For most people, a PSMA PET/CT scan requires no fasting and no change to regular medicines, but the imaging center’s instructions always take priority. Plan to drink water as directed, bring a complete health and treatment history, allow time for an intravenous (IV) tracer injection and about an hour of tracer uptake, then expect roughly 30 minutes of scanning. The whole visit is longer than the scan itself because it includes check-in, preparation, the uptake period and image review by staff.
What a PSMA PET/CT scan does
PSMA PET/CT is an imaging examination commonly used in people being evaluated or treated for prostate cancer. A technologist injects a small amount of radiotracer into a vein. The tracer circulates through the body and attaches to prostate-specific membrane antigen (PSMA) proteins. Areas where the tracer accumulates can then be shown on a PET scan.
PET shows the distribution of the tracer. The CT portion supplies anatomical detail, helping the specialist locate the signal in relation to organs, bones and other structures. The two sets of images are interpreted together.

Some normal organs can take up the tracer, and very small tumors may not be visible. An imaging specialist interprets the images and writes a report; your treating clinician considers that report alongside your symptoms, PSA results, previous imaging, biopsy or diagnosis information, treatments and other health conditions.

How you prepare before the appointment
Read the appointment letter first
Follow the written instructions from your imaging center even when they differ from general patient guidance. Centers can use different tracers and protocols, and your medical circumstances may require an individualized plan. If anything in the letter is unclear, call the service before the appointment rather than guessing.
Fasting and regular medicines
General EANM/SNMMI guidance says fasting is not required and regular medicines may be taken. The professional guideline states: “Patients do not need to fast and may take all their medications.” That is general guidance, not permission to disregard a center’s specific directions. Keep taking, stopping or timing a medicine exactly as the imaging team or prescribing clinician instructs.
| Preparation question | General guidance | What takes priority |
|---|---|---|
| Do I need to fast? | Usually no fasting is needed. | The imaging center’s appointment instructions and any patient-specific medical advice. |
| Can I take my medicines? | Usually yes, including regular medicines. | Instructions from the imaging service or the clinician managing a time-sensitive medicine. |
| How much should I drink? | Adequate hydration is encouraged. | The center’s directions on how much and when to drink; there is no universal volume in the available guidance. |
| What should I wear? | Comfortable clothing is practical. | Any clothing or changing instructions supplied by the facility. |
Hydrate according to the center’s directions
The joint EANM/SNMMI guideline advises adequate hydration. Ask the imaging service when to start drinking and whether it has a target amount. Do not assume that a generic internet schedule applies to you, particularly if you have a condition or treatment that limits fluids.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsGive the team a complete medication and allergy history
Tell the staff about prescriptions, over-the-counter medicines, vitamins and supplements. Report allergies and any previous reaction to an injection or contrast agent. If you have been given instructions for an anxiety medicine, ask whether it affects driving and whether another adult must take you home. Do not arrange your transport on assumptions; confirm the center’s policy.
Report devices, metal and claustrophobia
Tell the team about implanted medical devices or metal and about claustrophobia. This lets staff plan the examination and explain the equipment and positioning. A PSMA PET/CT appointment is not the place to conceal a device, implant or prior procedure because the team needs accurate information before imaging begins.

Make sure the referring team has the relevant history
The interpreting specialist needs the clinical context supplied with the referral. This can include your diagnosis and risk information, PSA history, previous prostate cancer treatment, symptoms, prior imaging and important coexisting conditions. Contact the referring office if a recent report or treatment detail is missing from your records.
Allow for a time-sensitive appointment
Some patients need a particular appointment time because a medicine or another part of their care is time-sensitive. If that applies to you, tell the imaging service when booking or as soon as the issue arises. Do not reschedule or arrive at a different time without checking first, because the service may have coordinated the tracer and staffing around your slot.
A practical preparation checklist
- Read the appointment letter and write down questions for the imaging center.
- Confirm whether the center wants you to eat normally, fast, or follow another diet instruction.
- Confirm which medicines to take and when, especially medicines that cannot be delayed.
- Drink water only in the amount and timeframe the service recommends.
- Prepare a list of prescriptions, nonprescription medicines, vitamins and supplements.
- Note allergies, previous injection reactions, implanted devices, metal and claustrophobia.
- Check that the referral includes relevant PSA, diagnosis, treatment and prior-imaging information.
- Wear comfortable clothing and arrange transport if an anxiety medicine may impair driving.
- Plan enough time for check-in, IV placement, tracer uptake and the scan itself.
What happens during the appointment
1. Check-in and safety questions
At arrival, staff verify your identity, referral and appointment details. They may review your medicines, allergies, medical history, implants and any instructions you were given. Use this opportunity to mention a new symptom, a change in treatment or a concern about lying still or being in the scanner.
2. Preparation for the injection
A technologist places an IV in a vein. The IV is used to administer the radiotracer. Tell the technologist if you have difficult veins, a history of fainting during injections or a previous reaction so the team can prepare appropriately.
3. Radiotracer injection
The technologist injects a small amount of PSMA-targeting radiotracer through the IV. The tracer then circulates and binds to PSMA proteins. The injection is a clinical procedure, so follow the staff’s instructions about where to sit or lie and what to do immediately afterward.

4. Uptake period of about an hour
SNMMI describes waiting about an hour after injection for the tracer to circulate and reach the tissues being imaged. This is an approximate patient-facing time, not a guarantee. The exact interval depends on the local protocol and your circumstances. Use the restroom or ask for assistance if needed, and follow the center’s directions about drinking during the wait.

5. Positioning on the scanner table
For image acquisition, you lie on a table that moves through the PET/CT scanner. Staff position you and explain how to remain still. Tell them before scanning starts if pain, anxiety or another problem could make the position difficult to maintain; solving it beforehand helps avoid motion-related problems and repeat images.
6. PET and CT image acquisition
The scanner collects the PET information showing tracer distribution and CT information showing anatomy. SNMMI says the scanning portion usually takes about 30 minutes. The duration can vary with the protocol and whether additional images are needed. Your practical task is to remain positioned and follow instructions from the technologist.
7. Completion and release
After the images are obtained, staff check that the examination is complete and explain any local instructions. Ask when and how your clinician will receive the report. If you were given an anxiety medicine, follow the transportation and activity advice you were given rather than driving based on how alert you feel.
How long should you plan for?
The scan itself is usually about 30 minutes, but the appointment is longer because of registration, questions, IV placement, injection and the uptake wait. A useful planning outline is:

| Part of visit | What to expect | Timing information available to patients |
|---|---|---|
| Check-in and preparation | Identity and history review, instructions, IV placement and positioning preparation. | Variable; the center should provide its arrival time. |
| Tracer uptake | Waiting while the injected tracer circulates and binds to PSMA proteins. | About one hour according to SNMMI; local protocols can differ. |
| Scanning | PET and CT image acquisition while you lie on the moving table. | Usually about 30 minutes according to SNMMI. |
| Total visit | All stages above, including any additional local steps. | Not a fixed universal duration; allow more time than the scan estimate. |
What to do if something changes before the scan
You received no clear fasting or medication instructions
Call the imaging center and ask. Do not invent a fasting period or stop a medicine simply because another patient was told to do so. General guidance is permissive, but the facility’s protocol and your medical needs control the appointment.
You take a medicine at a fixed time
Tell the service as soon as possible. The center may need to preserve a particular appointment time or provide a specific timing instruction. Bring the medicine list so staff can reconcile what you took and when.
You are anxious or claustrophobic
Tell staff when you book and again at check-in. Ask what support is available and whether any prescribed anxiety medicine would require a driver. Do not take an unapproved sedative or arrange to drive yourself after a medicine that may impair alertness.
You have an implant or metal
Report it before positioning. The team can document the device and determine the appropriate workflow. Do not assume that a device is irrelevant because you have had other scans in the past.
You have had a previous reaction
Tell the technologist and the referring team what happened, when it happened and which injection or medicine was involved. This information helps staff decide what precautions or explanations are needed for your appointment.

What happens after my procedure or examination?
Leaving the imaging center
Follow the center’s post-appointment directions, including any instructions about drinking, activity or transportation. Continue to ask the service if you are unsure what applies to your tracer or protocol; a general instruction from another facility may not match yours.
Image interpretation and the report
An imaging specialist, such as a nuclear medicine physician or radiologist, reviews the PET/CT images and prepares a report. The specialist considers where tracer uptake appears, how it corresponds to CT anatomy and whether the pattern requires qualification. Normal organs can show uptake, and very small tumors may be below the scan’s ability to resolve.
How your clinician uses the result
Your treating doctor reviews the report with the rest of your health information. The scan is one part of clinical decision-making, not a stand-alone verdict. Depending on the findings and your history, an unclear or unexpected result may lead to further evaluation. Ask your clinician how the report fits with your PSA trend, prior treatment, symptoms and other tests.
Questions worth asking the imaging center
- Should I eat and drink normally, fast, or follow a different instruction?
- Which medicines and supplements should I take on the appointment day?
- Does my time-sensitive medicine affect the appointment time?
- How much water should I drink and when?
- Should I bring a medication list or prior imaging reports?
- What should I do if I am claustrophobic or may need help remaining still?
- If an anxiety medicine is prescribed, must someone drive me home?
- When will the report be sent to my treating clinician, and whom should I contact for results?
Key points to remember
Use your imaging center’s instructions as the final authority. In general, PSMA PET/CT does not require fasting and regular medicines can be taken, while adequate hydration is encouraged. The appointment includes an IV tracer injection, about an hour for uptake and a scan that usually lasts about 30 minutes. Bring complete medical information, disclose implants, allergies and claustrophobia, and remember that tracer uptake must be interpreted in clinical context rather than treated as a diagnosis on its own.
