The reliable way to stop confidential data appearing in AI output is to use several controls together: send less sensitive data, enforce user- and task-level access, treat retrieved content as untrusted, inspect prompts and context, scan responses before release, and monitor every read, write, and action. A system prompt that says “do not reveal secrets” is useful guidance, but it is not a security boundary.
Information can leak from more than model training. Prompts, retrieved documents, vector stores, conversation history, caches, agent memory, tool results, and logs can all place sensitive material in the path that leads to an answer. The controls below cover that entire path and show what to do for a personal task, an enterprise assistant, or a custom AI application.
Where sensitive information enters an AI response
An AI model can only use information made available to it directly or indirectly. That information may be confidential even when the user never typed it into the final prompt.
| Potential source | How exposure happens | Primary control |
|---|---|---|
| Prompt and uploaded files | A user pastes credentials, personal records, source code, or an internal document into a service. | Classify data before submission; prohibit or de-identify high-risk data. |
| Retrieval and vector stores | Search returns a document the requesting user is not allowed to see, or returns more text than the task needs. | Enforce authorization at retrieval time and minimize returned fields and passages. |
| Conversation history and caches | A later turn, user, or session receives earlier confidential context. | Separate users and sessions; define retention, deletion, and cache boundaries. |
| Persistent memory and agent state | An assistant stores a fact and later recalls it for the wrong person, task, or tenant. | Use explicit write and read policies, scoped identities, lineage, and correction workflows. |
| Tools and external services | A model receives private records from a connector or sends data to an over-privileged tool. | Give tools short-lived, least-privilege identities and validate every consequential call. |
| Logs and telemetry | Prompts, retrieved text, outputs, or scan findings are copied into broadly accessible logs. | Minimize, redact, encrypt, restrict access, and set retention limits. |
| Model memorization | A model may reproduce information learned during training or fine-tuning. | Use approved data, privacy controls, testing, and output monitoring; do not assume training is the only risk. |
Microsoft’s guidance on sensitive-information disclosure identifies persistent memory, retrieval, context, tools, and logging as exposure paths. OWASP’s 2025 guidance treats sensitive-information disclosure as a distinct large-language-model risk. A clean test in which a user types a safe prompt therefore does not prove that the surrounding application is safe.

Start with a data policy and reduce exposure
Classify before data reaches the model
Define three practical categories for every workflow:
- Prohibited: information that must never be sent to an AI service, such as unredacted credentials, private keys, or data covered by a legal or contractual restriction.
- Approved only in a governed environment: internal records, customer information, regulated data, or source code that may be processed only with an approved service, contract, identity controls, and retention settings.
- Allowed with safeguards: public or low-risk material, or de-identified data whose remaining re-identification risk is acceptable.
Write the policy in terms employees can apply at the moment of use. Tell them that a prompt and its attachments are disclosures to the service receiving them. Provide a sanctioned tool for approved sensitive work; a prohibition without a workable alternative tends to push use into ungoverned channels.
Minimize the information in context
Give the model only the fields, records, and time range needed for the task. Replace names, account numbers, exact addresses, and identifiers with stable tokens when the task does not require identity. Retrieve a small relevant passage instead of an entire file, and remove secrets before text is placed in a prompt, memory entry, or tool request.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
For application builders, keep credentials and other high-risk values outside model-visible context whenever possible. A tool should obtain a secret from a protected store using its own scoped identity, not receive that secret in a prompt. Short-lived credentials reduce the damage if an agent call or log is exposed.
Set persistence rules before deployment
Document what the system may store, how long it remains, who can read or delete it, and how stale or poisoned entries are corrected. Apply the same decision to conversation history, retrieval indexes, caches, agent state, and debugging traces. If a feature cannot explain why a piece of data is retained, disable that persistence or redesign it.
Enforce identity, tenant, and task boundaries
Least privilege must apply to both data and actions. An assistant that can retrieve or remember information must be limited to the requesting user, the current task, and the current session.

Authorize retrieval as the user
Do not rely on a broad application service identity to search every document and then ask the model to behave. The retrieval layer should evaluate the requester’s roles, tenant, document permissions, and purpose before returning content. Apply authorization again when a result is fetched from a cache or vector store; an index created under one user’s authority must not become a shared answer bank.
Quick wins for a faster PC:
Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Repair Windows errors before they cause bigger problemsFix Now →Isolate sessions and tenants
- Use a distinct conversation and memory namespace for each user and tenant.
- Bind every retrieval, memory read, and tool call to a session and task identifier.
- Prevent cache keys from omitting tenant or authorization context.
- Make deletion remove derived copies where policy requires it, including indexes and memory records.
- Record lineage so an administrator can identify which source created a memory entry and who changed it.
Scope tools as carefully as documents
A read-only assistant should not receive a write-capable identity. Give each connector the smallest set of operations and records it needs, with short expiration and explicit audience restrictions. Separate preparation from execution: the model may draft an email or database change, while a policy engine or human approves the actual operation.
| Boundary | Question to enforce |
|---|---|
| User | Is this person authorized for the source record and the requested action? |
| Tenant | Can data from one customer, business unit, or workspace cross into another? |
| Task | Is the information necessary for this request, or merely available to the application? |
| Session | Could a later conversation inherit an earlier user’s context? |
| Tool | Does the connector have more read or write authority than the action requires? |
Treat outside content as untrusted input
Web pages, email, PDFs, tickets, and uploaded files can contain instructions aimed at the model. This is indirect prompt injection: the user’s request may be harmless while the retrieved content tells the assistant to disclose data, ignore policy, or call a tool. Direct prompt injection occurs when a user deliberately tries to override the system’s rules.
Separate instructions from data
Tell the model that retrieved material is evidence to analyze, not an authority that can change its rules. Keep system and developer instructions outside retrieved text, label untrusted content, and do not let a document redefine permissions. These measures reduce confusion but do not make the content trustworthy.
Reduce the attack surface
- Retrieve only sources relevant to the task and user.
- Strip hidden markup, active content, and unnecessary metadata before modeling.
- Limit the tools available during retrieval and analysis.
- Require independent validation before a response sends data, changes access, executes code, or makes a high-impact decision.
- Use a human approval step for actions whose impact cannot be reversed easily.
Microsoft’s prompt-injection guidance recommends layered controls because filters can miss attacks. OWASP likewise notes that restrictions placed only in a system prompt may be bypassed.
Inspect the complete data path
Scanning only the final answer misses leaks that occur earlier or in a tool response. Build checks at each transition.

- Before submission: detect prohibited secrets, personal information, and regulated fields in the user prompt and attachments. Block, redact, or request an approved workflow.
- After retrieval: re-check authorization and scan returned passages. Remove fields that are not needed for the task.
- Before memory writes: decide whether the fact is allowed to persist. Attach owner, purpose, source, and expiry metadata.
- Before memory reads: verify the current user, tenant, session, and purpose. Do not treat a prior memory as proof that access is still valid.
- Before tool calls: validate arguments, destination, scope, and required approval. Use a constrained operation rather than a free-form command.
- After tool calls: inspect returned data before it is placed in the model context or shown to a user.
- Before release: scan the generated answer and any attachments for secrets, personal data, confidential classifications, and policy-defined markers.
- At the destination: encode or sanitize the result for its exact sink, such as HTML, a database, a ticket, or an API. Never execute free-form model text as a command or control instruction.
Each check needs a defined response: allow, redact, block, or route to approval. Log the decision without copying more sensitive content than the audit requires.
Validate and constrain the output
Match validation to the destination
Text safe for a human reader may be unsafe in another context. Escape output for HTML, parameterize database operations, and use an allowlist for fields passed to another API. If the destination expects structured data, require a narrow schema with approved field names, types, ranges, and enumerated values. Reject unexpected fields rather than silently passing them downstream.
Keep model text away from direct execution
Do not let a model-generated string become shell code, a database query, an access-control rule, or an email recipient without deterministic validation. Prefer typed function arguments and fixed operations. A model can propose an action; application code should decide whether it is valid and permitted.
Use approval for consequential actions
Require explicit confirmation before sending information outside the organization, changing permissions, executing code, deleting records, or making a high-impact decision. Preserve enough provenance to show the input, retrieved sources, policy decisions, approvals, tool calls, and final action. The provenance record itself must follow the same access and retention rules as other sensitive data.
Microsoft’s output-safety guidance, published August 1, 2026, emphasizes validation, data-loss-prevention checks, safe rendering, schema constraints, and confirmation before consequential actions.
Log and monitor without creating a second leak
Auditing is essential, but unrestricted logs can become a sensitive store. Record policy decisions, scan results, retrieval and memory events, approvals, destinations, and final actions while minimizing raw prompt and response content.

Protect audit data
- Restrict log access by role and purpose.
- Redact secrets and unnecessary personal data before storage.
- Encrypt logs in transit and at rest.
- Set a retention period and an enforceable deletion process.
- Separate operational metrics from transcripts when full text is not required.
- Track who viewed, exported, or changed an audit record.
Alert on behavior, not just keywords
Useful signals include repeated extraction attempts, cross-user retrieval, unusual memory reads, sudden increases in blocked fields, access to high-risk tools, and sensitive markers in outputs sent to external destinations. Review false positives and false negatives; a scanner that blocks everything will be bypassed or disabled, while one that never challenges users provides little protection.

Choose controls for the type of AI workflow
| Workflow | Minimum practical controls | Additional requirements |
|---|---|---|
| Small, non-sensitive personal task | Do not enter information that should not be shared; remove identifiers and use a service with understandable retention settings. | Review attachments and copied chat history before submission. |
| Enterprise assistant using internal data | Approved service, data classification, identity-aware retrieval, tenant and session isolation, retention controls, output scanning, and audit access restrictions. | Incident response, DLP policy enforcement, memory governance, and testing against cross-user and indirect-injection cases. |
| Custom AI application | Prompt and context handling, scoped tool identities, schema validation, safe rendering, and application-level authorization. | End-to-end tests for retrieval, memory, caches, tool outputs, downstream actions, and log exposure. |
Microsoft names Purview Data Loss Prevention and Information Protection as examples of classification and policy-enforcement capabilities, and describes Azure AI or approved enterprise deployments as options for governed sensitive workflows. These are vendor-specific examples, not universal requirements; confirm current features, geography, retention behavior, plan limits, and contractual terms before relying on them.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Evaluate a guardrail or DLP implementation
Compare implementations by capability rather than by a single accuracy claim. Ask these questions during design and procurement:
- Which data sources and AI services are covered: prompts, files, retrieval, memory, tools, outputs, and logs?
- Can policy enforcement block, redact, warn, require approval, and merely audit as separate outcomes?
- Do checks run both before prompt submission and before output release?
- Are user, tenant, role, session, and retrieval permissions enforced at the data source?
- What are the retention, deletion, export, and access controls for prompts, memory, and logs?
- Can downstream tools receive only approved, typed fields rather than arbitrary model text?
- Can administrators inspect policy decisions, false positives, false negatives, and the source of a finding?
The available guidance establishes these as important control axes, but it does not establish a cross-vendor performance ranking. Test the exact deployment and data types you intend to use.
Test before trusting the system
Build a threat-oriented test set
Include direct requests for secrets, attempts to retrieve another user’s records, documents containing hidden instructions, poisoned memory entries, stale permissions, malicious tool arguments, and outputs containing regulated or confidential markers. Test both allowed and blocked cases so the system does not simply fail closed for every useful request.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsFollow the data through every component
For each test, record what entered the prompt, what retrieval returned, what memory stored, which tools were called, what came back, what the model generated, and what reached the final destination. A prompt-only test cannot reveal a cache-key bug, an over-broad service identity, or a downstream rendering vulnerability.

Retest after changes
Repeat the suite when you change a model, retrieval index, connector, memory policy, prompt template, scanner, or retention setting. Keep test records free of real secrets; use synthetic values that exercise the same detection and authorization rules.
Respond to a suspected disclosure
- Stop the affected workflow or revoke the exposed tool identity.
- Preserve minimal, access-controlled evidence needed to determine what data was exposed and where it traveled.
- Invalidate credentials, tokens, links, cached entries, and memory records that may contain the material.
- Identify every recipient, log, index, transcript, and downstream system that received the output.
- Apply notification, regulatory, contractual, and internal incident procedures as required.
- Fix the control failure, add a regression test, and review whether retention or permissions allowed unnecessary spread.
Deleting one chat is not a complete response if the same information was copied into retrieval indexes, memory, logs, exports, or a third-party tool.
Common approaches that fail
- Relying on a system-prompt prohibition: prompt instructions can be ignored or bypassed through direct or indirect injection.
- Filtering only the user’s text: sensitive data may enter through retrieval, memory, a connector, or a tool response.
- Using one broad application identity: the assistant can retrieve records the user is not entitled to see.
- Assuming a vector database is automatically private: indexes and caches still need tenant, role, and retention controls.
- Returning free-form text to an executable sink: a plausible answer can become an unsafe command, query, or instruction.
- Keeping full transcripts forever: debugging convenience increases the number of places a disclosure can persist.
- Adding a scanner without an approval path: users may bypass a control that blocks legitimate work without explaining the decision.
Use layered prevention, detection, and response. The goal is not to promise that a model can never produce a secret; it is to make unauthorized data unavailable, make suspicious output detectable, and prevent a generated answer from causing uncontrolled downstream harm.
What the guidance says about prompt restrictions
OWASP’s LLM02 2025 document states: “Adding restrictions within the system prompt about the data types the LLM should return can provide mitigation against sensitive information disclosure. However, such restrictions may not always be honored and could be bypassed via prompt injection or other methods.” Treat that as a useful layer, not as authorization, filtering, or proof of safety.
OpenAI’s safety best-practices guidance also emphasizes constrained inputs and human verification for higher-risk uses. Those practices complement, rather than replace, identity controls, data minimization, output validation, and protected logging.
