Effective AI-agent safety uses monitoring as one layer in a control system, not as a substitute for enforcement. Give an agent only the tools and permissions it needs, validate each proposed action outside the model, require approval for high-impact operations, and record enough telemetry to reconstruct what happened. Then use traces, anomaly detection, and human response to find behavior that is suspicious, out of scope, or drifting from tested baselines.
Logs and alerts help you discover and investigate unwanted actions. They cannot guarantee that the first harmful action will be blocked. Preventive controls—least privilege, downstream authorization, complete mediation, rate limits, and action-level approval gates—must operate before an action reaches a tool or downstream system.
What AI-agent monitoring is—and what it is not
An AI agent can interpret a goal, choose tools, call APIs, update memory, delegate work, and continue through several steps without a person reviewing every decision. Monitoring makes that execution observable and reviewable. A useful monitoring program answers four questions for every significant event:
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
- What task was the agent attempting?
- What action did it propose or perform, against which target, and with which identity?
- Which model, tool, permission, policy, or downstream service made the action possible?
- Was the action allowed, blocked, approved, retried, or escalated—and what happened next?
Monitoring normally includes event collection, distributed tracing, rules and anomaly detection, alert routing, investigation, and response. It may reveal tool abuse, privilege escalation, sensitive-data exfiltration, memory poisoning, goal hijacking, excessive autonomy, high-impact action abuse, or cascading failures. OWASP treats logging and monitoring as ways to identify undesirable actions and respond to them, while separately recommending controls that reduce the agent’s authority.

A monitor that examines an event after execution can provide warning, evidence, and containment value. It cannot retroactively make an irreversible transfer, deletion, publication, or disclosure safe. The control that decides whether the action is permitted must run at the execution boundary, independently of the model’s own explanation.
1. Define the agent’s mandate and risk tiers
Write the agent’s allowed purpose before selecting dashboards or alerts. Document the resources it may access, the tools it may call, the identities it may act for, the data it may read or write, and the actions that are explicitly out of scope. Treat this as an enforceable contract for the runtime, not merely a prompt.
Separate reversible work from high-impact work
Use risk tiers so that approval and monitoring effort match the consequences of an error. A read-only search with no external side effect is different from changing an access policy or sending a message to thousands of customers.

One free scan finds every outdated or missing driver and matches the right update for your exact hardware.Free scan · exact hardware match| Risk tier | Typical characteristics | Minimum execution control | Monitoring emphasis |
|---|---|---|---|
| Low | Read-only, reversible, limited to approved data and tools | Least-privilege identity; schema and scope validation | Normal traces, errors, latency, and unusual volume |
| Moderate | Writes to internal systems, repeated operations, or access to sensitive records | Downstream authorization, complete mediation, input and output checks, rate limits | Target changes, retries, data volume, policy decisions, and baseline drift |
| High | External publication, financial or administrative changes, disclosure of sensitive data, or difficult-to-reverse actions | Independent validation and human approval bound to the exact action and target | Real-time alerting, preserved evidence, reviewer identity, and pause or revoke capability |
OWASP recommends validating scope, privilege, and approval status independently before execution. An approval should identify the concrete operation, target, parameters, identity, and time window. “Approve the agent” is too broad; “approve posting this exact text to this exact account until 15:00 UTC” is reviewable and enforceable.
Turn threat categories into monitoring hypotheses
For each threat, define the observable signal and the preventive control. For example:
- Tool abuse: alert on a tool outside the task’s allowlist; block unknown tools at the gateway.
- Privilege escalation: alert when a token, role, or scope changes; require downstream authorization for the new privilege.
- Sensitive-data exfiltration: detect unusual destinations, data volume, or pattern matches; deny the transfer unless an approved policy permits it.
- Memory poisoning: record every memory or knowledge-base write; require provenance, validation, and restricted write permissions.
- Goal hijacking: compare the current objective and tool sequence with the signed task context; pause when instructions or scope change unexpectedly.
- Excessive autonomy: detect long chains, repeated retries, or actions outside the declared plan; enforce step and time budgets.
- Cascading failure: correlate errors and call volume across agents and services; apply circuit breakers and rate limits.
2. Constrain actions before they reach a tool
Prevention starts with the interface exposed to the agent. A narrow function such as create_refund(order_id, amount) is easier to authorize and audit than unrestricted shell execution, arbitrary database access, or a general-purpose URL fetcher. Design tools around one business operation, explicit parameters, bounded values, and a known target.
Use least privilege and downstream authorization
Grant only the permissions required for the current task, and make credentials short-lived and scoped where practical. When the agent acts for a person, execute on behalf of the authenticated user when appropriate so the downstream service can apply that user’s policy. The service receiving the request must verify authorization itself. Do not ask the model to decide whether its own proposed operation is allowed; model output is untrusted input to the authorization layer.
Apply complete mediation: check every call, not only the first call in a session. Re-check the target, resource, parameters, identity, and current policy after retries, delegation, tool handoffs, or a change in task state. A previously approved action must not silently become approval for a different target or payload.

Add independent validation and fail-closed behavior
Validate arguments against a strict schema, permitted ranges, resource ownership, and current state. Reject ambiguous identifiers and unexpected extra fields. If the policy service, approval service, identity provider, or validation dependency is unavailable, fail closed for high-impact operations rather than allowing the model to continue on a best guess.
Require human approval for consequential actions
Place a review step immediately before an externally visible, financially consequential, administrative, or difficult-to-undo action. Show the reviewer the exact operation, target, content, permissions used, expected effects, and relevant trace context. Bind the approval cryptographically or through an unambiguous transaction identifier so an agent cannot substitute a different payload after approval.
Posting social-media content is a straightforward example: the user should review and approve the proposed post before publication. The same pattern applies to deleting records, changing access controls, issuing refunds, sending regulated communications, or transferring data outside an approved boundary.

Limit speed, volume, and blast radius
Rate limits cap how many undesirable actions can occur and create time to discover them. Use per-agent, per-user, per-tool, and per-target limits where the risk warrants it. Combine them with maximum task duration, maximum tool calls, bounded retries, concurrency limits, and circuit breakers. A limit should produce a clear event and, for high-risk operations, pause the task for review rather than quietly dropping requests.
3. Instrument the complete execution path
Collect enough context to reconstruct the task from intent through outcome. Singapore’s 2026 addendum on securing agentic AI identifies the following telemetry categories as useful starting points:
| Event category | Record | Why it matters |
|---|---|---|
| Agent and model activity | Agent identity, model and software version, prompt or task reference, plan revisions, and handoff identifiers | Shows which component made a decision and whether behavior changed after a deployment or handoff |
| Tool and API calls | Tool name and version, arguments, target resource, caller identity, authorization result, response status, and returned data classification | Connects an observed side effect to the capability and permission that enabled it |
| Inputs and outputs | Relevant user input, tool input, model output, tool output, redactions, and content hashes where full content is unnecessary | Supports investigation while allowing sensitive material to be minimized |
| Internal state changes | Memory writes, knowledge-base updates, retrieved context identifiers, policy state, and delegation state | Exposes poisoning, unexpected persistence, and scope changes that ordinary request logs miss |
| Errors and exceptions | Error type, stack or safe diagnostic, retry count, timeout, fallback path, and circuit-breaker state | Distinguishes a deliberate action from a recovery loop or cascading failure |
| Time and duration | Timestamp with a consistent clock, start and end time, queue delay, and tool latency | Allows sequence reconstruction, rate analysis, and correlation with downstream logs |
| Task and session context | Task, session, workflow, user, tenant, parent-child span, and correlation identifiers | Lets investigators follow an action across services and agent handoffs |
Capture both the decision and the enforcement result. A record saying “tool called” is incomplete without whether policy allowed it, which rule matched, which approval was presented, and whether the downstream service accepted or rejected it.
Trace across agents, tools, and services
Use a trace identifier that survives model calls, tool calls, queues, delegated agents, and downstream APIs. The OWASP Agent Observability Standard describes agents as instrumentable, traceable, and inspectable. In practice, an investigator should be able to move from a suspicious API request to its parent task, model output, tool wrapper, permission decision, software version, and accessible data. OpenTelemetry and OCSF are cited approaches for tracing and event structure; MCP and A2A are relevant instrumentation points when agents use those protocols.

Prompts, tool arguments, outputs, identifiers, and memory contents may contain credentials, personal information, proprietary code, or regulated data. Decide what security review actually requires. Use field-level redaction, tokenization, hashing, encryption in transit and at rest, role-based access, and retention limits. Keep an auditable record of who viewed a sensitive trace. Singapore’s guidance specifically advises considering privacy regulations and related guidance when logging inputs.

Do not make redaction destroy investigative value. Preserve stable identifiers, event order, policy decisions, data classifications, and content hashes when the original payload can be withheld. Store the unredacted form only where the legal basis, access controls, and retention policy justify it.
4. Detect behavior that is suspicious or out of baseline
Detection should combine deterministic rules, tested baselines, and human review. No single anomaly score can understand every legitimate workflow or every attack.
Rules for clear policy violations
Rules are appropriate when the prohibited condition is explicit. Examples include a call to a disallowed tool, an attempt to access another tenant’s resource, a missing approval token, a request to send data to an unapproved destination, or a second action using an expired approval. Block the action at the enforcement point and emit an alert with the rule, identity, target, and trace.
Baselines for drift and unusual behavior
Build baselines from tested, known-good tasks. Useful dimensions include tools per task, sequence of calls, data volume, destinations, retry and error rates, latency, memory-write frequency, concurrency, and time-of-day patterns. Singapore’s addendum recommends continuous monitoring and logging for behavior that drifts from tested baselines, along with periodic permission audits.
A baseline is not a permanent allowlist. Revalidate it after model, prompt, tool, policy, data, or deployment changes. Separate legitimate seasonal or workflow changes from unexplained deviations, and document who can approve a baseline update.
Signals worth correlating
- Unauthorized or rarely used tools invoked by an otherwise routine task.
- Scope, role, destination, or identity changes in the middle of a workflow.
- Unusual access to sensitive records or a sudden increase in returned data.
- Memory or knowledge-base writes that lack provenance or occur outside the task’s purpose.
- Repeated failed actions, retries, timeouts, or fallback chains.
- High-volume or high-concurrency behavior inconsistent with the task.
- Tool sequences that contradict the declared objective or bypass an approval step.
- Coordinated anomalies across several agents, tenants, or downstream services.
Correlate signals by task and identity rather than alerting on isolated events only. A single failed call may be normal; a sequence of denied calls followed by a privilege change and a large export is materially different.
5. Make alerts operationally useful
An alert should start an investigation, not merely add a red dot to a dashboard. Include the trace, task and session identifiers, affected user or tenant, action and target, policy decision, approval details, preceding and following events, and the reason for escalation.

- Preserve evidence. Freeze or retain the relevant trace, policy version, model and tool versions, configuration, and downstream response. Record the time using synchronized clocks.
- Contain the activity. Pause the task, revoke or narrow credentials, disable the affected tool, or apply a target-specific block. Choose the least disruptive control that stops further harm.
- Identify scope. Search for the same agent, identity, tool, destination, payload hash, and behavior across related tasks and tenants.
- Assess impact. Determine whether data was read, changed, published, transferred, or merely attempted. Preserve downstream audit records.
- Review and decide. A designated human reviewer confirms whether the activity was legitimate, a policy error, a model failure, compromise, or an abuse attempt.
- Recover and learn. Rotate credentials, correct policy or tool schemas, remove poisoned state, notify affected parties when required, and add a regression scenario to evaluation.
Define on-call ownership and escalation thresholds before an incident. A monitor cannot help if no one can pause an agent or revoke its access outside business hours.

6. Evaluate observability products without confusing visibility with enforcement
Tracing products, evaluation systems, policy gateways, and security information platforms solve different problems. The Singapore addendum names MLflow, Databricks, OpenLLMetry, Helicone, Langfuse, LangSmith, Azure Agent Monitoring, and AWS Bedrock Trace Events as examples of monitoring and logging implementations. Their mention does not establish a ranking, feature parity, pricing, or a guarantee that any one product blocks unsafe actions.
When assessing an AI-agent observability tool or agent tracing platform, ask for evidence against the following criteria:
| Evaluation axis | Questions to ask |
|---|---|
| Trace coverage | Does it capture model calls, tool calls, handoffs, retries, memory and knowledge-base changes, policy decisions, and downstream outcomes? |
| Context linkage | Can an alert be connected to the complete task, session, user, tenant, identity, approval, and execution path? |
| Framework and standards support | Which agent frameworks and protocols are instrumented? Does it export or ingest OpenTelemetry or OCSF-compatible events? |
| Data handling | What can be redacted or tokenized? Who can view traces? What encryption, retention, residency, and deletion controls exist? |
| Deployment model | Can the system run in a required cloud, private network, or self-managed environment, and what data leaves your boundary? |
| Detection and workflow | Can teams define rules and baselines, route alerts, assign reviewers, preserve evidence, and connect incidents to existing response systems? |
| Runtime enforcement | Does it actually block or delay a tool call, require approval, enforce rate limits, or revoke access—or does it only record and evaluate activity? |
Test the enforcement claim with a controlled denied action. A dashboard that displays an unauthorized call after the fact is observability, not prevention. Keep authorization and approval decisions in a trusted runtime boundary even when a separate product provides excellent traces.

7. Test the monitor against realistic and adversarial scenarios
Evaluate detection with replayable tests that include normal work, edge cases, deliberate misuse, and ambiguous instructions. Measure whether the system captures the complete trace, blocks actions that should be blocked, alerts on behavior that should be reviewed, and lets responders pause or revoke access.
Test cases to include
- An agent calls a tool outside its allowlist.
- An approved payload is changed after the reviewer approves it.
- A user asks the agent to access another tenant’s data.
- A tool returns instructions that attempt to hijack the original goal.
- A memory write inserts an untrusted instruction that affects a later task.
- A downstream service is unavailable and the agent retries rapidly.
- A task gradually increases data volume or changes destination.
- Several delegated agents create a loop or duplicate an irreversible action.
Track false positives, false negatives, time to alert, time to human acknowledgment, time to containment, and the percentage of high-impact actions with valid approval. Re-run the tests after model, prompt, policy, tool, or infrastructure changes.
What the OpenAI internal example shows
OpenAI’s report on monitoring internal coding agents, published March 19, 2026, describes an asynchronous monitor that reviews coding-agent interactions and flags suspicious activity for human review. Approximately 1,000 moderate-severity conversations triggered alerts; many involved deliberate internal red-teaming. OpenAI reported that the monitor had matched every employee escalation known to the organization at that time, while cautioning that subtler issues may remain and that false-negative rates on open-ended real-world traffic could not yet be confidently quantified.
“That’s why we treat monitoring as one layer in a defense-in-depth stack—investing in monitorable reasoning traces and pairing monitors with complementary evaluations and preventative controls.”
Do these 3 things before closing this tab:

The alert count describes one internal deployment. It is not an industry-wide incident rate, a prevalence estimate, or a general accuracy score. Use it as an illustration of human-in-the-loop monitoring and its stated limits, not as proof that a monitor catches every unsafe action.

8. Roll out monitoring in stages
- Inventory capabilities. List every model, agent, tool, credential, data store, handoff, and downstream side effect.
- Set risk tiers and policies. Define allowed scope, approval requirements, rate limits, retention, and emergency controls for each tier.
- Instrument before optimizing. Add correlation IDs, policy decisions, tool arguments, outcomes, state changes, errors, and timestamps across the execution path.
- Enforce at boundaries. Put authorization, schema validation, complete mediation, approval checks, and rate limits in gateways or downstream services.
- Establish baselines. Replay representative tasks and record normal sequences, volumes, destinations, and failure behavior.
- Start with high-confidence alerts. Route clear policy violations and high-impact actions to named responders before adding noisier anomaly rules.
- Exercise containment. Verify that responders can pause tasks, revoke credentials, disable tools, and preserve evidence without logging into the agent’s own environment.
- Review continuously. Audit permissions periodically, update tests after every material change, and retire telemetry that no longer supports a security or operational decision.
Common monitoring failures and fixes
“We log everything, so the agent is safe.”
Complete logs improve investigation but do not stop an unauthorized action. Add least-privilege credentials, downstream authorization, approval gates, complete mediation, and rate limits.
“The model will refuse actions outside its instructions.”
Models can misinterpret goals, follow hostile tool output, or be manipulated by retrieved content. Treat model decisions as proposals and enforce policy independently.
“An approval happened earlier in the workflow.”
Approval must bind to the exact action, target, parameters, identity, and validity window. Revalidate after retries, handoffs, and state changes.
“Our dashboard shows tool calls but not memory changes.”
Instrument internal state and knowledge-base writes with provenance and policy decisions. Persistent state can alter future behavior even when the immediate tool call looks harmless.
“An anomaly detector alerts constantly.”
Use tested baselines, risk-tiered thresholds, correlation by task and identity, and a human tuning process. Do not lower thresholds blindly or disable alerts without recording the decision.
“The observability vendor says it supports agents.”
Ask whether support includes handoffs, internal state, approvals, denied actions, and downstream outcomes. Confirm whether the product enforces runtime policy or only records events.
What good looks like
A mature system makes unsafe behavior difficult, visible, and containable. The agent receives a narrow capability set and short-lived permissions. Every call is authorized by a trusted service, validated against the current task and target, and rate-limited. High-impact actions stop for a human decision tied to the exact payload. Traces connect the task, model, tool, state changes, policy checks, and downstream result. Rules and baselines identify suspicious deviations, while responders can preserve evidence, pause execution, revoke access, and recover.
NIST’s AI Risk Management Framework remains a voluntary, cross-lifecycle framework and is under revision. Its February 2026 Agent Standards Initiative emphasizes interoperable standards, open protocols, and agent security and identity research. These efforts reinforce the need for portable telemetry and explicit controls, but they do not replace an organization’s own authorization, approval, and incident procedures.
