type creates another exposure and deletion requirement. |
| Where does it come from? | Your typing, camera, microphone, photo library, wearable, electronic-health-record connection, contacts, location, or advertising identifier. | A connected source may continuously send information even when you are not actively chatting. |
| Why is it used? | Providing the answer, safety monitoring, analytics, product improvement, advertising, fraud prevention, or legal compliance. | “Service operation” may be broader than the task you intended. |
| Who receives it? | Cloud hosts, model providers, analytics vendors, support staff, contractors, advertisers, health-care partners, or authorities under a legal process. | Third-party sharing can create obligations and risks outside the app’s interface. |
| How long is it kept? | Active account retention, deleted-chat retention, backups, security logs, de-identified copies, and legal holds. | Deleting a chat from the screen may not immediately erase every copy. |
| How is it removed? | Per-chat deletion, account deletion, data-export and correction tools, support requests, and stated deletion times. | You need a usable route to remove or correct information before relying on the service. |
If the policy cannot answer these questions in plain language, treat that uncertainty as a reason not to upload a full record. A privacy interface that clearly exposes these choices is safer to evaluate than one that hides them in a generic policy.

Minimize and de-identify every prompt
The FTC’s mobile-health guidance starts with “Minimize data.” Remove details that do not change the answer you need. For example, ask, “What questions should an adult with a new rash ask a clinician?” rather than pasting a named patient’s complete chart.
- Remove names, usernames, postal addresses, phone numbers, email addresses, and medical-record numbers.
- Remove insurance or member IDs, exact birth dates, and appointment or admission dates unless the calculation genuinely requires them.
- Generalize location to a region or time zone instead of a street address or precise coordinates.
- Replace a person’s identity with a neutral label such as “Patient A,” and avoid unusual combinations of facts that could identify them.
- Crop documents and images to the relevant section; cover barcodes, faces, labels, and account numbers before uploading.
- Use age ranges instead of exact ages when an exact age is not medically necessary.
De-identification is not the same as anonymity. Rare conditions, dates, occupations, family relationships, and images can identify someone when combined. If a clinician or caregiver needs to recognize the person, use the clinician’s approved portal or workflow instead of a general-purpose chatbot.
Do not paste an entire longitudinal record to answer one narrow question. Send the smallest excerpt, with context sufficient to avoid a misleading answer. Check the response yourself; removing identifiers does not make an AI-generated medical conclusion accurate or safe.
Review model training, human review, and sharing controls
Model-improvement use
Find out whether prompts, uploads, transcripts, or feedback are used to train or improve models. The answer may differ between consumer, business, education, and health-care accounts, and between an optional setting and a contractual commitment. Look for an in-product control that is specific about which data it covers, when it takes effect, and whether previously collected data is affected.
Human access
Some services permit trained reviewers, support personnel, or safety teams to inspect conversations or uploaded material. Determine the purpose, the categories of staff or contractors involved, and whether you can opt out. A statement that data is “not sold” does not by itself answer whether people can view it for review or support.

Pick the symptom - the matching free tool is one click away.
Third-party disclosure
Check whether information is sent to cloud infrastructure, analytics providers, advertising partners, model suppliers, health-care organizations, or other affiliates. Distinguish a necessary processor from optional advertising or cross-service sharing. Read the policy’s definitions of “share,” “sell,” “service provider,” and “de-identified,” because those terms can change the practical scope of a promise.

Retention and deletion
Determine whether chats are retained until you delete them, for a stated period, or indefinitely in backups and security logs. Check whether deleting an item removes uploaded files, voice recordings, derived profiles, and copies held by vendors. Save a screenshot of the setting and the date you changed it; policies and interfaces can change.
Turn off permissions the app does not need
The FTC’s mobile-health best practices say to “Limit access and permissions.” On your phone or computer, open the operating system’s app-permission page and deny optional access to contacts, location, microphone, camera, photo library, Bluetooth, advertising identifiers, and health-platform data. Re-enable one permission only when a feature requires it, then disable it again if the feature is temporary.
A symptom-chat feature may need typed text but not contacts or continuous location. An image-analysis feature may need one selected photo rather than the entire photo library. A wearable integration may need a specific measurement rather than every category in your health account. Check permissions again after major app updates, because an update can add or reset access.

Secure the account, device, and connections
Authentication and software maintenance protect the data that remains after minimization. The FTC specifically advises users to “Keep authentication in mind.” Apply this sequence:
- Use a unique, long password for the app. Do not reuse a password from email, banking, or your health portal.
- Enable multi-factor authentication, preferably with an authenticator app or security key when the service supports it. Store recovery codes in a protected place.
- Review active sessions and connected applications. Sign out devices you no longer use and revoke integrations that are unnecessary.
- Keep the operating system, browser, app, and security software current. Updates often repair vulnerabilities that could expose stored chats or tokens.
- Use a screen lock and device encryption. Avoid entering health information on shared or unmanaged computers.
- Be cautious on public networks and with links in support messages. Open the app or provider’s known website directly instead of submitting credentials through an unexpected link.
Encryption, access controls, and incident response are important, but a policy should explain them at a useful level. “Bank-grade security” is not a substitute for naming authentication options, transmission and storage protections, account recovery, and breach-notice procedures.

| Comparison axis | Questions to ask | Prefer |
|---|---|---|
| Data collected | Does it accept text, files, images, voice, location, contacts, or health-platform feeds? | The narrowest collection that supports the feature. |
| Training and improvement | Are prompts or uploads used for model training or product improvement? Is there an effective opt-out? | A clear, granular control with a stated scope. |
| Third-party sharing | Which processors, model providers, advertisers, or partners receive data? | Specific recipients and purposes, with optional sharing disabled. |
| Retention and deletion | Can you delete individual chats and the account? What remains in backups? | Visible controls and a defined deletion process. |
| Human review | Can staff or contractors inspect content, and for what reason? | Limited, explained review with an opt-out where feasible. |
| Permissions | Does the app request contacts, location, microphone, camera, or broad health access? | Permission requests tied to an understandable feature. |
| Security disclosure | Are encryption, authentication, access control, and recovery described? | Concrete technical and account protections rather than slogans. |
| Breach response | How will the provider notify users and what support is offered? | A documented incident channel and clear notice commitments. |
| Regulatory role and geography | Is it operating for a covered health-care organization, and which law governs your account? | A precise explanation rather than a blanket HIPAA claim. |
| Privacy-interface clarity | Can an ordinary user find and understand the controls? | Plain language, dated settings, and confirmation of changes. |
If one app cannot answer a comparison question, mark it “not stated” rather than assuming the most favorable interpretation. Choose the service with the narrower data flow and clearer controls for the particular task.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minuteUsing a general-purpose chatbot safely
A general-purpose chatbot is designed for broad conversation, not necessarily for handling a clinical record. Avoid uploading full records, insurance documents, prescription labels, or identifiable photographs when a clinician portal or a purpose-built service with explicit health-data controls is available.
For low-risk educational questions, use a de-identified prompt and ask for general information, questions to discuss with a licensed professional, or an explanation of terminology. Do not treat the answer as a diagnosis, emergency triage, medication change, or substitute for a clinician. For urgent symptoms, contact local emergency services or a qualified health professional rather than relying on an app.
Check the account’s privacy mode and history controls before use. A setting that prevents future model improvement may not erase existing chats, and deleting visible history may not remove backups or data already disclosed to a provider. Confirm the scope in the service’s current policy and settings.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Delete, export, or correct your information
Deletion is a process, not merely tapping a trash icon. Use this order:
Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →- Export information you are legally entitled to keep if you need a copy for a clinician, dispute, or record.
- Delete individual conversations, uploaded files, images, and voice recordings from the app’s history.
- Disable optional model-improvement, advertising, analytics, and connected-health sharing controls.
- Revoke third-party connections and sign out active sessions.
- Submit an account-deletion or privacy request through the provider’s documented channel, specifying the account identifiers needed to locate your data without sending unnecessary medical details.
- Ask what will remain in backups, security logs, legal holds, or de-identified datasets, and how long those copies persist.
- Save confirmation messages, ticket numbers, screenshots, and dates. If inaccurate information affected an output or profile, request correction and retain the response.
Some data may be retained where a provider has a legal or security reason, but the provider should explain the category and retention period. If the answer is vague, do not assume the deletion is complete.

What to do if the app reports a breach
A breach notice may describe unauthorized acquisition, unauthorized disclosure, or both. Read the notice for the affected data, dates, account types, protective measures, and instructions. Under the FTC’s 2024 Health Breach Notification Rule changes, a qualifying incident can trigger notice duties for certain health apps even when HIPAA does not apply.
- Use the provider’s verified incident page or support channel, not a link in an unsolicited message, to confirm what happened.
- Change the app password immediately, and change any other account password where it was reused. Enable multi-factor authentication.
- Revoke active sessions, API tokens, health-platform connections, and third-party apps you do not recognize.
- Review account, email, payment, insurance, and health-portal activity for unauthorized changes. Keep copies of relevant notices and dates.
- Follow the provider’s instructions about credit or identity monitoring when offered, but do not treat monitoring as a replacement for password and session changes.
- Ask what information was accessed, whether it was downloaded or disclosed, how the provider contained the incident, and what remediation is available.
- Follow instructions from applicable regulators and contact local law-enforcement or consumer-protection authorities if identity theft or fraud occurs.
Do not publish sensitive details from the breach notice in a public forum. If the provider cannot explain the scope or response, document your questions and seek advice from the relevant regulator or a qualified privacy professional.
What organizations should learn from NIST guidance
Organizations using AI with electronic protected health information should distinguish consumer privacy settings from a compliance program. NIST published SP 800-66r2, Implementing the HIPAA Security Rule: A Cybersecurity Resource Guide, on February 14, 2024. It is primarily an implementation guide for regulated entities, but its themes explain the controls users should expect: risk assessment, authentication, least-privilege access, secure transmission and storage, auditability, contingency planning, and incident response.
A clinic or employer should verify vendor contracts, permitted uses, access logs, workforce training, retention, deletion, and breach procedures before allowing identifiable data into an AI system. A consumer app’s privacy toggle cannot replace those organizational safeguards.
Red flags that should stop an upload
- The app claims “HIPAA Certified” or “HIPAA Secure” without identifying its legal role, covered services, or concrete controls.
- The policy says data may be used for “any business purpose” or model improvement without a meaningful choice.
- Deletion is available only by closing an account, with no explanation of backups, vendors, or retained logs.
- The app requests contacts, continuous location, microphone, camera, or full health-platform access for a feature that does not need them.
- There is no clear incident-notice channel, account-recovery method, or list of third-party recipients.
- Support asks you to send a full medical record when a de-identified excerpt would answer the question.
When a red flag appears, stop, remove any test data you entered, revoke permissions, and choose a workflow with clearer limitations. Privacy clarity is itself a useful security feature: if you cannot tell what happens to the data, you cannot make an informed risk decision.
A repeatable privacy check before every use
- Purpose: State the one question the app must answer.
- Minimum data: Remove identifiers, unrelated history, and unnecessary images or files.
- Permissions: Allow only the sensor, library, or connection required for that task.
- Policy: Confirm retention, model use, sharing, human review, deletion, and breach response.
- Account: Use a unique password, multi-factor authentication, current software, and a private device.
- Afterward: Delete the chat or upload when appropriate, revoke temporary access, and record any privacy-setting change.
This workflow reduces exposure without assuming that any single label, vendor, or technology guarantees confidentiality. It also gives you evidence of the choices you made if the service later changes its policy or reports an incident.
