Prevent blisters by treating new flats as a fit-and-friction problem, not a test of endurance. Try them with the hosiery you will actually wear, make sure your foot is neither squeezed nor sliding, and start with brief indoor wears. Inspect your skin after each wear. At the first hot spot, stop, dry the area and add padding or a blister dressing. If the shoe keeps rubbing, replace or refit it; no break-in routine can turn a fundamentally poor fit into a comfortable one.

There is no medically validated, one-size-fits-all number of hours for breaking in ballet flats. Comfort and your skin’s response should determine when to extend each wear.

Why new flats cause blisters

A blister forms when repeated friction makes the upper layers of skin separate and fill with fluid. New flats can create that friction in several ways: a stiff heel counter scrapes the back of the foot, a seam presses a toe, or a loose shoe lets the foot slide on every step. Both tight and loose shoes can therefore be a problem. The NHS lists rubbing from shoes as a common cause and advises choosing footwear that fits well: NHS blister guidance.

Flat shoes often have little adjustable fastening. If the opening is too wide, your heel may lift and your forefoot may slide forward. If the toe box is narrow, toes can press against one another or the upper. A shoe that already pinches, slips or rubs during a fitting is unlikely to become suitable simply because you wear it longer. Guy’s and St Thomas’ NHS Foundation Trust advises trying new shoes indoors so you can identify problems before a long outing: its shoe-fitting guidance.

Check the fit before the first outdoor wear

Use the hosiery you really plan to wear

Try the flats with bare feet, tights, liners or socks according to how you will normally use them. A pair that fits over thin tights may become tight with socks; a pair that feels secure barefoot may slide when a smooth liner is added. Walk on a hard indoor surface for several minutes and notice whether your heel lifts, your foot moves forward or any edge feels sharp. Do not rely on standing still.

Check length and toe room

Your toes should not be forced against the front or folded under one another. Walk, bend your knees and go up and down a step if possible. Pressure that appears only when your foot flexes is still a fit warning. Extra length is not automatically safer: too much room can let the foot move and increase rubbing.

Check width, seams and the opening

Look and feel around the little-toe side, the top of the toes and the widest part of the forefoot. Run a finger around internal seams, decorative edges and stiff material. A prominent seam over a bony area is a likely hot spot. The topline should hold the shoe on without cutting into the ankle or instep.

Check the heel and sole

A heel that repeatedly lifts can abrade the skin even when the shoe feels roomy. A heel that is clamped tightly can create pressure blisters. Walk enough to see whether the sole bends where your foot bends; if the shoe forces your foot to flex at an awkward point, stop and reassess rather than expecting pain to disappear.

Fit is the foundation. Mayo Clinic’s prevention advice emphasizes well-fitting footwear and padding at high-friction areas: Mayo Clinic, “Blisters: First aid”.

Break the flats in gradually, indoors first

Georgeanne Botek, DPM, a foot specialist quoted by Cleveland Clinic, summarizes the safe approach: “Break shoes in slowly before you wear them for long periods of time.” She also recommends easing into new shoes “by wearing them indoors in short spurts.” Source: Cleveland Clinic, “5 Ways To Avoid Blisters (and the Best Way to Treat Them)”, June 11, 2024.

No source establishes a fixed ballet-flat schedule, so use a symptom-based progression rather than a promised number of days. The following sequence gives you control without pretending that every foot or shoe behaves the same way.

  1. Prepare the test. Put on the intended hosiery, trim rough toenails and make sure your feet are dry. Keep a blister dressing or moleskin nearby so you can stop immediately if a hot spot appears.
  2. Start with a short indoor wear. Wear the flats at home while you can sit down, remove them and inspect your skin. Avoid beginning with commuting, shopping or an event where you cannot change shoes.
  3. Inspect both foot and shoe. Look for redness, warmth, stinging or a shiny rubbed patch. Check the corresponding shoe area for a seam, edge or wrinkle. Early redness is a reason to intervene, not a sign that you should push through.
  4. Repeat only while comfortable. Use another brief indoor wear after the skin has returned to normal. Extend the duration in small, comfortable increments; if discomfort appears sooner, go back to the previous comfortable duration or stop.
  5. Test a short errand. Once several indoor wears remain comfortable, choose an outing with an easy return route and carry a backup pair. Do not make the first outdoor test an all-day event.
  6. Increase demands one at a time. Add distance, standing time or different hosiery separately. If a change creates rubbing, you can identify the cause instead of blaming the entire shoe.

Breaking in means allowing materials to settle while your skin remains intact. It does not mean tolerating pain, open skin or repeated blisters. If a shoe hurts during every trial, discontinue it and reconsider the size, width, shape or construction.

Control moisture without making the fit tighter

Damp skin is more vulnerable to friction. Keep feet clean and dry before putting on the flats, and let shoes dry fully between wears. If the style has enough room, breathable moisture-wicking socks or low-profile liners can move perspiration away from the skin. NHS and Mayo Clinic guidance supports moisture-wicking hosiery and changing damp socks; the hosiery must still fit comfortably and must not crowd the toes or make the shoe tight.

Carry a spare pair of socks or liners when socks are part of your outfit. Change them when they become damp rather than waiting for irritation. If the flats are designed for bare feet and adding hosiery changes the fit, do not force socks into them; use shorter wears, drying breaks and a better-fitting shoe instead.

Protect a known hot spot

Choose padding that matches the problem

At a location that repeatedly rubs, apply a piece of moleskin, medical tape or a gel blister bandage before the wear begins. Mayo Clinic describes moleskin as a durable fabric for high-friction areas and recommends moleskin or gel-filled blister bandages for padding. Place the material on clean, dry skin, smooth the edges and check that it does not create a new ridge.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Padding is an optional friction aid, not a way to rescue a shoe that is too small, too wide or structurally painful. If you need increasingly thick layers to tolerate the flats, the fit deserves more attention than the dressing.

Apply protection before the long day

Do not wait until an event to discover whether a dressing stays in place. Test it during an indoor wear. A strip that wrinkles, rolls or shifts can increase friction. Replace it if it becomes damp or loose, following the product’s instructions.

Reduce the source of rubbing

Inspect the shoe after each trial. A folded lining, rough thread, hard edge or removable insole may be responsible. If the manufacturer’s design allows a safe adjustment, smooth or remove the offending feature; do not cut structural material or create a sharp edge. When the problem is the shoe’s shape, return or exchange it instead of layering on more padding.

Act at the first sign of a hot spot

A hot spot may feel warm, tender, prickly or burning before a visible blister develops. Stop the friction as soon as you notice it. Remove the flats, dry the skin, and cover or cushion the area with a suitable bandage, tape or moleskin. If you cannot stop immediately, shorten your stride and change shoes as soon as possible; continuing to rub a hot spot can turn intact skin into an open blister.

Afterward, identify what touched that exact location. Compare the left and right shoes, check the hosiery, and look for sliding or a seam. A recurring hot spot despite protection is evidence that the fit or construction remains unsuitable.

Care for a blister that has already formed

Leave the skin roof intact when possible

The skin covering helps protect the raw tissue beneath. Do not deliberately break a fresh blister yourself. Gently wash the area, pat it dry and cover it with a protective dressing. NHS guidance notes that hydrocolloid blister dressings can protect a blister, reduce pain and support healing: NHS, “Blisters.”

A dressing should cushion the area from the shoe and remain clean. Follow its package directions for changing it, and avoid wearing the offending flats until the skin is comfortable and closed. Choose footwear that does not press the dressing or make the injury worse.

Know the warning signs

Seek medical advice if the area becomes increasingly red, hot or swollen, if pain worsens, or if you see green or yellow pus. NHS guidance identifies hot skin and green or yellow pus as signs that require medical attention. Fever, red streaks or feeling generally unwell also warrant prompt care. Do not use a painful, infected-looking area as a break-in experiment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

If the blister opens accidentally

Wash your hands, rinse the area gently with clean water, leave the remaining skin in place and cover it with a clean dressing. Keep the shoe away from the wound until it has healed. If drainage, redness or pain increases, contact a clinician rather than treating it as ordinary shoe irritation.

When to stop trying to break in the flats

Return, exchange or retire the pair when the same rubbing happens on each trial, when your foot slides despite normal hosiery, or when the shoe causes pain before you have walked any meaningful distance. A brief period of material softening cannot correct the wrong length, width, heel shape or toe-box geometry. Repeated blisters are feedback about the shoe, not a personal failure to “toughen” your feet.

Do not size down merely to stop heel lift if the smaller size squeezes the toes. Do not size up to create toe room if the larger size lets your heel move. Instead, look for a different last or a fastening that can hold the foot securely. Any insert or liner should be used only if it preserves comfortable toe room and does not create pressure elsewhere.

Extra caution for diabetes or poor circulation

People with diabetes, reduced sensation or poor circulation can develop serious foot problems from a small blister and may not feel the early warning signs. Mayo Clinic and NHS guidance advise extra care and individualized foot-care advice; NHS England provides specific high-risk diabetes foot-care information at its high-risk diabetes foot-care leaflet.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

If this applies to you, ask your clinician or podiatry team how to assess new footwear before wearing it. Inspect your feet after every trial, avoid self-treating a wound without professional guidance, and seek prompt advice for any break in the skin, discoloration, swelling or drainage. A gradual break-in plan does not replace your individualized medical instructions.

Quick troubleshooting guide

What you notice Likely contributor What to do next
Heel lifts and the back feels hot Opening or heel shape allows sliding Stop, protect the spot, test a different size or shape; do not rely on repeated long wears.
Toes feel compressed or numb Insufficient width or toe-box height Remove the flats and seek a roomier shape. Padding cannot create missing space.
One edge rubs at every step Seam, stiff trim or folded lining Inspect and smooth only a non-structural wrinkle; otherwise exchange or retire the pair.
Feet feel damp and skin softens Perspiration or damp hosiery Dry feet, change hosiery and let the shoes dry; use moisture-wicking hosiery only if it does not tighten the fit.
Redness appears during a short indoor wear Friction has started before a blister is visible Stop immediately, cover the hot spot and reassess the shoe before another trial.
Blister is painful, hot or draining Possible infection or significant irritation Protect it and seek medical advice, especially with diabetes or circulation problems.

Carry a simple prevention kit

For the first few wears, pack the items that let you respond before a hot spot becomes a wound:

  • A pair of comfortable backup shoes.
  • Moleskin, medical tape or a gel blister bandage for a known rubbing point.
  • Clean spare socks or liners when your outfit uses hosiery.
  • A small clean dressing for an existing blister.
  • A way to dry your feet if hosiery becomes damp.

Use the kit to end friction early, not to justify wearing a pair that repeatedly hurts. The most effective prevention remains a comfortable fit, short indoor trials and stopping at the first warning from your skin.
