Protect an AI application from prompt injection by assuming that every user message, retrieved document, file, web page, memory entry, and tool result may contain hostile instructions. Then limit what the model can access, enforce permissions in application code, require approval before consequential actions, and test the complete workflow—including content the model reads indirectly. No prompt wording or filter can guarantee that an LLM will resist every injection.

What prompt injection is—and why your application is at risk

Prompt injection is an attempt to manipulate a large language model (LLM) with crafted input so it follows an attacker’s intentions. The attack may try to change an answer, expose sensitive context, or make the application misuse a connected capability. The impact depends not just on the model, but on the data, tools, and permissions surrounding it.

A direct injection arrives in the user’s input—for example, a request that tells the model to disregard the task and reveal information it should not disclose. An indirect injection arrives in material the application asks the model to process, such as a webpage, uploaded document, retrieved passage, or tool result. The user may not see that material as an instruction, but the model may still parse it as one.

This matters especially in retrieval-augmented generation (RAG) and tool-using applications. A RAG system adds retrieved content to the model’s context; an agent may also be able to call APIs, read files, update records, or send messages. If hostile content influences the model, the consequences depend on what those connected capabilities allow. OWASP’s Gen AI Security Project describes both direct and indirect prompt injection in its LLM01:2025 Prompt Injection guidance.

LLMs commonly process natural-language instructions and data together. Separators, labels, and carefully written system prompts can help communicate intended roles, but they are not a dependable security boundary by themselves. OWASP states that “there is no fool-proof prevention within the LLM.” Treat defenses as layers that reduce risk and limit the damage a successful injection could cause—not as a promise that an attack will never work.

Start with the security boundary, not a stronger prompt

The most important design question is not whether the model can be persuaded to ignore malicious text. It is what the application will let the model do if it is persuaded. Keep authorization, credential handling, and permission checks in application or tool code. Do not make access control depend on the model correctly interpreting a natural-language instruction.

OWASP’s LLM01: Prompt Injection guidance recommends least privilege: “restricting the LLM to only the minimum level of access necessary for its intended operations.” In practice, scope access to the task, the user, and the specific operation wherever possible. A model that only drafts a reply should not also have a tool that sends it. A workflow that summarizes one authorized document should not have unrestricted access to a user’s entire file store.

Consider each capability as an authority granted to the workflow. Reading a record, changing it, publishing it, and deleting it are distinct abilities with distinct consequences. Separate them where practical, and make the application decide which are permitted for the current user and operation. If a model proposes an action outside that authority, reject it in code rather than asking the model to reconsider.

Map what enters the model and what it can affect

Before changing prompts or adding a classifier, draw the data and action paths for each AI workflow. This inventory is an implementation method based on OWASP’s distinction between direct and indirect inputs and its recommendations on privileges, testing, and action control.

  1. List every input source. Include the user’s prompt, conversation history, retrieved passages, uploaded files, webpages, memory, search results, and output from tools or APIs. Note which sources are controlled by the user, an external party, or your own application.
  2. List every capability available. Record which tools can read data, write data, contact other systems, or trigger external side effects. Identify what each tool can access and under whose identity it operates.
  3. Mark sensitive data and consequential actions. Identify information that should not be disclosed in a response, as well as operations that are privileged, destructive, hard to reverse, or externally visible.
  4. Trace the enforcement point. For every protected read or action, find the application or tool code that checks identity, authorization, scope, and required approval. If the only safeguard is an instruction in a prompt, the boundary is not independently enforced.

Use the map to find mismatches. A read-only summarization task may not need write-capable tools. A tool may be more broadly privileged than its current workflow requires. A retrieved passage may cross into a more privileged agent without a clear boundary. These are concrete design issues to fix even if no attack is currently known.

Enforce least privilege outside the model

Give each workflow only the data access and tool permissions necessary for its task. Apply the same principle to individual tool calls: check whether the current user may perform the requested operation on the requested resource, and reject calls that exceed that scope. Where possible, use permissions scoped to the end user and operation instead of broad, long-lived access that applies to every request.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Keep credentials out of model context. Store secrets in the application’s protected configuration or credential system, and have application code use them when an authorized tool call is allowed. Do not place a secret in a prompt and expect the model to keep it confidential.
  • Authorize every operation at execution time. A model’s claim that an action is permitted, or that approval has already been granted, is not proof of authorization. The executing code should verify the relevant permission itself.
  • Scope tools to the workflow. Provide a workflow with the small set of tools it actually needs. Separate read and write capabilities when that helps limit consequences.
  • Validate arguments in code. Check the target resource, operation, and relevant parameters before a tool performs work. Do not let a plausible-sounding model explanation substitute for those checks.
  • Fail closed on authorization problems. If the application cannot establish that an operation is allowed, it should not perform that operation. Handle the refusal as an application outcome, not as an invitation for the model to find another route.

These controls do not prevent a model from generating an unsafe suggestion. They reduce the authority available to turn that suggestion into a harmful result. OWASP’s prompt-injection guidance and its LLM Prompt Injection Prevention Cheat Sheet both emphasize least privilege and layered defenses.

Handle retrieved, uploaded, and external content as untrusted

Content does not become trustworthy because it came from a search index, document store, browser, or internal-looking tool. Treat external and user-controlled material as data to analyze, not as a source of authority over the application. Make its origin and role clear in the workflow, and separate it from trusted instructions where the architecture permits.

Clear labeling and structural separation can help the model distinguish a document to summarize from instructions that govern the task. They are supporting measures, not guarantees: the model may still follow hostile text embedded in material it processes. RAG does not eliminate prompt injection, and fine-tuning does not make an application immune. Neither replaces access controls at the application and tool boundaries.

For workflows that handle risky external material, one possible layer is a quarantined parsing or classification stage with no tool access. It can extract or classify content before a more privileged component uses a constrained result. This limits what that stage can do directly, but it does not make its output inherently safe: a later component may still be influenced by content passed along. Decide what information crosses the boundary, label it, and continue to enforce authorization downstream.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Input and output filtering can add another layer. For example, an application may inspect content or model output for patterns that warrant blocking or review. A filter can miss attacks or flag benign content, so do not treat a classifier, regular expression, delimiter, or second model as the control that grants permission. Whether a call is allowed must remain a decision enforced by the application.

Put human approval at the action boundary

Require human approval before high-impact, privileged, destructive, or irreversible operations. Examples include sending or deleting messages, publishing material, making a purchase, or changing an important record. The examples describe kinds of actions to evaluate; the right threshold depends on what your application does and what authority it has.

The approval checkpoint belongs in the code path that performs the operation. Showing a warning to the model or asking it to request approval in a prompt is not enough if another route can execute the action without that check. Likewise, a model-generated statement that a person approved an action should not count as approval.

  1. Have the model propose a specific action. Represent the proposal in a form the application can inspect, including the operation and its target.
  2. Check authorization and policy in code. Confirm that the current user may request that operation and that it falls within the workflow’s allowed scope.
  3. Show the reviewer what will actually happen. Present the action and its relevant arguments clearly enough for a person to assess the target and consequence. Do not ask for approval of a vague summary while hiding the executable details.
  4. Execute only after a valid approval. Bind approval to the specific proposed action and verify it at the execution boundary. If the proposal changes, obtain the approval required for the changed action.
  5. Record the outcome where appropriate. Keep an application-level record of the proposal, authorization and approval result, and execution outcome if your operational and privacy requirements call for it.

Approval is a human control, not a substitute for least privilege or authorization. A person should not be presented with an action the application could already have rejected as unauthorized. OWASP’s prompt-injection and excessive-agency guidance recommends approval for privileged or high-impact actions and places the check at the implementation boundary.

Build tests around the properties the application must preserve. Use harmless test data and instrumented tool substitutes so that a test cannot send real messages, alter real records, or expose real secrets. OWASP’s AI/LLM Application Security Testing and Red Teaming guidance recommends adversarial simulations, including indirect injection through tool outputs, with safe test setups.

  • Direct-input cases: provide user messages that try to override the task, request protected information, or induce an operation outside the user’s authority.
  • Indirect-content cases: place hostile instructions in a test document, webpage, retrieved passage, or other content the application processes.
  • Tool-output cases: have a test tool return content that attempts to redirect the model, request data disclosure, or prompt another tool call.
  • Authorization cases: check that the model cannot expand its permissions, access another user’s resources, or use an unapproved capability just by requesting it.
  • Approval cases: verify that a consequential operation is blocked until the required approval is recorded for the exact action being executed.
  • Failure cases: test how the application behaves when a permission check, approval service, or policy decision cannot be completed.

Observe both the answer and the application’s actions. A response that says “I can’t do that” is not proof that no tool was called; conversely, an unsafe-sounding draft is not by itself proof that a protected operation executed. Verify tool invocation, arguments, authorization decisions, approval state, and resulting side effects using test instrumentation.

Repeat the relevant tests when prompts, tools, data sources, memory behavior, or authorization rules change. A control that worked for one version of a workflow should not be assumed to work after its permissions or inputs have changed.

Choose controls by authority, enforcement, and coverage

Compare designs on the boundaries they enforce rather than on whether they sound like a complete fix. There is no measured effectiveness comparison in the cited OWASP guidance that establishes a universal winner among filtering, quarantined parsing, approval gates, or least privilege. Their roles differ: some help handle content, while others constrain or review actions.

Design question What to verify
Authority granted Which tools, data, and side effects are available to this workflow?
Trust boundary Is external content identified and handled separately from trusted instructions where practical?
Enforcement location Does application or tool code make the authorization decision, or does the design only ask the model to obey a prompt?
Human checkpoint Which actions require review, and does the check happen before the operation executes?
Attack coverage Do tests cover direct inputs, indirect content, and tool outputs?
Operational burden What deployment, review, and maintenance effort does the design require relative to the impact it is intended to limit?

The last question is a local engineering trade-off; OWASP’s cited guidance does not provide comparable quantitative estimates of operational effort or risk reduction. Assess the answer against your own workflow, permissions, and consequences rather than relying on a claimed universal percentage.

Common defenses that fail when used alone

  • A stronger system prompt: useful for expressing the intended task, but not an independent authorization boundary when hostile data is processed in the same model context.
  • Delimiters around untrusted text: useful for signaling content boundaries, but not proof that the model will treat everything inside them only as data.
  • A regex, classifier, or second model: may help flag or filter some content, but should not be the sole control deciding whether a privileged action is allowed.
  • RAG or fine-tuning: neither removes the underlying risk that model behavior can be influenced by crafted input or external content.
  • Secrets in the prompt: a prompt is not a secret store, and an instruction not to reveal a credential does not replace keeping it out of model context.
  • A warning before a tool call: a warning has little security value if the tool can execute without an application-enforced authorization or approval check.

OWASP’s guidance is prescriptive rather than a comparative efficacy study. It does not establish a universal attack-success rate or prove that one of these layers reduces risk by a particular percentage. The actual exposure depends on the model, data, tools, permissions, and implementation.

A practical implementation order

  1. Map inputs, tools, data, and side effects. Identify direct and indirect content sources, sensitive information, and operations with consequential outcomes.
  2. Remove unnecessary authority. Restrict each workflow’s data and tools to what the task needs, and keep credentials and permission decisions outside model context.
  3. Put checks in code. Enforce user- and operation-specific authorization at the tool or application boundary; validate the requested target and arguments there.
  4. Separate and label untrusted content. Use clear provenance and boundaries, and consider a no-tool parsing stage for suitable workflows without treating its output as guaranteed safe.
  5. Add approval for consequential operations. Make reviewers inspect the actual proposed action and ensure execution depends on approval at the action boundary.
  6. Exercise the entire path with safe simulations. Test direct, indirect, and tool-output attacks using harmless data and instrumented tools; verify that unauthorized effects do not occur.
  7. Retest when the system changes. Revisit the relevant cases whenever prompts, tools, content sources, or permission rules change.

Use this order to prioritize architecture and enforcement before relying on model behavior. Filtering and prompt structure can support a defense-in-depth design, but the decisive safeguards for access and consequential actions must remain enforceable outside the LLM.
