Use CISA’s Known Exploited Vulnerabilities (KEV) Catalog as a high-priority threat signal, not as proof that every listed flaw exists in your network. The defensible process is to match each entry to an accurate asset inventory, confirm exposure and business impact, select a tested patch or mitigation, deploy it in a controlled way, and retain evidence that the vulnerability is closed. Federal civilian agencies have additional directive-driven obligations; private organizations should adopt the workflow without presenting federal deadlines as universal legal requirements.
What the KEV Catalog tells you
CISA’s KEV Catalog tracks vulnerabilities that are known to have been exploited in the wild. That exploitation evidence makes a KEV entry a stronger prioritization signal than a severity score alone. A critical vulnerability that has never been observed in attacks may deserve urgent work, but a lower-severity flaw with confirmed exploitation can create more immediate risk when it affects an exposed, important system.
A catalog entry still does not establish that your organization is vulnerable. You must determine whether the affected product and version are deployed, whether the vulnerable configuration is enabled, whether an existing mitigation already applies, and whether an attacker can reach the asset. Treat the catalog as the front of the queue for investigation and action, then apply local evidence to decide the order among applicable findings.
Why severity alone is inadequate
Vulnerability-management teams face more findings than they can fix in one maintenance window. CISA’s November 2021 overview reported 18,358 new cybersecurity vulnerabilities in 2020, including 10,342 classified as critical or high severity—an average of 28 critical or high findings per day. Those are historical figures, not current-year totals, but they illustrate why a severity-only queue is impractical.

KEV status supplies threat context. Inventory, reachability, asset importance, available fixes, change risk, and the strength of compensating controls determine the operational priority.
Does a CISA KEV deadline apply to your organization?
Binding requirements depend on organizational scope. Binding Operational Directive (BOD) 22-01 applies to Federal Civilian Executive Branch agencies. CISA encourages public and private organizations to review and adopt relevant practices, but that encouragement does not make the directive legally binding on every company, school, nonprofit, or state and local government.
CISA’s FY 2025 IG FISMA Metrics Evaluation Guide describes federal agencies remediating KEVs added in 2021 or earlier within six months and other KEVs within two weeks. Those periods are assessment expectations in that guide’s federal context. The guide is not a substitute for the current BOD 22-01 text, addenda, or agency-specific instructions. Confirm the current directive before publishing a compliance statement, contract requirement, or internal service-level target.
| Organization or use case | How to interpret the timeline |
|---|---|
| Federal Civilian Executive Branch agency | Follow the current BOD 22-01 requirements, addenda, and agency instructions. The FY 2025 assessment guide describes six months for KEVs added in 2021 or earlier and two weeks for other entries. |
| Private, nonprofit, state, local, tribal, or territorial organization | No universal BOD 22-01 legal deadline follows from the directive. Use the catalog and CISA guidance to set a risk-based internal deadline, contract obligation, or regulator-specific requirement. |
| Supplier supporting a federal customer | Check the contract, system-authorization boundary, and customer instructions. Do not assume that a customer’s federal obligation automatically applies to every supplier asset. |
Build an inventory that can answer a KEV question
Prioritization fails when the organization cannot reliably match a catalog entry to a real asset. Maintain records that are specific enough to distinguish products, editions, and versions, and connect each asset to an owner who can approve and execute a change.
- Product name, vendor, edition, and exact version or build.
- Hostname, cloud or on-premises location, environment, and network segment.
- Business, mission, safety, or data criticality.
- Internet exposure, remote-access paths, and important trust relationships.
- Technical owner, business owner, maintenance window, and recovery contact.
- Installed security controls, configuration state, and known compensating mitigations.
- Evidence source and collection date, such as endpoint management, cloud inventory, package data, authenticated scanning, or a configuration record.
CISA assessment material treats asset discovery as part of flaw management. Reconcile multiple inventories rather than trusting a single scanner: unmanaged devices, appliances, inherited cloud images, and software embedded in products are common sources of false negatives.

A repeatable KEV prioritization and remediation workflow
1. Monitor the catalog and related advisories
The KEV Catalog is a living resource. Assign responsibility for monitoring catalog changes and relevant vendor advisories, and record the date and entry details when a new item is discovered. CISA’s response materials describe monitoring CISA information and external threat or vulnerability feeds as part of vulnerability response.
Subscribe to the vendor’s security notices for affected products. A catalog record may identify the product and remediation date, while the vendor advisory supplies the exact fixed version, workaround, restart requirement, or configuration change.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errors2. Establish applicability
For each entry, compare the affected product and version with your inventory. Record a decision such as applicable, not present, fixed already, mitigated by configuration, or unknown pending evidence. Include the query, scan result, package list, configuration export, or owner confirmation supporting the decision.
Do not close an item merely because a broad asset search returned no match. Define the search scope, account for aliases and bundled components, and investigate systems that cannot report their software inventory. An “unknown” result is an inventory task, not a safe exemption.
3. Rank the applicable findings
Put applicable KEVs ahead of ordinary backlog items, then compare them using the dimensions below. CISA’s cited materials support prioritization and analysis but do not prescribe one universal numerical formula. A transparent local rubric is preferable to an unexplained score.

| Decision factor | Questions to answer | Priority increases when… |
|---|---|---|
| Deployment | Is the affected product and vulnerable version actually installed? | Evidence confirms the vulnerable build on multiple or important assets. |
| Reachability | Can an attacker reach the service directly or through a trusted path? | The asset is internet-facing, remotely accessible, or adjacent to high-value systems. |
| Asset criticality | What business, mission, safety, or regulatory harm follows from compromise? | The system supports essential operations, sensitive data, safety functions, or privileged access. |
| Exploitability in context | Are the vulnerable feature and required attack conditions enabled? | The deployed configuration matches known attack paths and no strong control blocks them. |
| Fix availability | Is a vendor patch or reliable mitigation available and tested? | A deployable fix exists and delay provides little operational benefit. |
| Change risk | Could remediation cause an outage or unsafe state? | The change is low risk or can be staged with a tested rollback. |
| Compensating controls | How long and how effectively do isolation, filtering, or access controls reduce exposure? | Controls are weak, unverified, temporary, or easy to bypass. |
| Verification quality | Can you prove the fix or mitigation was applied? | Reliable version, configuration, and rescanning evidence is available. |
4. Select a patch or mitigation
Use the vendor’s supported fixed version where practical. If an immediate patch is unavailable or unsafe during an emergency window, apply the vendor’s documented workaround or another defensible mitigation, such as disabling the vulnerable feature, restricting access, isolating the system, or adding a narrowly scoped detection and blocking rule.

Repair common Windows errors and clear accumulated junk for a smoother, more stable PC - no reinstall needed.Free scan · no reinstallDocument what the mitigation protects, what it does not protect, who approved it, and when it expires. A network rule that blocks one route is not equivalent to removing the vulnerable code; keep the KEV open until the durable fix is installed or an accountable risk owner accepts the residual risk.
5. Test before production deployment
CISA’s assessment guide includes patch testing and patch management. Test the exact product edition, version, plugins, integrations, and operating conditions used in production. Check authentication, data flows, scheduled jobs, backups, monitoring, and dependent applications.
For fragile or safety-critical systems, use a representative staging system or a canary group. Define success criteria, outage thresholds, rollback commands, backup validation, and an on-call escalation path before starting the change. Record test results rather than relying on an informal approval.
6. Deploy in a controlled sequence
- Confirm the change record, owner, maintenance window, backup or recovery point, and communications list.
- Apply the patch or mitigation to a small canary set.
- Check service health, logs, authentication, integrations, and security controls.
- Expand to additional systems in risk-ranked waves.
- Pause and roll back when defined failure criteria are met; investigate before continuing.
- Update the asset record and change record with the installed version or mitigation state.
7. Verify and close
Closure requires evidence. Confirm the installed version or configuration, rescan with an authenticated method where possible, and compare the result with the affected-version range. For an access-control mitigation, verify the rule from the relevant network paths; for isolation, verify segmentation and permitted exceptions.

Close the KEV record only when the evidence supports the decision. Retain the original applicability analysis, test result, deployment record, verification output, approver, and closure date. If a scanner still reports the flaw, determine whether it is a stale result, a missed host, a bundled component, or a genuine failed change.
Handling exceptions without losing control
Some systems cannot be patched immediately because of safety certification, vendor support limits, operational continuity, or an unavailable maintenance window. An exception is a managed risk decision, not a deletion from the queue.
- Name an accountable technical and business risk owner.
- State why the patch cannot be applied and identify the affected assets.
- List compensating controls, their verification method, and their limitations.
- Set a target date or trigger for the durable fix.
- Review the exception at a defined interval and after material threat or configuration changes.
- Keep the item visible in reporting until remediation evidence supports closure.
Measure whether the process works
Track time from catalog identification to applicability decision, mitigation, patch deployment, and verified closure. Also track overdue applicable KEVs, unknown inventory matches, failed or rolled-back changes, recurring asset-discovery gaps, and exceptions past their review date.
Review trends by business unit and asset class. A rising number of “not present” decisions may indicate good hygiene—or an inventory that cannot see the environment. Compare records with independent discovery and scanning results before drawing conclusions.

Pick the symptom - the matching free tool is one click away.
Use the results to improve the full lifecycle CISA describes: identification, analysis, remediation, and reporting. For example, repeated missed appliances may justify a new discovery source; repeated failed patches may require better staging or rollback automation; recurring exceptions may indicate an unsupported platform that needs replacement planning.

Practical operating model for small and large teams
Small IT or security team
Assign one owner for catalog monitoring, maintain a simple but authoritative asset register, and use a standard KEV record with applicability, owner, due date, change evidence, and verification fields. Hold a short weekly review of new entries and open exceptions. Automate collection where possible, but require human confirmation for critical systems.
Enterprise or managed environment
Integrate catalog monitoring with asset management, endpoint or cloud inventory, vulnerability scanning, change management, and incident-response channels. Route applicable findings to the system owner with a risk-ranked due date. Separate discovery and verification data so that a single stale scanner result cannot silently prove its own success.
Safety-critical or highly regulated environment
Include safety, reliability, legal, and business owners in the decision. Pre-approve emergency change paths, maintain tested recovery procedures, and use staged deployment. Document why a mitigation is sufficient for the interim and how the organization will demonstrate that the residual risk was actively managed.

- “It is in KEV, so every host is vulnerable.” Match the affected product and version to evidence from your own environment.
- “The CVSS score determines the queue.” Add exploitation status, reachability, asset criticality, configuration, and fix availability.
- “A scanner says fixed, so we are done.” Confirm the installed build, service health, and any remaining vulnerable components.
- “The workaround is permanent.” Give mitigations an owner, expiry or review date, and a path to the supported fix.
- “A federal deadline applies to our private company.” Check legal scope, contracts, regulators, and customer requirements before making that claim.
- “We cannot patch during this window, so we will remove the ticket.” Keep an approved, time-bounded exception with compensating controls and a next action.
- “The inventory is complete because the dashboard is green.” Test coverage with independent discovery and investigate unmanaged, embedded, and transient systems.
A KEV record that stands up to review
For every catalog entry, retain at least:
- Catalog identifier, affected product and version range, and date discovered.
- Inventory query and evidence showing applicable, not present, mitigated, or unknown status.
- Asset owner, business criticality, exposure, and selected priority rationale.
- Vendor patch or mitigation reference, test results, and change approval.
- Deployment scope, timestamps, rollback outcome if used, and operational checks.
- Verification evidence, closure approver, or exception owner and review date.
This record makes the decision reproducible and lets auditors, incident responders, and new team members understand what was known at the time and why the organization acted as it did.
Bottom line for prioritizing CISA KEVs
Start with every new KEV, but do not stop at the catalog. Prove whether the affected product and version exist, rank the exposure and consequences in your environment, deploy a tested patch or mitigation, and verify the result with evidence. Apply BOD 22-01 timelines only within their federal scope, while other organizations use the same disciplined lifecycle to set risk-based and contract-specific deadlines. CISA describes the purpose of BOD 22-01 as helping federal agencies and public and private organizations improve vulnerability management and reduce exposure to cyberattacks; the practical way to achieve that goal is a repeatable, auditable process rather than a severity label or an unverified dashboard.
