Preventing enterprise AI failures requires two control planes working together: protect information at every point where it is stored or moved, and test model answers and actions against evidence before anyone relies on them. Retrieval, citations, filters, and guardrails reduce risk; they do not make a language model infallible or guarantee zero disclosure.
The practical approach is to map the full data path, enforce identity and authorization at every boundary, treat retrieved content and model output as untrusted, evaluate the deployed system with realistic and adversarial tests, and keep an auditable response loop.
What you are protecting: two different failure classes
A data leak is an authorization or handling failure: information reaches a person, model, connector, tool, log, or downstream system that should not receive it. A hallucination—called confabulation in the National Institute of Standards and Technology (NIST) Generative AI Profile—is an unsupported or incorrect output presented as if it were useful information. One system can suffer from both at once. For example, a retrieval assistant may quote a confidential document to an authorized employee but invent a policy exception, or it may produce a factually correct answer from a document the employee was not allowed to see.

Every data-path surface can expose information
Authorization must cover more than the chat window. Inspect each of these surfaces:
- User prompts and uploaded files, including hidden metadata and pasted credentials.
- Retrieved documents, search indexes, vector stores, embeddings, and reranking services.
- Conversation memory, summaries, agent state, caches, and temporary files.
- Tool results from email, ticketing, databases, code repositories, web fetchers, and business applications.
- Model responses, intermediate traces where retained, exports, analytics, debugging records, and security logs.
- Third-party models, connectors, agent runtimes, and hosting locations outside the organization.
A boundary that is secure at ingestion can still fail when a broad service account reads a vector index, when a cache is shared between sessions, or when a response is copied into an unrestricted log. Map the complete path before selecting a control.
1. Map the system and its data before deployment
Start with an inventory that can be maintained, not a one-time list of chatbots. Microsoft governance guidance describes an AI asset inventory and an AI bill of materials (AI-BOM) as records of system components and dependencies. Extend that record to every model, prompt, dataset, connector, retrieval index, memory store, tool, owner, version, and deployment location.
What to record
| Inventory area | Questions to answer | Security consequence |
|---|---|---|
| Models and versions | Which provider, model build, endpoint, fallback model, and region are used? | Changes can alter retention, behavior, permissions, or evaluation results. |
| Data sources | Which databases, files, messages, tickets, code, and web sources are reachable? | Classification and ownership determine who may retrieve each item. |
| Prompts and policies | Which system instructions, templates, filters, and refusal rules are active? | Secrets or authorization logic hidden in prompts are difficult to protect and audit. |
| Retrieval and memory | Where are chunks, embeddings, summaries, conversation state, and caches stored? | Cross-user reads and stale permissions are common disclosure paths. |
| Tools and agents | What functions can be called, with which identity and write permissions? | Excessive agency can turn a misleading answer into an irreversible action. |
| Operations | Who owns the service, approves releases, reviews alerts, and handles incidents? | Unowned components remain in production after their risks change. |
Classify the information each component can access, including personal data, credentials, regulated records, confidential strategy, source code, and customer content. Record whether data is copied, transformed, retained, or sent to another provider. Include development, test, support, and disaster-recovery environments; a nonproduction index with production data is still a production exposure.
Define trust boundaries on a diagram
Draw the request from identity provider to application, model, retrieval layer, memory, tools, and output destinations. Mark where authentication, authorization, sanitization, encryption, logging, and human approval occur. Show both synchronous calls and background jobs. The diagram should answer which identity is used for each read and write, not merely which service makes the call.
2. Establish data and identity boundaries
Use an approved enterprise deployment for sensitive workflows only after reviewing the exact service, tier, region, and configuration. Check its data-use, retention, tenant-isolation, logging, training, support-access, and deletion terms. A general statement about a vendor is not proof that every product or setting has the same policy.

Keep secrets out of prompts
Do not place API keys, passwords, signing material, private authorization rules, or long-lived tokens in system prompts or retrieved text. Store secrets in a dedicated secrets manager and issue short-lived credentials to the component that needs them. Prompts are application data: they may be exposed through debugging, prompt-leakage attacks, exports, or provider-side diagnostics.
Apply least privilege to every identity
- Give the model only inference access; give retrieval services only the indexes they need.
- Use separate identities for ingestion, search, memory, and write actions where practical.
- Scope connectors to specific repositories, fields, rows, folders, or operations rather than an entire tenant.
- Run retrieval in the requesting user’s authorization context when document permissions must be honored.
- Isolate user, session, task, agent, and tenant memory unless an explicit policy allows sharing.
- Require a new authorization decision for each sensitive retrieval and tool call; do not rely only on a decision made when a session started.
Audit memory writes and reads, retrieval decisions, tool outputs, and repeated extraction attempts. A user who is allowed to read one document is not automatically allowed to ask an agent to aggregate every document in the repository.
3. Secure ingestion, retrieval, and memory
Prompts, uploaded files, web pages, email, retrieved documents, memory, and tool results are all potentially untrusted inputs. Content can contain direct instructions aimed at the model or indirect instructions hidden in a document, image, spreadsheet, or web page.
Make search permission-aware
Use document-level access control or security trimming so the index returns only items the requesting identity may see. Enforce authorization at query time, not just when documents are initially indexed. Recheck permissions after sharing changes, role changes, and revocation. Test deleted, moved, and newly restricted documents; stale embeddings can preserve content after the source permission has changed.
Validate provenance and content
- Record the source identifier, owner, classification, timestamp, and permission decision for each retrieved chunk.
- Reject or quarantine malformed files, unexpected encodings, hidden instructions, and content that attempts to alter system policy.
- Separate instructions from evidence in the prompt format and tell the model which fields are data rather than commands.
- Limit the number, size, and source diversity of retrieved items to reduce irrelevant or malicious context.
- Prevent a document from causing an automatic tool call merely because it contains imperative text.
Protect vector stores and memory
Embeddings can reveal relationships among sensitive records even when the original text is not displayed. Encrypt stores, isolate tenants, restrict administrative queries, and apply retention and deletion rules to vectors, metadata, summaries, and caches. When a user asks to forget information, identify every derived copy that must be deleted or re-indexed. Test whether similarity search can return another user’s content through shared namespaces, predictable identifiers, or broad service credentials.
Test prompt injection and extraction
Include direct attacks such as requests to reveal hidden instructions, and indirect attacks embedded in retrieved content. Add encoding tricks, multilingual requests, malicious markup, poisoned documents, oversized context, and attempts to make the agent call an unauthorized tool. A passing test means the deployed authorization and handling path behaved safely; it does not prove that all future inputs will be safe.

4. Ground answers without treating retrieval as proof
Retrieval-augmented generation (RAG) can steer an answer toward approved material and make its sources inspectable. It does not prove that the retrieved text is current, complete, relevant, or correctly interpreted. A citation can point to a real document while failing to support the claim, and a user may be unauthorized to see the cited passage.
Require evidence checks
- Retrieve only documents permitted for the requesting identity.
- Pass source identifiers and relevant excerpts in a clearly separated evidence section.
- Ask the model to distinguish supported facts, inferences, and missing information.
- Check that each citation actually entails the associated statement and that the citation is visible to the user.
- Refuse, ask for clarification, or escalate when evidence is absent, conflicting, stale, or outside the task’s authority.
Do not invent a universal confidence threshold. The appropriate action depends on the consequence of an error, the quality of evidence, and the organization’s approval policy.
Build a deployment-like evaluation set
Use representative enterprise tasks rather than generic question-answering benchmarks. Include normal requests, ambiguous requests, rare edge cases, conflicting policies, revoked permissions, empty search results, stale records, and adversarial prompts. Measure answer correctness, citation support, refusal behavior, unauthorized retrieval, sensitive-output rates, tool-call correctness, latency, and cost where those measures matter to the service.

NIST’s Generative AI Profile states: “AI system performance or assurance criteria are measured qualitatively or quantitatively and demonstrated for conditions similar to deployment setting(s). Measures are documented.” Attribute this sentence to NIST, Artificial Intelligence Risk Management Framework: Generative Artificial Intelligence Profile (NIST AI 600-1, 2024).
Rerun tests after material changes
Re-evaluate after changing a model, prompt, retrieval algorithm, index, connector, memory policy, tool, safety filter, or permission model. Keep the prior results, test inputs, model version, retrieved context, and policy decisions so a regression can be explained. A model upgrade that improves factual accuracy can still weaken refusal behavior or expose a previously isolated index.
5. Constrain outputs and agent actions
Model output is untrusted until the application validates it. This applies before rendering it in a browser, storing it, sending it to a user, or passing it to another system.

Validate format and destination
- Use strict schemas and type validation for structured responses.
- Encode content for its destination to prevent HTML, SQL, shell, template, or command injection.
- Run sensitive-data detection before exporting, emailing, displaying, or persisting a response.
- Check recipients, tenant identifiers, record ownership, and transaction limits in application code.
- Reject extra fields, unsupported commands, malformed references, and output that exceeds a defined scope.
Reduce agent authority
Expose narrowly scoped functions instead of open-ended code execution or unrestricted database access. Separate read and write tools. Require explicit parameters, bounded queries, rate limits, and idempotency keys. Execute a tool using the authenticated user’s access context when the target resource must honor that user’s permissions. Never let the model choose a stronger identity because the requested task is difficult.

Require a human or a separately authorized service to approve payments, access changes, customer communications, production deployments, deletion, legal commitments, and other high-impact actions. Show the proposed action, source evidence, affected resources, identity, and reversibility before approval. Log the approver and the exact payload that was authorized. Human review is a control for consequence, not a substitute for basic authorization.
6. Monitor, respond, and improve
Logging must be detailed enough to reconstruct an event and protected because it may contain the same sensitive information the system is meant to guard.
Capture an auditable event record
- Initiating user, service, agent, tenant, session, and correlation identifiers.
- Relevant prompt and policy version, with secrets and unnecessary personal data redacted.
- Model name and version, application release, retrieval query, source identifiers, and permission decisions.
- Tool calls, arguments after redaction, results, approvals, output, destination, and completion status.
- Safety classifications, refusal or escalation decisions, and evaluation or policy failures.
Set retention, access, integrity, and deletion rules for these records. Restrict log readers, encrypt data in transit and at rest, and monitor attempts to alter or export logs.
Detect suspicious behavior
Alert on cross-user or cross-tenant reads, repeated requests for hidden prompts or credentials, unusual bulk retrieval, sensitive-output markers, sudden tool-call volume, permission-denied spikes, and repeated attempts using different encodings or languages. Correlate alerts across the model, retrieval, identity, and application layers; a single component may not show the full pattern.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitchesUse a response runbook
- Contain the path: disable the connector, revoke a token, isolate a tenant, or pause the affected agent.
- Preserve the relevant prompts, context, model version, permissions, tool calls, approvals, and outputs with access controls intact.
- Determine whether data was merely exposed in memory, written to a log, delivered to a user, or sent to an external system.
- Revoke or rotate credentials, remove poisoned content, correct permissions, and invalidate unsafe caches or indexes.
- Notify the required privacy, security, legal, and business owners according to the organization’s incident process.
- Add the failure to adversarial tests and change the access, retrieval, prompt, tool, or approval control that allowed it.
Frameworks that organize the control program
NIST AI Risk Management Framework
NIST describes AI RMF 1.0 as voluntary guidance for incorporating trustworthiness into AI design, development, use, and evaluation. It was released on January 26, 2023. NIST’s AI RMF page says, “The AI RMF 1.0 is being revised as part of the White House AI Action Plan.” Cite the version and date in internal policies so a future revision is not confused with the 1.0 edition.

NIST AI 600-1, Generative AI Profile
Released July 26, 2024, the profile suggests actions for managing generative-AI risks in line with organizational goals and priorities. It addresses confabulation, data privacy, information security, evaluation, and human-centered safeguards. Use its actions to expand a risk register and test plan rather than as a certification or guarantee.
OWASP Top 10 for LLM and Generative AI, 2025
OWASP’s 2025 list is an application risk taxonomy covering prompt injection, sensitive information disclosure, supply chain, data and model poisoning, improper output handling, excessive agency, system prompt leakage, vector and embedding weaknesses, misinformation, and unbounded consumption. Map each applicable category to an owner, preventive control, detection signal, test case, and response action. The list does not prove that one mitigation fully solves any category.

Common approaches that fail
- Relying on a system prompt as a security boundary. Prompts can leak, be overridden by injected content, and cannot replace application authorization.
- Indexing everything and filtering later. Broad indexes and shared service identities can expose restricted material before a response filter runs.
- Assuming a citation proves truth. The source may be stale, irrelevant, contradictory, or unauthorized for the reader.
- Passing model output directly to tools. A plausible-looking string can contain an unsafe command, wrong identifier, or unauthorized target.
- Testing only ordinary questions. Security failures often appear in edge cases, revocations, malformed files, indirect injections, or conflicting evidence.
- Logging everything without protection. Debug traces can become a secondary repository of prompts, documents, credentials, and personal data.
- Treating a model upgrade as a quality-only change. New behavior can alter data handling, refusals, tool selection, and retrieval interactions; rerun the full evaluation set.
Operational checklist
Before production approval, verify that the team can answer yes to the following:
- Every model, prompt, data source, index, memory store, tool, owner, version, and location is inventoried.
- Data classifications and authorization rules are enforced at ingestion, retrieval, memory access, tool use, and output delivery.
- Secrets and authorization logic are outside prompts and retrieved documents.
- Indexes and caches are isolated, permission-aware, and tested after revocation or deletion.
- Retrieved content and model output are treated as untrusted and validated for their destination.
- Agents have only the functions and permissions required for the task.
- High-impact actions require explicit approval with an auditable payload.
- Evaluation sets include representative, adversarial, ambiguous, stale, conflicting, and unauthorized cases.
- Results are measured under conditions similar to deployment and rerun after material changes.
- Logs can reconstruct incidents while minimizing and protecting sensitive content.
- Alerts, containment steps, notification responsibilities, and improvement actions are documented.
This layered program addresses leakage and unsupported output as operational risks rather than as defects that one model setting can eliminate. Its effectiveness depends on the actual architecture, permissions, data, and deployment conditions, so controls must be tested and maintained as those conditions change.
