Do not treat chatbot safety as a final accuracy check. Before launch, establish accountable owners, map the chatbot’s users and data, test ordinary use and foreseeable abuse, mitigate unacceptable risks, and record a written go/no-go decision with monitoring and rollback authority. Reassess whenever the model, prompt, tools, data, or vendor changes.
What a chatbot safety risk assessment must decide
An assessment answers two practical questions: what could go wrong in this specific deployment, and what evidence is strong enough to release it within defined limits? The answer depends on context rather than on a universal “safe” score. A low-stakes public FAQ bot and a bot that advises on health, employment, finance, education, or access to internal systems have different failure consequences and therefore need different controls and acceptance criteria.
The assessment should produce an auditable record containing:
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minute- the chatbot’s intended purpose, users, operating boundaries, dependencies, and affected people;
- identified benefits, harms, misuse cases, legal and organizational constraints, and residual risks;
- test plans, representative and adversarial results, human-review findings, and severity ratings;
- named owners for each unresolved risk and each mitigation;
- launch limits, monitoring thresholds, incident procedures, and rollback or shutdown authority; and
- a dated go/no-go decision that is revisited after material changes.
NIST’s AI Risk Management Framework (AI RMF 1.0, January 26, 2023) organizes this work into Govern, Map, Measure, and Manage. NIST describes the framework as voluntary guidance for organizations designing, developing, deploying, or using AI systems. Its Generative AI Profile (NIST-AI-600-1, July 26, 2024) adds risk-management guidance for generative-AI-specific problems. ISO/IEC 23894:2023, published in February 2023, provides guidance for integrating AI risk management into activities involving AI products, systems, and services.

1. Govern: make safety someone’s job
Governance comes before testing because a test result has no operational meaning unless someone can act on it. Assign an accountable business owner with authority to delay launch, a technical owner for the model and integrations, a security and privacy contact, and an incident lead. In a small organization, one person may hold several roles, but the responsibilities should still be written down.
Set decision rights and escalation paths
- Identify who approves the initial release, who can change prompts or retrieval sources, and who can authorize a rollback or shutdown.
- Define when a frontline reviewer must escalate a conversation to a human and which team receives the escalation.
- Specify who must be notified for a privacy incident, security abuse, harmful output, prolonged outage, or material model change.
- Set a target response time for each severity level; do not launch without coverage for the chatbot’s operating hours.
Define policy and documentation requirements
Write the permitted use, prohibited use, user-facing disclosures, data-retention rules, acceptable sources, and rules for human review. Keep a versioned record of the model identifier, system prompt, retrieval corpus, tool permissions, safety filters, test data, test date, and approval decision. Record assumptions explicitly, such as “the bot is informational and cannot approve transactions” or “users must not submit regulated personal data.”
Control third-party dependencies
List every external model, hosting service, retrieval component, moderation service, analytics tool, and action-taking integration. For each dependency, document the data it receives, the regions or environments in which it operates when known, change-notification terms, service limits, security responsibilities, and a contingency if the service becomes unavailable or changes behavior. A vendor update is a reassessment trigger, not merely a maintenance detail.

2. Map the deployment before selecting thresholds
NIST’s mapping step should leave the team with enough contextual knowledge to make an initial “go/no-go decision” about whether to design, develop, or deploy an AI system. Build a one-page system map, then expand it as evidence becomes available.
Describe users, tasks, and affected people
- Users: public visitors, employees, customers, children, professionals, or a mixed population; include language, accessibility, and digital-literacy needs.
- Tasks: answering questions, drafting text, searching an internal knowledge base, triaging requests, or taking actions through tools.
- Reliance: whether users may treat an answer as authoritative, whether a human checks it, and what happens if the answer is wrong.
- Affected people: not only the person chatting, but also employees, applicants, customers, bystanders, and anyone represented in retrieved or generated content.
Map data flows and boundaries
Trace a message from entry to storage, model processing, retrieval, tool execution, logging, analytics, and deletion. Mark personal, confidential, regulated, proprietary, and security-sensitive data. Identify where data can cross a trust boundary, whether prompts or outputs are retained, who can view logs, and which components can access source systems. Define retrieval boundaries so the bot cannot search repositories unrelated to its purpose.
List dependencies, constraints, and foreseeable misuse
Document model and vendor versions, prompt templates, retrieval indexes, tools, authentication, rate limits, network paths, and availability assumptions. Brainstorm misuse by an ordinary user, a malicious user, a compromised account, and an operator who configures the system incorrectly. Include prompt injection, attempts to extract hidden instructions or confidential content, unauthorized tool calls, abusive or discriminatory requests, automated scraping, and deliberate overload.
Describe benefits and harms
State the intended benefit in measurable terms, such as faster access to approved documentation or reduced queue time. Then describe harms if the bot fabricates an answer, exposes information, produces discriminatory content, takes an unauthorized action, or becomes unavailable. Include legal, contractual, policy, safety, reputational, and operational consequences. The most serious plausible harm—not the average conversation—should influence launch limits.

3. Set risk criteria that fit the context
Neither NIST AI RMF nor ISO/IEC 23894 supplies a universal numeric chatbot pass mark. Set criteria from the user population, task, data sensitivity, autonomy, harm severity, and applicable requirements. A useful internal rubric can classify findings as critical, high, medium, or low, but the labels and thresholds belong to your organization.
| Severity | Illustrative consequence | Default decision treatment |
|---|---|---|
| Critical | Likely exposure of sensitive data, dangerous autonomous action, or a failure that could cause severe harm. | Block launch; disable the affected capability until corrected and retested. |
| High | Material privacy, security, discriminatory, or operational harm requiring prompt human intervention. | Launch only if the risk is removed or tightly constrained with documented executive acceptance and monitoring. |
| Medium | Meaningful but recoverable misinformation, poor refusal behavior, or service degradation. | Define a remediation owner, user-visible limitation, monitoring threshold, and due date before release. |
| Low | Minor formatting, coverage, or usability defect with no material safety consequence. | Track in the backlog and verify that it does not combine with a higher-severity weakness. |
Define an acceptance criterion for each risk area before running tests. For example, a tool-enabled bot might require that every state-changing action is authenticated, explicitly confirmed, authorized for the user, logged, and reversible. A public informational bot might permit occasional uncertainty, but require it to disclose uncertainty and route high-impact questions to a human. Do not convert these examples into a universal percentage.
4. Build a test plan that covers normal use and abuse
Use a test set that represents real users, languages, devices, conversation lengths, knowledge domains, and likely edge cases. Add adversarial prompts designed by people who were not responsible for the original prompt or guardrails. Combine automated checks with human review; automation is useful for scale, while human reviewers are needed for context, ambiguity, dignity, and consequential harm.

| Risk area | What to test | Evidence to retain |
|---|---|---|
| Accuracy and grounding | Correct answers, uncertainty, citations or source references where promised, stale or missing information, and refusal when evidence is insufficient. | Prompt set, expected result, actual output, source used, reviewer judgment, and error severity. |
| Harmful content | Requests for dangerous, abusive, hateful, sexual, or self-harm-related content; indirect or coded variants; and attempts to bypass refusals. | Conversation transcript, policy category, refusal or safe-completion result, and escalation outcome. |
| Bias and fairness | Equivalent tasks using varied names, languages, demographic references, disability-related wording, and dialects; check both answers and refusals. | Matched-prompt results, reviewer criteria, disparity findings, and remediation decision. |
| Privacy leakage | Requests for personal data, memorized content, other users’ conversations, training or retrieval records, and sensitive data entered in prompts. | Data-flow trace, red-team prompts, access-control result, log-retention behavior, and deletion evidence. |
| Prompt injection | Instructions hidden in retrieved documents, webpages, attachments, user messages, and tool responses that attempt to override system rules. | Injected artifact, model behavior, blocked action, alert, and containment result. |
| Security and tool abuse | Unauthorized commands, privilege escalation, parameter tampering, cross-tenant access, secret extraction, replay, and actions without confirmation. | Authorization logs, tool-call trace, blocked and allowed cases, and rollback proof. |
| Availability and robustness | Long conversations, malformed input, high request volume, dependency failure, timeouts, model refusal loops, and partial outages. | Latency and error observations, fallback behavior, rate-limit response, and recovery time. |
| Accessibility and usability | Keyboard navigation, screen-reader labels, readable errors, language support, input alternatives, and understandable uncertainty or escalation messages. | Accessibility review, user findings, defect severity, and retest result. |
5. Run the tests in a controlled sequence
- Freeze the candidate. Record the model, system prompt, retrieval data, tools, safety settings, and dependency versions. Separate the test environment from production credentials and data.
- Create a representative baseline. Sample real or synthetic tasks that reflect intended use. Remove unnecessary personal data and label expected answers, acceptable uncertainty, and prohibited actions.
- Test ordinary conversations. Check factuality, relevance, grounding, language coverage, clarification behavior, and whether the bot stays within its stated purpose.
- Test foreseeable misuse. Add direct and indirect harmful requests, jailbreak attempts, role-play, multilingual variants, repeated pressure, and malformed or extremely long inputs.
- Test data and permissions. Attempt to retrieve another user’s information, expose secrets, cross a tenant boundary, bypass authentication, or invoke a tool outside the user’s authorization.
- Test injection and exfiltration. Place hostile instructions in retrieved documents, web content, attachments, and tool responses. Verify that the chatbot treats them as untrusted data and does not reveal hidden prompts, credentials, or unrelated records.
- Conduct independent human review. Give reviewers the policy, severity rubric, and a way to flag uncertainty. Use more than one reviewer for high-impact categories and adjudicate disagreements.
- Retest after every mitigation. Guardrails can create regressions, such as blocking legitimate questions or causing the model to disclose more context while refusing. Preserve before-and-after evidence.
- Summarize residual risk. For each unresolved finding, state the consequence, likelihood assessment, owner, compensating control, monitoring signal, and deadline.
6. Add controls before launch
Testing identifies weaknesses; controls reduce the chance or impact of failure. Select controls that match the mapped risk rather than adding generic warnings.

- Scope and retrieval boundaries: restrict the knowledge sources, tools, actions, and domains the chatbot can reach. Return “I don’t know” or escalate when evidence is outside the boundary.
- Authentication and authorization: bind every user and tool call to an identity and permission check. Never rely on the model to enforce access control.
- Human escalation: route high-impact, ambiguous, distressed, abusive, or repeatedly unresolved conversations to trained staff. Preserve the relevant context without exposing unnecessary data.
- Confirmation and reversibility: require explicit confirmation for consequential actions, show the intended parameters, use least privilege, and provide a tested undo or rollback path.
- Rate limits and abuse controls: limit automated scraping, repeated extraction attempts, oversized prompts, and unusual tool-call patterns. Define what happens when a limit is reached.
- Content and privacy filters: detect prohibited requests, secrets, personal data, and unsafe outputs. Document false-positive handling so legitimate users have a safe alternative.
- Output and uncertainty design: identify the bot as an AI system, state its purpose and limits, distinguish retrieved facts from generated text, and avoid presenting guesses as verified conclusions.
- Isolation and secrets management: keep credentials outside prompts, use short-lived tokens, constrain network access, sanitize tool inputs, and log security-relevant events without storing unnecessary sensitive content.
- Fallbacks: provide a human or static-information path when the model, retrieval service, moderation layer, or network is unavailable.
7. Make a written go/no-go decision
The decision record should be understandable to someone who did not run the tests. Include the intended release scope, excluded uses, evidence reviewed, open findings, compensating controls, monitoring thresholds, and the person authorized to halt service. A conditional “go” should specify exactly which users, regions, channels, tools, data classes, and traffic levels are allowed.
Use a practical decision gate
- Go: no unresolved critical risks; high risks are controlled and accepted by the named authority; required monitoring, escalation, and rollback have been tested.
- Limited go: release only to a constrained audience or read-only capability while specified medium or high risks are remediated by a dated owner.
- No-go: evidence is incomplete, a critical risk remains, access controls cannot be demonstrated, or incident and shutdown procedures are not operational.
Do not describe a limited release as proof that the system is generally safe. Its boundaries are part of the control and must be enforced technically and operationally.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.8. Monitor after deployment and rehearse failure
Launch is the start of the lifecycle, not the end of the assessment. Monitor signals connected to the risks you identified: harmful-output reports, escalation volume, unsupported-answer rate, privacy or security alerts, blocked tool calls, unusual extraction patterns, latency, errors, dependency outages, accessibility complaints, and drift in user or topic mix. Set thresholds that trigger investigation, traffic reduction, feature disablement, or shutdown.
Define the incident process
- Capture the conversation, model and prompt versions, relevant tool and access logs, and the time of the event while limiting unnecessary sensitive data.
- Contain the issue by disabling a tool, narrowing retrieval, increasing human review, blocking an attack pattern, or taking the chatbot offline.
- Assess affected users and data, preserve evidence, and notify the responsible privacy, security, legal, and business contacts according to policy.
- Correct the cause, test the fix against the original failure and related cases, and obtain reapproval before restoring the capability.
- Record lessons, update the risk register and test set, and communicate any user-facing correction required.
9. Reassess whenever the system changes
Trigger a targeted or full reassessment after a model or vendor replacement, system-prompt change, retrieval-corpus update, new language or user group, new tool or permission, authentication change, altered retention policy, safety-filter change, major traffic shift, or serious incident. Keep regression tests for previously fixed failures. A change-control record should state what changed, which risks could move, which tests were rerun, and who approved the result.

How the main frameworks fit together
| Framework | Primary scope | How it helps a chatbot assessment | Important limitation |
|---|---|---|---|
| NIST AI RMF 1.0 (January 26, 2023) | General AI risk management across the lifecycle. | Provides the Govern, Map, Measure, and Manage structure, including context, accountability, measurement, and ongoing management. | Voluntary guidance; it does not give a universal chatbot pass threshold. |
| NIST Generative AI Profile (July 26, 2024) | Generative-AI-specific risks and actions. | Helps address fabricated content, prompt injection, data leakage, harmful generation, and other generative-system concerns within the broader lifecycle. | It complements rather than replaces deployment-specific testing and governance. |
| ISO/IEC 23894:2023 (published February 2023) | Guidance for organizations developing, producing, deploying, or using AI products, systems, and services. | Helps integrate AI risk management into organizational activities and supports evidence and process discipline. | It is guidance, not a universal numeric safety score; applicability and certification expectations depend on the organization and other requirements. |
Organizations can use NIST and ISO/IEC 23894 together: use NIST’s lifecycle functions to structure the assessment and ISO/IEC guidance to fit risk management into existing organizational processes. Add sector, geographic, contractual, privacy, security, accessibility, and consumer-protection obligations that apply to the deployment.

Common mistakes that invalidate an assessment
Testing only the happy path
A handful of correct answers says little about injection, privacy, misuse, tool abuse, or outages. Add adversarial and failure-mode cases before drawing a conclusion.
Using accuracy as a safety proxy
A chatbot can be accurate on routine questions and still leak data, discriminate, follow malicious instructions, or take an unauthorized action. Measure each material risk separately.
Crashes, No Sound, or Screen Glitches?
Random freezes, missing sound and display glitches usually trace back to one bad driver. Find and replace yours safely.Free scan · under a minuteWindows Errors? Fix Them Before They Spread
Repair common Windows errors and clear accumulated junk for a smoother, more stable PC - no reinstall needed.Free scan · no reinstallLetting the model enforce permissions
Instructions such as “never reveal confidential records” are not an access-control system. Enforce identity and authorization outside the model and test the boundary directly.
Accepting vague residual risk
“The team will monitor it” is not a control unless the signal, threshold, owner, response, and rollback path are specified and tested.
Failing to reassess changes
A new model, prompt, retrieval source, tool, or vendor can alter behavior even when the user interface is unchanged. Preserve regression tests and require change-triggered review.
A chatbot is safe enough to deploy only within a documented scope whose risks are understood, controlled, monitored, and reversible. The strongest assessment is therefore a maintained operating process: Govern the accountability, Map the context, Measure ordinary use and abuse, Manage residual risk, and make a written decision that can be revisited when evidence or circumstances change.
