Use CISA’s Known Exploited Vulnerabilities (KEV) Catalog to identify vulnerabilities with real-world exploitation, then match each relevant entry to your own asset inventory, follow the affected product vendor’s remediation guidance, and verify the result on every affected system. The catalog helps establish priority; it does not tell you which systems you own, whether a particular version is affected, or how to deploy a fix safely in your environment.
A reliable enterprise response connects asset discovery, risk decisions, accountable owners, an emergency change path, and evidence of closure. NIST defines enterprise patch management as identifying, prioritizing, acquiring, installing, and verifying patches, updates, and upgrades. That lifecycle is a practical backbone for handling KEV entries, whether your organization patches through routine maintenance or an accelerated process.
What a KEV entry tells you—and what it does not
CISA describes the KEV Catalog as its authoritative source of vulnerabilities exploited in the wild. That makes an entry a strong prioritization signal: exploitation is not merely theoretical. CISA recommends using KEV as an input to vulnerability-management prioritization.
A catalog entry is not, by itself, proof that your organization is exposed. You still need to establish whether you run the affected product, whether the installed version or configuration is in scope, and whether the affected asset is reachable or important to your business. The product vendor’s advisory is the source to consult for affected versions, available patches or workarounds, and product-specific installation requirements. Use the catalog to set urgency and the vendor’s instructions to determine applicability and remediation.

KEV should inform your risk framework, not replace asset discovery or local risk decisions. An organization with no affected assets has a different response from one with an affected, internet-facing system that supports a critical business process. Record the basis for that distinction so that the decision can be reviewed later.
Build an inventory that can answer “where do we run this?”
You cannot reliably prioritize, deploy, or verify a fix if you cannot find affected assets. Maintain an authoritative inventory that can connect product and version information to a specific system, its owner, its exposure, and its business importance. Treat inventory quality as part of patch management rather than as a separate housekeeping task.
Capture the fields needed to make a decision
- Asset identity: a stable device, workload, appliance, or service identifier and its environment or location.
- Product details: vendor, product name, version, and relevant firmware or configuration information where applicable.
- Accountability: a technical owner who can investigate and deploy, plus a business or mission owner who can explain impact and participate in risk decisions.
- Exposure and importance: whether the asset is externally exposed or otherwise reachable, and how consequential an outage or compromise would be.
- Support status: whether the product is supported and whether the vendor provides a patch or mitigation for the version in use.
- Evidence and freshness: when the information was last observed and which management, scanning, or inventory source supplied it.
Reconcile endpoint-management and vulnerability-scanning results with inventories for cloud workloads, network equipment, appliances, and operational technology when those systems are in scope. A single endpoint tool may not see every technology an enterprise operates. Gaps should be visible rather than silently treated as proof that no affected systems exist.

For each KEV entry, compare the affected product and version ranges in the vendor advisory with the records in your inventory. A product family name alone may be too broad: different versions, editions, configurations, or deployment models can have different applicability. If the advisory does not make your installation’s status clear, route the question to the product owner or vendor support rather than assuming either that every deployment is vulnerable or that yours is safe.

Keep the result of the match, including the advisory or version information used, with the affected-asset list. This gives responders a defensible starting point for deployment and makes it possible to compare post-remediation evidence against the same original population.
Turn each KEV entry into an owned response
A catalog alert becomes actionable when someone is responsible for deciding applicability, choosing a response, coordinating deployment, and confirming closure. NIST recommends that leadership, business or mission owners, and security and technology management jointly create an enterprise patch strategy. Translate that strategy into named roles and an operational path for urgent cases.
Assign decisions before the next urgent entry
- Security or vulnerability management: monitor KEV and vendor notices, identify potentially relevant entries, and coordinate the risk assessment.
- Asset or product owner: confirm whether the product and version are present, determine applicability, and identify technical dependencies.
- Business or mission owner: explain operational impact, participate in decisions about maintenance or service disruption, and make risk acceptance decisions within the organization’s authority.
- Change authority: approve the appropriate routine or emergency change path and any exception to normal procedure.
- Deployment team: acquire the vendor remediation, execute the approved change, and report failures or systems that could not be reached.
- Verification owner: validate the outcome against the affected-asset list and track unresolved devices or exceptions to closure.
One person may fill more than one role in a smaller team, but the decisions should still be explicit. Specify who can authorize an emergency deployment, who can approve a maintenance exception, who communicates service impact, who may accept residual risk, and who independently checks the result where your process requires it.
Set an internal target that reflects your exposure
Establish internal response targets for KEV findings rather than waiting to negotiate urgency during each incident. Base the target and escalation path on whether your assets are affected, their exposure and business criticality, the exploitation signal, vendor guidance, and whether a safe mitigation is available. Define what happens when an owner misses a target or cannot deploy within it.

NIST SP 800-40 Rev. 4 describes the two-week remediation period for new KEV entries under Binding Operational Directive 22-01 as a requirement for federal agencies. NIST also says other organizations may voluntarily use KEV to help prioritize patching. The federal period is not a universal deadline or service-level agreement for private enterprises. Private organizations should set and document targets appropriate to their own risk, obligations, and operating model.
Follow a repeatable KEV-to-remediation workflow
Use one case record for each relevant vulnerability or coordinated response. The record should preserve the catalog entry, vendor advisory, affected assets, owners, decision, deployment activity, and verification evidence. The precise tools and approval sequence depend on your environment; the control objective is to move from identification to verified remediation without losing track of systems or decisions.
- Review the live KEV Catalog. Check CISA’s current catalog and identify entries that could apply to products in your environment. Do not rely on a copied list or old alert as a substitute for checking the current entry.
- Read the vendor advisory. Confirm affected product and version ranges, the supported patch or workaround, and any stated installation, restart, or deployment requirements. Keep the advisory with the case record.
- Search and reconcile the inventory. Identify candidate assets across the relevant endpoint, cloud, network, appliance, and operational-technology sources. Ask owners to resolve ambiguous product or version matches and note inventory gaps.
- Classify the affected set. Separate confirmed affected assets from candidates that need investigation and assets confirmed out of scope. Record the evidence supporting each status; do not label an unobserved device “not affected.”
- Rank and assign. Prioritize confirmed affected systems using exposure, business consequence, exploitation status, vendor guidance, and available mitigations. Name a technical owner and a business or mission contact for each deployment group or service.
- Choose the response path. Decide whether normal maintenance is sufficient or whether the risk calls for emergency change handling. Select the vendor-supported patch when available; use a vendor-described workaround or other appropriate temporary mitigation when the patch cannot yet be applied.
- Acquire and validate the remediation. Obtain the patch or mitigation from the vendor’s supported channel. Where feasible, validate it on a representative pilot or canary group, or use a tightly scoped emergency change. The amount of validation should reflect urgency and potential impact; an extended test cycle should not become an unexamined reason to leave an actively exploited exposure open.
- Deploy and communicate. Execute through routine or emergency channels with explicit authorization, owner visibility, and communications appropriate to the service impact. Record deployment results, including systems that were offline, failed, or require a different maintenance window.
- Verify and reconcile. Check installation or mitigation status using evidence suitable for the technology, then compare the results with the original affected-asset set. Re-scan or otherwise validate that the exposure is removed, and investigate assets for which evidence is missing or contradictory.
- Close or track an exception. Close only when the required verification is complete. For anything unresolved, record the accountable owner, current mitigation, reason remediation is delayed, review or expiry date, and next action.
Choose between routine patching, emergency deployment, and mitigation
NIST’s enterprise patch-management guidance supports a lifecycle that includes acquiring and installing fixes, while NIST SP 1800-31 demonstrates commercial tools for routine and emergency patching and temporary alternatives to patching. It does not prescribe one product or one universal deployment sequence. Select a response path according to the exposure and operational risk, and make the decision visible to the people accountable for both security and service continuity.
Use routine maintenance when the risk allows it
Route a KEV remediation through the normal maintenance process when the organization’s risk assessment supports that timing and the service can remain appropriately protected. Preserve the ordinary approval, scheduling, communication, and verification controls. A routine channel is not a reason to defer indefinitely: the case should still have a target, owner, and escalation if the planned window slips.

Accelerate through an emergency change when delay is riskier
Use an emergency path when the assessed risk warrants action sooner than the normal cycle. The path should make clear who authorizes the change, what systems are in scope, how owners and affected operations are notified, and how deployment failure or service disruption will be handled. Include a recovery or rollback plan appropriate to the technology, but do not treat rollback as a substitute for deciding how to contain the vulnerability if the change must be reversed.
Testing should be proportionate to the situation. A representative pilot, canary group, or narrowly scoped deployment can provide useful signal where feasible. For an actively exploited issue, teams may need to balance a shorter validation window against the risk of delay. Document the rationale, any compensating control, and who approved the residual risk rather than presenting either prolonged testing or immediate broad deployment as universally correct.
Use a temporary mitigation when the patch is unavailable or cannot be installed
If the vendor has not supplied a patch, or a system cannot be patched immediately, consult the vendor’s supported mitigation instructions. Determine which affected assets the measure covers, how it changes exposure, and how the organization will verify that it is in place. Track the mitigation as temporary: assign an owner, set a review or expiry date, identify the next action, and revisit it when the patch or a safer maintenance opportunity becomes available.

A mitigation is not proof that the underlying vulnerability is permanently resolved. Keep mitigated assets in the case until a supported remediation is installed or an authorized risk decision determines another outcome. If no vendor workaround is established, do not invent one or claim protection without evidence; escalate for a technical and risk decision using the information available.

Verify remediation on the systems that were actually at risk
Deployment success in a management console is not enough if some affected devices were offline, unreachable, or outside that tool’s coverage. Verification is an explicit part of NIST’s definition of enterprise patch management. Select evidence appropriate to the platform and retain it with the case.
Use evidence that answers the remediation question
- For a software patch: confirm the installed package or product version on the asset, or use an appropriate vulnerability scan or other technical validation.
- For firmware or infrastructure: use a source that can establish the running version or configuration on the relevant appliance or system.
- For a mitigation: verify the required setting or control is active on each covered asset, and preserve evidence of its scope.
- For a failed or offline asset: record the deployment result and arrange a follow-up check when the asset is reachable or its service owner can act.
Reconcile verification results against the original affected-asset set, not just the devices that reported success. Investigate mismatched versions, scan findings that remain, incomplete telemetry, and assets that disappeared from the latest inventory. A missing result means the status is unresolved, not automatically remediated.
Report the outcome in a way that distinguishes confirmed remediation, verified mitigation, pending deployment, and unknown status. Track exceptions to closure with a responsible owner, mitigation where applicable, a review or expiry date, and a next action. This gives leadership a more useful picture than a single “patch complete” percentage that hides unreachable or unverified systems.

What NIST’s 2026 NVD update means for response teams
NIST announced that, beginning April 15, 2026, the National Vulnerability Database (NVD) would prioritize enrichment for KEV entries, software used within the federal government, and critical software as defined by Executive Order 14028. NIST stated a goal of enriching KEV entries within one business day of receipt. That is an NVD data-enrichment goal, not a deadline for enterprises to patch.
For operational decisions, continue to consult CISA’s live KEV Catalog and the relevant product vendor’s current notice. The NVD update may affect how quickly additional vulnerability information is enriched there; it does not establish whether your asset inventory is complete, whether your installed version is affected, or what remediation the vendor supports.
Select tools around coverage and proof, not a product label
If you are evaluating patch-management or endpoint-management tools, assess whether they can support the full workflow across the technologies you actually operate. NIST SP 1800-31 demonstrates commercial tools in routine and emergency patching scenarios, but the guidance does not endorse a specific vendor. A tool that reports a deployment job as successful may still leave coverage gaps or fail to provide the evidence needed to close a KEV case.
- Inventory coverage: can the process account for endpoints, cloud workloads, appliances, network equipment, and other in-scope systems, or must results be reconciled from separate sources?
- Deployment reach: can teams deploy to the operating systems and infrastructure they manage, including systems with different ownership or maintenance constraints?
- Control over urgent changes: can responders target a defined group, identify failures, and make an emergency deployment visible to the appropriate owners?
- Mitigation handling: can teams record and track a temporary measure when patching is not immediately possible, even if the mitigation itself is applied through another control?
- Verification and reporting: can the organization retain asset-level results, identify offline or failed devices, and reconcile the outcome against the affected population?
- Operational fit: does the workflow support normal maintenance windows as well as an authorized emergency route, without obscuring approvals or service impact?
Tool selection should follow the required operating process and asset coverage. The reviewed NIST materials demonstrate a category of commercial tooling; they do not establish that one platform fits every enterprise or that purchasing a tool alone creates a complete patch program.
