Prevent prompt injection from becoming a security incident by assuming that every prompt, retrieved passage, document, webpage, email, tool result, conversation turn, and memory entry may be adversarial. Keep that content separate from authoritative instructions, and make application code—not the model—enforce identity, data access, tool permissions, argument validation, and approval requirements. Detection and model training can reduce risk, but only independent controls stop a malicious instruction from turning into an unauthorized action.
What the AI control boundary is
An AI system’s control boundary is the set of enforceable rules between information the model can read or propose and actions the application permits. Crossing the boundary means untrusted content changes what the system retrieves, discloses, modifies, sends, purchases, deletes, or executes without the required authority.
Prompt injection has two main entry paths. A direct injection is placed in the user’s message. An indirect injection is carried by a resource the system processes. NIST defines it as “A type of prompt injection executed through resource control rather than through user-provided input as in a direct prompt injection.” A webpage, uploaded document, email, retrieved record, calendar event, tool response, or memory value can therefore carry an attack even when the user’s request is harmless. See the NIST glossary definition.

The model may interpret hostile text as an instruction, but interpretation alone is not the security failure. The failure occurs when the surrounding application accepts the model’s proposal without independently checking who is asking, which data may be used, which operation is allowed, and whether a person must approve it. Message roles, delimiters, and phrases such as “treat this as untrusted” provide context; they are not authorization mechanisms. OWASP’s prevention guidance explicitly warns that labeling content does not enforce a trust boundary.

Map every route by which instructions can enter
Start with a data-flow diagram that follows content from ingestion to the final side effect. Mark each item as untrusted unless a separate, auditable process gives it authority. Include:
- User messages, pasted text, form fields, and uploaded files.
- Web pages, search results, email bodies, calendar entries, issue trackers, and chat transcripts.
- Retrieved passages, vector-store metadata, database rows, and documents supplied by connectors.
- Tool output, API responses, error messages, logs, and generated summaries.
- Conversation history, profile fields, long-term memory, and any state the model can write for a later run.
- Model-generated plans, tool arguments, SQL, HTML, shell commands, and other output sent to downstream systems.
Attach source, owner, timestamp, access classification, and transformation history to each item. Provenance lets policy code distinguish a user’s authorization from a sentence found in a document and makes poisoned memory or a manipulated source traceable.
Keep data and instructions in separate channels
Preserve roles and provenance
Keep system policy, developer configuration, user intent, retrieved data, and tool output in distinct fields. Do not concatenate raw external text into a system or developer message. Pass source identifiers and trust classifications as structured metadata that the application can inspect. If a connector returns HTML, retain the URL and document identity outside the text that the model reads.
Free tools Windows power users keep installed

Normalize and segment external content
Parse files with a format-aware reader, remove hidden markup when it has no business purpose, decode suspicious encodings, and place each source in an explicit data segment. Extract only schema-valid fields needed for the task instead of forwarding an entire document. Normalization reduces opportunities for an attacker to hide instructions in styling, comments, invisible characters, or alternate encodings, but it does not prove that the remaining text is safe.
Use labels as context, not as a gate
A label such as untrusted_document can help the model distinguish evidence from policy. It cannot grant or revoke permissions. The execution layer must still reject an unauthorized read, write, disclosure, or external call even if the model presents a convincing explanation.
Enforce authorization outside the model
Every tool call should pass through deterministic policy code. Check the caller’s identity, tenant, role, resource ownership, purpose, and current authorization at the moment of execution. Do not rely on a previous model decision or on a permission implied by the user’s wording. A model refusal is useful behavior, but it is not an access-control decision.
Use least privilege and scoped identities
Give an agent only the operations and records required for its current task. Separate read and write capabilities, restrict queries by tenant and document permissions, and prefer task-specific identities over a shared administrator credential. Short-lived grants reduce the time available for an injected instruction to abuse a valid capability. General least-privilege guidance is described in Microsoft’s identity and access guidance.

Validate every argument
Define typed schemas for tool inputs. Enforce allowed operations, resource identifiers, destination addresses, maximum result sizes, time ranges, and rate limits in code. Reject extra fields and ambiguous values. For database access, generate parameterized queries from validated fields rather than allowing the model to supply arbitrary SQL. For file operations, resolve paths against an approved directory and prevent traversal. For network calls, use an allowlist of hosts and protocols.
Recheck at each hop
If one tool returns data that another tool will consume, apply policy again at the second boundary. A search result does not authorize a download; a downloaded file does not authorize sending an email; and a draft message does not authorize transmission. Carry the original principal and purpose through the chain so a later component cannot silently widen access.
Require approval for consequential actions
Pause actions that can materially affect a person, organization, account, or external system. Examples include sending messages, deleting records, purchasing, changing permissions, publishing content, transferring funds, or submitting a legal or compliance filing. The approval screen should show the concrete operation and arguments: recipient, attachment, amount, records affected, destination, and any irreversible consequence. “Approve this action?” without details is not meaningful review.
Approval is a backstop, not a replacement for narrow permissions. A reviewer can miss a subtle manipulation, so the tool must still enforce identity, scope, validation, and rate limits. OpenAI’s prompt-injection discussion and OWASP’s guidance both support action-specific human oversight for high-impact operations.

Constrain retrieval and memory
Apply permissions during retrieval
Use permission-aware indexes and document-level filters so unauthorized material never reaches the context window. Enforce the same check when a document is opened, quoted, summarized, or exported. Relevance is not permission: a highly relevant record can still belong to another user, team, or tenant.
Keep provenance with every passage
Return document ID, owner, access class, source location, and retrieval time alongside text. Display or log that provenance when the model cites evidence or proposes an action. This makes it possible to identify which source introduced an instruction and to revoke or quarantine it without deleting unrelated data.
Protect memory writes and reads
Treat memory as a writeable security-sensitive store. Validate fields before writing, reject attempts to alter policy or identity attributes, require explicit user confirmation for durable preferences, and version every change. At read time, apply the same authorization and tenant filters used for ordinary data. Keep rollback capability so a poisoned entry can be removed from future contexts.

Use multiple controls because each catches a different failure mode. Microsoft states, “No single solution is sufficient—combine probabilistic and deterministic defenses.” A detector can flag suspicious wording, a data-marking scheme can highlight untrusted text, a planner monitor can identify drift, and a sandbox can limit what succeeds. None should be treated as the sole boundary.
| Control location | Typical control | Primary value | Important limitation |
|---|---|---|---|
| Ingestion | Format parsing, hidden-markup removal, normalization, malware and content checks | Reduces concealed or malformed instructions before context assembly | Clean-looking text can still contain a persuasive attack |
| Retrieval | Permission-aware indexes, metadata filters, provenance, document-level checks | Prevents unauthorized or untraceable content from entering context | Relevant content may still be malicious |
| Model context | Role separation, data labels, spotlighting or other data marking, prompt shields | Improves source distinction and detects suspicious patterns | Probabilistic detection can miss novel or transformed attacks |
| Planning and tools | Plan-drift monitoring, typed schemas, allowlists, sandboxing, runtime policy | Limits what a compromised plan can request or execute | Additional checks add latency and operational complexity |
| Approval | Action-specific human confirmation | Provides a review point before high-impact side effects | People can approve a manipulated request; permissions must remain narrow |
| Downstream sink | Parameterized queries, safe rendering, typed interfaces, destination validation | Stops generated content from becoming injection in SQL, HTML, shell, or another tool | Output filters alone do not establish authorization |
Choose layers according to data sensitivity, action impact, latency budget, and operational capacity. Layering can increase false positives and reduce legitimate task completion, so measure those effects rather than disabling controls blindly.
Validate output at every downstream sink
Model output is untrusted data even when it came from an apparently trusted instruction. Before inserting it into SQL, HTML, a shell, a template, or another API, use destination-specific safeguards.
- Use parameterized database queries and allowlisted column or sort names.
- Escape or sanitize HTML and render untrusted text as text unless markup is explicitly allowed.
- Never execute model-generated shell commands directly; map approved operations to fixed functions and run them in a restricted environment.
- Use typed API clients, strict JSON schemas, and reject unknown fields.
- Validate URLs, recipients, file paths, and attachment types against policy before transmission.
- Apply size, rate, and time limits so a successful injection cannot create an unbounded workload.
A reference execution flow
The following sequence keeps the model useful while reserving security decisions for deterministic components.

- Assemble context. Preserve message roles, place external material in data fields, normalize formats, and include only the minimum necessary content.
- Generate a typed plan. Ask the model for an intended operation and schema-valid arguments, not executable code or unrestricted credentials.
- Run policy checks. Verify identity, resource scope, operation allowlists, argument constraints, rate limits, and current authorization outside the model.
- Request approval when required. Show the exact side effect and arguments to an authorized reviewer.
- Execute with a scoped identity. Use the least-privileged tool credential in a sandbox or constrained runtime.
- Validate returned data and outputs. Apply sink-specific checks before writing, rendering, sending, or passing results to another tool.
- Log the chain. Record source IDs, policy decisions, tool arguments, approvals, side effects, and errors without storing unnecessary secrets.
Testing whether the boundary actually holds
Test each attack route separately
Place direct attacks in the user-input channel. Place indirect attacks in the webpage, document, email, retrieved record, or tool-result channel that the production system processes. An indirect payload pasted as a user message tests a different boundary and can produce a misleading result.
Use harmless but observable fixtures
Use dummy secrets, synthetic records, and instrumented tool substitutes. Configure the substitute to record attempted calls and state changes without affecting real users. Observe whether an unauthorized tool call occurred, whether dummy state changed, and whether test data reached an instrumented destination. A polite refusal in the final response does not prove that no side effect happened earlier.

Include benign tasks
Run legitimate retrieval, summarization, editing, and tool-use tasks alongside attacks. Report completion quality, latency, false refusals, policy decisions, and side effects. A defense that blocks every request is not a usable security control.
Interpret small test sets honestly
OWASP presents 14 hand-picked attacks and seven benign prompts as illustrative smoke tests, not a representative sample of application traffic or attacks. Do not call that set a benchmark or infer a general attack-block rate from it. Absence of a test marker only shows that the exact marker was not observed; transformed disclosure or other prompt content may still have escaped. Expand tests with application-specific documents, languages, encodings, tool chains, and memory behavior, and rerun them after every policy or model change.

“The system prompt says to ignore instructions in documents.”
Why it fails: The model may still follow a persuasive or obfuscated instruction, and the prompt does not control tool permissions.
Fix: Keep documents in a data channel, validate every tool call in code, and restrict the credential used for execution.
“We added a trusted/untrusted label.”
Why it fails: Labels provide context but do not enforce authorization.
Fix: Make policy decisions from identity, provenance, resource scope, and typed arguments outside the model.
“The retrieval system returned the passage, so the agent may use it.”
Why it fails: Relevance is not permission, and a retrieved passage can be malicious.

- Maintain an inventory of every model, connector, memory store, tool, credential, and downstream sink.
- Assign an owner and data classification to each connector and tool.
- Review permission scopes and allowlists when a workflow changes, not only when a model changes.
- Log policy denials, approval decisions, source provenance, tool arguments, and side effects with correlation IDs.
- Alert on unusual plan drift, repeated denied calls, unexpected destinations, excessive retrieval, and memory changes.
- Rotate or revoke credentials quickly when a connector, memory store, or tool is suspected of poisoning.
- Keep a rollback path for memory, configuration, permissions, and generated artifacts.
- Retest after model, prompt, retrieval, parser, connector, or policy updates.
Can prompt injection be prevented completely?
No. Prompt injection is an ongoing failure mode of systems that combine probabilistic language interpretation with external content and real-world tools. There is no generalizable attack-prevalence or defense-efficacy percentage established here, and no detector can guarantee that every novel, encoded, multilingual, or context-dependent attack will be recognized.
The practical objective is to prevent an injection from crossing the control boundary even when the model is persuaded. Keep untrusted content and provenance intact, enforce authorization and argument checks in application code, constrain retrieval and memory, require approval for consequential actions, validate outputs at sinks, and test observable side effects with layered controls. A missed detector should then produce a contained, auditable event rather than an unrestricted breach.
